ABSTRACT Background: Helicobacter pylori has been extensively studied since 1982 it is estimated that 50% of the world population is affected. The literature lacks studies that show the change of its prevalence in the same population over time. Aim: To compare the prevalence of H. pylori in 10 years interval in a population that was submitted to upper endoscopy in the same endoscopy service. Method: Observational, retrospective and cross-sectional study comparing the prevalence of H. pylori in two samples with 10 years apart (2004 and 2014) who underwent endoscopy with biopsy and urease. Patients were studied in three consecutive months of 2004, compared to three consecutive months of 2014. The total number of patients was 2536, and 1406 in 2004 and 1130 in 2014. Results: There were positive for H. pylori in 17 % of the sample as a whole. There was a significant decrease in the prevalence from 19.3% in 2004 to 14.1% in 2014 (p<0.005). Conclusion: There was a 5.2% reduction in the prevalence of H. pylori comparing two periods of three consecutive months with 10 years apart in two equivalent population samples.

Background: Dermatophytosis is a common fungal infection affecting 20-25% of the world population. Aims: Our study was aimed to assess its impact on health-related quality of life(QoL), mental health, and various variables. Materials and Methods: A cross-sectional study was done from April 2019 to September 2019 on 174 patients of dermatophytosis of aged more than 16 years with their informed consent. The impact of infection on the quality of life was assessed by using the Dermatology life quality index questionnaire and General health questionnaire-12 was used to assess psychological impact. A visual analogue scale was used to assess the severity of pruritus. Appropriate statistical tests were applied. Results: Males to females ratio was 1.4:1. The age group of 21-30 was having the highest number of patients with the mean age of 27.8±9.97. Most patients had BSA under 10%. The mean value of DLQI and GHQ-12 were found 15.989±7.407 and 2.8563±2.8964, respectively. We found that dermatophytosis had a very large effect on the quality of life as the maximum number of patients(39%) were within this category. The “work and school” part in the questionnaire gained maximum importance(52.8%). The mean VAS score was 6±2.733 with most patients(32.7%) had moderate itching. We found a positive correlation between VAS and DLQI, VAS and GHQ-12, DLQI, and GHQ-12 with the statistical significance. Conclusion: In our study dermatophytosis affected the quality of life as well as the psychological health of patients. Therefore proper treatment of superficial dermatophtytosis is essential to prevent it from further complications

CONTEXT AND OBJECTIVE: Helicobacter pylori infection is mainly acquired during childhood, and is associated with significant morbidity in adults. The aim here was to evaluate the seroprevalence and risk factors of H. pylori infection among children of low socioeconomic level attended at a public hospital in São Paulo, Brazil. DESIGN AND SETTING: Cross-sectional study, among patients attended at an outpatient clinic. METHODS: 326 children were evaluated (150 boys and 176 girls; mean age 6.82 ± 4.07 years) in a cross-sectional study. Patients with chronic diseases or previous H. pylori treatment, and those whose participation was not permitted by the adult responsible for the child, were excluded. The adults answered a demographic questionnaire and blood samples were collected. The serological test used was Cobas Core II, a second-generation test. Titers > 5 U/ml were considered positive. RESULTS: H. pylori infection was diagnosed in 116 children (35.6%). Infected children were older than uninfected children (7.77 ± 4.08 years versus 5.59 ± 3.86 years; p < 0.0001). The seroprevalence increased from 20.8% among children aged two to four years, to 58.3% among those older than 12 years. There were no significant relationships between seropositivity and gender, color, breastfeeding, number of people in the home, number of rooms, bed sharing, living in a shantytown, maternal educational level, family income or nutritional status. In multivariate analysis, the only variable significantly associated with H. pylori seropositivity was age. CONCLUSION: Infection had intermediate prevalence in the study population, and age was associated with higher prevalence.

