In treatment of 87 patients (main group) with diaphysial shin fractures new techniques and devices for optimization of transosseous extrafocal osteosynthesis by Ilizarov were applied. All techniques and devices, i.e. devices for the perfection of Ilizarov apparatus stability, for bone fragment reposition, console compression arrangement for extrafocal osteosynthesis in commi­nuted fractures, devices for damper compression of bone fragments, for prevention of lower extremity edema at extrafocal osteosynthesis, for provision of early axial loading in Ilizarov apparatus, for activization of osteogenesis using asymmetrical dynamic compression were elabo­rated by the author. Control group (108 patients) was treated by traditional Ilizarov technique. Treatment results were assessed at terms from 4 months to 2 years. Data of clinical, X-ray and functional examinations as well as terms of fracture healing, duration of hospitalization and terms of working ability restoration were better in the main group as compared to the control one. In the main group good results were achieved in 47.1%c, satisfactory — in 49.4%, poor — in 3.5% of cases, in control group — 30.6%, 63.0%, 6.4%, respectively. Author believes that further perfection of Ilizarov technique is reasonable.

The aim: The research was to evaluate the effectiveness of acute traumatic pulpitis treatment with the use of bioceramics according to the results of a clinical study. Materials and methods: Acute traumatic pulpitis of 25 teeth in 25 patients aged 18-25 years were treated by biological direct pulp capping. In 13 patients of the main group, bioceramics was placed on the exposed pulp, and in 12 patients of the control group, calcium hydroxide paste was applied on the exposed pulp. Treatment effectiveness was evaluated according to the following criteria: the absence of clinical signs of pulpal inflammation, on X-ray – signs of dentinal bridge formation, the absence of periapical changes. Results: The use of bioceramic material in patients of the main group provided a positive dynamic of treatment in 12 teeth. The need for re-treatment was diagnosed only in 1 clinical case (7.7%), in the control group – in 4 cases (33.3%). Conclusions: Pulpitis treatment with the use of bioceramics provides preserving the functional properties of pulp, creation of a calcified barrier – dentin bridge and prevents the periodontal complications compared to the pulp capping with a material based on calcium hydroxide. The effectiveness of treatment in the main group was 92.3%, in the control – 66.7%.

Purpose of the study — to evaluate the degree of correction of acetabular spatial position after triple pelvic osteotomy using patient-specific templates in children with dysplastic hip subluxation. Materials and methods. Study included 30 patients (30 hips) with dysplastic hip subluxation aging from 14 to 18 years (mean age of 16.3±1.4). All children were divided into two groups — main group consisted of 15 children (15 hips) who underwent triple pelvic osteotomy using patient-specific templates produced by 3D modeling and prototyping technology; control group consisted of 15 children (15 hips) who underwent triple pelvic osteotomy based on standard preoperative planning. X-ray and CT of hip joints was made in all patients. Results. Parameters of acetabular spatial position as well as integral parameters which define the ratio between acetabulum and femoral components and hip stability were in the range of normal variation in patients of the main group. Similar average parameters in patients of control group were below the lower margin of the physiological norm (p<0.05). Besides, the values of standard deviation of abovementioned X-ray parameters in some patients of control group demonstrated pronounced variability characterized by hyper- and hypo-correction values of spatial position of acetabular fragment. Conclusion. In contrast to standard preoperative planning the use of patient-specific templates during re-orienting triple pelvic osteotomy in children with dysplastic hip subluxation allows to level out possible errors in three-dimensional acetabulum positioning towards hyper- and hypo-correction (p<0.05) of pelvic component. This creates conditions to ensure adequate hip stability which is confirmed by key x-ray indices with values within normal reference range.

Introduction. Triple pelvic osteotomy is an effective surgical treatment for dysplastic subluxation of the hip in children aged above 12 years. However, modern studies did not assess the indicators of spine-pelvis ratios, sagittal profile of the spine in children with dysplastic subluxation of the hip following surgical treatment, and possibility of change on these values during the operation. Aim. This study aimed to evaluate the effectiveness of the improved technique of performing triple pelvic osteotomy on children with dysplastic subluxation of the hip. Materials and methods. We analyzed the x-ray images and surgical treatment of 35 female patients (44 hip joints) aged 13 to 18 years with dysplastic subluxation of the hip between 2016 and 2018. The patients were divided into two groups: the main group consisted of 20 patients (25 hip joints) who underwent surgical treatment that had taken into account the state of sagittal spine-pelvis ratios according to the improved method using personalized navigation templates, and the control group consisted of 15 patients (19 hip joints) who received surgical treatment according to the generally accepted method. Results. In addition to the typical clinical and radiological abnormalities of the dysplastic subluxation of the hip in 90% of patients in both groups, there were changes in the sagittal balance in the form of excessive pelvic anteversion and lumbar hyperlordosis. One year postoperatively, patients in the main group showed significant changes (p < 0.05) in the state of sagittal spine-pelvis ratios in the form of a decrease in the pelvic base angle, which led to the achievement of the average values of the angle of inclination of the sacral slope (SS) and the value of global lumbar lordosis. On the other hand, these radiological parameters in patients in the control group remained within the preoperative values. Conclusion. The improved technique of triple pelvic osteotomy provides conditions for the reduction of pelvic anteversion and restoration of the sagittal profile of the spine (p < 0.05). The use of personalized navigation templates allows for the most accurate multiplane correction of the acetabulum. It is necessary to include a specialized x-ray examination in the preoperative planning to assess the state of sagittal spine-pelvis ratios.

