The Effect of Casein Phosphopeptide-Amorf Calcium Phosphate and Acidulated Phosphate Fluoride Gel on Dental Erosion in Primary Teeth: An in Vitro Study

Objective: This study aimed to investigate the effect of acidulated phosphate fluoride (APF) gel and casein phosphopeptide/amorphous calciumphosphate (CPP-ACP) on the dental erosion produced by carbonated soft drink in primary teeth. Study Design: This study evaluated by an in vitro model the effect of APF gel and CPP-ACP on the dental enamel previously subjected to erosive challenge with carbonated soft drink. Sixty sound human primary molars were prepared by embedding the crown sections in acrylic resin blocks leaving the enamel surfaces exposed. The surface roughness of the enamel was measured with prophilometry at baseline. Specimens were randomly divided into three treatment groups (n:20): artificial saliva, CPP-ACP, 1.23% APF gel. All specimens were then exposed to an erosive challenge of carbonated soft drink and artificial saliva for 20 cycles of 20 seconds each. Demineralization-remineralization cycles was repeated twice at eight-hour intervals and roughness values were measured. Enamel samples were treated with artificial saliva, CPP-ACP, 1.23% APF gel applied for 10 min after erosive challenge. The arithmetic average roughness (Ra) readings were recorded after remineralization agents were applied. Results: The mean surface roughness in all groups increased significantly after erosion process and decreased after remineralization treatment. After treatment, the mean surface roughness of the 1.23% APF gel group was significantly less than the other groups and the mean surface roughness of the artificial saliva group was significantly more than the other groups. 1.23% APF gel showed the highest protective effect against erosive enamel loss. Conclusions: Under the conditions of this study, artificial saliva, CPP-ACP and 1.23% APF treatments were able to reduce erosive enamel loss produced by carbonated soft drink in primary teeth. However, 1.23% APF gel showed the highest protective effect against erosive enamel loss.

Introduction The aim of this in vitro study was to assess and compare the effect of different intermediary bases on microleakage between tooth and a nanocomposite interface in Class II box cavities in primary teeth. Methods Standard Class II box cavities were prepared in 52 primary molars and randomly divided into 9 groups according to the intermediary base used (Multicore Flow, Fuji II LC, SDR, Smart Dentin Replacement, and Biodentine). All specimens were subjected to thermocycling and prepared for microleakage testing and evaluation. Results There was significant difference in the mean ranks of microleakage between the 9 groups, which was observed in the gingival side (p<0.0001) and the occlusal side (p<0.0001). The mean ranks microleakage was significantly higher with experimental SDR, experimental Multicore Flow, and positive control materials when compared with the other 6 groups. The microleakage mean ranks were statistically significantly lower in experimental Fuji II LC, experimental Biodentine, and all negative control groups when compared with the other 3 groups. Conclusions Microleakage is affected by the application of intermediate material. Experimental Biodentine and Fuji II LC showed the lowest microleakage while experimental SDR and experimental Multicore Flow showed the highest microleakage.

BACKGROUND: Erosion is a widespread phenomenon with higher predilection in primary dentition. AIM: The aim of the present study is to assess the remineralising effect of Er,Cr:YSGG laser application combined with CPP-ACPF after erosive demineralisation by Coca-Cola in primary teeth. METHODS: Fifty teeth (n = 10) were divided into; Group I: Artificial saliva, (Saliva natural, Medac, UK), Group II: CPP-ACPF (MI Paste Plus, GC Corp, USA), Group III: Er,Cr:YSGG (Waterlase iPlus, USA), Group IV: CPP-ACPF + Er,Cr:YSGG, Group V: Er,Cr:YSGG + CPP-ACPF. Teeth were immersed in Coca-Cola for 10 min, 5 times/day for 5 days. DIAGNOdent (DD) measurements were taken before and after the experiment. RESULTS: There was a significant increase in DD readings after erosive-treatment cycles in all test groups. The highest reading was in samples immersed in artificial saliva, and the lowest was in those subjected to combined CPP-ACPF and Er,Cr:YSGG laser application, regardless of the sequence used. There was no significant difference between samples immersed in artificial saliva, and after CPP-ACPF application. Similarly, there was no significant difference between samples treated by combined treatment of CPP-ACPF and Er,Cr:YSGG application. However, there was a significant difference between samples immersed in artificial saliva or treated with CPP-ACPF application and those subjected to combined treatment CPP-ACPF along with Er,Cr:YSGG. CONCLUSION: Combining Er,Cr:YSGG laser and CPP-ACPF paste significantly increased enamel remineralisation, regardless of the sequence implemented. Saliva naturally and CPP-ACPF application had a comparable effect on remineralisation.

