Comparative evaluation of methods of surgical treatment of posterior marginal fractures of the tibia based on the analysis of vascular supply

December 23, 1930 in the State. a 37-year-old patient was delivered to the trauma institute. An hour before admission, she twisted her right foot and received a posterior marginal fracture of the tibia from the outside. Large hemorrhage and rapidly growing swelling in the ankle joint prevented the reposition of the fragments, and a two-fold attempt to establish the displaced fragments along the axis was unsuccessful. At the same time, X-ray showed the presence of two fragments in the region of the posterior edge of the tibia, between which there is a gap 2.5 cm wide. The ratio of the bones forming the fork is broken. Despite the fixation of the limb with a plaster cast, unbearable pains remained along the entire limb, the patient did not sleep.

Introduction. Triple pelvic osteotomy is an effective surgical treatment for dysplastic subluxation of the hip in children aged above 12 years. However, modern studies did not assess the indicators of spine-pelvis ratios, sagittal profile of the spine in children with dysplastic subluxation of the hip following surgical treatment, and possibility of change on these values during the operation. Aim. This study aimed to evaluate the effectiveness of the improved technique of performing triple pelvic osteotomy on children with dysplastic subluxation of the hip. Materials and methods. We analyzed the x-ray images and surgical treatment of 35 female patients (44 hip joints) aged 13 to 18 years with dysplastic subluxation of the hip between 2016 and 2018. The patients were divided into two groups: the main group consisted of 20 patients (25 hip joints) who underwent surgical treatment that had taken into account the state of sagittal spine-pelvis ratios according to the improved method using personalized navigation templates, and the control group consisted of 15 patients (19 hip joints) who received surgical treatment according to the generally accepted method. Results. In addition to the typical clinical and radiological abnormalities of the dysplastic subluxation of the hip in 90% of patients in both groups, there were changes in the sagittal balance in the form of excessive pelvic anteversion and lumbar hyperlordosis. One year postoperatively, patients in the main group showed significant changes (p < 0.05) in the state of sagittal spine-pelvis ratios in the form of a decrease in the pelvic base angle, which led to the achievement of the average values of the angle of inclination of the sacral slope (SS) and the value of global lumbar lordosis. On the other hand, these radiological parameters in patients in the control group remained within the preoperative values. Conclusion. The improved technique of triple pelvic osteotomy provides conditions for the reduction of pelvic anteversion and restoration of the sagittal profile of the spine (p < 0.05). The use of personalized navigation templates allows for the most accurate multiplane correction of the acetabulum. It is necessary to include a specialized x-ray examination in the preoperative planning to assess the state of sagittal spine-pelvis ratios.

Aim of study: To investigate the short- and long-term outcome of patients with isolated lateral malleolar fracture type B treated with a single hemicerclage out of metallic wire or PDS cord. Methods: Over an 8-year period 97 patients were treated with a single hemicerclage for lateral malleolar fracture type B and 89 were amenable to a follow-up after mean 39 months, including interview, clinical examination and X-ray controls. Results: The median operation time was 35 minutes (range 15-85 min). X-ray controls within the first two postoperative days revealed an anatomical restoration of the upper ankle joint in all but one patient. The complication rate was 8%: hematoma (2 patients), wound infection (2), Sudeck's dystrophy (2) and deep vein thrombosis (1). Full weight-bearing was tolerated at median 6.0 weeks (range 2-26 weeks). No secondary displacement, delayed union or consecutive arthrosis of the upper ankle joint was observed. All but one patient had restored symmetric joint mobility. Ninety-seven percent of patients were satisfied or very satisfied with the outcome. Following bone healing, hemicerclage removal was necessary in 19% of osteosyntheses with metallic wire and in none with PDS cord. Conclusion: The single hemicerclage is a novel, simple and reliable osteosynthesis technique for isolated lateral type B malleolar fractures and may be considered as an alternative to the osteosynthesis procedures currently in use.

