ABSTRACT Objective: to compare electronic and manual prescriptions of a public hospital of Brasilia, identifying risk factors for the occurrence of medication errors. Method: descriptive-exploratory, comparative and retrospective study. Data collection occurred from July 2012 to January 2013, using an instrument for the review of the information contained in medical records related to the medication process. A total of 190 manual and 199 electronic records composed the sample, with 2027 prescriptions each. Results: compared to the manual prescription, a significant reduction was observed in the risk factors after implantation of the electronic prescription, in items such as "lack of the form of dilution" (71.1% to 22.3%) and "prescription with brand name" (99.5% to 31.5%). Conversely, the risk factors "no check" and "lack of CRM of the prescriber" increased. The lack of the allergy registration and the occurrences related to medication were the same for both groups. Conclusion: generally, the use of the electronic prescription system was associated with a significant reduction in risk factors for medication errors, concerning the following aspects: illegibility, prescription with brand name and presence of essential items that provide a safe and effective prescription.

ABSTRACT Objective: To analyze and characterize the use of night beds in a Psychosocial Attention Care Center for Alcohol and Drugs (Centro de Atenção Psicossocial Álcool e Drogas – CAPS ad). Method: It is a quantitative, documental, descriptive and retrospective study. Data were gathered from 565 medical records. An analysis of continuous variables was performed. Results: When admitted to the beds, most users (87.6%) consumed multiple substances daily and were vulnerable, specially in street situation (68.3%). These users were admitted on an average of two times, undergoing a previous evaluation by the nurse (85.8%), usually for detoxication or due to the vulnerable condition. They stayed in the center for an average of seven days and 31.1% did not finish what was proposed. For a few cases, hospital support was needed. Overall, discharges were planned, but the return happened without booking. Conclusion: Social issues cut through the use of night beds, however, it is a therapeutic resource that meets significant demands and is present in the daily lives of vulnerable users as a comprehensive care.

Medication errors are one of the major problems mostly seen in the hospitals. Manual prescription of medicine is difficult now days, so electronic prescription came into form. It is the alternative to the current method of manually prescription in hospitals. This paper introduces a simple and easy classification technique that can be used to prescribe drugs according to the symptom parameters and if higher rate of symptoms exist then a better hospital for the treatment is suggested for the patient. It has many benefits for those who prescribe and dispense the medicines, and also for maintenance of medical records. The complexity of the usage of medications has increased enormously. Due to the higher demand it can lead to a greater risk of errors. Hence the usage of E-prescribing has been developed out of it with less error rate. It provides better and more reliable information about the patient’s medication. It saves staff time and improves the availability of patient information when needed. It also reduces the time spent by rewriting the charts of prescription etc.

The objective of this retrospective study was to identify risk factors for screw migration after triple pelvic osteotomy (TPO) in clinical patients. The medical records, radiographs made immediately after surgery, and follow-up radiographs documenting a healed osteotomy were reviewed for 52 dogs treated with unilateral TPO and 38 dogs treated with bilateral TPO. Signalment, surgeon expertise, length of surgery, sequence of surgery in dogs treated bilaterally, use of ischial or ilial wires or both, screw depth in the sacrum, and screw migration were documented for each of the 128 pelvic osteotomies. Screws placed in the first and second plate hole, securing the cranial portion of the plate, loosened most frequently. Factors associated with decreased screw migration included use of an ischial hemicerclage wire and increased depth of sacral purchase with the first and second cranial screws.

Abstract Background: The outbreak of coronavirus disease 2019 (COVID-19) becomes an enormous threat to all human beings. Via this retrospective study conducted on medical records of confirmed COVID-19 pneumonia patients on admission, we investigate the CT manifestation and clinical and laboratory risk factors associated with progression to severe COVID-19 pneumonia and assessed the association among clinical and laboratory records, CT findings, and epidemiological features. The medical records and radiological CT Features of 236 confirmed COVID-19 patients were reviewed at one public hospital and one respiratory clinic in Quom, from 1 August to 30 September 2020. Results: Among a total of 236 confirmed Covid-19 cases, 62 were infected with moderate to severe COVID-19 disease and required hospital admission, and 174 were followed up on outpatient bases. A significant difference was verified in the mean age between outpatients and hospitalized groups. The incidences of bilateral lung involvement, consolidation, linear opacities, crazy-paving pattern, air bronchogram sign, and the number of lobe involvement were significantly higher in hospitalized groups. However, only the crazy-paving pattern was significantly associated with an SpO2 level lower than 90%, with clinical sign of cough severity. Our data indicate that this pattern is also significantly associated with inflammatory levels and the presence of this pattern along with SpO2 level lower than 90%, older age, diabetes, on admission are independent risk factors for COVID-19 progression to severe level.Conclusions: The crazy-paving pattern can predict the severity of COVID-19, which is of great significance for the management and follow-up of COVID-19 pneumonia patients. The clinical factors of aging, male gender, and diabetes, may be risk factors for the crazy-paving pattern, whereas severe coughing is considered to be the most important clinical symptom related to this pattern, and SpO2 level lower than 90%, which is a matter of more severity.