Aim. To assess the effect of dynamic electroneurostimulation in treatment of community-acquired pneumonia in preschool children. Methods. The main group consisted of 45 preschool children with community-acquired pneumonia, in whom 7-day treatment course of dynamic electroneurostimulation was included as the component of the complex treatment. The control group included 55 children in whom conventional tactics was applied. Results. General intoxication syndrome was resolved at 3.2±1.1 day in children of the main group compared to 7.7±1.4 day in the control group. The use of dynamic electroneurostimulation reduced the duration of respiratory distress by more than 1.5 times (6.8±1.3 day), regression of physical lung changes was observed at 10.3±1.4 on average, compared to the end of the second week at the control group. Resolving of the clinical symptoms in children of the main group was accompanied by statistically significant (p 0.05) reduction of basic parameters of systemic inflammation (increased white blood cells count, high neutrophil count, left shift) by 7th day of treatment. The level of C-reactive protein reduced by 7 times in children of the main group, compared to 2.3 times in comparison group (p 0.05). Chest X-ray on the 7th day showed complete resolving of inflammatory changes in 20 (44.4%) patients of the main group, while no such cases were observed in control group (p 0.001). At the same time, no positive change of X-ray picture was revealed in 25 (45.5%) children of the control group, while there were no such cases in the main group (p 0.001). On the 11th day of treatment complete resolving of inflammatory changes was found in 43 (95.6%) children of the main group, compared to 34 (61.8%) children of the control group (p 0.05). Conclusion. Including of dynamic electroneurostimulation in complex treatment of community-acquired pneumonia in children reduces the recovery time, hospital stay and treatment cost.

From 1990 until 2010 retrospective study was carried out to investigate the occurrence of cystic echinococcosis (CE) of liver in children at Stavropol Regional children hospital in clinical wards of Stavropol State Medical Academy. We treated 60 children from 3 to 15 years with hepatic CE. In 22 (36.7%) patients had hepatic CE, in 38 (63.3%) — combines CE of the liver and lungs. Patients were divided into two equal groups: main and control. In the main group was used omentoplastiсa of residual cavity after closed echinococсectomy. In the control group — capitonage. In main group performed pre-and postoperative chemotherapy with nemozol at a dose of 10–15mg/kg/day and wobenzym in the dose of 1 tablet per 6 kg of body weight three times per day. In the main group complications occurred in 6.7% of children. In the control group, the complication rate was 23.2%. The number of relapses in the control group was 6 cases, in the main group is not mentioned. Accordingly the combined use of omentoplastiсa of residual cavity after closed echinococсectomy with nemozol and polyenzyme therapy to avoid recurrence of the CE, as well as shorten the duration of hospitalization and number of postoperative complications in children.

Summary. The aim of the study was to compare open and endovascular interventions in patients with infrainguinal arterial disease (IAD) against a metabolic syndrome (MS). Materials and methods. Results of treatment of 73 patients with IAD on MS background were studied. Patients were divided into two groups: 48 patients made up the basic group, they underwent endovascular interventions; 25 patients entered the control group they performed open operations. The value of bed-day, the degree of oxygenation of tissues, the dynamics of trophic disorders were analyzed. The duration of hospitalization of patients in the main group was less than in patients with open surgery. The dynamics of elimination of trophic disorders is more pronounced in patients of the main group. Conclusion. Endovascular interventions have a number of advantages over open operations in the treatment of IAD patients against the background of the metabolic syndrome: in relation to the number of so-called. “Large” complications, shunt interventions look less preferable.

Results of extraosseous osteosynthesis in 160 patients with fractures and pseudarthroses of lower extremities long bones were analyzed. In 80 patients from the main group the plate was additionally fixed to the bone with ring-shaped tightening clamp with shape memory to prevent screw migration.. In 65 (81.3%) patients from control group consolidation of bone fragments with excellent results was achieved in 66.3% of cases. In 15 (18.7%) patients with mainly peri- and intraarticular femoral fractures the migration of screws and plate destruction were noted. In main group the fractures consolidated in anatomically correct position in 76 (95.0%) and good functional results of treatment were achieved.