SummaryThe treatment of femoral head necrosis of adults is still rather problematic. Conservative treatment has been reported relatively unsuccessful and surgical treatment does not show convincing results either. The most effective seems to be a surgical treatment in early stages of the disease, however, the diagnosis still remains relatively complicated. For the late stages (2B and above) the most effective treatment option is represented by core decompression and vascular grafting. However, drilling and plombage (especially when using press-fit technique) seems to be successful, although not excellent. The authors describe their own method of drilling and plombage of the necrotic zone of the femoral head in 41 patients with X-ray detected necrotic changes of the femoral head. The pain measured by VAS was seen to decrease after surgery in all patients significantly. The Jacobs score was also observed to have increased (from fair to good outcome). We have not observed any large femoral head collapse after surgery, moreover, in some cases an improvement of the round shape of the femoral head was seen. It is important to mention that in all cases femoral heads with existing necrotic changes (flattening or collapse) were treated. Although the clinical improvement after surgery was not significantly high, the method we describe is a safe and simple method of diminishing pain in attempt to prepare the femoral head for further treatment in a future, without significant restriction of the indication due to necrosis (osteochondroplasty, resurfacing, THR).

In addition to its established oncological indications the sensitivity of bone scintigraphy is of steadily increasing significance in traumatology. Inactivity- induced osteoporosis plays a major role during the immobilization period in the plaster cast. In the region of the joints remodelling intensity may reach such a high level that the non-injured bone shows a higher rate of accumulation than the fracture. This process already begins between the third and fourth week of immobilization. The highest uptake is found after fracture of the scaphoid bone at the end of twelve weeks of immobilization. Control scintigraphies at intervals of several days are indicated to differentiate between various clinical conditions (pseudoarthrosis, activated osteoarthrosis, algodystrophy in case of doubtful x-ray results).

In a competitive variety trial, promising selection lines of sugar sorghum were studied in the Lower Volga region. Based on a set of signs, the best lines were identified, which are recommended for transfer to the State Variety Testing. Breeding lines, which are distinguished by high values of commercially valuable traits, can be used in planned crosses to create highly productive varieties and hybrids of sugar sorghum.

In conditions of limited financial resources of the state, the task of assessing the appropriateness and choosing rational options for the joint use of traditional and new types of high-tech products is topical. The paper proposes a method for substantiating rational options for the joint use of traditional and new products of high-tech products, based on the criteria of their comparative technical and economic assessment, namely, comparing the achieved efficiency and the required cost of performing a fixed set of tasks. The dialectical foundations of the method are presented, in particular, it is established that the law of development of high-tech products fully corresponds to the well-known classical law of «denial of denial». The structure of the method, the order of formation of the set of Pareto-optimal options for the joint use of traditional and new products, as well as the algorithm for choosing a rational option are considered.

Purpose. Create a summary classification of exophoria. To propose an optimal algorithm for optometric and surgical methods of treatment. A modern view of the problem. It is optimal to subdivide exophoria according to the state of the vergent-duction balance into divergence excess, basic exophoria, convergence insufficiency, divergence pseudo-excess, lateral gaze incomitance. By the degree of compensation for compensated, subcompensated, uncompensated, decompensated. In combination with alphabetic pattern for exophoria without pattern, exophoria in combination with horizontal type A pattern, exophoria in combination with vertical type A pattern, exophoria in combination with horizontal type V pattern, exophoria in combination with vertical type V pattern. Features of optimal optical and prismatic correction depend on the state of the vergent-duction balance. In cases of exophoria without pattern surgical treatment is carried out during the transition from a state of subcompensation to non-compensation. In case of exophoria with horizontal-type alphabetical pattern, combined horizontal-transpositional surgery is optimal: elimination of exophoria, the protocol is based on the amount of deviation in the direct gaze position; elimination of the pattern, the protocol is based on vertical transposition of muscles of horizontal action. In cases of exophoria with vertical-type pattern, it is necessary to perform staged vertical-horizontal surgery: stage 1 – elimination of vertical heterotropy in adduction; stage 2 – elimination of exophoria (the protocol is based on the amount of deviation in the direct gaze position). Conclusions. The treatment protocol should be based on a reliably diagnosed diagnosis and consists at the pre-surgical stage in the optimal optical and prismatic correction, the appointment of orthopto-diplopto-prismatic treatment. The effectiveness of treatment is assessed by the dynamics of the disease: the magnitude of exodeviation and the stage of compensation. The protocol of surgical treatment must be reasonable and timely. Key words: exophoria, divergence excess, convergence insufficiency, basic exotropia, lateral gaze incomitance, alphabet pattern.