Abstract Background: Clinical assessment and monitoring of dental erosion in vivo is challenging, although a number of clinical indices have been proposed but these are unlikely to be sensitive enough to quantify erosion with sufficient sensitivity to allow comparison of different therapies. In this respect, some of the techniques predominantly employed for the assessment of dental caries such as Quantitative light- induced fluorescence (QLF) and Optical coherence tomography (OCT) may also have utility for the assessment for erosion both in vivo and in vitro. The aim of the present study was to test three techniques (QLF, OCT and SMH) for the assessment of dentine erosion in a product testing model, using dentifrices to reduce erosion susceptibility. Dentine erosion was evaluated comparing the methods. Methods: Human dentine specimens were treated with one of two dentifrice slurries or mineral water (control group), followed by an erosive challenge and storage overnight in artificial saliva. These procedures were performed over 5 days. The following two dentifrices were tested: Duraphat™ 5000 (5000ppm Sodium Fluoride) and Colgate Sensitive Pro-Relief™ (8% arginine plus calcium carbonate and 1450ppm Fluoride). All groups showed a progressivedecrease in SMH and increase in scattering for OCT (P<0.05) indicating mineral loss. Results: The two dentifrice groups demonstrated significantly less softening than the control group (P<0.05). No statistically significantdifferences were detected for dentine erosion using QLF (P>0.05). Conclusion: SMH and OCT techniques were able to detect a significant protective effect against in vitro erosion whensamples were pre-treated with either dentifrice formulation. Dentifrices containing high concentrations of fluoride andarginine associated with calcium carbonate and fluoride have a protective effect against dentinal erosion.

SUMMARY Resin-based materials that show promising effects for preventing the progression of erosion have been studied. This in vitro study evaluated the effects of applying resin-based materials, including resin infiltration, on previously eroded enamel subjected to erosive challenges. The influence of enamel surface etching prior to application of the material was also studied. Bovine enamel blocks were immersed in hydrochloric acid (HCl), 0.01 M (pH 2.3), for 30 seconds in order to form a softened erosion lesion. The blocks were then randomly divided into nine groups (n=12) and treated as follows: C = control without treatment; Hel = pit & fissure resin sealant (Helioseal Clear); Adh = two-step self-etching adhesive system (AdheSe); Tet = two-step conventional adhesive system (Tetric N-bond); and Inf = infiltrant (Icon). The Helno, Adhno, Tetno, and Infno groups received the same materials without (or with no) surface conditioning. The depth of the material's penetration into softened erosion lesions was qualitatively analyzed using reflection and fluorescence confocal microscopy. After application of the materials, the blocks were immersed in HCl for two minutes; this step was followed by immersion in artificial saliva for 120 minutes four times a day for five days (erosive cycling). Both the enamel alteration and material thickness were analyzed using profilometry, and the results were submitted to Kruskal-Wallis and Dunn tests (p&gt;0.05). Images from the confocal microscopy showed minimal penetration of Adh/Adhno and deep penetration of Inf/Infno into the erosive lesions. The groups Hel, Adh, Inf, Tetno, and Infno resulted in the formation of a layer of material over the enamel, which was effective in inhibiting the progression of erosion. In conclusion, the infiltrant, with or without etching, was able to penetrate and protect the enamel against dental erosion. The other resin-based materials, except for the two-step conventional adhesive, were able to penetrate and inhibit the progression of erosive lesions only when they were applied after enamel etching.

