Background: It is very important to keep the Glucose levels under control continuously and without any holidays. Type 2 diabetes is increasingly common in the industrialized world. Tight glycemic control attempts to rigidly glucose control levels (A1c: 6.5% -7.0% or lower). Maintaining tight glycemic control is lifesaving. Proper counselling, proper nutrition and if regular exercise is done it can result is good glycemic control.Methods: In the present study a total of 350 diabetic patients were selected out of which only 208 participants were eligible for study. Inclusion criteria in the study were 1) Age: men and women between age group 30 years to 70 years and 2) Type 2 Diabetes mellitus with HbA1c between 8 to 10%. Exclusion criteria in the study were 1) Type 1diabetes mellitus, Pre-existing renal, hepatic or cardiac disease, Hba1c >10%. A quadriad was established between patients, personal health worker, dietician and doctor.Results: Participants had regularly followed up and were divided randomly into cases (n = 112) and controls (n = 96). Patient were followed up as per study design it was observed that at the end of 3 months period mean FBS (case group 168.2±26.4 control group 200.8±38.3 p value <0.001) , Mean PPBS (case group 204.8±53.0 control group 271.0±45.5 p value <0.001) and Mean Hba1c (case group 8.7±0.5 control group 8.9±0.6 p value .003) in cases was significantly lower than control group.at the end 6 months it was observed that the mean cholesterol, mean triglyceride, mean LDL and mean VLDL was significantly lower in the cases compared to controls.Conclusions: A systematic approach and close monitoring that increased the adherence to medication, diet, and counselling would help in better glycemic control and prevent long term complication.

Background. Subjects with type 2 diabetes mellitus (DM2) require an adequate glycemic control to avoid diabetic complications. Currently, saliva biomarkers are used as a diagnostic tool and can be indicative of the degree of progression and control of various diseases. Several studies indicate thatα-2-macroglobulin levels are elevated in diabetic patients.Methods. 120 subjects with DM2 were enrolled and classified into two groups according to their glycemic control (percentage of glycated hemoglobin-A1c (HbA1c), <7% adequate glycemic control group; >7% inadequate glycemic control group). The relationship betweenα-2-macroglobulin levels from saliva samples and HbA1c was subsequently evaluated.Results. We found a positive correlation betweenα-2-macroglobulin and HbA1c (r=0.778andP<0.0001). Area under the receivers operating characteristic (ROC) curve ofα-2-macroglobulin indicated a positive discrimination threshold ofα-2-macroglobulin (AUC = 0.903, CI 95%: 0.847–0.959,P<0.0001) to diagnose glycemic control.Conclusions. Our data strongly suggest that the level of salivaα-2-macroglobulin is an indicator for the degree of glycemic control in diabetic patients and represents a promising alternative method to evaluate this parameter.

Background: Hypothyroidism is an endocrine disorder resulting from deficiency of thyroid hormones, and Diabetes is a metabolic disorder that share the phenotype of hyperglycaemia. Both the endocrinopathies have been found to be associated with dyslipidaemia and atherosclerosis that result into various complications. Our aim was to assess the difference of dyslipidaemia in patients suffering from hypothyroidism with diabetes as compared to diabetes alone.Methods: Study was conducted in department of medicine in SGRDIMSR, Sri Amritsar. A total of 120 patients were enrolled for the present study, the one who presented to hospital from January 2018 to August 2019 diagnosed with diabetes mellitus type 2. The patients were divided into two groups. 60 patients having type 2 diabetes without hypothyroidism (control group). 60 patients having type 2 diabetes with hypothyroidism (study group). Fasting lipid profile were performed in both the groups and were analysed for the study.Results: Dyslipidaemia was seen more in study group as compared to control group with mean cholesterol of 488.3(±144.43) and mean triglyceride levels of 354.30(±128.57) in study  group as compared to control group with mean cholesterol and triglyceride levels of 179.7(±81.47) and 177.08(±118.18) with p-value of 0.001 and 0.01 respectively which were significant. Also, obesity and diabetic complication were more in patients of dual endocrinopathies as compared to diabetes alone which were also significant statistically.Conclusions: From our study it is concluded that type 2 diabetes with hypothyroidism causes more dyslipidaemia as compare to type 2 diabetes alone. So proper screening of thyroid profile of type 2 diabetic patients and its correction helps in achieving better lipidemic control which further prevent complications.

