Phenotypes of bronchial asthma during the health resort period and personalized programs of medical rehabilitation

The purpose of study was to elaborate methodology of assessment of effectiveness of a medical rehabilitation in patients with the bronchial asthma with use of the International Classification of Functioning, Disability and Health (ICF). Under observation there were 420 patients with bronchial asthma who received sanatorium treatment on the Southern coast of the Crimea. All patients received the clinical, laboratory and functional trial. For assessment of a condition of patients according to provisions of ICF, a set of domains was developed for patients with bronchial asthma. The health resort medical rehabilitation in the climatic resort leads to reliable increase of asthma control. Changes of continuants of the offered domains before carrying out sanatorium medical rehabilitation had reliable positive dynamics. A proposed methodology can be used for assessment of functional condition of patients with bronchial asthma under health resort medical rehabilitation.

The article is devoted to new possibilities of using halotherapy in the prevention, treatment and medical rehabilitation of children. In recent years, active work has been carried out to develop new effective methods of reproducing the aerodispersed medium in the form of haloingalation therapy or by group method — in the conditions of a halochamber. The therapeutic effect of halotherapy is determined by the biophysical properties of a dry highly dispersed aerosol of sodium chloride. The paper presents the biophysical properties of the main active factor of halotherapy, i.e. a dry highly dispersed aerosol of sodium chloride, and reveals some aspects of the formation of the mechanism of therapeutic action. The anti-inflammatory, draining, mucolytic, immunomodulatory, sanogenetic effect of haloaerosol is shown. In pediatric practice, haloaerosol therapy is used for medical rehabilitation of children with respiratory diseases (bronchial asthma, bronchitis), ENT pathology (rhinosinusitis, tonsillitis, pharyngitis), skin diseases. Halotherapy is the method of choice in the prevention of acute respiratory diseases and improving the health of children in general education institutions. The high efficiency of the complex application of halotherapy in various diseases in children has been established. A promising direction for the development of halotherapy is the development of combined exposure technologies. The high efficiency of the use of haloingalation therapy in combination with magnetic therapy in the treatment of acute rhinosinusitis in children has been proven. Based on the conducted studies, a favorable effect of the use of halotherapy and flutter therapy on clinical and functional parameters in children with bronchial asthma was revealed. The data on the possibility of combined use of halotherapy with a pulsed low-frequency electrostatic field in the medical rehabilitation of children suffering from bronchial asthma are presented. Simplicity, safety, the possibility of personalization of exposure and good tolerability by children expands the possibilities for using halotherapy in pediatric practice.

Over the last few years, the German Pension Insurance has implemented a new method of quality assurance for inpatient rehabilitation of children and adolescents diagnosed with bronchial asthma, obesity, or atopic dermatitis: the so-called rehabilitation treatment standards (RTS). They aim at promoting a comprehensive and evidence-based care in rehabilitation. Furthermore, they are intended to make the therapeutic processes in medical rehabilitation as well as potential deficits more transparent. The development of RTS was composed of five phases during which current scientific evidence, expert knowledge, and patient expectations were included. Their core element is the specification of evidence-based treatment modules that describe a good rehabilitation standard for children diagnosed with bronchial asthma, obesity, or atopic dermatitis. Opportunities and limitations of the RTS as a tool for quality assurance are discussed.

The current state of medical rehabilitation of servicemen at the sanatorium and health resort stage is outlined, the main issues of rehabilitation measures are shown. To increase the effectiveness of measures aimed at strengthening and preserving the health of military personnel, the order of their referral to medical rehabilitation in a sanatorium after completion of treatment in a day hospital (previously - only from military hospitals after inpatient treatment with a round-the-clock stay) was further defined. The further development of sanatorium-resort organizations of the Ministry of Defense of the 2nd stage of medical rehabilitation (somatic diseases, pathology of the peripheral nervous system and musculoskeletal system) seems promising, which will require taking measures to form medical rehabilitation units, equip them with medical equipment, and train doctors.

