The article describes a clinical case of the successful use of transnasal pterygopalatine ganglion blockade for the treatment of postdural puncture headache. The conditions for the application of this technique, the mechanism of its action, and the results are discussed. Results reveal the simplicity of the technical implementation of pterygopalatine ganglion blockade and the almost absent contraindications to its use, except for cases involving individual intolerance to topical anesthetic drugs. Hence, the technique can be an effective method for the complex therapy of postpunctional pain.

A clinical case of the development of classic Dressler syndrome in a young patient with acute diffuse myocarditis is described. Timely diagnosis, administration of glucocorticoid and long-term complex therapy using beta-blockers (carvedilol), mineralocorticoid receptor blockers (eplerenone) not only led to the disappearance of Dressler’s syndrome, but also to a fairly rapid recovery of the patient from the underlying disease.

Objectives. To compare the effectiveness of the topical administration of benzocaine and EMLA on oral pain and tactile sensitivity. Materials and Methods. A randomized, double-blind, split-mouth clinical trial was carried out with 20 volunteers. The sensorial and quantitative tests were applied before the contact with topical anesthetic and after the application. Results. In the superficial tactile perception test, when we compared each group singly, there were statistically significant values in the decrease of superficial tactile perception when compared to the moment prior to the application of anesthetic agents. For the sensitivity to mechanical pain, no statistical significant difference was observed at evaluated times. In the needle penetration test, in an intergroup analysis, we found a decrease in the pain sensitivity to needle penetration at 5 min (p=0.053) and at 10 min (p=0.019) after the contact of the anesthetic drug with the oral mucosa. Conclusion. The application of topical anesthetic drugs reduces the discomfort associated with this procedure, mainly until the first 10 minutes. Only the needle penetration sensitivity test showed sufficient sensitivity to reveal a difference in the anesthetic effect between EMLA and benzocaine. This trial is registered with RBR-2N2GSW.

The relevance of monitoring the course of coronavirus infection COVID-19 in combination with comorbid pathology is obvious, since this new infection with immunopathological pathogenesis is not fully understood.The article presents a clinical case of COVID-19 disease in a 14-year-old teenager from the Moscow region, who developed against the background of severe ulcerative ileocolitis of moderate activity. The new coronavirus infection contributed to the recurrence of ulcerative colitis, which required observation and treatment in a hospital setting.The examination revealed: mild hypochromic posthemorrhagic anemia, signs of hypercoagulation characteristic of the course of ulcerative colitis; excluded: opportunistic infections, pneumonia, acute intestinal infections and intestinal bleeding.The standard complex therapy of ulcerative colitis without the inclusion of immunosuppressors made it possible to avoid a more severe and complicated course of the new coronavirus infection. COVID-19 in the teenager was mild, antiviral therapy was not used.

The article presents the experience of using infusion thrombolytic agents in the complex therapy of patients with complicated diabetic foot syndrome. Presented was a vivid clinical case. This method made it possible to optimize the treatment of this group of patient.

Headache is one of the most common complications after lumbal punction or epidural anesthesia. The article presents a clinical case of post dural puncture headache in a 26-year-old woman, 25 days long. An intracranial hypotension was confirmed by the help of magnetic resonance imaging of the brain, a pituitary gland with a slightly convex upper contour and a rounded form of transverse venous sinus were found. Positive effect of rehidratation, analgesics, non-steroid anti-inflammatory and antiemetic drugs and drugs of caffeine in the treatment of post-dural puncture headache was noted.

