Social longevity of elderly people is largely determined by their dental health. The ability to talk freely without feeling socially awkward due to bad breath or visible absence of teeth greatly facilitates the lives of elderly people engaged in work. Unfortunately, the majority of elderly people have compromised dentitions, restored by dental prosthetic structures that meet social and functional requirements to varying degrees. The high need of elderly people in stomatological orthopedic treatment and the demand for comprehensive dental rehabilitation determined the relevance and purpose of this publication. Research methods. The object of the study was elderly people aged 60-74 years who sought dental orthopedic care in dental medical organizations, the sample size was 408 persons. Results. The maximum number of patients in the 60-74 age group was found to have complete absence of teeth – 26.5%, the total number of patients with the first, second, third and fourth classes according to Kennedy classification was 13,3%, 24,7%, 24,9%, 10,6% accordingly. The total number of patients with the first Kennedy subclasses (class 1-3) was 135 persons (33.1%), with the second subclasses – 69 persons (16.9%), with the third subclasses – 53 persons (13.0%). Conclusions. All types of orthopedic dental structures were manufactured for elderly patients. The choice of the dental prosthetic design used in orthopedic dental rehabilitation was determined by the type of dentition defect and met the principles of rational prosthetics.

Relevance. The social profile of senile people has some differences from other age categories of patients. People of the older age group dream of finding harmony with the outside world, preserving the love of the nearest and dearest and relatives and the opportunity to communicate with the opposite sex. Dental health and the resulting quality of life contributes to this in many ways. A smile, laughter, the ability to talk without feeling embarrassed by splashing saliva, bad breath or visible absence of teeth, taking your favorite food together with the nearest and dearest are the main indicators of their life quality dependent on dental health, without which it is impossible to imagine a modern person High prevalence of dental diseases among senile people cause a high need for making various dental prosthetic structures. Studying dental health of this category of citizens can become a promising area of practical healthcare, a comprehensive study of this issue has determined the relevance and purpose of this publication.Materials and methods. The object of the study was senile people, 75-89 years old, who sought dental orthopedic care in municipal healthcare institutions of Cheboksary, the Chuvash Republic. The sample size was 481 people.Results. It was found that the maximum number of patients in the age group of 75-88 years - 188 persons (39.1%) - had complete absence of teeth, the total number of patients with the first, second, third and fourth classes according to the Kennedy classification of dentition defects was 56 persons (11.6%), 143 persons (29.7%), 79 persons (16.4%), 15 persons (3.2%), respectively. The total number of patients with the first Kennedy subclasses (class 1-3) was 135 persons (33.1%), with the second subclasses – 69 persons (16.9%), with the third subclasses - 53 persons (13.0%). The analysis of dental prosthetic structures mounted in elderly patients showed that the total number of orthopedic structures amounted to 4,615 units, among which there were pin structures - 1,328 units (28.8%); the total number of artificial dental crowns was 1,662 units (36.0%); the total number of prosthetic bridges – 620 units (13.4%); removable laminar dentures in partial loss of teeth – 445 units (9.6%); clasp prostheses – 184 units (4.1%); removable laminar dentures in complete loss of teeth – 376 units (8.1%).Conclusions. All types of dental orthopedic diseases are commonly found in patients of senile age, they had all kinds of dental prosthetic structures made. The choice of the dental prosthetic design used in orthopedic dental rehabilitation was determined by the type of dentition defect and met the principles of rational prosthetics.

ABSTRACTSThe effects of smoking on human health are serious and in many cases, deadly. There are approximately 4000 chemicals in cigarettes, hundreds of which are toxic. The ingredients in cigarettes affect everything from the internal functioning of organs to the efficiency of the body’s immune system; the effects of cigarette smoking are destructive and widespread. All forms of tobacco including cigars, smokeless tobacco, and huka pose dental health concernsOBJECTIVES:To Assess the knowledge/awareness, attitude and practices (KAP) of people about adverse effects of smoking on their health particularly oro-dental healthMETHODOLOGY:The study was conducted in two teaching hospital of Peshawar i.e. KCD (Khyber college of dentistry and SBDC (Sardar begum dental college).it was a cross sectional descriptive study carried out over400 patients who attended the OPD of these hospitals for some reasons. Approval of the study was taken from the ethical committees of both the hospitals. Non probability sampling technique was adopted for the study. All relevant information about the study was gathered with the help of semi structured questionnaire. Purpose of the study was explained to the patients. The study was conducted from May 2016 to November 2016. Patients with systemic diseases that could affect oro-dental health were excluded from the study. Data was presented in the form of tables and graphs.RESULTS:According to their dentist 52% of study patients had adverse oral hygiene. About 74.5% smokers were aware of adverse effects of smoking. 55% of the people who smoke cigarette are 21-30 years of age. 26.5% people smoked 1-3 times a day. . In a significant number of patients 41% people started smoking because of peer pressure. Mass media had a significant role in developing this habit. About 74.5% are aware of adverse effects of smoking and they consider that it causes bad effects on respiratory system and oral hygiene. Most of the smokers use tooth paste to get rid of bad breath. About 59% people visited dentist only when some oro-dental problem arises. According to dentist 39.5% of smokers have adverse oral health status.CONCLUSION:Despite of knowledge of adverse effects of smoking and complaining of bad breath and other conditions related to oro-dental hygiene, majority of them were engaged in regular smoking and they are still satisfied from their oral health ,

