Counseling for the Catastrophically Disabled: A Critical Need in the Medical Model

The intent of this article is to create greater awareness of a continuing problem in the medical rehabilitation of the catastrophically disabled. That problem is a need for counseling throughout the rehabilitation program. It is noted that without this element in the medical model there are numerous problems related to later adjustment, such as socialization, and sexual functioning. The authors comment on these areas of concern, and discuss what might be implemented to facilitate the total rehabilitation process.

The urgency of the problem is due to the significant increase in recent years in the number of young and middle-aged people who have passed the war, the need to develop adequate measures of social assistance and protection. An important task in the organization of the rehabilitation process is to determine the structure of the needs of servicemen with disabilities in various types of medical and social care and the development of individual rehabilitation programs (IRP) based on them. The purpose of the study: to determine the structure of the needs of servicemen with disabilities in medical and social rehabilitation depending on the severity of disability. Statistical data from 25 administrative territories of Ukraine for 2018 are analyzed, the needs of participants of military service with disabilities in medical and social rehabilitation measures, their structure are calculated. The data of the information base of the centers and the bureau of medical and social examination of the regions were used. Processing of the primary material was performed using the universal statistical package "Excel". In 2018, medical and social expert commissions (MSEC) of Ukraine for the first time and re-certified and recognized persons with disabilities 7843 combatants. Of these, disability of group I (IA and IB) was established in 2.6 %, II – in 29.2 %, III – in 68.2 % of cases. All victims for MSEC were formed IRP. The dependence of the needs of combatants in medical and social rehabilitation measures on the severity of disability has been established. For persons with disabilities of groups II and III, priority is given to medical and professional rehabilitation, group I – social rehabilitation and technical means of rehabilitation with medical support. Of the medical rehabilitation services for persons with II and III groups of disability, sanatorium treatment is significant, and group I – rehabilitation therapy. Among vocational rehabilitation services, employment in production conditions is important – for persons with group III disabilities, in specially created conditions – for persons with group II disabilities, at home – for persons with group I disabilities. Vocational training was offered to a small number of people with disabilities of all groups. Among social rehabilitation services and technical means of rehabilitation, the services of social workers of territorial social service centers were significant for representatives of all disability groups, simpler means of transportation for persons of group III disability and more complex means for persons of group I. Determining the characteristics of the needs of servicemen with disabilities depending on its severity allows MSEC specialists to better form the IRP, develop targeted rehabilitation programs at the regional level, assess shortcomings in the organization of the rehabilitation process and build a rehabilitation system for combatants.

The relevance of the problem of children with upper limb trauma medical rehabilitation is determined by the high frequency elbow joint fractures; the risk of complications development, disabilityc of the patient. A comprehensive medical rehabilitation program assumes a personalized approach and a differentiated prescription of physical factors, depending on the time period of rehabilitation. Early physical rehabilitation is a key link in the complete recovery of the function of a patient’s limb with damage to the elbow joint. The purpose of this work is to analyze literature and summarize the results of our own research to determine the optimal approaches and methods for medical rehabilitation of children with upper limb injury. The medical rehabilitation program includes various methods of kinesiotherapy, robotic mechanotherapy, massage, a wide range of apparatus physiotherapy to improve the trophism of the periarticular tissues, to maintain mobility in joints free from immobilization; gain of the full range of motion in the damaged joint; normalizing tone and strengthening the muscles of the upper limb. Medical rehabilitation of children with elbow joint injury is carried out from the earliest stage, in stationary conditions, during the period of immobilization. The rehabilitation measures continue in outpatient and polyclinic conditions, during the entire period of immobilization (2–4 weeks) and then in the post-immobilization period until the limb function is fully restored. It is necessary to monitor the main indicators of the function of the upper limb during the entire period of rehabilitation to assess the effectiveness of rehabilitation measures, objectify the course of the rehabilitation process and the continuity of rehabilitation programs. Definition of the tasks of medical rehabilitation, differentiated for each stage; the choice of modern, pathogenetically grounded rehabilitation technologies with an assessment of their effectiveness contribute to a significant increase in the effectiveness of comprehensive rehabilitation programs for children with upper limb trauma.

Introduction. The problem is actual due to the increasing of cystic fibrosis children quantity: according to neonatal screening, there were 124 children with cystic fibrosis in 2018 (72.9 % from all identified cases for the reporting year). Medical rehabilitation is the main method of the pathogenetic therapy and prevention of cystic fibrosis exacerbation. The new strategy of medical procuring for patients with orphan diseases, which is necessary to develop expert methods for diagnostics of rehabilitation potency and objectify individual rehabilitation programs, gives particular relevance to the problem.The objective was the development of new rehabilitation programs for early aged children with cystic fibrosis, the search of efficiency assessment criteria.Methods and materials. Physical and motor growth, clinical and functional parameters were estimated in 79 early aged children of main and control groups.Results. It was proved that the weight index (absolute and relative), quantity of children with harmonic growth increased due to physical methods of rehabilitation while the quantity of children with acutely disharmonic growth reduced in main group. After applying the developed rehabilitation program, the formed motor skills amounted 83 — 92 % from age norm in the main group of children and 60 — 85 % in the control group. Normotonic reaction to physical activity after the physical rehabilitation program was registered in 73.2 % of the main group and 47.8 % of the control group. In dynamics, the rehabilitation efficiency coefficient amounted (1.68±0.10) in the control group of children and (1.97±0.14) (p<0.05) in the main group that could be corresponded as «improvement». That means the quality of clinical and laboratory shifts, dynamism of physical growth and development rate of motor skills is significantly higher in the main group of children.Conclusion. The possibility of physical rehabilitation use in different rehabilitation programs of early aged children with cystic fibrosis was justified. The rehabilitation efficiency coefficient, the rehabilitation potential were offered, the effectiveness of chosen rehabilitation programs was proved.