Disorders in the periodontal tissues in generalized periodontitis have a significant negative impact on the patient’s somatic and psycho-emotional health, causing not only dental, but also medical and social difficulties. Although scientific research has been studied, the search for new drugs for treatment and prevention of periodontal tissue diseases, the introduction of new methods have been provided, but the prevalence of generalized periodontitis includes 88-100% among adults. Destructive periodontal changes in chronic generalized periodontitis (CGP) are often irreversible and, in combination with dysfunction of the dento-alveolar apparatus and premature loss of teeth, they impair the quality of patients’ life and decrease their social adaptation. SIgA is the most important external secretory immunoglobulin. The functions of sIgA are the following: agglutination of bacteria, change of bacterial metabolism, delayed colonization of microorganisms, neutralization of viruses, bacterial toxins, enzymes, decrease the virulence of infectious agents and increase the opsonization of microorganisms. Receptors of slgA molecules have an affinity for some proteins of pathogens and, therefore, provide the formation of passive immunity against bacterial and viral infections. The aim of the work was to evaluate the clinical efficacy of the proposed immunomodulatory treatment for patients with severe generalized periodontitis. To achieve this goal, a clinical case of the drug Derinat as an addition to the complex therapeutic treatment of severe generalized periodontitis in a patient with a deficiency of secretory immunoglobulin A in the oral fluid (54, 6 mg/l) was used and presented by us. The patient’s treatment was done comprehensively, purposefully, individually including local and general therapy, and effective, conservative, surgical and orthopedic measures in the conditions of outpatient supervision of patients were also performed. The periodontal protocol included following steps: 1) a comprehensive examination of general and dental status; 2) professional oral hygiene, elimination of aggravating factors, selective grinding of teeth; selection of individual care products; 3) the appointment of the antibacterial drug such as Azithromycin 500 mg - 7 days; 4) immunomodulatory therapy with the use of Derinat by applications in periodontal pockets for 15 minutes - 7 days; 5) prosthetics with partial removable plate prostheses on the upper and lower jaws; 6) evaluation of intermediate results of treatment: repeated clinical examination and completion of a control periodontal card. The data, which has been obtained, indicated a significant positive trend in treatment, the absence of complaints on 2nd day of treatment and clinical improvement on 3rd or 4th days. Dental status data (hygiene index (1,0), PMA index (0%), periodontal index (1,2), bleeding index (1) and depth of periodontal pockets (2-3 mm)) has been significantly improved after the proposed treatment. The extension of the remission period is more than a year. Conclusions. The use of immunomodulatory drugs in the complex therapy of patients with chronic generalized periodontitis can significantly improve the results of conservative treatment, reduce the treatment time and prolong the period of remission due to the pathogenetic effect on the key links of the immune defense in the oral cavity.

Background. Ependymomas are a group of glial tumours, usually occurring in the posterior cranial fossa, less often — in the lateral ventricles and spinal cord. Most often, the recurrence of ependymomas occurs in primary sites, or in the central nervous system (CNS). Ependymoma metastasis beyond the craniospinal system occurs rarely if ever.Description of a Clinical Case. A clinical example of extraneural metastasis to the bones and bone marrow in a 10-year-old patient with supratentorial anaplastic ependymoma after complex therapy has been presented. A review of published cases of the development of extraneural ependymoma metastasis in children has been presented. An attempt was made to consider possible risk factors for their development. Conclusion. Ependymal tumours are capable of extraneural metastasis to the bone and hematopoietic systems. Continued growth and metastasis lead to extremely unfavourable prognosis for the disease.

Idiopathic pulmonary fibrosis (IPF) is usually characterized by a chronic and slowly progressive course. According to several studies, a small number of patients with IPF (about 515%) develops an acute deterioration of deasese exacerbation of IPF. Exacerbations of IPF can occur at any time of the disease and sometimes becomes the first manifestation of IPF. Pulmonary hypertension in IPF is a fairly frequent complication, which leads to severe violations of gas exchange and reduced tolerance to physical stress. Currently, proven effective treatments for exacerbations of IPF do not exist, the management of this condition is based on supportive therapy (oxygen, respiratory support) and interventions with inadequate evidences (corticosteroids, immunosuppressant). During exacerbation of IPF a careful search of all the possible triggers is justified. In the presented clinical case of exacerbation of IPF there was demonstrated the efficacy of complex therapy including antifibrotic therapy, PAH-specific medicines and enhanced oxygen therapy.

The article presents a clinical case of a 23-year-old patient with an extremely severe congenital form of chronic intestinal pseudoobstruction coupled with a neuromyopathy,colon malrotation, malabsorption, bacterial overgrowth syndrome, cholelithiasis and gastrostasis, which excluded bowel transplantation. Long-term treatment in the intensive care unit with combined, mainly parenteral nutrition for 6 months, using antibiotics, prokinetics, intestinal decompression allowed to achieve partial stabilization of the patients condition and transfer to home treatment with the continuation of adequate complex therapy.