The effect of solutions and gels containing a sugarcane-derived cystatin (CaneCPI-5) on the protection against enamel and dentin erosion in vitro was evaluated. Bovine enamel and dentin specimens were divided into two groups (n=135 and 153/group for enamel and dentin, respectively) that were treated with solutions or chitosan gels containing 0.1 or 0.25 mg/ml CaneCPI-5. The positive controls for solutions and gels were Elmex Erosion Protection™ solution and NaF gel (12,300 ppm F), respectively. Deionized water and chitosan gel served as controls, respectively. The solutions were first applied on the specimens for 1 min and the gels for 4 min. Stimulated saliva was collected from 3 donors and used to form a 2 h acquired pellicle on the specimens. Then, the specimens were submitted to an erosive pH cycling protocol 4 times/day for 7 days (0.1% citric acid pH 2.5/90s, artificial saliva/2h, artificial saliva overnight). The solutions and gels were applied again during pH cycling, 2 times/day for 1 min and 4 min, respectively, after the first and last erosive challenges. Enamel and dentin losses (µm) were assessed by contact profilometry. Data were analyzed by 2-way ANOVA and Tukey´s test (p <0.05). All the treatments significantly reduced enamel and dentin loss in comparison with controls. Both CaneCPI-5 concentrations had a similar protective effect against enamel erosion, but only the higher concentration was as effective against dentin erosion as the positive control. Regarding the vehicles, only the 0.1 mg/ml gel performed worse than the positive control for dentin. CaneCPI-5 reduced enamel and dentin erosion to a similar extent as the fluoride-containing vehicles. However, dentin requires higher CaneCPI-5 concentrations, in the case of gels. Solutions or gels containing CaneCPI-5 might be a new approach to protect against dental erosion.

Dental bleaching occurs due to an oxidation reaction between the bleaching agents and the macromolecules of pigments in the teeth. This reaction is unspecific and the peroxides can also affect the dental matrix causing mineral loss. On the other hand, recent studies have suggested that the thickener agent carbopol can also cause mineral loss. Thus, the objective of this study was to evaluate in vitro the effect of at-home dental bleaching on dental enamel microhardness after the use of bleaching agents with and without carbopol as a thickener agent. Bovine dental slabs with 3 x 3 x 3 mm were obtained, sequentially polished, and randomly divided into 4 groups according to the experimental treatment: G1: 2% carbopol; G2: 10% carbamide peroxide with carbopol; G3: carbowax; G4: 10% carbamide peroxide with poloxamer. Bleaching was performed daily for 4 weeks, immersed in artificial saliva. Enamel microhardness values were obtained before the treatment (T0) and 7 (T1), 14 (T2), 21 (T3), 28 (T4), and 42 (T5) days after the beginning of the treatment. ANOVA and Tukey's test revealed statistically significant differences only for the factor Time (F = 5.48; p < 0.01). All bleaching and thickener agents caused no alterations on the enamel microhardness.