Introduction. Glaucoma is one of the most significant eye diseases. It is often diagnosed, not always amenable to therapy, and can lead to a complete loss of visual functions. In recent years, the method of osteopathic correction has become widespread as one of the effective methods of treatment and rehabilitation of patients with pathologies of various body systems. In the pathogenesis of glaucoma, it is customary to distinguish a dystrophic concept, which considers primary open-angle glaucoma as a result of dystrophic changes in the connective tissue, as well as in the endothelial lining of the trabeculae and Schlemm′s canal, especially destructive changes in mitochondria and the alteration of their functional activity. A vascular concept is also distinguished. According to this concept, the central link in the pathogenesis of glaucoma is circulatory disorder in the ciliary vessels, ocular artery, and major vessels of the head and neck, it can be assumed that osteopathic correction in the treatment of patients with open-angle glaucoma will be pathogenetically substantiated and will have a positive effect on intraocular pressure and trophicity of the optic nerve. The goal of research — to study the influence of in osteopathic correction on the nature of unoperated glaucoma (stage IIA) and to substantiate the possibility of using osteopathic correction in the complex treatment of patients with this pathology.Materials and methods. A prospective controlled randomized study was conducted at 52 city polyclinics, branch 3, Moscow, from January 2018 to January 2019. 40 patients (70 eyes) aged 50 to 75 years with primary open-angle glaucoma IIA stage were examined. At this stage of the disease, patients most often seek medical care and the issue of conservative management is primarily considered. All patients were divided into two groups of 20 people: the main group and the control group. The treatment in the main group included hypotensive drug therapy and osteopathic correction. Patients of the control group received only drug therapy. All patients underwent ophthalmic (visometry, tonometry, perimetry) and osteopathic examination twice: before the treatment and after 3 months.Results. For patients with primary open-angle IIA non-operated glaucoma, regional (most often regions of the head, neck, dura mater) and local (abdominal diaphragm, iliac bones, hip and knee joints) somatic dysfunctions were the most typical. In the main group a statistically significant decrease in the frequency and severity of dysfunctions at all levels was stated. Also, in patients receiving osteopathic correction, a significant decrease in the level of intraocular pressure and perimetric indices was noted. In patients of the control group, no reliable changes in these indicators were obtained.Conclusion. The results obtained indicate that osteopathic correction is clinically effective in the complex treatment of patients with primary open-angle II A glaucoma.

In order to increase the pregnancy rate in the assisted reproductive technology, the selection of one embryo with the highest implantation potential it is very important. Time-lapse microscopy (TLM) is a tool for selecting quality embryos for transfer. This study aimed to assess the benefits of single-embryo transfer of autologous oocytes performed on day 5 of embryo incubation in a TLM-equipped system in IVF and ICSI programs. Single-embryo transfer following incubation in a TLM-equipped incubator was performed in 282 patients, who formed the main group; the control group consisted of 461 patients undergoing single-embryo transfer following a traditional culture and embryo selection procedure. We assessed the quality of transferred embryos, the rates of clinical pregnancy and delivery. The groups did not differ in the ratio of IVF and ICSI cycles, average age, and infertility factor. The proportion of excellent quality embryos for transfer was 77.0% in the main group and 65.1% in the control group (p = 0.001). In the subgroup with receiving eight and less oocytes we noted the tendency of receiving more quality embryos in the main group (р = 0.052). In the subgroup of nine and more oocytes the quality of the transferred embryos did not differ between two groups. The clinical pregnancy rate was 60.2% in the main group and 52.9% in the control group (p = 0.057). The delivery rate was 45.0% in the main group and 39.9% in the control group (p > 0.050).

The objective: the study of immunomodulatory effects of a probiotic, which contains lyophilized Lactobacillus (Lactobacillus rhamnosus) – 13 mg (2,0ґ109 CFU) and lyophilized bifidobacteria (Bifidobacterium lactis) – 4 mg (2,0ґ109 CFU) the level of serum immunoglobulin IgA as a marker of local immunity in the plasma of women of reproductive age with the violation of the biocenosis of the vagina. Patients and methods. The study involved 86 patients of reproductive age with the violation of the vaginal biocenosis, which were divided into two groups according to received treatment. A survey was conducted for all patients in both groups: determine the level of serum IgA, measuring pH of vaginal environment and the quantification of lactobacilli and pathogenic flora with the help of test-system «Florotsenoz» before treatment and in 6 weeks after treatment. The state of vaginal microbiocenosis in both groups before treatment was homogeneous. Patients in both groups as therapy at the first stage of treatment received, if necessary antimicrobial therapy depending on the selected flora. In the second stage (restoration of microflora) patient of the main group received systemic probiotic combined with a complex prebiotic local action, patients in the control group, the probiotic localy in the form of the vaginal candles or tablets. Results. The research stated the increasing level of serum IgA in blood plasma of patients of the main group compared to control group at 20%, normalizing the pH of the vaginal environment in the main group in 94% of cases, which indicates an increase of immunity in mucosal. Conclusion. The inclusion of the systemic probiotic in the scheme of treatment of disorders of biocenosis of the vagina system enhances the increasing of immunity of the mucous membranes, and the vaginal tablets prebiotic of local action restores the own normal microflora of the vagina.
