The novel coronavirus infection (COVID-19), which quickly became a pandemic in 2020, has presented new challenges for healthcare system. COVID-19, being a contagious infectious disease predominantly affects respiratory system, causes cardiovascular, neurocognitive, renal, gastrointestinal, hepatic, metabolic and mental damages, which is in fact a multisystem disease. Patients with COVID-19, primarily with moderate and severe forms, need appropriate rehabilitation. This article establishes need of development of rehabilitation programs for COVID-19 patients, identifies their problems that leads to restrictions on everyday life, self-service, mobility, communication, interpersonal relationships, and professional activities. Goals and general principles of medical rehabilitation in context of a pandemic at all three stages are presented. Features of rehabilitation approaches are highlighted and important components of individual rehabilitation program for COVID-19 patients are highlighted.

The purpose of the study is to develop a medical rehabilitation program for the patients who have come through a new coronavirus infection in the resort of Pyatigorsk. Material and methods. In the conditions of the health center named after S.M. Kirov in Pyatigorsk, a branch of FSBI NCFSCC FMBA of Russia, there have been observed 52 patients who had the new coronavirus infection COVID-19 of moderate severity, at the age from 18 up to 65 years old. Two groups have been formed by simple randomization: in the main group, the patients received normoxic barotherapy against the background of traditional spa therapy for pulmonary patients; in the control group, the patients had only conventional therapy. Results. The inclusion of normoxic barotherapy in the complex of medical rehabilitation of patients who came through the novel coronavirus infection contributed to the increase of tolerance to exercise and improved blood oxygen saturation in 92,9 % of cases, restoration of impaired functions of bronchopulmonary apparatus in 89,3 % and physical and mental health in 85.7 % of cases, which was significantly higher in comparison to similar indicators in the control group. Conclusion. Medical rehabilitation of patients after the new coronavirus infection in the resort is justified and advisable.

The results of evaluating the effectiveness of the use of kumis treatment in 64 patients who have had pneumonia associated with the novel coronavirus infection COVID-19 are presented. It was found that the use of kumis treatment in addition to the basic complex in 32 patients of the main group has led to an increase in exercise tolerance by 13.6 % (p < 0,05), a decrease in the severity of shortness of breath by 36.3 % (p < 0.05), and contributed to the improvement of the parameters of the function of external respiration with an increase in the vital capacity of the lungs by 14,4 % (p < 0,05), the volume of forced expiration — by 24,5 % (p < 0,05). Against the background of kumis treatment, the patients of the main group showed a significant decrease in the severity of depression and anxiety — by 62.5 % (p < 0.05) and by 62.3 % (p < 0,05), respectively, an improvement in the parameters of «well-being» by 62,2 % (p < 0.05), «activity» — by 37,6 % (p < 0,05), «mood» — by 35,5 % (p < 0,05), compared to the initial values. The dynamics of parameters in the patients of the control group against the background of the basic complex turned out to be less significant in comparison with the main group.

At present, in the context of mass spread of the new coronavirus infection COVID-19 in the territory of the Russian Federation, one of the main directions of further professional education of health workers is the improvement of professional competences (streamlining the existing ones) and acquisition of new knowledge and skills in order to implement measures to prevent and reduce the risks of the spread of the new coronavirus infection COVID-19 in adults and children in the practical activities of medical personnel, necessary for independent professional activities within the existing qualifications.

