Obstructive jaundice (OJ) or blockage of the bile duct code K83.1 (according to ICD 10), occurs in approximately 45-50% of cases of all varieties of jaundice, it can be both non-tumor and tumor genesis. The functional pathway plays a special role in the genesis of complications of breast the activity of neutrophils as key effector cells responsible for the development of the inflammatory process in the breast. Investigation of the metabolic mechanisms of the functioning of neutrophils allows us to identify intracellular targets, when exposed to It was possible to modulate the level of cell reactivity.The study used data from 47 men with obstructive jaundice of non-tumor origin and 45 men with obstructive jaundice of tumor origin (stage I-II of the tumor process). As a control, data from 100 practically healthy men were used. A pronounced change in the kinetics of the chemiluminescent response of neutrophils in men with obstructive jaundice was found, consisting in an increase in the time to reach the maximum intensity, maximum intensity, area under the curve and activation index for both spontaneous and luminol-dependent induced chemiluminescence. The development of the tumor process in this category of patients was accompanied by a decrease in the area parameter under the curve during spontaneous and induced reactions, time to maximum, intensity maximum and activation index during spontaneous chemiluminescence. The data obtained indicate a marked increase in the values of the functional activity of neutrophils in patients with obstructive jaundice of benign origin, as well as a sharp decrease in their values in the presence of a pathological process of malignant origin.

Background Most published literature using SELDI-TOF has used traditional techniques in Spectral Analysis such as Fourier transforms and wavelets for denoising. Most of these publications also compare spectra using their most prominent feature, ie, peaks or local maximums. Methods The maximum intensity value within each window of differentiable m/z values was used to represent the intensity level in that window. We also calculated the ‘Area under the Curve’ (AUC) spanned by each window. Results Keeping everything else constant, such as pre-processing of the data and the classifier used, the AUC performed much better as a metric of comparison than the peaks in two out of three data sets. In the third data set both metrics performed equivalently. Conclusions This study shows that the feature used to compare spectra can have an impact on the results of a study attempting to identify biomarkers using SELDI TOF data.

The purpose of research. Predicting risk of infection of acute destructive pancreatitis. Material and Methods. The dynamics of functional activity of granulocytes in 27 (58,7%) patients with sterile and 19 (41,3%) patients with infected pancreatic necrosis. An objective assessment of the severity of their condition carried out by performed using of the scale of the integrated assessment APPACHE-II. The exclusion criterion from the study was the revealing of accompanying acute and chronic diseases. Results.Scale APPACHE-II can be used mainly for screening assessment of the degree of polyorgan insufficiency, as well as predicting the development of infected pancreatic necrosis forms, but, unfortunately, only in the later stages of development of pathological process. Dynamics of changes in the functional activity of leukocytes in patients with pancreatic necrosis can be used as an additional test, which characterizes the moment of transition of sterile pancreatic necrosis to infected, and, consequently, in deciding on the timing of terms and nature of performance of operational allowance. Conclusions. 1. In patients with sterile pancreatic necrosis (SPN) after a moderate decrease in the activity of myeloperoxidase (MPO), the content of the cation proteins (CP) in the first three days in subsequent periods of observation, on the background of complex conservative therapy, noted, compared with the established "Normal" gradual increase or stabilization at close to the indicators of the functional activity of neutrophils. The activity of KF and alkaline phosphatase from patients with SPN, after the increase in performance in the first 11 days, then gradually declines, reaching the normal level. 2. Patients with infected pancreatic necrosis(IPN) sharply, starting with 11 days of the study, by 35-40%, reduce the activity of MPO and the content of CB and Acid Phosphatase activity and Alkaline Phosphatase, on the contrary, it had progressively pronounced growth after 11 days by 35-40%.