ENGLISHThe prevalence of hypertension in type 2 diabetic patients is 1,5-3 times higher than in nondiabetic The objectives of this research is to explain the risk factors affecting hypertension in type 2 diabetic patients. The research used an observational studies with case-control study design in Primary Healthcare Centers patients in Pati Regency of 2014. Case group were 57 patients with hypertension in type 2 diabetes, while control group were the type 2 diabetes patients without hypertension. Data were obtained from medical records and qualitative interviews. Chi-square test in bivariate and multiple logistic regression in multivariate analysis. This study has been obtained ethical clearance from The Ethical Committee of Health Research Medical Faculty of Diponegoro University or dr. Kariadi Hospital. Results : factors that influence hypertension in type 2 diabetic patients were physical activity (OR=6.4; 95% CI: 2.18-18.77; p=0.001), diabetes duration ≥ 5 years (OR=5.4; 95% CI: 1.97 – 14.704; p=0.001), and medication adherence (OR=3.6; 95% CI: 1.32-9.83; p=0.012). Other risk factors that not significantly influenced were age ≥45 years, male, diet compliance, history of hypertension, smoking, salt consumption, coffee consumption, and sleep duration. INDONESIAPrevalensi hipertensi pada penderita DM tipe 2 lebih tinggi dibandingkan non DM tipe 2. Tujuan penelitian untuk menjelaskan faktor yang berpengaruh terhadap terjadinya hipertensi pada penderita DM tipe 2 di wilayah kerja Puskesmas Kabupaten Pati tahun 2014. Jenis Penelitian observasional dengan rancangan studi kasus kontrol pada pasien Puskesmas. Kelompok kasus adalah 57 pasien DM tipe 2 dengan hipertensi sedangkan kelompok kontrol adalah 57 pasien DM tipe 2 tanpa hipertensi. Data diperoleh dari observasi catatan medis dan wawancara. Uji chi-square pada analisis bivariat dan analisis multivariat dengan regresi logistik ganda. Penelitian ini telah memdapatkan Ethical clearance dari Komisi Etik FK UNDIP/RSUP dr. Kariadi Semarang. Hasil Penelitian : faktor yang berpengaruh terhadap terjadinya hipertensi pada penderita DM tipe 2 adalah aktivitas fisik kurang (OR=6,4; 95% CI: 2,18 - 18,77; p=0,001), lama menderita DM ≥5 tahun (OR=5,4; 95% CI: 1,97 - 14,704; p=0,001), dan kepatuhan minum obat DM (OR=3,6; 95% CI: 1,32 - 9,83; p=0,012). Faktor yang tidak berpengaruh adalah : usia ≥45 tahun, jenis kelamin laki-laki, kepatuhan diet DM, riwayat hipertensi, kebiasaan merokok, kebiasaan makan asin, kebiasaan minum kopi, dan lama waktu tidur.

Objective: : High intensity interval training (HIITs) can induce weight control, lowering blood pressure and beneficial effects on cardiovascular health in type 2 diabetic patients. The effect of different volumes of these exercises is unclear in type 2 diabetic patients. The aim of this study was to compare the effect of low volume and high volume of short-term intensive training on glycemic indexes of men with type 2 diabetes. Materials and Methods: Thirty type 2 diabetes male patients who were referred to Yazd diabetes research center (30 - 46 years old) were selected and randomly divided into 3 groups of low and high volume HIITs groups and control group. The intensity of the exercises in low volume was up to 110% and in high volume up to 80% of the maximum heart rate. The two training groups performed exercises 3 days of week for 8 weeks. Glycemic factors and lipids profile were measured before and after the last training session. Data were analyzed by covariance and paired T-test. Results: Low volume HIIT exercises significantly decreased the glucose ( P -value: 0.01), HbA1c ( P -value: 0.01), insulin ( P -value: 0.005), insulin resistance ( P -value: 0.001), and triglyceride ( P -value: 0.04). Low volume HIIT in the insulin resistance had a significant difference with the control group ( P -value: 0.04). High density lipoprotein in high volume group had a significant difference with the control group ( P -value: 0.021). Conclusion: Low-volume HIIT exercises can be a nonpharmacological approach to improving glycemic factors in type 2 diabetic patients.

