BACKGROUND: Wheelchair skills training is an essential part of rehabilitation of the disabled persons with lower limb amputation (LLA) due to peripheral artery disease (PAD). However, nowadays this issue is poorly addressed in our country, there are no standardized technologies. AIM: Investigation of dynamics of the extent of wheelchair usage among disabled persons with LLA due to PAD during adjustment to the wheelchair using the Wheelchair Skills Program (WSP, Canada). MATERIALS AND METHODS: Altogether, 350 patients with lower limb stump [unilateral tibial stump ― 116 (33.1%), hip stump ― 223 (63.7%), bilateral tibial stumps ― 6 (1.7%), hip stump ― 5 (1.4%)] were examined in clinical settings at the Federal State Budgetary Institution Novokuznetsk Scientific and Practical Centre for Medical and Social Expertise and Rehabilitation of Disabled Persons Ministry of Labour and Social Protection of the Russian Federation. Male ― 268 (76.6%), female ― 82 (23.4%). Mean age 63.10.52 years. Cause of LLA: obliterative PAD (lower limb atherosclerotic arterial disease) ― 226 (64.6%), type 2 diabetes mellitus ― 124 (35.4%). WSP effectiveness was assessed in 227 disabled persons. Wheelchair skills retention in 1.1 (0.591.87) years was assessed in 48 persons, 25 persons have undergone training second time. RESULTS: It was found that 86% of the disabled persons with LLA needed the wheelchair skills training. Upon the completion of the course the level of individual wheelchair skills was increased in all participants. This contributed to an increase in their activities and participation, which was indirectly evidenced by 1.8 fold increase in the proportion of those using the wheelchair outdoors by the time of second hospitalization. CONCLUSION: The program provides an individual approach to training, based on the results of a study of the initial level of proficiency in wheelchair skills, the wishes of the user himself, his physical, somatic and psychological condition. Thanks to this, novice users were able to master basic skills, and relatively experienced ones ― more complex, and thereby increase their level of safe use of the wheelchair, as well as the degree of independence and everyday activity, which can indirectly be evidenced by a significant increase in the share of wheelchair users outside the house.

The article deals with issues related to the problem of vocational rehabilitation of disabled people. The competence of the Russian Federation in the fi eld of rehabilitation of disabled persons in the person of its Federal authorities and management, subjects of the Russian Federation and local self-government bodies is specifi ed. The content of such concepts as "services for professional rehabilitation of disabled people" in accordance with the state standard GOST R 53873-2010 Rehabilitation of disabled people is disclosed. Professional rehabilitation services for the disabled; "professional rehabilitation program" and "options for professional rehabilitation". Despite the fact that professional rehabilitation and adaptation in the workplace is an integral part of the state policy in the fi eld of social protection of persons with disabilities, there are many unresolved problems and diffi culties in this area.

Execution of documentation is an essential component of the vocational work of experts engaged in the provision of medical and social assistance to the disabled, from the clinical diagnostic to rehabilitation. In the article there is presented the questionnaire of the assessment of health and social status of the elderly disabled person, which allows to reflect the existence of violations of the functions and limitations of life activity, the degree of their severity, needs in certain types of rehabilitation, including with the use of technical means of compensation of disturbedfunctions. This type of documentation allows to reduce (speed up) the time on the execution and to increase the level of the vocational competence of the professionals engaged in medical and social activities as well in health institutions as social protection institutions. With properfilling ofpreprinted forms (layout, template) it will be received medical and social portrait ofa disabled elderly person. Questionnaire can also be used in execution ofscientific research.

Objective. In order to study the motion recognition intention of lower limb prosthesis based on the CNN deep learning algorithm. Methods. A convolutional neural network (CNN) model was established to reconstruct the motion pattern. Before the movement mode of the affected side was converted, the sensor was bound to the healthy side. The classifier was employed to extract and classify the features, so as to realize the accurate description of the movement intention of the disabled. Results. The method proposed in this research can achieve 98.2% recognition rate of the movement intention of patients with lower limb amputation under different terrains, and the recognition rate can reach 97% after the pattern converted between the five modes was added. Conclusion. The deep learning algorithm that automatically recognized and extracted features can effectively improve the control performance on the intelligent lower limb prosthesis and realize the natural and seamless conversion of the intelligent prosthesis in a variety of motion modes.

Objective: to develop an algorithm for customized selection of a wheelchair for the disabled persons with limited mobility followed by design of a software allowing to automate the process of wheelchair selection. Materials and methods. Analytical review of English literature was performed to address the issues in prescription and utilization of the assistive devices and technologies (ADT). The regulatory documents governing the ADT prescription in the Russian Federation (RF) were studied. Russian market of the wheelchairs for the disabled persons was analyzed. Ten disabled persons with traumatic spinal cord injury to the department of medical rehabilitation of the Centre were examined to pilot the software for automated wheelchair selection. Results. After the literature review results were structured, the major groups of factors that should be considered when selecting a wheelchair were identified: 1) anthropometric measurement, 2) physical and functional capabilities of the disabled person, 3) level of everyday activity, 4) availability and physical capabilities of the caregivers, 5) particularities of the environmental factors. The algorithm for customized selection of wheelchair was designed. The database comprising the technical characteristics of 141 wheelchair models that are taken as a basis of the software was developed. IBM-compatible computer was used to develop the software. Programming language: Delphi, data base management system: Microsoft Access 2013. Piloting results showed the effectiveness of the software. In all cases appropriate wheelchairs were selected. Conclusion. Within current study the software for automated process of customized wheelchair selection was piloted. The software takes into consideration the level of activities and participation of the disabled persons and significant environmental factors which allows to customize the ADT prescription.

Background: Diabetic foot ulcer (DFU) is one of the leading complications of type-2 diabetes mellitus. It isassociated with neuropathy and peripheral arterial disease of the lower limb in patients with diabetes. Basically, there are four stages of wound healing namely hemostasis phase, inflammatory phase, proliferative phase and maturation phase. In case of DFU, all these stages are disturbed which lead to delay in healing and consequently to lower limb amputation. Traditionally the dosage forms like tablets, creams, ointments, gels and capsules have been used for the treatment of diabetic foot ulcer from many years. Introduction: In this review the global prevalence as well as etiopathogenesis related to diabetic foot ulcer has been discussed. Potential role of various synthetic and herbal drugs as well as their conventional dosage form for the effective management of diabetes foot ulcer has been highlighted. Methods: Structured search of bibliographic databases for previously published peer-reviewed research papers was explored and data was culminated in terms of various approaches that are used for the treatment of diabetic foot ulcer. Results: About 142 papers including both, research and review articles, were included in this review in order to produce a comprehensive as well as readily understandable article. A series of herbal and synthetic drugs have been discussed along with their current status of treatment in terms of dose and mechanism of action. Conclusion: DFU has become one of the most common complications in patients having more than ten years of diabetes. Hence, understanding the root cause and its successful treatment is a big challenge because it depends upon multiple factors such as judicious selection of drug as well as proper control of blood sugar level. Most of the drugs that have been used so far either belong to the category of antibiotics, antihyperglycaemics or, they have been repositioned. Moreover, in clinical practice, much focus has been given towards dressings that have been used to cover the ulcer. The complete treatment of DFU is still a farfetched dream to be achieved and it is expected that a combination therapy of herbal and synthetic drug with multiple treatment pathway could be able to overcome the disease.