Abstract: Non-infectious skin disease is a skin disease that is not caused by pathogenic bacteria or by transmission. Many non-infectious skin diseases showed different manifestations in infants and children. This study aimed to obtain the patterns of non-infectious skin diseases in children at Dermatovenorology Department Prof. Dr. R. D. Kandou General Hospital period 2009-2011. This was a retrospective study. Data of the annual cases, gender, and ages were obtained from the medical records. From a total of 595 patients in 2009-2011 there were 323 (54.29%) children with non-infectious skin diseases, females (167 patients, 51.70%) were more frequent than males, and most commonly in the age group of 5 to 14 years (59.13%). Conclusion: Non-infectious skin diseases were still common in Dermatovenorology Department Prof. Dr. R. D. Kandou Manado periode 2009-2011.Metode Penelitian dilakukan secara retrospektif pada pasien anak dengan penyakit kulit non-infeksi yang datang ke Poliklinik Kulit dan Kelamin pada tahun 2009-2011. Data diperoleh dari rekam medik mengenai jumlah kasus pertahun, jenis kelamin, dan umur. Hasil penelitian memperlihatkan dari sejumlah 595 kunjungan pada tahun 2009-2011 terdapat 323 (54,29%) pasien anak dengan penyakit kulit non-infeksi, distribusi jenis kelamin memperlihatkan perempuan yang terbanyak 167 (51,70%), dan ter banyak pada kelompok umur 5-14 tahun (59,13%). Hydatiform mole is an important disease, with a high incidence, many risk factors, and equity spreading. To know and compareb-hCG levels among patients with hydatiform mole before and after curretage was investigated. A retrospective study from January2002 to December 2005 at Dr. Wahidin Sudirohusodo Public Hospital was performed comprising b-hCG levels of hydatiform molepatients before and after curettage. b-hCG level (Elisa’s method) were grouped by age of pregnancy. Among 72 patients, 43 patients withhydatiform mole were found. At trimester I among 10 patients (23.3%), b-hCG level was higher 3 (7.0%), normally 4 (9.3%) and lower3 (7.0%). At trimester II, among 33 patients (76.7%), b-hCG level was higher 12 (27.9%), normal 13 (30.2%) and lower 3 (7.0%).After curretage, b-hCG level was decreased in 35 (81.4%), and increased in 7 (16.3%). Diagnosis of hydatiform mole was establishedin 36 (83.7%) and Gestational Trophoblastic Tumours (GTT) 7 (16.3%). Patients with hydatiform mole had the highest incidence intrimester II with normal b-hCG level and b-hCG level decreased after curretage.

Abstract Background Isolated vertigo attack history preceding the acute stroke were frequently accompanying with other focal neurological symptoms. To clarify the different clinical characteristics between isolated vertigo attack and vertigo symptom accompanying hemiplegia preceding stroke, we performed this 4-year retrospective study. Methods Medical records of 1283 patients hospitalized with vertigo symptom had been screened. Patients were divided into two groups: isolated vertigo attack history preceding the stroke defined as IVA group, vertigo symptom accompanying hemiplegia attack defined as VAH group. Clinic characteristics including ABCD2 score, infarction volume and location, relative risk factors and the following medical intervention were compared between the group. Results Patients featured with VAH had higher extracranial stenosis (21.2% vs. 9.0%, P < 0.01) and ABCD2 score (3.7 ± 1.9 vs. 2.3 ± 1.5, P = 0.03), patient with IVA showed a higher diabetic prevalence (40.9% vs. 29.7%, P = 0.02). The frequency of vertigo events tended to be more commonly in patient with VAH (median 3.1 vs. 5.5, p < 0.03). The total cerebral infarction volume in IVA group tended to be larger than VAH with a median of 4.56 cm3 versus 2.32 cm3 (p = 0.02). Additionally, less patients with IVA sought medical intervention when vertigo symptom occurred. Conclusions Clinical characteristics including ABCD2 score, total cerebral infarction volume and the location were different between AVH and IVH group. In addition, less patients in IVH cohort sought medical intervention when vertigo symptom occurred.

