Comparison of electro-gustometry data and the results of blood glucose studies, sodium and potassium content in saliva and blood, daily excretion of 17-ketosteroids in healthy and sick patients showed that electro-gustometry can be used as a simple approximate method for detecting adrenal insufficiency.

Abstract Background: Adrenal crisis is an emergency condition in endocrinology that commonly found in primary adrenal insufficiency but also occur in chronic adrenal insufficiency triggered by various conditions such as sepsis, infection, trauma, burns, surgery, and myocardial infarction. In chronic adrenal insufficiency, adrenal crisis can be induced by excessive reductions or inadequate discontinuation of steroid treatment. Case Illustration: A 40-year old-man admitted with chief complaint abdominal pain since seven days before admission. He felt pain in the umbilical area and slowly radiated to all the part of abdomen. Other than that, he also felt nausea, had vomitus, fever, and constipation for five days. He was immobilized for four months, with muscles weakness and atrophy. He was diagnosed with Focal Segmented Glomerulosclerosis and had a high dose of methylprednisolone (48 mg) for 6 months, 40 mg for the next 2 months and methylprednisolone 12 mg for the last 2 months. The last two month, he began to have severe general weakness and hypotension. On physical examination we found hypotension, fever (38.1o C), pale conjunctiva, moon face, buffalo hump, slightly distended and tenderness of abdomen, normal bowel sound, and purple striae all over the abdomen and extremities. On laboratory examination, Hb was 8.2 (n 11.7 – 15.5 g/dl), leukocytes 10,400 (5.00 – 10.00 x 103/μL), Na 123 (n 135 – 147 mmol/L), random blood glucose 74 (n 70 – 140 mg/dL). On abdominal X ray, there was prominent faecal material and no signs of ileus. No sign of infection found in urinalysis. He had sodium correction, packed red cell transfusion, symptomatic therapy including laxative, methylprednisolone 12 mg, but no improvement of signs and symptoms beside be able to defecate. The abdominal ultrasound gave a normal result. The morning cortisol level was then examined, with the result 14.4 (n 3.7–19.4). The patient was then diagnosed with adrenal crisis based on the clinical manifestations and had hydrocortisone therapy 100 mg a day for 2 consecutive days. After hydrocortisone administration, the symptoms improved, no fever and abdominal pain, he had normotension, increased sodium level 132 (n 135 – 147 mmol/L) and blood glucose level 118 (n 70 – 140 mg/dL). On the third day the patient discharged with oral hydrocortisone 15 mg in the morning and 10 mg in the afternoon. Conclusion: Adrenal crisis was generally found in primary adrenal insufficiency but could also occur in secondary adrenal insufficiency due to inappropriate tapering off process of long term glucocorticoid use.

Background. Sini Decoction (SND) is composed of Aconitum carmichaelii Debeaux, Zingiber officinale Roscoe, and Glycyrrhiza uralensis Fisch, having been used in China for centuries for collapsing phrase of disease. Studies reported that SND could alleviate inflammatory response, ameliorate microcirculatory disturbances, and improve shock reversal and adrenal gland glucocorticoid stress response during sepsis shock, yet the underlying mechanism is still elusive. Toll-like receptor (TLR) 4 is demonstrated to be crucially correlated with the corticosterone secretion and the impaired adrenal glucocorticoid responses in sepsis. Materials and Methods. SND at dose of 10 g/kg (in low-dose SND group, LD-SND) and 20 g/kg (in high-dose SND group, HD-SND) was administered to CLP rats. Four days later, overall survival rates of rats were calculated; rat serum and adrenal glands were collected. Basic serum corticosterone levels were determined, and the increase of corticosterone after 0.8 ug/kg ACTH injection was checked to detect the adrenocortical sensitivity to ACTH. The protein and mRNA expression of TLR4 in adrenal glands were measured to study the impact of SND on TLR4 expression. mRNA levels of IL-10 and TNF-a in adrenal glands and IL-10 and TNF-a levels in serum were also determined to study the cytokines profile. Results. SND improved the cumulative survival rate of CLP rats up to 4 days (P < 0.05 with HD-SND) and adrenocortical sensitivity to 0.8 ug/kg ACTH stimulation (P < 0.05 at 60 mins, 31.02 ± 19.23 ng/ml in LD-SND group and 32.18 ± 14.88 ng/ml in HD-SND group versus 5.03 ± 13.34 ng/ml in CLP group), with a significant decrease of protein (P < 0.05, 29.6% in LD-SND group and 27.8% in HD-SND group), mRNA expression of TLR4 (P < 0.05, 32.9% in LD-SND group and 36.1% in HD-SND group), mRNA expression of IL-10 (P < 0.05, 32.0% in LD-SND group and 29.6% in HD-SND group), TNF-a in adrenal glands (P < 0.05, 26.0% in LD-SND group and 25.3% in HD-SND group), and TNF-a level in serum (P < 0.05, 100.20 ± 19.41 pg/ml in LD-SND group and 92.40 ± 11.66 pg/ml in HD-SND group versus 134.40 ± 27.87 pg/ml in CLP group). Conclusion. SND increased overall survival rate within 4 days and attenuated adrenal insufficiency in septic rats by downregulating TLR4 mRNA and protein expression in adrenal tissue, inhibiting adrenal production of TNF-α and IL-10, and improving adrenal responsiveness. Our results suggest that SND is able to ameliorate adrenal stress responses in a local immune-adrenal crosstalk way involving downregulated expression of TLR4 in adrenal tissue. SND might be a promising treatment for adrenal insufficiency prevention in prolonged sepsis.