Acute toxic hepatitis was caused by intraperitoneal administration of carbon tetrachloride in vaseline oil to white rats at the rate of 0.4 ml per 100 g of body weight for 3 days once. As a result, the animals had a violation of the cytoplasmic membranes of hepatocytes, which was accompanied by an increase in serum transamination enzymes and alkaline phosphatase, a sharp decrease in glu-cose and total protein. The use of hypoxen stopped this pathological process. After using the drug, the activity of transamination enzymes and alkaline phosphatase in the blood serum of animals de-creased to physiological values, the amount of protein and glucose increased, and the physiological state of white rats improved. Thus, hypoxen can be used in animals as a hepatoprotective agent at a dose of 50.0 mg/kg of body weight.

The study involved 63 children with granulating periodontitis of permanent teeth, aged 12-1 and determined the activity of liver enzymes: arginase, cholinesterase, aspartate aminotransferase (AST) and alanine aminotransferase (ALT) of serum. A marked increased activity of arginase and decrease in cholinesterase activity in case of chronic granulating apical periodontitis of permanent teeth and in case of the exacerbation of this pathological process have been revealed. It was also found a significant increase of ALT and AST parameters in case of the exacerbation of disease. The results obtained suggest that periodontal pathology affects the functional activity of the liver cells.


This paper has studied the liver`s functional activity in acute epiglottitis (AE) using such biochemical indicators as: Weltmann test, the activity of asparagine transferase and alanine transferase, thymol test, and alkaline phosphatase. The pathological process, which involves the epiglottis, aryepiglottic folds and surrounding tissues, changes the main clinical and biochemical blood and urine values in patients with acute epiglottitis. 86 patients with epiglottitis aged 20 to 81 years old stayed at the CCH No. 30 in of Kharkiv (clinical site of the ENT-department of Kharkiv Medical Academy of Postgraduate Education) were the subject of our study. It has been found that almost all patients with AE have elevated values even a month after recovery. De Ritis ratio, in particular AST/ALT ratio should be paid special attention. Even after 30 days of recovery, De Ritis ratio remained high, 32.25% higher if compared to healthy people. The Weltmann test was increased by 12.8%, being associated with an increased percentage content of α1- and α2-globulins in the serum; the test tends to increase in the case of acute inflammatory conditions. The highest activity of LF was determined in patients with abscessed form. After 30 days of the occurrence of the disease, the activity of LF remained increased by 84.3% (infiltrative form) and by 52.9% in the case of abscessed form. An increased LF activity in the blood is a sing of damaged hepatocytes under conditions of endogenous intoxication. There was found a disorder of protein-forming and enzyme-forming functions of the liver (according to the liver tests) in patients with acute epiglottitis that requires a long-term therapy with the use of medications aimed at stabilizing the structure and function of the liver.

Abstract Background Sarcopenia, an age-related disease, has been implicated as both a cause and consequence of type 2 diabetes mellitus (T2DM) and a symbol of poor prognosis in older adults with T2DM. Therefore, early detection and effective treatment of sarcopenia are particularly important in older adults with T2DM. We aimed to investigate the prevalence of sarcopenia in Chinese older T2DM patients and explore whether homocysteine and inflammatory indexes could serve as biomarkers and participate in the development process of sarcopenia. Methods T2DM patients aged over 60 years were consecutively recruited from the ward of department of Endocrinology, Xuanwu Hospital between April 2017 and April 2019. Sarcopenia was defined based on the standard of the Asian Working Group of Sarcopenia, including muscle mass, grip strength and gait speed. Logistic regression was used to explore the association between biochemical indicators and sarcopenia. Receiver operating characteristic (ROC) curves were applied to determine the diagnostic effect of these clinical indicators. Results Totally 582 older adults with T2DM were characterized and analyzed in the study. Approximately 8.9% of the older T2DM patients had sarcopenia. After adjusting for age, sex, body mass index (BMI) and hemoglobin A1c (HbA1c), increased concentrations of homocysteine [odds ratio (OR): 2.829; 95% confidence interval (CI), 1.064–7.525] and high-sensitive C-reactive protein (hs-CRP) (OR: 1.021; 95% CI, 1.001–1.042) were independent predictors of sarcopenia; but not interleukin-6. The combination of age, sex, BMI and HbA1c provided a discriminatory effect of sarcopenia with an area under the curve (AUC) of 0.856, when homocysteine was added to the model, the value of the ROC curve was further improved, with an AUC of 0.861. Conclusion In the current study, we demonstrated a positive correlation of homocysteine, hs-CRP with sarcopenia in older adults with T2DM and the relationship remained significant even after adjustment for HbA1c. These biomarkers (homocysteine and hs-CRP) may play important roles in the pathological process of sarcopenia.