Subject. An interdisciplinary approach in the science and practice of the dental industry has long become an established reality. The direct and indirect effect of a huge number of somatic diseases on the dental health of patients, the tactics of their treatment and subsequent follow-up observation was studied. One of these diseases is diabetes. On the other hand, the number of lawsuits against orthopedic dentists is growing every year and they are often associated with an insufficient, in the patient's opinion, period of comfortable use of the manufactured construction. When analyzing the causes of patients re-seeking dental orthopedic care, we revealed a significant discrepancy in their occurrence in patients with and without diabetes. In this article, we tried to describe the causes and consequences of such a phenomenon and propose ways to solve the discovered problem. The goal is to reduce the number of complications of dental orthopedic treatment in patients with diabetes mellitus. Methodology. We analyzed data from 304 patients who applied to dental clinics for the purpose of repeated prosthetics of teeth and dentitions. One of the research methods was a specially designed questionnaire, which included a number of questions for the reasons for replacing the prosthesis. Clinical and paraclinical methods for examining patients that meet the standards of care for established diagnoses were also used. Results. A clear dependence of the life of dentures on the presence of a history of diabetes was revealed. The main complaints of patients with repeated visits to the orthopedic dentist are formulated. The most common causes of early abandonment of the use of dentures were identified and the peculiarities of adaptation to them in patients with the pathology under consideration were determined. Findings. The obtained data can be applied in predicting the immediate and long-term results of prosthetics of teeth and dentitions, and on the basis of them, the period of clinical observation of patients with diabetes can be changed.

Relevance. The success of treatment of periodontal diseases directly depends on the patient’s response to the therapy, therefore, the characteristics of the person’s personal characteristics can affect both the effectiveness of treatment and the prevention of relapse of the disease.Purpose. To study the features of the internal picture of the disease in the process of complex treatment of adult patients suffering from chronic generalized periodontitis.Materials and methods. The generally accepted comprehensive treatment of chronic generalized periodontitis in 69 middle-aged and elderly men was carried out taking into account the personal response of patients Solovyov «Psychosensory-anatomical-functional maladaptation syndrome».Results. When patients were discharged from the hospital, there was a difference in the phenomena of maladaptation among the elderly and middle-aged: in middle-aged people, sufficient adaptation to the conditions of existence was determined; in elderly people, due to the existing comorbid pathology, a state of maladaptation was diagnosed, which was caused by the presence of complaints of defects in the dentition.Conclusion. In elderly people, as soon as possible after completion of treatment in a hospital, dental rehabilitation cannot be considered completed, which requires the adoption of organizational measures for their timely provision of dentures.

Abstract Microtubules in eukaryotic cells participate in a variety of nuclear and cytoplasmic structures, reflecting functional requirements and cell cycle position. We are studying the cellular regulation of microtubule assembly and organization in the yeast Saccharomyces cerevisiae. We screened for genes that when overexpressed suppress the growth phenotype of conditional mutants in alpha-tubulin that arrest with excess microtubules at the nonpermissive temperature (class 2 mutations). Here we describe one such suppressing element, called ATS1 (for Alpha Tubulin Suppressor). Overexpression of this gene rescues both the growth and microtubule phenotypes of all class 2 mutations, but not the cold-sensitive mutations that arrest with no microtubules (class 1 mutations). Deletion of ATS1 confers a modest slow growth phenotype which is slightly enhanced in strains containing both a deletion of ATS1 and a class 2 tub 1 mutation. The predicted ATS1 protein contains 333 amino acids and has considerable structural homology to the products of both the mammalian mitotic control gene RCC1 and the S. cerevisiae gene SRM1/PRP20. Overexpression of SRM1/PRP20 also suppresses class 2 mutants. The results suggest that this family of genes may participate in regulatory interactions between microtubules and the cell cycle.

