Introduction: Several studies suggest increasing rates of antibiotic resistance among adult populations with Urinary tract infections (UTI). Escherichia coli (E. coli), is the predominant bacterium both in the community and in hospital environments causing uropathogenic infections. This study aimed to estimate the common uropathogen bacteria that cause UTI among outpatients as well as to determine the antibiotic resistance pattern of E. coli isolates among outpatients with UTI infections at Shaafi hospital, Mogadishu, Somalia. Methodology: A cross-sectional study was conducted at Shaafi Hospital, Mogadishu, Somalia. A total of 128 samples were collected from outpatients suspected of UTI and tested through bacteriological investigations and antimicrobial susceptibility tests following the Kirby-Bauer agar disc diffusion method. Results: E. coli was isolated in 34 (41%) out of the total 83 samples that showed growth followed by Staphylococcus aureus 22 (26.5%), Pseudomonas aeruginosa, 13 (15.7%), Klebsiella pneumoniae 8 (9.6 %) and Proteus spp. 6 (7.2%). E. coli was highly sensitive to nitrofurantoin, 29 (85.3%), and ciprofloxacin (n = 23, 67.6%), and had the highest resistance rate of ceftriaxone, 33 (97.1%). The odds of having UTI were higher in patients with a history of UTI (Odds ratio OR = 0.211, 95% confidence interval CI: 0.080, 0.553) and history of antibiotic use (OR = 0.322, 95% CI: 0.113, 0.917). Increased resistance rate of E. coli against cephalosporins could be due to its excessive use as empirical therapy. Conclusion: The study indicates that outpatients with UTI could be at high risk of antibiotic resistance, suggesting regular surveillance and monitoring of antibiotics.

The aim of this study was investigating the drug resistance patterns of bacteria isolated from pregnant women with urinary tract infections (UTIs). This study included 1140 pregnant women attending two of main hospitals in Baghdad, Iraq, between September 2016 and August 2017. The isolation and identification of uropathogenic bacteria conducted by standard microbiological methods and Vitek2 system. The disc diffusion test was used to determine the antimicrobial resistance patterns of the isolated bacteria. The present study revealed that three hundred and fifty six isolates were positive for significant bacterial growth. Escherichia coli were the predominant bacteria (34 %) followed by Staphylococcus aureus (22.2 %), Klebsiella spp. (14.6 %), non-coagulase Staphylococcus (11.5 %), Proteus spp. (4.5 %), Pseudomonas spp. (3.7 %), Acinetobacter spp. (2.8 %), Citrobacter (2.8 %), Enterococcus (2.5 %) and Enterobacter (1.4 %). High resistance to Ampicillin (85.6 %), Co-trimoxazole (72.2 %) and Tetracycline (71.3 %) was observed. Also, It was found a moderate resistance to Ceftazidime, Ciprofloxacin, Amoxicillin-clavulanic acid and Ceftriaxone. Imipenem was the most active antibiotic against isolated uropathogens. Among the 356 uropathogenic isolates, 196 (55 %) were from women in the 21 to 30 years old age group, and this rate was higher than other age groups. In conclusion, urinary tract infections are frequent among pregnant women. Early detection and continuous monitoring of antibiotic resistance pattern is very important to determine the appropriate treatment of UTIs among pregnant women without complications.

Pseudomonas aeruginosa is one of the most important opportunistic pathogens responsible for various types of hospital infections. High prevalence of antibiotic resistance in P. aeruginosa strains of human clinical samples cause more severe diseases for a longer period of time. The current research was done in order to study the distribution of blaIMP-1 gene among the imipenem-resistant P. aeruginosa strains isolated from burn and urinary tract infections of hospitalized patients. Two-hundred and forty-three P. aeruginosa isolates recovered from the cases of burn and urinary tract infections of inpatients and outpatients were analysis for antibiotic resistance pattern using the disk diffusion method. Then, imipenem-resistant isolates were further analyzed for distribution of blaIMP-1 gene using the PCR. Of 243 P. aeruginosa isolates, 146 strains (60.08%) were taken from outpatients and 97 strains (39.91%) were taken from inpatients. P. aeruginosa isolates harbored the highest levels of resistance against streptomycin (100%), nalidixic acid (100%), aztreonam (100%), cotrimoxazole (95.47%), ciprofloxacin (88.47%), cefotaxime (84.36%) and gentamycin (83.95%). Inpatients had a relatively higher levels of antibiotic resistance. One-hundred and twenty-one out of 126 (96.03%) imipenem-resistant P. aeruginosa isolates harbored the blaIMP-1 gene. Inpatients also had a relatively higher prevalence of blaIMP-1 gene. High prevalence of blaIMP-1 gene and also imipenemresistant P. aeruginosa are important public health issue. Clinical laboratories should consider the detection of the blaIMP-1 gene among the P. aeruginosa isolates of clinical samples.