This in vitro study was designed to compare the forces generated by commercially available elastomeric chains and NiTi closed coil springs, and to determine their force decay pattern. Forty elastomeric chains and forty NiTi closed coil springs were divided into 4 groups according to the following manufacturers: (1) Morelli®, (2) Abzil®, (3) TP Orthodontics® and (4) American Orthodontics®. The specimens were extended to twice their original length and stored in artificial saliva at 37°C. Initial force was measured by means of an Instron universal testing machine and then at 1, 4, 7, 14, 21, and 28 days. The results revealed that the elastomeric chains delivered a mean initial force of 347 g for Morelli®, 351 g for American Orthodontics®, 402 g for Abzil®, and 404 g for TP Orthodontics®. The NiTi closed coil springs generated a mean initial force of 196 g for American Orthodontics®, 208 g for TP Orthodontics®, 216 g for Abzil®, and 223 g for Morelli®. The mean percentage of force decay observed after 28 days for the elastomeric chains was 37.4% for TP Orthodontics®, 48.1% for American Orthodontics®, 65.4% for Morelli®, and 71.6% for Abzil®. After 28 days, the NiTi closed coil springs presented a mean percentage of force decay of 22.6% for American Orthodontics®, 29.8% for Abzil®, 30.6% for Morelli®, and 45.8% for TP Orthodontics®. At the end of the study, significant differences were observed between the elastomeric chains and the NiTi closed coil springs. The results indicated that the studied NiTi closed coil springs are more adequate for dental movement than the elastomeric chains.

An acidified dialyzed gelatin gel system was used to determine the caries resistance of a variety of restorative materials used to obturate the canal orifice of overdenture abutment teeth. The restorative materials used were Tytin, Tytin + Copalite, P30 + Scotchbond, Fuji Ionomer-Type II, and Miracle Mix. Polarized light microscopy and microradiography were used to examine the caries-like lesions adjacent to the restorations. The lesions formed in the Fuji Ionomer-Type II and Miracle Mix groups appeared arrested at the wall adjacent to the restoration, and did not penetrate apically down the wall as did those associated with the other restorative materials. The mean depths of lesions adjacent to Fuji Ionomer-Type II and Miracle Mix restorations were significantly less than those of Tytin, Tytin + Copalite, or P30 + Scotchbond.

ABSTRACT Objective: To investigate the effect of beverages′ temperature on the surface roughness, hardness, and color stability of a composite resin. Materials and Methods: Fifty specimens of the Filtek Z250 composite (3M ESPE, Dental Products, St.Paul, MN, USA) were prepared and initial roughness, microhardness, and color were measured. Then the specimens were randomly divided into five groups of 10 specimens each: Coffee at 70°C, coffee at 37°C, cola at 10°C, cola at 37°C, and artificial saliva (control). After the samples were subjected to 15 min × 3 cycles per day of exposure to the solutions for 30 days, the final measurements were recorded. Results: After immersion in beverages, the artificial saliva group showed hardness values higher than those of the other groups (P < 0.001) and the microhardness values were significantly different from the initial values in all groups except for the control group. Both cola groups showed roughness values higher than the baseline values (P < 0.05), while the other groups showed values similar to the baseline measurements. When ΔE measurements were examined, the 70°C coffee group showed the highest color change among all the groups (P < 0.05). Conclusion: High-temperature solutions caused alterations in certain properties of composites, such as increased color change, although they did not affect the hardness or roughness of the composite resin material tested.

The production of aldosterone and other corticosteroids by adrenal glands surgically removed from 5 normotensive subjects with renal disease of various types, 11 patients with arterial hypertension, and 2 with Cushing's syndrome was investigated in vitro by the incubation chromatographic technique. The rate of steroid formation per unit weight of tissue was markedly lower in severe and malignant hypertension and slightly higher in benign hypertension as compared with the rate in normotensive controls. The amount of steroid released varied widely from one gland to the other and showed marked overlapping between the various groups; these variations were most prominent in benign hypertension, less in the normotensive group, and least evident in severe and malignant hypertension. The response of steroidogenesis to ACTH stimulation in vitro was slightly reduced in severe and malignant hypertension. The mean output of aldosterone by adrenal glands from hypertensives was slightly above the mean value obtained with normotensive control glands. The percentage of aldosterone formation in respect to total steroid production was roughly correlated with the severity of hypertension. In four hyperplastic adrenals obtained from two cases of Cushing's syndrome, the in vitro formation of steroids per unit weight of tissue and the response to ACTH did not differ significantly from that found in glands obtained from the other patients under study. The value of in vitro studies for the assessment of the functional capacity of the adrenal cortex is discussed.