In the following review article, the principles and approaches of medical rehabilitation in patients with implanted pacemakers are discussed. It was shown that total benefit results from cooperation among medical, physical and psychological components of rehabilitation program and optimisation of the mode and parameters of cardiac pacing. Social adaptation of the patients with implantable pacemakers is of paramount importance and should be directed on avoidance of external electromagnetic fields and their influence on pacemaker functions, which in turn may be life-threatening for patient and the staff as well. The indications to diagnostic and treatment regimens including physiotherapeutic procedures should be clearly defined in order to avoid potential influence of electromagnetic interference on cardiac pacemaker. The well-structured medical rehabilitation programs need to be organised in order to improve quality of life and patients survival with regard to growing tendency of implantation of cardiac pacemakers.

Stroke is a clinical syndrome, and one of the leading causes of death and disability, occurring under the influence of a large number of risk factors. The symptoms of a stroke are in correlation with the size, time and location of the lesion. Rehabilitation, which involves the application of conventional and supplementary methods, relates to the assessment of the ability, the level of damage of the affected functions, and an adequately designed rehabilitation program. The use of dance, as a rehabilitation procedure in post-stroke patients, requires listening and active participation of an individual with a specific motor reaction to the stimulus. The primary aim of this article is to demonstrate the importance and effects of the application of dance in the rehabilitation process in post-stroke patients, with a special emphasis on the process of improving motor skills and functions. In accordance with the set criteria and purpose of study, scientific research papers were collected and analysed from the PubMed/MEDLINE, Science Direct and Oxford Academic databases, all published in the period between 2010 and 2019, and all indicating the importance of the application of dance when treating post-stroke conditions. The obtained results suggest that dance, which is basically a sports-recreational activity, when applied for therapeutic purposes in persons after stroke, contributes to the development and recovery of motor abilities, balance, mobility, endurance, coordination, motor skills, stance, walking and precision. Also, dancing is of great importance for the improvement of the functioning of the whole organism, which directly or indirectly affects the recovery of motor functions and the overall welfare of an individual.

The article describes the experience of medical rehabilitation of patients after a complex treatment of cerebellar medulloblastoma with metastases in the spinal cord membranes and substance. Substantiates the role of medical rehabilitation funds in correcting the effects of the disease, and cancer treatment, the essence of which is determined by the damage to the structure and function of various organs and systems. The most common effects of the three-treating medulloblastoma are postoperative deficits: ataxia and paresis; radiation myelopathy, polyneuropathy chemotherapy-induced, deformations of the skeletal system, reduced functional performance and adaptation. Actualized value of interdisciplinary interaction: oncologists, pediatricians, neurologists, orthopedists, medical rehabilitation specialists; phasing and integrated use of all available methods of rehabilitation. Presented personalized rehabilitation program to study techniques of individual lessons therapeutic exercises, therapeutic swimming, gidrokinezioterapii, procedures, motor praxis, training paretic muscles of the selected feedback, massage. It is shown that multimodal rehabilitation treatment of patients at high risk of being in remission, is effective in its action in assessing both subjective: improvement of health, emotional tone, sleep, increased appetite, and objective data: normalization of body weight, improving coordination and increasing the range of motion, increase muscle strength, strength endurance muscle corset. Currently, medical rehabilitation is an essential component of cancer patients routing.

The novel coronavirus infection (COVID-19), which quickly became a pandemic in 2020, has presented new challenges for healthcare system. COVID-19, being a contagious infectious disease predominantly affects respiratory system, causes cardiovascular, neurocognitive, renal, gastrointestinal, hepatic, metabolic and mental damages, which is in fact a multisystem disease. Patients with COVID-19, primarily with moderate and severe forms, need appropriate rehabilitation. This article establishes need of development of rehabilitation programs for COVID-19 patients, identifies their problems that leads to restrictions on everyday life, self-service, mobility, communication, interpersonal relationships, and professional activities. Goals and general principles of medical rehabilitation in context of a pandemic at all three stages are presented. Features of rehabilitation approaches are highlighted and important components of individual rehabilitation program for COVID-19 patients are highlighted.

The Social Insurance Institution (ZUS) has been implementing a medical rehabilitation program as part of disability prevention since 1996. The purpose of medical rehabilitation is to improve the health condition of the insured in terms of regaining or maintaining the ability to work. Medical rehabilitation is carried out in those diseases that most often cause inability to work, and thus generate the greatest costs for the social insurance system. Currently, ZUS is referring to medical rehabilitation in: diseases of the musculoskeletal system, cardiovascular diseases, respiratory system diseases, psychosomatic diseases, people after treatment of breast cancer, voice organ diseases, people who, during an accident, in particular an accident at work, suffered an injury to the musculoskeletal system , diseases of the central nervous system. Its role is limited to supplementing the therapeutic process with medical measures aimed at improving the body’s efficiency, enabling the insured person or pensioner to return to employment. Hence, the subjective scope of its activities covers the insured at risk of total or partial incapacity for work and who are promising to start gainful employment as a result of its completion. The concept of incapacity for work means that a person who has lost the ability to work completely or partially due to a violation of the body’s efficiency is incapable of work and does not expect to regain the ability to work after retraining. A person who has lost the ability to perform any work is completely incapable of work. A person partially incapable of work is a person who has largely lost the ability to work in accordance with the level of their qualifications.