Background: Diabetes mellitus (DM) type 2 is one of the most common endocrine disorders affecting more than 135 million people in the world. The etiology of the disease is not fully understood, but recently subclinical hemochromatosis has been considered as one of the probable causes of DM. This study was carried out to examine the relationship between serum ferritin as a marker of iron overload with DM and HbA1c.Materials & Method: This study was conducted in the Biochemistry department of Sir Salimullah Medical College, Dhaka; over a period of 18 months from July 2013 to December 2014. In this case control study, 46 patients with type 2 diabetes were taken as case, who were referred to theoutpatient department of "Ibrahim General Hospital & Diabetic care & Educational Center"(DCEC). 46 normal individuals were included as the control group, who were matched with the case group regarding age, sex, BMI and Hb%. Ferritin, hemoglobin, HbA1c and fasting plasma sugar were measured in blood samples. Exclusion criteria included anaemia, or any other disease or drug that could affect ferritin levels.Result: Results were analyzed statistically by Chi-square test, Student's t-test, Pearson correlation coefficient test and Odds ratio.Mean serum ferritin was significantly higher in diabetics than in the control group (197.97±75.99 µgm/L vs. 64.24±27.83 µgm/L, p<0.001). There was significant positive correlation between serum ferritin and HbA1cin diabetic patients (p<0.001). In this study, OR of 11.64 was also found.Conclusion: Serum ferritin is positively correlated with type 2 Diabetes Mellitus. Android Based Diabetic Manager (ABDM) is an application used for diabetic patients to control their diet. This application is an easy way to remind them in consuming foods and giving them alert when its necessary calories have been achieved. This study was conducted in response to technological developments in the Era 4.0 to help people with Type 2 Diabetes Mellitus through diet control that can be done independently. This study was conducted in the city of Magelang aimed to evaluate whether using ABMD was effective to improve patients adherence in their diet and to control their blood glucose levels. This experimental study was using pre test and post test with control group design. By using simple random sampling, 52 people suffering from Type 2 Diabetes Mellitus recruited in this study devided into two groups, ABDM group and control group. The Pearson Chi-Square test result showed that there was significantly different between ABDM group and control group in the status of blood glucose control and patient adherence with p value was 0.048and 0.000 respectively (p value <0.05). This result indicated that H0 was rejected and H1 was accepted, which could be interpreted that Android based Diabetic Managerapplication influenced the status of blood sugar control and improved the patient adherence in managing their diet. Recomendation was made to conduct future study in the wider Diabetes community by adding more complete types of menu list and its variations in the Android based Diabetic Manager.

Background: People with type 2 diabetes compared to those without seem to have more chances of developing depressive symptoms. Diabetes leads to depression or vice versa, remains an unsolved puzzle. Diabetes and depression together leave deep psychological and physical imprints on their victims leading to functional limitation, poor quality of life and raised mortality rate. Diabetic patients should be warned by the physicians to keep a watch for the development of depressive symptoms in them. The study aimed to investigate the relationship between type 2 diabetes mellitus and depression. Methods: The cross sectional study was conducted at Ziauddin University Hospital over a period of six months. Sample size of 100 was taken. Details of participant’s demographics along with Body Mass Index were recorded. Questionnaires were filled by researchers. SRQ 20 depression scale was used to diagnose depression. Data was entered and analyzed by version 20 of SPSS. Results: Different demographic and social variables were assessed in this study. 100 patients with HbA1c <6.5% were taken and 100 with ≥ 6.5%. 73 (36.5%) patients out of the total sample were depressed, almost from the good glycemic control group. Only a quarter of patients that had normal HbA1c levels became depressed however 48% patients with poor glycemic control suffered depression. Conclusion: Research concludes that co-morbid diabetes and depression is not a rare finding. Risk factors must be ruled out and the chances to develop depression should be identified at an earlier stage before complications worsen the condition.

