The healthcare and social cost associated with diabetic peripheral neuropathy and the foot can be devastating. There are more than 300,000 hospitalizations in the US each year for diabetic foot complications and there were more than 92,000 lower extremity amputations of people with diabetes in 1999.1The problems of ulceration and amputation have a profound effect on the quality of life of those who are affected.

Traditional Chinese medicine (TCM) practitioners assess body constitution (BC) as a treatment basis for maintaining body homeostasis. We investigated patterns in spontaneous brain activity in different BC groups using resting-state functional magnetic resonance imaging (rsfMRI) and determined the relationship between these patterns and quality of life (QOL). Thirty-two healthy individuals divided into two groups (body constitution questionnaire (BCQ)-gentleness [BCQ-G] and BCQ-deficiency [BCQ-D]) based on the body constitution questionnaire (BCQ) underwent rsfMRI to analyze regional homogeneity (ReHo) and the amplitude of low-frequency fluctuation (ALFF). The World Health Organization Quality of Life Instruments (brief edition) scale was used to evaluate the QOL. The BCQ-G group (n = 18) had significantly greater ReHo values in the right postcentral gyrus and lower ALFF values in the brainstem than the BCQ-D group (n = 14). In the BCQ-D group, decreased ReHo of the postcentral gyrus correlated with better physiological functioning; increased ALFF in the brainstem correlated with poor QOL. BCQ-subgroup analysis revealed a nonsignificant correlation between ReHo and Yang deficiency/phlegm and stasis (Phl & STA). Nonetheless, the BCQ-D group showed a positive correlation between ALFF and Phl & STA in the parahippocampus. This study identified differences between BCQ-G and BCQ-D types of healthy adults based on the rsfMRI analysis. The different BCQ types with varied brain endophenotypes may elucidate individualized TCM treatment strategies.

Objective. This study aimed to investigate the effects of traditional Chinese medicine (TCM) on biological behavior and magnetic resonance imaging and recurrence rate of patients with bladder cancer. Method. Forty-seven postoperative bladder cancer patients treated in our hospital who met the criteria were selected and randomly divided into the TCM treatment group (observation group) and the group without TCM treatment (control group). In the TCM treatment group, the prescription was slightly adjusted according to the different symptoms, and the main prescription remained unchanged. According to the treatment plan, patients continued to undergo bladder irrigation chemotherapy plus TCM treatment, while the control group was only treated with bladder irrigation chemotherapy. The number of patients with recurrence at 3 and 6 months and 1 year, the effects on patients’ clinical symptoms, and quality of life were observed, respectively. The changes in MRI images, blood routine, immune function, and leukocyte level and other related indexes before and after treatment were compared between the two groups. Results. After the patients in the observation group were treated with traditional Chinese medicine, the patients’ quality of life significantly improved. The patients’ CD3+, CD4+, and CD4+/CD8+ indexes were significantly higher than those of the control group. The levels of Hb and PLT of the patients in the observation group were significantly lower than those of the patients in the control group. Patients in the observation group had higher leukocyte levels and a lower recurrence rate than patients in the control group. Conclusion. TCM with chemotherapy drugs can effectively improve patients’ immune function, increase the level of T-lymphocyte subpopulation, and improve bone marrow hematopoietic function, which has a significant effect on the prevention and treatment of bladder cancer recurrence after surgery.