The purpose of the article is to conduct a study of the main problems of delivery of orthopedic care in dentistry. It is known that dental health determines the overall health of the body. The comfort of life of the population depends on their condition, since damaged teeth negatively affect the state of the digestive system and respiratory organs. As for the aesthetics of the appearance, here teeth have a special role, since they are able to provide both proper speech and the necessary level of human sociability. Thus, improving the quality of delivery of medical care is an important task for the industry today. The problem of the availability of orthopedic dentists exists only in those areas of the region where there is no orthopedic care encounters at all, or orthopedic care encounters are carried out by part-time doctors. Accordingly, it can be said that municipal dental clinics are generally provided with the necessary personnel. In this regard, one can talk about the need to improve the quality of treatment of dental diseases in polyclinics, primarily in terms of orthopedic care for the population. However, today there are frequent cases of return visits for orthopedic care due to its poor-quality during initial treatment. And the doctor’s task during second denture treatment is to avoid mistakes made earlier and to provide competent and highquality orthopedic services. The author concludes that the results of a study of the work of orthopedic units of the region showed a steady growth of most indicators year by year, but a number of economic problems were found during the analysis of the profitability reserves of orthopedic dental care. So, it is necessary to create a unified system for calculating the financial plan for the correct assessment of the status of orthopedic dental care for the population, to analyze the quality indicators for subsidized denture treatment, to introduce the concept of “prosthesis working lifespan”, which will establish the reasons and justify the terms of the second denture treatment.

The importance of maxillary canine impaction is that it may compromise dental health , particularly because a small but significant proportion of impacted canine is associated with the resorption of the roots of neighbouring teeth. A few cases of impacted upper canines, premolar and central incisors of different ages are presented. Although the best treatment alternative for impacted teeth is extraction, the same cannot be directly applied to canines and central incsor especially. Since canines present with them a high demand on aesthetic and functional requirements. Thus, alignment of impacted canines into the arch is the best treatment approach. The purpose of this study is to evaluate the impacted maxillary canines that were surgically exposed, followed by orthodontic alignment into occlusion. The main cause of prosthetic stomatitis belongs to the chemical and toxic action of the residual monomer of the prosthesis base, which is a protoplasmic poison. Occurrence of prosthetic stomatitis depends not only on quality of production of prosthesis in laboratory though at non-observance of technology indicators of residual monomer can reach 2-5%, but also at individual intolerance at its minimum concentration in a prosthesis after polymerization - 0,2-0 .5%. The aim of our study was to increase the effectiveness of orthopedic treatment of patients with prosthetic stomatitis by coating the plastic of removable prostheses with nanoscale materials. Materials and methods. To solve this goal, we studied the condition of the tissues of the prosthetic place of patients with prosthetic stomatitis with prosthetic removable prostheses with modified plastic. Orthopedic dental treatment of 50 people was examined and performed, including 25 people (the second group, prostheses were not covered with nanoparticles) and 25 people (the third group, prostheses were covered with nanoparticles). The first control group consisted of 10 people without signs of pathology. Prior to treatment, all patients had removable acrylic plastic dentures. The reason for seeking orthopedic care was a violation of masticatory function and the inability to use previously made prostheses due to the development of pain in the soft tissues of the prosthetic place. Complaints of pain were observed in all patients of varying intensity, impaired fixation and stabilization of the prosthesis due to swelling of the mucous membrane of the soft tissues of the prosthetic place, heartburn and dryness were observed in 90% of patients. Complaints were also about speech and aesthetic defects. Patients of III group after two weeks of using prostheses were coated with the inner surface of the prosthesis, which is in direct contact with the mucous membrane of the prosthetic place with molecules of fullerene C60, by magnetron sputtering. For this purpose, the prostheses were removed from the patients for several days and returned after the coating with the nanomaterial, after which the observation was continued. The results. After coating the prostheses in patients of group III with Fullerene C60, we observed the disappearance of inflammation of the mucous membrane under the prosthesis and patients noted the absence of discomfort. Рatients in II group had a negative dynamics in 80% and had diffuse inflammation of the mucous membrane under the prosthesis. Within 3 months of use, 18 patients (72%) in II group reported that they stopped using removable dentures during the day, due to unpleasant pain under the prosthesis, and used only during meals and during conversations. In contrast to II group, patients in III group did not notice discomfort when using plate prostheses. Conclusions. Obtained in the course of the work convincingly prove the effectiveness of the use of removable plate prostheses with nanocoating for the treatment and prevention of prosthetic stomatitis in patients. This is evidenced by the data of objective examination and the disappearance of complaints from patients.

Aim The aim of this study was to determine the possible effects of the 2006 National Health Service General Dental Services contract changes on the referral pattern to the orthodontic department at St George's Hospital, South West London. Method This study was carried out on a retrospective basis. The notes of consecutive patients referred between 1st May and 30th September in 2005 and 2008 were assessed, and the patient's Index of Orthodontic Treatment Need (IOTN) and the source of referral noted. Results The total numbers of referrals increased from 260 in 2005 to 405 in 2008. The number of referrals from general dental practitioners decreased slightly from 165 to 156, as did the numbers of referrals from other sources, such as tertiary referrals. The number of referrals made by specialist practitioners increased from 41 in 2005 to 207 in 2008, representing an increase from 16% to 51% of overall referrals. Overall, the number of patients being referred with an IOTN dental health component grade of 5 increased from 27% to 55%. Conclusion The increase in referrals from specialist practitioners may be partly due to the changes brought to the commissioning of orthodontic services for specialist practitioners. Overall, the number of cases being referred with IOTN grades 4 and 5 remains high at St George's Hospital, indicating that appropriate referrals are being made.