The development of the tumor process involves the involvement of various functional elements of innate and adaptive immunity. A special role in the reorganization of tissues, changes and evolution of the properties of cancer cells is given to cytokines, the study of the role of which in these processes with various types of neoplasms and, in particular, with nonmetastatic gastric cancer is an urgent task. The aim of the work was to determine the characteristics of the cytokine status in the tissues of patients with non-metastatic gastric cancer. Materials and methods. In the extracts of tumor tissue, perifocal zone, omentum, peritoneum of patients with gastric cancer without metatstases (T3-4aN0-3M0, n = 20, 58.9 ± 9.7 years), the concentration of IL-1P, IL-2, IL -6, IL-8, IL-18, TNF-a, IL-10 and IL-1RA in tumor tissue, the perifocal zone, as well as in the omentum and peritoneum. Results and discussion. In the tumor tissue, an increase in the content of IL-1P, IL-8 and a decrease in IL-2, TNF-a and IL-10 were noted. In the perifocal zone, a decrease in the concentration of IL-1P, IL-6, IL-8 was revealed with an increased content of IL-2, IL-1RA, IL-10, TNF-a and IL-18. Against this background, the omentum and peritoneum compared with the tumor, the concentration of IL-1P, IL-18 and IL-1RA is reduced, against the background of an increase in the concentration of IL-2, IL-6, TNF-a and IL-10. Findings. It was shown that tumor tissue has some features in the cytokine profile compared with unaffected tissues, which can be used to further evaluate the development of the pathological process.

The infusion of yerba mate (YM) Ilex paraguariensis, with its typical bitterness, is traditionally consumed in South America as a mild stimulant beverage. Two types of YM, with sticks (YM-S) and without sticks (YM-L) and three brands for each type were evaluated for caffeine content and the time course of bitterness. The chemical analysis of the six YM infusions at 5% w/v made by high performance liquid chromatography (HPLC) showed that caffeine levels were higher for YM-L brands. The kinetic study of YM bitterness assessed by time-intensity (TI) curves revealed that the presence of sticks lowered maximum intensity, total duration and area under the curve of bitter perception.

The aim of this study was to compare the performance of a deep-learning convolutional neural network (Faster R-CNN) model to detect imaging findings suggestive of idiopathic Parkinson’s disease (PD) based on [18F]FP-CIT PET maximum intensity projection (MIP) images versus that of nuclear medicine (NM) physicians. The anteroposterior MIP images of the [18F]FP-CIT PET scan of 527 patients were classified as having PD (139 images) or non-PD (388 images) patterns according to the final diagnosis. Non-PD patterns were classified as overall-normal (ONL, 365 images) and vascular parkinsonism with definite defects or prominently decreased dopamine transporter binding (dVP, 23 images) patterns. Faster R-CNN was trained on 120 PD, 320 ONL, and 16 dVP pattern images and tested on the 19 PD, 45 ONL, and seven dVP patterns images. The performance of the Faster R-CNN and three NM physicians was assessed using receiver operating characteristics curve analysis. The difference in performance was assessed using Cochran’s Q test, and the inter-rater reliability was calculated. Faster R-CNN showed high accuracy in differentiating PD from non-PD patterns and also from dVP patterns, with results comparable to those of NM physicians. There were no significant differences in the area under the curve and performance. The inter-rater reliability among Faster R-CNN and NM physicians showed substantial to almost perfect agreement. The deep-learning model accurately differentiated PD from non-PD patterns on MIP images of [18F]FP-CIT PET, and its performance was comparable to that of NM physicians.