Lumbar Foraminal Stenosis (LFS) dapat secara signifikan mengurangi fungsi dan kualitas hidup pasien dan Magnetic Resonance Imaging (MRI) adalah alat pendukung yang umum digunakan untuk mengukur beratnya stenosis. Lee score umumnya digunakan untuk mengukur derajat LFS pada MRI sagital. Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) dan Oswestry Disability Index (ODI) digunakan untuk menilai disabilitas dan skor fungsional pada pasien LFS. Penelitian ini bertujuan untuk mengetahui korelasi antara derajat LFS pada MRI sagital dengan kualitas hidup pada pasien dengan LFS. Penelitian ini merupakan penelitian analitik observasional yang melibatkan 25 pasien dengan gejala klinis LFS di RS. X Surakarta. Pasien dinilai dengan mengisi kuesioner JOABPEQ dan ODI, kemudian dilakukan evaluasi MRI sagittal lumbar untuk menentukan derajat Lee score, kemudian melakukan uji korelasi pada data yang diperoleh. Penelitian ini menunjukkan korelasi yang signifikan antara Skor Lee dengan JOABPEQ dan ODI. Tingkat LFS berdasarkan Lee Score memiliki korelasi yang signifikan dengan tingkat disabilitas menggunakan JOABPEQ dan ODI. JOABPEQ memiliki korelasi yang lebih signifikan dengan Skor Lee dibandingkan dengan ODI. Kata Kunci : Lumbar Foraminal Stenosis, Lee Score, JOABPEQ, ODI Lumbar Foraminal Stenosis (LFS) can significantly reduce the patient’s function and quality of life and Magnetic Resonance Imaging (MRI) is commonly used supporting tool to measure the degree of stenosis. Lee Score is commonly used to measure the degree of LFS on sagittal MRI. Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) and Oswestry Disability Index (ODI) to assess disability and functional scores in LFS patients. This study was conducted to determine the correlation between the degree of LFS on sagittal MRI images with quality of life in patients with LFS.This study is an observational analytics study involving 25 patients with clinical symptoms of LFS in X Hospital Surakarta. Patients were assessed by filling JOABPEQ dan ODI questionnaires, then performed sagittal lumbar MRI evaluation to determine the degree of Lee Score, then performed correlation test on the data obtained.This study shows a significant correlation between Lee Score with JOABPEQ and ODI. The degree of LFS based on Lee Score has a significant correlation with the degree of disability using JOABPEQ and ODI. Abstract Background Endogenous Cushing’s syndrome (CS) results in increased cardiovascular (CV) morbidity and mortality. So far, most studies focussed on distinct disease entities rather than the integrity of the CV system. We here describe the design of the Cardiovascular Status in Endogenous Cortisol Excess Study (CV-CORT-EX), a study aiming to comprehensively investigate the health status of patients with endogenous CS (with a particular focus on CV phenotypes, biochemical aspects, quality of life, and psychosocial status). Method A prospective non-interventional cohort study performed at a German tertiary referral centre. At the time of enrolment, patients will be categorised as: (1) newly diagnosed overt CS, (2) recurrent overt CS, (3) CS in remission, (4) presence of mild autonomous cortisol excess (MACE). The target cohorts will be n = 40 (groups 1 + 2), n = 80 (group 3), and n = 20 (group 4). Patients with overt CS at the time of enrolment will be followed for 12 months after remission (with re-evaluations after 6 and 12 months). At each visit, patients will undergo transthoracic echocardiography, cardiac magnetic resonance imaging, 24-h electrocardiogram, 24-h blood pressure measurement, and indirect evaluation of endothelial function. Furthermore, a standardised clinical investigation, an extensive biochemical workup, and a detailed assessment of quality of life and psychosocial status will be applied. Study results (e.g. cardiac morphology and function according to transthoracic echocardiography and cardiac magnetic resonance imaging; e.g. prevalence of CV risk factors) from patients with CS will be compared with matched controls without CS derived from two German population-based studies. Discussion CV-CORT-EX is designed to provide a comprehensive overview of the health status of patients with endogenous CS, mainly focussing on CV aspects, and the holistic changes following remission. Japanese encephalitis is an important cause of encephalitis in Southeast Asia. Survivors may suffer from various movement disorders leading to disability, presumed to be due to involvement of basal ganglia and thalamus. Oromandibular dystonia is a rare complication of Japanese encephalitis and treatment is unsatisfactory in severe cases. We report a child with JE who developed markedly severe oromandibular dystonia in subacute phase of illness. His Magnetic Resonance Imaging revealed involvement of basal ganglia and thalami. Oral antidystonic medications were used without much avail. In view of severe disabling oromandibular dystonia he was treated with botulinum toxin without adverse effects and had improved quality of life. Botulinum toxin may be considered as a therapeutic option in severe oromandibular dystonia following Japanese encephalitis.

A 73-year-old exsmoker with Global initiative for chronic Obstructive Lung Disease stage III chronic obstructive pulmonary disease underwent airway bypass (AB) as part of the Exhale Airway Stents for Emphysema (EASE) trial, and was the only EASE subject to undergo hyperpolarized3He magnetic resonance imaging for evaluation of lung function pre- and post-AB.3He magnetic resonance imaging was acquired twice previously (32 and eight months pre-AB) and twice post-AB (six and 12 months post-AB). Six months post-AB, his increase in forced vital capacity was <12% predicted, and he was classified as an AB nonresponder. However, post-AB, he also demonstrated improvements in quality of life scores, 6 min walk distance and improvements in3He gas distribution in the regions of stent placement. Given the complex relationship between well-established pulmonary function and quality of life measurements, the present case provides evidence of the value-added information functional imaging may provide in chronic obstructive pulmonary disease interventional studies.
