Abstract Access to health care is a sensitive issue in low population density territories, as these areas tend to have a lower level of service provision. One dimension of access is accessibility. This paper focuses on measuring the accessibility to urgent and emergency care services in the Portuguese region of Baixo Alentejo, a territory characterized by low population density. Data for the calculation of accessibility is the road network, and the methodology considers the application of a two-level network analyst method: time-distance by own mean (car or taxi) to the urgent care services and the time distance to emergency services as a way to get assistance and to go to urgent care services. While urgent care accessibility meets the requirements stipulated in the Integrated Medical Emergency System’s current legislative framework, the simulation of different scenarios of potential accessibility shows intra-regional disparities. Some territories have a low level of accessibility. Older adults, the poorly educated, and low-income population, also have the lowest levels of accessibility, which translates into dually disadvantageous situation since the potential users of emergency services are most likely to belong to this group of citizens.

Central to this article is the issue of choosing sites for where a fieldwork could provide a better understanding of divergences in health care accessibility. Access to health care is critical to good health, but inhabitants may experience barriers to health care limiting their ability to obtain the care they need. Most inhabitants of low-income countries need to walk long distances along meandering paths to get to health care services. Individuals in Malawi responded to a survey with a battery of questions on perceived difficulties in accessing health care services. Using both vertical and horizontal impedance, we modelled walking time between household locations for the individuals in our sample and the health care centres they were using. The digital elevation model and Tobler’s hiking function were used to represent vertical impedance, while OpenStreetMap integrated with land cover map were used to represent horizontal impedance. Combining measures of walking time and perceived accessibility in Malawi, we used spatial statistics and found spatial clusters with substantial discrepancies in health care accessibility, which represented fieldwork locations favourable for providing a better understanding of barriers to health access.

Objetivo: Conhecer o comportamento de saúde de mulheres e os motivos para buscar assistência em um pronto atendimento. Método: Estudo descritiva e exploratória, de natureza qualitativa com 18 mulheres atendidas em um pronto atendimento municipal no noroeste do Paraná. Os dados foram coletados em novembro de 2015 por entrevista semiestruturada e submetidos à análise de conteúdo, modalidade temática. Resultados: Foram identificadas duas categorias empíricas: “Comportamentos de mulheres diante de intercorrências na saúde”, mostra que as condutas iniciais das mulheres, em situações de adoecimento, são automedicação e protelamento em procurar serviços de saúde; e “Motivos para procurar o serviço de pronto atendimento”, onde identificou-se que a procura deste nível de atendimento é motivada pela percepção de maior resolutividade, eficácia e agilidade, além de proximidade do lar. Conclusão: É comum às mulheres retardarem a procura por atendimento em virtude das responsabilidades inerentes ao gênero, e quando o fazem, preferem serviços mais resolutivos.Descritores: Serviços de Saúde; Assistência Ambulatorial; Saúde da Mulher.

An emergency can happen anywhere and anytime, especially in developing countries with a high potential for emergencies, such as Eastern European countries as well as Indonesia. This study aimed to find out the quality of PSC 119 Si Slamet as a prehospital emergency service innovation. The data collection in this study was carried out in a location, namely, Batang Regency, Indonesia, in May–June 2018. The qualitative data collection methods used in this study are in-depth interviews and document reviews. This study was using Service Quality (Servqual) questionnaire. The results show that PSC 119 Si Slamet provides easy access to emergency services to the community 24 hours a day and 7 days a week by simply calling 119 numbers, sending messages via SMS and WhatsApp, or using the Android-based application, with a maximum response time target of 10 minutes. Batang is one of the regencies (rural area) in Central Java province, located on the main coastline, with a hilly geographic condition with many derivatives, climbs, and sharp curves, which is one of the causes of the high number of traffic accidents in the area. This emergency care information systems, with Android-based application, was aimed at improving the quality of services in the health sector, especially emergency services. This service is of good quality as seen from the tangible, reliability, responsiveness, assurance, and empathy dimensions. However, in the implementation, the socialization aspect is not the best to some people. The recommendation given was the need to increase the PSC 119 socialization of Si Slamet not only regionally but also internationally to be massive, especially in developing countries.

India, with its billion inhabitants, requires better emergency services to meet the growing demand for faster critical care facilitation. The scientific advances in the field of information and communication technology have contributed to the implementation of various e-health initiatives by various state governments within the country to improve the quality, access, and delivery of emergency care. “108 Emergency Response Service” is an e-health project established by the government of Tamilnadu state in India to render emergency services to the people. A qualitative study of the effectiveness of Information and Communication technologies in this Emergency Response Service (108 Emergency Service) in Coimbatore district of Tamilnadu state (http://www.coimbatore.tn.nic) was done and recommendations to increase the effectiveness were provided. The results reveal that the efficiency and effectiveness of 108 ERS could be greatly enhanced by providing better telecommunication facilities in rural areas and by deploying Global Positioning System (GPS) and Geographic Information System (GIS) and Automatic Vehicle Location (AVL) technologies to reduce the response time of the emergency vehicles.