Purpose: The aim of this study was to assess the correlation of hemoglobin A1c (HbA1c) levels with choroidal thickness in patients with type 2 diabetes mellitus (DM) using spectral domain optical coherence tomography (SD-OCT). Methods: In this prospective case series, 180 eyes from 90 patients with type 2 DM were classified into three study groups based on HbA1c values: group 1 included patients with good glycemic control (HbA1c ≤ 7%), group 2 included patients with moderate glycemic control (HbA1c between 7% and 8%), and group 3 included patients with poor glycemic control (HbA1c ≥ 8%). Additionally, 50 eyes from 25 non-diabetic subjects were enrolled to group 4 as a control group. Sub-foveal, nasal, and temporal choroidal thickness were measured and compared. Results: Mean central, nasal, and temporal choroidal thicknesses in diabetic patients (247.80, 238.63, and 239.30.

Background :Alkaline phosphatase (ALP) was a widely used marker for skeletal and hepatobiliary disorders, but its activity was also increased in atherosclerosis and peripheral vascular disease. Several study has showed that ALP activity was increased in the sera of diabetic patients. The current study was conducted to evaluate ALP activity in type 2 diabetic patients and optimum conditions for enzyme activity in their sera.Methods: This study was carried out at in AL-Yarmok hospital(diabetic center) between February /2009 and April /2009. Fifty two patients with type 2 diabetes have been enrolled. Besides BMI, WHR, serum fasting blood glucose, ALP, HbA1C,uric acid and lipid profile levels have been performed .The relationship between ALP and other biochemical factors have been studied.Results: From a total 52 cases, FBG, HbA1C and ALP were significantly elevated P value < 0.01 while Uric acid, Cholesterol, TG, HDL, LDL,VLDL and LDL/HDL were significantly different P value < 0.05 in diabetic patients when compared with that found in control group . ALP was significantly associated with LDL (P < 0.05) and significantly negative correlation with HbA1C (P

Introdution: Adiponectin is a peptide secreted from fat cells that plays an important role in the development of insulin resistance and type 2 diabetes. A number of single nucleotide polymorphisms in the adiponectin gene are associated with lower adiponectin expression and T2DM. The aim of this study was to determination the frequency of rs 2241766 polymorphism in adiponectin gene and its relation with adiponectin level, insulin resistance, type 2 diabetes and in pre diabetes/diabetes and healthy individuals. Methods: This study was performed in a case -control method. 73 prediabetic/diabetic patients with fasting blood sugar >100 mg/dl was selected as the case group without drug history, and 76 individuals with FBS 70-100 mg/dl as the control group. Biochemical and anthropometric variables were measured by standard methods and the levels of adiponectin and insulin hormone were measured using commercially available human ELISA kit. The determination of genotyping was done using PCR-RFLP method. Data were analyzed by SPSS software version 19. Results: Regression analysis showed that polymorphism rs2241766 in the adiponectin gene has no significant relation with diabetes and insulin resistance. No significant difference was found between TT and TG carriers for any of the variables in the two groups (pre diabetes/diabetes and Non-diabetic). A negative correlation was seen between adiponectin and insulin resistance ( r=-0.225, p< 0/01). Conclusion: According to this study, adiponectin gene polymorphism rs2241766 is not associated with adiponectin levels, insulin resistance and type 2diabetes, but there was a correlation between adiponectin and insulin resistance.
