Morbidity with malignant neoplasms offemale genital organs in Amur region makes up 38-45per 100000 women. Cervical and uterine carcinomae dominate through all malignant tumors. The authors point to the low rate оfcase detecting as to women with genital neoplasms. Late diagnostics lead to unfavourable prognosis. General cancer mortality decreased for the last three years but still remains high. To reduce the rate of oncological diseases a regional programme including tumor and pretumor screening is to be elaborated.

Pancreatic cancer is a lethal condition with poor outcomes and an increasing incidence. The unfavourable prognosis is due to the lack of early symptoms and consequent late diagnosis. An effective method for the early diagnosis of pancreatic cancer is therefore sought by many researchers in the field. Heparan sulfated proteoglycan-related genes are often expressed differently in tumors than in normal tissues. Alteration of the tumor microenvironment is correlated with the ability of heparan sulfated proteoglycans to bind cytokines and growth factors and eventually to influence tumor progression. Here we discuss the importance of glypicans, syndecans, perlecan and extracellular matrix modifying enzymes, such as heparanases and sulfatases, as potential diagnostics in pancreatic cancer. We also ran an analysis on a multidimensional cancer genomics database for heparan sulfated proteoglycan-related genes, and report altered expression of some of them.

The osteoarticular tuberculosis is a uncommon diease, which is accout for 0.1%-0.3% of Tuberculosis (TB). The involoment of foot and ankle is only 0.01%-0.03% of TB, particularly the bilateral feet and ankles are affected is extremely rare. Futhermore, its clinical manifestation is atypical and similar with autoimmune disorder, subacute or chronic pyogenic arthritis, osteochondrosis, benign bone tumors and so on, which conduce to the delay of its diagnose and therapy along with unfavourable prognosis. Here we present a case of a 52-year-old man with bilateral feet and ankles tuberculous arthritis, misdiagnosed successively Rheumatoid Arthritis (RA) and Poncet’s diease. It is hope that can enhance the alertness about osteoarticular tuberculosis. Keywords: Osteoarticular tuberculosis; ankle; foot; misdignose.

In this review we presented the information about pathogenesis of mycotic keratitis and the most characteristic clinical signs that can help ophthalmologists to suspect mycotic etiology. In conditions of poor accessibility and informativeness of laboratory and instrumental diagnostic tests, the analysis of the anamnesis and the clinical signs remain the only information that can be used by a physician while making a diagnosis and choosing treating strategies. This review contains the results of a number of studies. In the review we show the progression phases of cornea mycotic infection, among them adherence, invasion, morphogenesis and toxigenicity. We also reveal that the most characteristic clinical signs of mycotic keratitis are scalloped rough edge of stromal defect, prominent dry crumby or caseous structured view of infiltrates and necrotic masses, satellite infiltrates, certain changes of color of the defected area. Heaviness of mycotic keratitis ulcer defect has a correlation with an unfavourable prognosis for disease. Mycotic keratitis during contagion can be complicated by mycotic glaucoma with a malignant disease course and endophthalmitis. There can be cases with mixed infection. While Almost No clinical sign is pathognomonic for keratomycoses, summing up all the signs can verify the diagnosis. Ophthalmologists need training in the recognition the clinical signs of infectious keratitis. Also new methods of fast diagnostics of infectious keratitis etiology and implementation on a large scale are needed.

Abstract Background Prenatally diagnosed pericardial teratoma present a rare finding with an unfavourable prognosis due to frequently associated Foetal hydrops and limited treatment options. We report a successful surgical resection of a prenatally diagnosed cardiac teratoma in a 1160 g neonate with severe Foetal hydrops and cardiac deterioration. Case summary The patient was transferred in utero to our institution due to prior diagnosed pericardial mass and severe foetal hydrops, which necessitated caesarean section one day after arrival at a gestational age of 28 + 0 weeks. After intubation, the patient was stabilized by surgical drainage of 60 mL of pericardial effusion. Further clinical worsening of the patient on the day of life 12 demanded urgent intervention, so that in toto resection of the tumour was performed at a bodyweight of 1160 g. Histopathological analysis revealed a teratoma and the patient is in excellent clinical condition one year after surgery. Discussion This case report demonstrates that an interdisciplinary, two-staged approach can be a feasible and promising treatment option in patients with prenatally diagnosed teratoma and severe Foetal hydrops in a critical circulatory state. Furthermore, it illustrated that resection of pericardial masses can be successfully performed at a bodyweight as low as 1160 g.

Abstract In the context of COVID-19 infection, 0.5–2% of affected patients will suffer a stroke. The strokes are usually severe with an unfavourable prognosis. Most patients suffer from occlusion of the large brain-supplying arteries caused by the COVID-19 induced coagulation disorders. In the context of COVID-19 infection, there has been a dramatic temporary decrease in the number of stroke patients treated in stroke units.

Pilocytic astrocytomas comprise the most common central nervous system tumour during childhood and have an excellent response to surgical treatment in this population. The tumour incidence decreases with age, whereas more aggressive behaviour tends to increase. Haemorrhage as a presenting feature of pilocytic astrocytomas is a rare phenomenon, especially in the adult population. We present a case of a 55-year-old patient with progressive headaches and dizziness. MRI confirmed a sellar and predominantly retrochiasmal suprasellar lesion with heterogeneous signal, enhancement and blood products. Management via transsphenoidal approach was performed, and histopathology revealed the unexpected diagnosis of haemorrhagic pilocytic astrocytoma. Haemorrhagic pilocytic astrocytoma is an infrequent entity in the adult population and it is essential to recognise the peculiarities regarding diagnostic evaluation and management, which differ from the paediatric population. During adulthood, this tumour carries an overall unfavourable prognosis, with higher rates of progression and recurrence.

Ultrasonic tips and anatomic finishing files have been used as strategies to improve the cleaning and disinfection of root canals, especially in complex anatomies. These instruments are also subjected to fractures, which can hinder the endodontic treatment and negatively affect the outcome. This article presents two cases of successful management of intracanal-fractured instruments with unfavourable prognosis, emphasizing on treatment protocols and outcomes. Two 14-year-old female patients underwent root canal treatment of teeth 37 (case 1) and 47 (case 2). During the final irrigation protocols, case 1 presented a fractured XP-Clean file within the mesiobuccal canal and within the distal canal in case 2, which also showed fractured E1-Irrisonic tips within the mesiobuccal and distal canals. Non-surgical management and bypassing retained instrument fragments were successful in both cases and they may be considered adequate approaches.