Objective: This study aims to find an association between hypospadias complicating factor for complications that occurred after hypospadias reconstruction, focusing on tubularized incised plate (TIP) as reconstruction techniques are most commonly used. Material & methods: The study data was obtained from medical records of urologic hypospadias patients in Cipto Mangunkusumo General Hospital, Jakarta. Samples are those who underwent hypospadias reconstruction in Cipto Mangunkusumo General Hospital, since April 2002 until May 2014 with a total sampling method. Data were analyzed using SPSS ver 14. Results: We collected 127 patients who underwent the TIP technique with a mean patient age and treatment duration was 7.58 years and 10.95 days. Fistule is the most common complication in patients with post-TIP hypospadias accounting for as high as 32 (25.2%). Bivariate analysis showed a significant correlation between the location of the hypospadias meatus (distal) with torsion of the penis (n = 2; p 0.019). Conclusion: On the basis of the technical complications TIP, such as fistules, meatus stenosis, urethral stricture, torsion of the penis, and glans dehiscence, then the indication for TIP should be clear or selective.

Abstract: Struma or goiter is the most widely thyroid gland disease in the world that causes enlargement of the thyroid gland. This disease can be classified based on clinical, physiological, and anatomical alteration. If the function of the thyroid gland is normal it is called as non-toxic struma that involves many risk factors. This was a descriptive retrospective study using the medical records of all non-toxic struma patients hospitalized at Prof. Dr. R. D. Kandou Hospital Manado from July 2014 to June 2016. There were 38 non-toxic struma patients in this study distributed based on sex, age, morphology, sosioeconomy, region of residences, laboratory results, and therapy. Conclusion: The majority of the patients were females, aged 41-50 years old, diagnosed as non-toxic uninodular struma, middle to low class of socio-economy, living in lowland areas, laboratory result as euthyroid, and treated with surgery.Keywords: struma, enlargement of the thyroid gland, normal Abstrak: Struma (goiter) merupakan penyakit kelenjar tiroid terbanyak di dunia yang menyebabkan pembesaran kelenjar tiroid. Penyakit ini dapat diklasifikasikan berdasarkan klinis, fisiologi, dan perubahan anatomi. Bila kerja kelenjar tiroid normal maka disebut struma non toksik dan melibatkan berbagai faktor risiko. Jenis penelitian ialah deskriptif retrospektif dengan menggunakan data rekam medik semua pasien rawat inap struma non toksik di RSUP Prof. Dr. R. D. Kandou Manado periode Juli 2014-Juni 2016. Terdapat 38 pasien struma non toksik yang didistribusi berdasarkan jenis kelamin, usia, perubahan anatomi, sosioekonomi, daerah tempat tinggal, hasil laboratorium, dan terapi. Simpulan: Majoritas pasien terdiri dari perempuan, usia 41-50 tahun, diagnosis struma non-toksik uninodusa, golongan ekonomi menengah ke bawah, tempat tinggal di dataran rendah, hasil pemeriksaan laboratorium cenderung eutiroid, dan terapi tindakan operatif. Kata kunci: struma, pembesaran kelenjar tiroid, normal.

The purpose of this retrospective study was to assess risk factors and complications affecting postoperative outcome of dogs with laryngeal paralysis treated by either unilateral arytenoid lateralization (UAL) or bilateral ventriculocordectomy (VCC). Medical records of all dogs having either UAL or VCC between 2000 and 2011 were analyzed. Twenty-five dogs had VCC and 20 dogs had UAL. The overall postoperative complications rates for VCC and UAL were similar (52% and 60%, respectively; P = .0887). Dogs that had UAL were more likely to have acute postoperative respiratory distress and aspiration pneumonia (P = .0526). Dogs with VCC were more likely to have chronic postoperative respiratory distress and aspiration pneumonia (P = .0079). Revision surgery was required in 6 dogs (24%) following VCC and 2 dogs (10%) following UAL. Sex, breed, presenting complaint, type of service provided, and concurrent diseases were not significantly associated with higher risk of either death or decreased survival time postoperatively with either procedure. Overall postoperative complication rates, required revision surgeries, and episodes of aspiration pneumonia were similar in dogs undergoing UAL and VCC surgeries. Dogs that had VCC appeared to have an increased risk of lifelong complications postoperatively compared with UAL; therefore, VCC may not be the optimal choice for treatment of laryngeal paralysis.