BACKGROUND: In the context of the COVID-19 pandemic, the relevance of rehabilitation measures for patients with COVID-associated pneumonia has increased. AIM: The study aimed to review the effect of bioacoustic stimulation of the respiratory system (BSRS) with high-intensity low-frequency sounds on the dynamics of functional indicators of the respiratory system and the cardiovascular system, as well as indicators of psychoemotional and somatic states, the severity of post-traumatic stress disorders (PTSD) and cognitive capabilities in patients who have had COVID-associated pneumonia in the process of their medical rehabilitation at a health resort immediately after treatment of the disease in a hospital. MATERIALS AND METHODS: The randomized controlled open parallel prospective clinical study involved 28 patients undergoing medical rehabilitation after suffering moderate to severe COVID-19 complicated by community-acquired bilateral polysegmental pneumonia (COVID-associated pneumonia) of moderate to severe severity. Spirometry, pulse oximetry, compression oscillometry, bioelectrography, Mississippi Scale (civil version), and cognitive speed test were used to assess the functional parameters of the respiratory and cardiovascular systems, as well as indicators of psychoemotional and somatic states, the intensity of PTSD and cognitive abilities of patients before and after the course of rehabilitation measures. RESULTS: According to significant differences in changes in the indexes of external respiration, cardiac activity, psychological status, and the intensity of PTSD in patients of the main and control groups before and after a course of rehabilitation measures, it has been reliably established that the use of BSRS as a part of a complex of rehabilitation measures significantly increases the effectiveness of medical rehabilitation at a health resort for patients after COVID-associated pneumonia. CONCLUSION: The possibility and feasibility of using medical technology of BSRS with high-intensity low-frequency sound in complex medical rehabilitation of patients after COVID-associated pneumonia at the health resort stage have been scientifically substantiated.

The purpose of the work is to identify the dependence of the effectiveness of medical rehabilitation (MR) of patients with cerebral atherosclerosis (CA) on the seasons at the Southern coast of Crimea (SCC). Materials and methods. A prospective, single-center clinical trial in 216 patients with CA. Assessment of the effectiveness of MR was carried out in accordance with the "International Classification of Functioning, Disability and Health" (ICF). Results. As a result of MR, the level of violations of all controlled functions (on average for all ICF domains) was reduced by 28-35% in different seasons of the year, which indicates the high efficiency of the year-round integrated health resort MR at the SCC. Findings. The effectiveness of MR in patients with CA is quite high in all seasons of the year, which testifies to the validity of indications for referring patients to the SCC health resorts throughout the year.

The aim of this work is to assess the influence of the climatic and weather characteristics of the South Coast of Crimea (SCC) on the effectiveness of health resort medical rehabilitation (MR) of patients with ischemic heart disease (IHD).Materials and methods. A study was carried out in a group of 276 patients with IHD. The influence of climatic and weather characteristics of the SCC were assessed using the methods developed by the «ARI n.a. I.M. Sechenov» - modified clinical index on weather pathogenicity (CIPP-M) and MR assessment according to the criteria of the» International classification of functioning, disabilities and health» (ICF).Results. Regression equations for the dynamics of the ICF b2401, b280, b4301 and b440 domains on the values of air temperature (AT-M), atmospheric pressure variability (APV), air temperature variability (ATV-M) wind speed (WS-M) and total CIPP-M have been developed, which allow to reliably form a rehabilitation prognosis for patients with IHD for all terms of health resort treatment in the SCC.Conclusions. The use of modernized clinical indices of weather pathogenicity allows one to adequately assess and predict the effect of weather dynamics and form a rehabilitation prognosis for patients with IHD.

In the structure of medical rehabilitation, the demand for restorative technologies with healing natural factors is steadily increasing. Rational exploitation of natural resources provides the core of the resort and recreation and sanatorium branches. Rehabilitation measures in the complex of sanatorium-resort therapy are optimally balanced and effective. A potential part of recovery techniques is the healing properties of climate, mineral water, natural heat carriers, etc. The Voronezh region is rich in reserves of mineral waters, brines and therapeutic mud in the operational waiting phase. Deposits of various clays are available everywhere. Argillotherapy developed based on gray clay (kaolinite) incision "Strelitsa Middle" at Latnoe settlement. Comparison is made of clinical effectiveness and economic effect of argillotherapy gray clay (kaolinite) in the complex of rehabilitation programs among patients of different ages in conditions local health resort.

Background. To study the influence of evergreen cypress blossoming period on the effectiveness of sanatorium and health resort treatment (HRT) of bronchial asthma (BA) patients in the South Coast of Crimea (SCC). Methods. The analysis of the data of 71 patients with BA, the residents of the Crimea was conducted. The survey of BA patients included the questioning, medical observation, physical examination, complete blood count, sputum cytology, lung function examination, motor (physical) abilities by 6 minute steptype test and quality of life. Results. Aeropalinological measurement in the SCC (Yalta) showed that the evergreen cypress’s blossoming period is continuing from February to April inclusive. The highest concentrations of cypress pollen were registered in March and in the first half of April. Efficiency of HRT of BA patients in the SCC did not depend on the pollen season of evergreen cypress. Conclusion. Pollen season of evergreen cypress (February - April inclusive) is not a contraindication for HRT of BA patients in the SCC.
