Introduction. According to different authors, 4 to 40% of the child and adult population suffer from chronic tonsillitis (СT), with many authors noting an increase in the number of cases and no tendency to decrease the frequency of this pathology.Objective. To optimize diagnostics and treatment tactic of taking patients with chronic tonsillitis (CT) using our own clinical and laboratory data and morphological data as well as literature data.Materials and methods. Аt the first stage of work, 166 patients were examined. They were divided into 4 categories (experimental groups) depending on clinical CT classification according to B.S. Preobrazhensky and V.T. Palchun. We also formed a group of patients who did not have CT symptoms. All patients underwent tonsillectomy or tonsillotomy. All tissue samples of palatine tonsils we got were investigated using immunohistochemical markers and default histological colorings. At the second stage, the dynamics of complaints, clinical symptoms and laboratory parameters were assessed in 247 patients diagnosed with CT Toxic-Allergic Form I (CT TAF I) before conservative treatment and 3 months after conservative treatment. Patients were divided into 3 groups depending on the conservative treatment regimen: a course of flushing the lacunae of the tonsils; antibiotic therapy with a retard form of clarithromycin; complex treatment combining the first two options.Results. Тhe chronic inflammatory process in the palatine tonsils changes their cytoarchitectonics. At the same time, there is a direct correlation between the presence and severity of changes with the clinical classification of chemotherapy according to B.S. Preobrazhensky and V.T. Palchun.Conclusion. With CT TAF II, the palatine tonsil not only loses its importance as an organ of the immune system, but also becomes a hotbed of infection in the body, which can cause severe complications. Only tonsillectomy can effectively protect the patient from these complications. At the stage of CT TAF I, the formation of metatansillar complications occurs. Patients with this form of CT require special attention, and correctly conducted conservative therapy can protect patients from local and general complications of CT. A variant of such conservative therapy that gives a lasting effect is complex therapy with washing the lacunae of the palatine tonsils with an antiseptic solution and simultaneous antibiotic therapy with 14-membered macrolides that can accumulate in the lymphadenoid tissue.

The aim of this work is to draw the attention of otorhinolaryngologists to the advantages of early conservative treatment of children with hypertrophy of the nasopharyngeal tonsil - adenoids. In childhood, a significant part of the protective and informational functions lies on the pharyngeal tonsil, since it is in the path of inhaled air. Unfavourable environmental conditions exacerbate its functional failure. Pathology of the lymphopharyngeal ring is highly prevalent and occupies a leading place among ENT diseases in children. Each person has a certain own microflora on the nasal mucosa from birth. In most cases, monoflora is determined on the mucous membrane of the nasal cavity, and only 2-3% is due to the combination of two types of microbial and fungal flora. For more than twenty years, we have been using the tactics of a sparing effect on the mucous membrane when using medicinal mixtures on a protective basis. Conservative treatment of 245 children aged 3 to 6 years with grade 1 – 2 adenoiditis was carried out, which made it possible to avoid surgical intervention in this group of children. When choosing a drug mixture, the sensitivity of the microflora of the patient’s nasal cavity was considered according to the results of a smear from the mucous membrane. Adenoidectomy is an operation with potentially undesirable consequences for the whole body, therefore, effective timely conservative therapy can reduce the risks and harms of surgery for pediatric patients.

