Background and Purpose: Superficial mycoses are the fourth most common cause of disease worldwide. It is not surprising that zoonotic transmission occurs to humans due to close contact with different animals, be it companion or farm animals. Therefore, the objective of this study was to determine the presence of asymptomatic dermatophyte carriers in the owner-pet pairs, identify the most common etiologic agents, and find the likely connection between the carrier status of an owner and the presence of dermatophytes in their pets. Materials and Methods: From May 2019 to January 2020, 21 cats and 115 dogs with their respective owners were selected for dermatophyte culture. All the dogs and cats included in the study were from the communities of southeastern Mexico. The samples were taken with a cotton swab, which was vigorously rubbed and twisted on the scalp or body of the pet four times and grown on Mycosel Agar. The isolates were identified based on macroscopic and microscopic characteristics. The prevalence of the binomial ranged from 0.73% in pet skin and human hands to 2.2% in human scalp. In humans, the agents were Trichophyton tonsurans and Trichophyton verrucosum, while in pets, a strain of Trichophyton sp was found. Conclusion: Different species of dermatophytes were found in the owner/pet pairs, which denotes that coexistence is not related in asymptomatic cases.

Abstract Background: The objectives of this study were to determine for the first time, in Morocco, the nasal carriage rate, antimicrobial susceptibility profiles and virulence genes of S.aureus isolated from animals and breeders in close contact.Methods: From 2015 to 2016, 421 nasal swab samples were collected from 26 different Livestock areas in Tangier. The antimicrobial susceptibility phenotypes were determined by disk diffusion according to EUCAST 2015. The presence of nuc, mecA, mecC, lukS/F-PV, and tst genes were determined by PCR for all isolates. Results: The overall S. aureus nasal carriage rate was low in animals (9.97%) and high in breeders (60%) with a statistically significant difference, (OR= 13.536; 95% CI = 7.070- 25.912; p< 0.001).In general, S. aureus strains were susceptible to the majority of antibiotics and the highest resistance rates were found against tetracycline (16.7% in animals and 10% in breeders). No MRSA was detected in animals and breeders. A high rate of tst and lukS/F-PV genes has been recovered only, from animals (11.9% and 16.7%, respectively).Conclusion: Despite the lower rate of nasal carriage of S. aureus and the absence of MRSA strains in our study, S. aureus strains harbored a higher frequency of tst and lukS/F-PV of virulence genes, which is associated to an increased risk of infection dissemination in humans. This highlights the need for implementing adequate approaches for prevention.Further larger and multi-center studies are needed to validate and confirm our findings.

Abstract Background: Front-line health workers (FLWs) are always at a very high risk of being infected by COVID 19 due to their close contact with patients; thus, proper protection is critical. Concerning the FLWs' safety, Bangladesh's haphazard health system preparedness response on the eve of the pandemic triggered an uncoordinated management crisis, making the FLWs even more vulnerable. This study aims to elicit FLWs' perceptions and experiences regarding personal safety against COVID-19 while attending suspected/confirmed cases in the pandemic's early days. Methods: A mixed-methods quick telephone survey of a cross-sectional design was conducted during April and May 2020. 60 FLWs of various categories and levels from 14 districts and 43 institutions participated in the study in two rounds (initial and follow up) by four weeks apart. The participants were chosen using a combination of purposive sampling and the snowball sampling method and responded to a semi-structured questionnaire. Descriptive analysis was done for quantitative data, and qualitative data were thematically analysed.Results: The FLWs were obvious about the necessity of using Personal Protective Equipment (PPEs) for suspected/confirmed COVID-19 cases with suspected/asymptomatic patients. All types and levels of FLWs did not get the needed formal training on COVID-19 management and prevention. During the initial round, only 75% and 57% FLWs received PPEs and training for their proper use, respectively. Concerns about spreading the infection to family members precipitated mental health problems for the FLWs, so they recommended some safety measures, e.g. separate accommodation, transport, modified duty roaster during COVID placement. After almost a month, the follow-up round showed that a total of 76% FLWs received PPEs, but only 20% received training about them during the last month. The recommended measures were found to be more beneficial for physicians than the other FLWs. Conclusion: COVID 19 highlighted the shortcomings of Bangladesh's health systems and the low priority given to public health. This hampered the COVID 19 response, which was late, fragmented, and poorly coordinated, putting the FLWs at risk even more. Nonetheless, a coordinated, integrated approach involving all relevant authorities is required, and the health ministry should direct an efficient and inclusive COVID response.

To date, data on COVID-19-related death cases among physicians from different medical specialties are incomplete and scattered. In Italy, available data highlight that general practitioners (GPs) are, apparently, the most heavily affected group. Indeed, they currently represent 44.1% of the total COVID-19 related death cases occurred among physicians, whereas they constitute about 15% of the total number of doctors. This high proportion is most likely the consequence of a work-related contagion happening especially during the first weeks of the epidemic, and persisting also in the following weeks, after the national lockdown. There are various reasons for these higher contagion rates: GPs perform a lot of medical examinations daily, usually in close contact with patients. Especially at the beginning of the epidemic, GPs might have had scant information on the specific safety procedures for the prevention of SARS-CoV-2 transmission (e.g., there was limited knowledge on the possibility of contagions deriving from asymptomatic patients) and, moreover, the availability of personal protective equipment was insufficient. Furthermore, the risk of infection is highly increased by the virus’ characteristics, like its survival for several hours/days on different surfaces and its persistence in the air after an aerosolization process, with possibilities to be transmitted over distances longer than two meters. Following these observations, and considering the high cost in term of GPs’ lives, the COVID-19 pandemic will probably revolutionize the approach to patients in general practice. Clear and effective guidelines are absolutely and urgently needed for the refinement of adequate measures to prevent SARS-CoV-2 infections among GPs.