Background: Helicobacter pylori causes a major health problem worldwide; more than half of the world’s population are infected with this pathogen. The diagnosis of the infection was initially made through invasive methods, but now non-invasive methods have been developed to make diagnosis easier. This study aimed to screen the presence of H.pylori antibodies and antigen among symptomatic and asymptomatic patients at Tamboul City in Gezira State. Methods: A cross-sectional study was conducted in Tamboul city, Gezira State, Sudan between March 2016 and December 2019 to compare between antigen and antibody tests results used for diagnosis of H. pylori infection among symptomatic and asymptomatic Sudanese patients. Stool and blood samples were collected and analyzed for presence of antigen and antibodies to H. pylori using immunochromatography (ICT) cards. Results: Serum and stool samples were collected from 100 patients; 50 were symptomatic and 50 were asymptomatic. In symptomatic patients, 18/50 (36%) were men (32; 64%, women) with mean age of 16.7±24.6 years. In this group, 35/50 (70%) showed positive results for stool antigen, while 30/50 (60%) were positive for serum antibodies. In asymptomatic patients, 19/50 (38%) were men (31; 62%, women) with mean age of 16.7±20.4 years. In this group, 18/50 (36%) were positive for stool antigen and 25/50 (50%) for serum antibodies. There was a significant association between antigen results and patient group (P=0.001), but there was an insignificant association between antibodies results and patient group (P=0.317). Age group, history of infected persons in the family, blood group, and previous treatment were all not associated with H. pylori infection (P≥0.05). Conclusion: The frequency of H. pylori antigen was higher than antibodies in symptomatic patients, while the frequency of H. pylori antibodies was higher than antigen in asymptomatic patients.

Abstract Aim There is insufficient knowledge on the * duodenal histology and Helicobacter pylori infection in malnourished Bangladeshi children. Therefore, we attempted to explore the prevalence of H. pylori infection and duodenal histopathology in 2-year-old chronic malnourished Bangladeshi slum-dwelling children and investigate their association with dyspeptic symptoms. Methods This cross-sectional study was conducted using the data of the Bangladesh Environmental Enteric Dysfunction study in an urban slum of Dhaka, Bangladesh. With a view to address the association of environmental enteric dysfunction (EED) with stunting, upper gastrointestinal endoscopy was performed on 54 chronic malnourished children {31 stunted [length-for-age Z-scores (LAZ) &lt;−2] and 23 at risk of stunting (LAZ &lt;−1 to −2)} aged between 12–24 months and the mucosal biopsies were subjected to histopathological examination after obtaining proper clinical history. Stool antigen for H. pylori (HpSA) was assessed to determine H. pylori status. Results In all, 83.3% (45/54) of the children had histopathological evidence of duodenitis. Chronic mild duodenitis was found to be the most prevalent form of duodenitis (53.7%) in the children. Only 8.9% (4/45) of the children with duodenitis had dyspepsia (p &lt; 0.05). The 14.8% (8/54) of the children were found positive for H. pylori infection. Logistic regression analysis revealed children positive for HpSA had significant association with dyspepsia (OR 9.34; 95% CI 1.54–56.80). Conclusions The number of chronic malnourished children suffering from duodenitis was found to be very high. Majority of these children was asymptomatic. Children positive for HpSA had significant association with dyspeptic symptoms.

Background. The most common cause of chronic gastritis is infection with Helicobacter pylori. Identifying the relationship between intensities of colonization and activity of gastritis helps the clinician in more effective treatment and posttreatment follow-ups. Methods. In this cross-sectional study, endoscopic gastric biopsy samples of 544 patients who complained symptoms of dyspepsia for more than three months referring to the laboratory were studied. To determine the colonization rate of H. pylori and other pathological findings, Giemsa and H&E stains were, respectively, used. Results. Among 544 subjects, 47 (8.64%) patients had no gastritis, 203 (37.32%) had mild gastritis, 278 (10.51%) suffered moderate gastritis, and 16 (2.94%) had severe gastritis. In this study, patients with mild H. pylori colonization rates had the highest level of mild activity (33.52%); in contrast, those with severe H. pylori colonization had the highest level of severe activity (43.75%). 93.96% of people with severe H. pylori colonization suffered from moderate and severe chronic gastritis. There is a significant statistical relationship between the intensity of H. pylori colonization and histopathological findings including intestinal metaplasia, atrophy, and lymphoid follicle formation. Conclusions. According to the present study, with increasing intensity of H. pylori colonization, chronicity and activity of gastritis and its complications increase.