Two-year-olds (15 individuals) and five-year-olds (15 individuals) of the Russian sturgeon × Siberian sturgeon (Acipenser gueldenstaedtii × Acipenser baerii) hybrid, kept in net cages in the Volga delta, were studied after wintering in order to establish the functional state of the organism according to the complex of indicators of the erythron system and metabolism substances. The results of the performed study found that the erythron system in five-year-olds is characterized by a higher content of erythrocytes and blood hemoglobin (0.58 ± 0.01×10 12 l –1 and 64.98 ± 1.29 g/l, respectively), compared with two-year-olds (0.49 ± 0.02×10 12 l –1 and 64.98 ± 1.29 g/l, respectively).The serum iron content in both groups was the same. A higher content of total protein and albumin in the blood, as well as a greater activity of ALT were found in five-year-olds, compared with two-year-olds. Thus, the content of total protein and albumin in blood serum in five-year-olds was at the level of 48.23 ± 1.54 and 21.58 ± 2.34 g/l, respectively, and in two-year-olds — 32.42 ± 0.92 and 13,84 ± 0.61 g/l, respectively (p < 0.05). The activity of serum ALT in five-year-olds was 1.07 ± 0.06 μkat/l, and in two-year-olds — 0.85 ± 0.03 μkat/l (p < 0.01). Serum cholesterol in five-year-olds (3.94 ± 0.04 mmol/l) and two-year-olds (4.4 ± 0.08 mmol/l) did not differ significantly (p > 0.05). The content of serum triglycerides in five-year-olds (4.50 ± 0.49 mmol/l) and in two-year-olds (5.36 ± 0.45 mmol/l) did not differ significantly (p > 0.05). The study of carbohydrate metabolism revealed a pronounced variability in the glycemic index in the younger age group (coefficient of variation 44.7 %) with an average blood glucose value of 2.69 ± 0.31 mmol/l. The blood glucose content of five-year-olds was higher than that of two-year-olds — 3.84 ± 0.24 mmol/l (p < 0.05). The established parameters of variation and the age specificity of the indicators of the erythron system, protein, lipid and carbohydrate metabolism can be used to assess the state of physiological adaptation of the hybrid after wintering in cages in the Volga delta.

Tuberculosis (TB) is a common infectious disease in developing countries like Bangladesh. Drug-resistant tuberculosis (DR-TB) poses a significant threat to the population. A relatively uncommon site of involvement of TB is adrenal gland, which is an important cause of chronic adrenal insufficiency. Here we present a case of a 40-year-old male with multidrug-resistant tuberculosis (MDR-TB) involving the adrenal glands. He was previously diagnosed as a case of disseminated TB involving lungs, right knee joint and lumbar spine and received anti-TB chemotherapy for several occasions and in different categories in last 12 years before this presentation. He presented with 4-month history of vomiting, diarrhea, generalized weakness, generalized pigmentation and postural hypotension. Investigations revealed hyponatremia with hyperkalemia. Adrenal insufficiency was suspected and serum cortisol was found to be low and serum adreno corticotropic hormone (ACTH) was high. On imaging there was bilateral adrenal enlargement. Fine needle aspiration cytology (FNAC) was done from adrenal gland which revealed granulomatous lesion suggestive of TB. Gene Xpert and culture-sensitivity of aspirated material for Mycobacterium tuberculosis revealed MDR-TB. Patient was finally diagnosed as a case of primary adrenal insufficiency due to MDR-TB of the adrenal glands and was treated with regime for MDR-TB for 20 months.Birdem Med J 2017; 7(3): 235-237

1. The sodium and potassium content of longissimus dorsi (lumbar), psoas major, rectus femoris, triceps (lateral head) and extensor carpi radialis from 6 pigs each at pork, bacon and manufacturing weights were determined: these varied significantly and systematically between muscles and between groups.2. Since the concentration of potassium may differ by at least 30% between muscles, it is concluded that assessment of total muscle mass may be inaccurate if based on the integration ofγ-ray emission from 40K.