The present paper is a case report of a one-year old nulliparous Alpine Goat belonging to a dairy goat farm in semi-arid region of Brazil. Both glands were naturally infected by α-hemolytic Staphylococcus simulans and evolved similar clinical signs. The mammary glands presented an acute catarrhal mastitis with systemic clinical signs that responded positively to treatment with gentamicin associated with amoxicillin. The present report suggests the importance of the pathogenic potential of non-aureus Staphylococci strains (NAS) as a cause of clinical mastitis also in nulliparous animals. The isolate showed resistance to tetracycline and contained staphylococcal toxin production genes (sec, sec and TSST-1). Moreover, it has been reported that Staphylococcus simulans is an emerging pathogen in humans causing cutaneous and osteoarticular infections, mainly in those in close contact with farm animals. To the best of our knowledge, this is the first report of a clinical mastitis in a nulliparous goat caused by Staphylococcus simulans.

IntroductionA new coronavirus, called Severe Acute Respiratory Syndrome-CoronaVirus-2 (SARS-CoV-2), has emerged from China in late 2019 and has now caused a worldwide pandemic. The impact of COVID-19 has not been described so far in a military setting. We therefore report a case series of infected patients in a recruit school in Switzerland and the herein associated challenges.MethodsRetrospective review of COVID-19 cases among Swiss Armed Forces recruits in the early weeks of SARS-CoV-2 pandemic in the canton of Ticino, the southernmost canton of Switzerland. Positive cases were defined with two positive PCR testing for SARS-CoV-2 from nasopharyngeal swabs. Serological testing was performed with a commercially available kit according to manufacturers’ instructions.ResultsThe first case was likely contaminated while skiing during weekend permission. He became symptomatic 4 days later, tested positive for SARS-CoV-2 and was put into isolation. He showed complete symptom resolution after 48 hours. Quarantine was ordered for all recruits with close contact in the past 2 days, a total of 55 persons out of 140 in the company. Seven out of nine recruits in one particular quarantine room became mildly symptomatic. SARS-CoV-2 PCR was positive in one of them. Seven days after initial diagnosis, the index patient and the other one from the quarantine retested positive for SARS-CoV-2, although they had been completely asymptomatic for over 96 hours. Serological testing revealed positive for both patients. All others showed negative IgM and IgG.ConclusionsYoung healthy recruits often showed a mild course of COVID-19 with rapid symptom decline but were persistent SARS-CoV-2 carriers. This illustrates how asymptomatic patients may be responsible for covert viral transmission. An early and prolonged establishment of isolation and quarantine for patients and close contacts is essential to slow down the spread of SARS-CoV-2, especially in the confined space of a military environment.

Fungi have become an increasingly important cause of peritonitis in patients undergoing continuous ambulatory peritoneal dialysis. The most common cause of fungal peritonitis is Candida. However, in recent years unusual and “nonpathogenic” fungi have been reported as etiologic agents of CAPD-associated peritonitis. We are reporting the first case of CAPD-associated peritonitis caused by Monilia sitophila. This organism had previously been considered to be non-pathogenic, and a troublesome laboratory contaminant. Our patient was successfully managed with intravenous and intraperitoneal amphotericin B, followed by oral itraconazole, without removal of her Tenckhoff catheter.

Abstract Background: COVID-19 is a newly emerging disease caused by a novel coronavirus (SARS-CoV-2), which spread globally in early 2020. Asymptomatic carriers of the virus contribute to the propagation of this disease, and the existence of asymptomatic infection has caused widespread fear and concern in the control of this pandemic.Methods: In this study, we investigated the origin and transmission route of SARS-CoV-2 in Anhui’s two clusters, analyzed the role and infectiousness of asymptomatic patients in disease transmission, and characterized the complete spike gene sequences in the Anhui strains.Results: We conducted an epidemiological investigation of two clusters caused by asymptomatic infections sequenced the spike gene of viruses isolated from 12 patients. All cases of the two clusters we investigated had clear contact histories, both from Wuhan, Hubei province. The viruses isolated from two outbreaks in Anhui were found to show a genetically close link to the virus from Wuhan. In addition, new single nucleotide variations were discovered in the spike gene.Conclusions: Both clusters may have resulted from close contact and droplet-spreading and asymptomatic infections were identified as the initial cause. We also analyzed the infectiousness of asymptomatic cases and the challenges to the current epidemic to provided information for the development of control strategies.

Since the WHO has declared the Covid 19 pandemic, number of cases is on rise but not attained peak in Pakistan. Although people are affected by COVID-19 throughout the world but the impact of, the disease is different in various countries, which may be attributed to different factors. Scientifically it is point to ponder that though the official figures are in thousands but there may be many asymptomatic patients in our communities due to closed family living systems in Pakistan. There are more than 12 to 13 people living in single small apartments in Karachi, who are in so close contact with each other and the local transmission cannot be avoided in them. However, the positive thing is that the severity of disease and number of deaths in Pakistan are not as much as in developed countries. There could be a number of reasons, which are needed to be explored by researchers in Pakistan and world over.