Abstract Background The association between H. pylori (Helicobacter pylori) infection and gastroesophageal reflux disease (GERD) is a complex and confusing subject, so we designed this study. Method In a cross-sectional study, all patients referred for endoscopy due to dyspepsia were enrolled. The diagnosis of erosive GERD was made by endoscopy. Patients with normal esophagus were selected as comparison group. Random gastric biopsies were taken from all participants to diagnose H. pylori infection. Result In total, 1916 patients were included in this study, of whom 45.6% had GERD. The mean age (SD) was 42.95 (16.32). Overall, 1442 (75.3%) patients were positive for H. pylori infection. The frequency of H. pylori infection in mild GERD patients was higher than the severe GERD, but this difference was not significant (P=0.214). Except for sociodemographic status (P < 0.001), other variables including gender, age, ethnicity, body mass index (BMI), smoking, and presence of hiatus hernia in patients had no significant association with the frequency of H. pylori infection. According to Robust Poisson regression models analysis, the association of H. pylori (PR 1.026;95%CI 0.990-1.064; P=0.158) and sociodemographic status were not significantly different between the two groups. But smoking, increased BMI, older age, presence of hiatus hernia, and peptic ulcer diseases were significantly associated with GERD compared with the non-GERD group. Conclusion In our results, there was no association between H. pylori infection and erosive GERD. Further studies are recommended, especially considering the effects of pathophysiological and other confounding factors on H. pylori and GERD.

Abstract Background: Blood culture negative sepsis is a diagnostic challenge to both clinicians and microbiologists. We aimed to investigate blood culture negative cases of community-acquired sepsis among children admitted at pediatric intensive care unit of Cairo University Specialized Children Hospital, Egypt. Methodology: Our cross-sectional study was carried out over a period of six months. Electrical cardiometry was used to assess the patients’ status. For blood culture negative samples, broad range bacterial and fungal PCR amplification and sequencing were performed. Results: Among 43 patients, 9 samples were positive to pan bacterial 16srRNA gene, while five samples (20,22,23,24,25) were identified as Helicobacter pylori positive with the following sequence accession numbers respectively (KT198991.1, JQ323580.1, APEL01000010.1, APEL01000010.1, CP007603.1). Patients H pylori positive higher C reactive protein, longer duration of ventilation, higher stroke volume variation and pre-ejection period (P value= 0.005, 0.005, 0.043 & 0.043) Conclusion: Detection of Helicobacter pylori in community acquired septic children alarms the necessity to conduct epidemiological studies on populations of septic shock in countries with high prevalence of H. pylori such as Egypt.

