Abstract Background Pharmacists are often the first healthcare professionals that patients contact with their illnesses and requests for medical information, which is enhanced following the recent COVID-19 pandemic. Community pharmacists are expected and required to possess a broad spectrum of knowledge and skills. Self-assessment of these competencies is needed for their self-improvement. Purpose of the study To assess pharmacists’ clinical knowledge and practice in the safe use of contraceptives, and to compare the scores obtained by external observation with pharmacists’ self-assessment of their knowledge as well as investigate the significance of preceptorship experiences. Contraceptives was chosen as the subject area in view of high rates of abortions as a means of contraception in Bosnia and Herzegovina. Methods A questionnaire approach was used. The questionnaire included the following: the first domain contained two case scenarios (safe use of contraceptives), which evaluated clinical knowledge, a second domain in which pharmacists self-assessed their knowledge to resolve cases from the first domain and a third domain that measured the demographics of pharmacists (including experience in preceptorship). Dispensing practice was evaluated in the second domain. The questionnaires were distributed to a convenient sample of 100 pharmacists at the Annual Meeting of Bosnia and Herzegovina Pharmacists. The results were presented as counts (%). The groups (preceptors and non-preceptors) were compared using Mann-Whitney U test, paired assessments were analyzed by Wilcoxon signed-rank test and Spearman’s correlation was used to assess the correlation between variables. Results Of the 100 pharmacists invited to participate, 84 completed the questionnaire (84 % response rate). There was no agreement between pharmacists’ real knowledge (average score - case 1: 2.71, case 2: 3.3) and their self-assessment (average score - case 1: 3.77, case 2: 3.91). There was no statistically significant difference in the actual knowledge of pharmacists (experienced/non-experienced in precepting), while the difference in the self-assessment was significant between these two groups. Conclusion Pharmacists appear to overrate themselves, which leads to self-enhancement bias, in which the experience in precepting has some influence. Pharmacists’ capability in performing an objective self-assessment of their clinical knowledge needs to be carefully studied in the future to fully benefit patients.

AimsThis project aimed to improve the knowledge and confidence of doctors at all levels when managing patients with eating disorders while on call.BackgroundA recent survey found just 1% of doctors have the opportunity for clinical experience on eating disorders. Anecdotally, a number of junior doctors within our trust had mentioned that they felt unsure when asked to manage patients with eating disorders during their out of hours shifts.MethodThis project aimed to ascertain levels of confidence with managing patients with eating disorders, and to collect suggestions to improve this. This was achieved using a survey sent out to 97 doctors working in a Mental Health Trust. We then utilised two of the suggestions to improve the identified areas of concern. The first method involved direct lectures. This was followed up with the creation of a poster highlighting the pertinent information which was displayed in key clinical areas. The second avenue was the creation of an information booklet covering key clinical information that is available to all on call doctors.ResultThe response rate for the survey was 37.11%. The survey found that doctors lacked confidence in the management of common conditions that arise in patients admitted with eating disorders. Refeeding syndrome was identified as the greatest area of concern by responding doctors. To assess the impact of the lectures, MCQs were given out before and after the presentation. The results were compared, and showed a clear improvement in overall knowledge, with results going from an average score of 56.6% to 80%.ConclusionBy using multiple methods to improve doctors confidence, (lectures, written information and visual posters), this quality improvement project achieved its aims in improving doctors knowledge, and through having easy access to important information, will have long term positive effects on patient care.

Abstract Introduction: Intervention of pharmacists in medication adherence can meaningfully contribute to achieving therapeutic outcomes. Exploring the real-life readiness and opportunities of pharmacists may result in the adoption of measures, which could be seen through improvement of patients’ adherence to pharmacotherapy. Aim: The aim of the paper was to make a survey on community pharmacists’ potential in medication adherence support in its connectivity to technical and personnel factors, which underline the capacities of pharmacies in dealing with medication adherence. Methods: The questionnaire survey was conducted from October to December 2014 and involved 158 pharmacists from 117 Czech (CZ) and 41 Slovak (SK) community pharmacies. The structured questionnaire surveyed both technical and personnel factors, including provision of consultancy services related to medication adherence. Non-adherence risk reduction was evaluated by adopting Morisky Scale modified from the pharmacist’s perspective. Questionnaires outcomes were summarised in contingency tables and analyzed for associations between respective categorical variables using χ2 or exact tests and association coefficients. All results are reported as significant at P≤0.05. Results: The average score of adherence support (CZ/SK 1.95/1.93) was significantly higher as compared to that of persistence or concordance (P<0.001). Reduction of non-adherence risk reached the score of a medium degree (P=0.73, average 2.29 in CZ and 2.22 in SK). These findings were significantly associated with personnel capacities (provision of consultancy, preference for the use of recommended procedures in CZ (P<0.001), number of years of practice in SK (P=0.029)), while significant association with technical equipment (consultancy room) in the SK (P=0.037). Conclusion: The pharmaceutical care is developing towards the improvement of medication adherence in both countries - assuming a medium degree of adherence support. Further progress may be observed in strengthening the pharmacists’ personnel capacities, and accelerated mainly using information technologies, i.e. through technical capacities.

Closed process groups (15 sessions) cofacilitated by experienced post-MSW female social workers within a generalist problem-solving framework were offered to 115 survivors of childhood sexual abuse. This quasi-experimental study compared group work intervention with a waiting-list comparison group on measures of depression and self-esteem. Survivors who completed the group work intervention (86.6%) were significantly less depressed and had significantly improved self-esteem as compared with their wait-listed counterparts. Consistent across the three outcome measures at immediate posttest, nearly three quarters of the intervention group members' scores were less than the waiting-list group's average score. At 6-month follow-up (75.6% completed), the positive effects tended to be even larger.