Introduction. The high incidence of chronic tonsillitis (CT), as well as the high risk of complications, make research in this area very relevant. The ambiguity of approaches to conservative therapy determines the need to search for new, reasonably effective, chemotherapy regimens.Objective. To carry out a comparative characteristic of various options for conservative therapy of chronic tonsillitis of toxicallergic form (TAF) I degree (CT TAF I).Materials and methods. The study included patients with CT TAF I according to the classification of B.S. Preobrazhensky and V.T. Palchun. The patients were divided into three clinical groups, depending on the therapy. Patients of the first group underwent a course of washing the lacunae of the tonsils with 1% dioxidine solution. Patients of the second group were prescribed antibiotic therapy with a retard form of clarithromycin. Patients of the third group underwent complex treatment – washing the lacunae of the palatine tonsils and simultaneously taking antibiotic therapy. The effectiveness of treatment was assessed by comparing the dynamics of changes in complaints, pharyngoscopic symptoms of CT, regional lymph nodes, and laboratory parameters.Results. Complex conservative therapy of CT TAF I provides early relief of patients’ complaints (in 93.2% of patients on the 7th day from the start of treatment), the maximum decrease in the severity of local CT symptoms (up to 1 point on the VAS at the end of treatment, with the exception of the Zak sign — 1.4 points) and the greatest positive dynamics of laboratory parameters.Conclusions. The analysis showed that for patients with CT TAF I, complex therapy, including a course of washing the lacunae of the palatine tonsils with 1% dioxidine solution and simultaneous administration of antibacterial therapy with a retard form of clarithromycin, is the most optimal therapeutic tactic that allows to achieve an earlier and stable reduction in the severity of symptoms HT, patient complaints, as well as the normalization of laboratory parameters.

Abstracts. Internal diseases.A. T. Ostrovsky. To the question of post-tuberculosis lesions of the larynx in pulmonary tuberculosis. Diss. S.-PB.1900 g.The author cites the literature that exists on the issue of interest to him, before and after the discovery of tuberous sticks by prof. Kos'om. At the present time, regarding the secondary tubercle of the larynx, there are already not at all many, each having its own supporters, from which some admit the penetration of tuberous sticks from the epithelial cover of the mucous membrane of the larynx, others hold the view that the vessels for the lymph from the inside through the third In view of the fact that up to the present time the issue of post-tuberculosis lesions of the larynx remains not fully clarified, the author decided to take on his work as much as possible.

BACKGROUND: Knee osteoarthritis (OA) is accompanied by inflammation and angiogenesis. Modifying angiogenesis through transcatheter arterial embolization (TAE) can be a potential treatment for knee OA. OBJECTIVE: We subjected five OA knees in three patients to TAE and report the results of our post-treatment observations. CASE DESCRIPTION: Three patients that had experienced knee pain for a minimum of one year prior to the study, and whose pain had persisted despite conservative treatment, were included in this study. Patients more often chose conservative treatment over surgical treatment. Pain and functional scales were evaluated before, immediately, and 1 month after TAE using the Numeric Rating Scale (NRS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). TAE was performed by an experienced interventional radiologist. The average values of NRS evaluated before and after 5 TAEs were 5.2 before TAE, 3 immediately after TAE, and 3.6 after 1 month of TAE, and the average values of WOMAC were 52, 38.4, and 36.4, respectively. There were no major adverse effects. CONCLUSION: The examined cases support the conclusion that TAE is an effective treatment for patients with knee OA. Substantial pain relief and WOMAC improvement were observed both immediately and one month after TAE.

Periodontal disease is known to be the most common and serious health problem in dogs today. Despite the rather large arsenal of medicinal substances, the problem of treatment and prevention of periodontal disease remains relevant. Based on this, the purpose of the work was to study the features of chronic catarrhal gingivitis and to develop an effective treatment regimen for dentistically ill dogs. For research, two groups of animals with chronic catarrhal gingivitis were formed in 5 dogs in each (control and experimental), selected on the principle of analogues in terms of age, character and localization of the inflammatory process. Animals of the control and experimental groups were performed tartar removal, irrigation of the oral cavity with water and drying of the mucosa with a sterile gauze swab. Dogs of the control group on the mucous membrane was applied 1 ml of septogel 2 times a day, and the experimental – argumentistin 2 times a day. Before and after the procedures on the mucous membrane was applied a 3 % solution of hydrogen peroxide. It was determined the prevalence of dental disease in dogs, the influence of microflora on the condition of the oral cavity of dogs with chronic catarrhal gingivitis, the clinical condition and the main indices and samples were studied, reflecting the intensity of the inflammatory process in the gums of sick dogs and was developed a method of treatment. The results of researches have shown that the use of argumentistin in the complex treatment of dogs with chronic catarrhal gingivitis helps to reduce the recovery period by 5 days.