Background: Helicobacter pylori is the most common chronic bacterial infection globally and is the underlying cause of many disorders, including dyspepsia. Also, the prevalence of gastric cancer in Zanjan city is high. Therefore, this study investigates the prevalence of H. pylori infection in patients with dyspepsia by the H. pylori stool antigen (HpSA) test. Materials and Methods: This cross-sectional study was performed on patients over 18 years of age who were referred to Shafieieh Special Clinic in Zanjan with dyspepsia during 2019-2020. First, the HpSA test was examined for H. pylori infection. Since the results of two separate tests are needed to prove that H. pylori infection is negative, the serological test was used in the patients with HpSA negative results. Results: Based on the demographic information of the present study, most patients (70.1%) were under 40 years old, and most (55.1%) were women. Smoking, alcohol, drug, and non-steroidal anti-inflammatory drugs (NSAIDs) use in patients were 16.5%, 6.3%, 7.9%, and 39.4%, respectively. The highest and lowest symptoms were abdominal pain (44.9%) and burp (22%), respectively. Except for the relationship between positive serological results and age (P˂0.001), no significant relationship was found between other variables (P≥0.05). Based on the HpSA test, H. pylori infection was seen in 56.7% of patients. By the serological test, another 29 patients had H. pylori infection. Therefore, the total number of patients was 101, and the overall prevalence of H. pylori increased to 79.5%. Conclusion: The results of the present study showed that the overall prevalence of H. pylori infection was high compared to previous studies. Also, above the age of 40 years, female gender in HpSA test and male gender in serology test, higher education level, and the presence of risk factors (smoking, alcohol, and NSAIDs) have a higher positive test for H. pylori.

Background In Gastroenterology practice, worldwide, the most common cause of dyspepsia is functional. Functional or non-ulcer dyspepsia is established by gastroduodenoscopy which rules out structural disorders in dyspeptic patients. Helicobacter pylori, a gram-negative bacterium in gastric mucosa is associated with non-ulcer dyspepsia, chronic gastritis, gastriculcer and cancer. Worldwide prevalence of Helicobacter pylori infection is higher but its association with non-ulcer dyspepsia is less clear.Material and Methods The aim of this study was to see the prevalence of H. pylori infection in non-ulcer dyspepsia. A cross-sectional study of 340 patients presented at Nobel Teaching Hospital in one year with dyspeptic symptoms underwent clerking, physical examination, gastroduodenoscopy and RUT. Symptomatic patients without any structural lesions were designated as functional dyspepsia. RUT when turned red indicated positive for H. pylori infection.Result Out of 340 patients, 180 (52.9%) were female and 160(47.1%) were male. Mean age of male and female patients was 35.88 ± 11.8 and 38.11 ± 11.7 respectively. Amongst all participants 150 (44.11%) were housewives and 69(20.3%) were students. Endoscopic findings showed gastritis 205(60.29%) and duodenitis 15(4.42%). RUT was found positive in 62% of gastritis and 86.7% of duodenitis patients (p value=0.001).Conclusion High prevalence of H pylori infection in present study may be one of the causative factors in producing symptomatic non-ulcer dyspepsia. Hence, early detection and complete eradication of H.pylori infection is mandatory. ObjectivePrevious clinical studies in adults from developed countries have implicated Helicobacter pylori infections in the development of thrombocytopenia. However, studies in children, particularly those from low-income countries, are unusually scarce. We examined the association between H. pylori infection and platelet indices in young Ethiopian school children.DesignCross-sectional studySettingThis study was conducted in five elementary schools located in central Ethiopia.ParticipantsBlood and stool samples were collected from 971 children across five elementary schools in Ethiopia. H. pylori infection was diagnosed using stool antigen and serum antibody tests, and haematological parameters were measured using an automated haematological analyser. An interviewer-led questionnaire administered to mothers provided information on demographic and lifestyle variables. The independent effects of H. pylori infection on platelet indices were determined using multivariate linear and logistic regressions.Study outcomesH. pylori-infected children had a lower average platelet count and mean platelet volume than uninfected after adjusting the potential confounders (adjusted mean difference: −20.80×109/L; 95% CI −33.51 to −8.09×109, p=0.001 and adjusted mean difference: −0.236 fL; 95% CI −0.408 to −0.065, p=0.007, respectively). Additionally, H. pylori-infected children had lower red blood cell counts (adjusted mean difference: −0.118×1012/L; 95% CI −0.200 to −0.036, p=0.005) compared with non-infected.ConclusionOur study from a developing country provides further support for an association between H. pylori infections and reduced platelet indices in young Ethiopian school children, after controlling for potential confounders. Further research is needed, particularly longitudinal studies, to establish causality.