Patients with a new coronavirus infection (COVID-19) need rehabilitation. Rehabilitation approaches of patients with another pathology also require to be reviewed under the conditions of the SARS-CoV-2 epidemic. It is necessary to develop clinical recommendations for the rehabilitation care delivery in patients with COVID-19 and other diseases during a pandemic. A group of experts analyzed the data of current reviews on rehabilitation with COVID-19, as well as previous studies on rehabilitation in patients with Post-intensive care syndrome (PICS) and acute respiratory distress syndrome of non-coronavirus etiology, and developed thereupon the main positions of clinical recommendations. We discuss the main rehabilitation problems in patients with COVID-19, in particular structures and functions impairments, leading to limitations and restrictions of self-care, mobility, domestic life, communication, interpersonal interactions and relationships, professional activities and those determining care requirement. The article also provides general recommendations on medical rehabilitation management in a pandemic at all three stages, including patients routing, ensuring the infectious safety of medical personnel and patients. The necessary individual rehabilitation program components for patients with COVID-19 at all stages of rehabilitation have been identified, including patients examination, nutritional deficiency correction, restoration of respiratory function, exercise tolerance, muscle strength, self-monitoring and training in new movement conditions, psychoemotional state disorders and cognitive functions, as well as independence in everyday life recovery. Medical rehabilitation during the COVID-19 epidemic should include all rehabilitation care components and helps to optimize vital functions, to prevent complications and to improve the patients life quality.

The article examines the psychological and psychiatric aspects of unusual living conditions caused by the pandemic of the novel coronavirus infection COVID-19. The features of the formation of psychopathological disorders in the population during an unfavorable epidemiological situation are analyzed. Five periods of their development are identified, which were characterized by their specific features depending on the influence of various factors (patterns of development of the infectious process, the nature of its information support, organizational measures taken, etc.). In the study of the psychological and pathopsychological characteristics of the response under conditions of an unfavorable epidemiological situation, it was established that their structure and clinical features had, along with the general patterns (characteristic for each of the periods of the development of the epidemic situation), a number of specific features. In the course of studying the psychological and psychiatric aspects of unusual conditions of existence (forced self-isolation), three groups of people were identified, according to their age and the nature of their professional activities. It was established that the psychological forms of response and the features of psychopathological manifestations in the conditions of an unfavorable epidemiological situation caused by COVID-19 were characterized by heterogeneity and polymorphism and were determined both by the course of the epidemic process and by the influence of various biomedical, socio-psychological, informational, and professional household pathogenic factors. Of particular importance was the fact that the degree of influence of these factors on the mental health of people was largely determined by the gradually increasing asteniс effect of each of them (the “vicious circle”), with the formation of a “favorable asthenic soil” that promotes the development of more marked and persistent mental and psychosomatic disorders.

In December 2019, a unique coronavirus infection, SARS-CoV-2, was first identified in the province of Wuhan in China. Since then, it spread rapidly all over the world and has been responsible for a large number of morbidity and mortality among humans. According to a latest study, Diabetes mellitus, heart diseases, Hypertension etc. are being considered important risk factors for the development of this infection and is also associated with unfavorable outcomes in these patients. There is little evidence concerning the trail back of these patients possibly because of a small number of participants and people who experienced primary composite outcomes (such as admission in the ICU, usage of machine-driven ventilation or even fatality of these patients). Until now, there are no academic findings that have proven independent prognostic value of diabetes on death in the novel Coronavirus patients. However, there are several conjectures linking Diabetes with the impact as well as progression of COVID-19 in these patients. The aim of this review is to acknowledge about the association amongst Diabetes and the novel Coronavirus and the result of the infection in such patients.

A review article is based on current foreign sources. The level of cytokines in the peripheral blood can be increased in many diseases, but in some cases there may be an excess of their normal concentration in tens, hundreds or more times with the development of a peculiar clinical picture, which is based on a systemic inflammatory reaction. In the literature this condition has received the figurative name «cytokine storm», which highlights an extremely violent reaction of the immune system with an unknown (often unfavorable) outcome. Close attention of the scientific world and the public to the problem of extremely high levels of cytokines in the peripheral blood (hypercytokinemia) was drawn due to the high frequency of the cytokine storm in the novel coronavirus infection.

The novel coronavirus infection (SARS-CoV-2), which first appeared in Wuhan, China in December 2019, has been declared a global pandemic by WHO. COVID-19 affects people of all age groups. The disease in children is usually asymptomatic or mild compared to adults, and with a significantly lower death rates. Data on kidney damage in children with COVID-19, as well as the effect of coronavirus infection on the course of diseases of the genitourinary system, are limited, the risks of contracting a new coronavirus infection in children with significant health problems, including those with chronic kidney disease, remain uncertain. The pandemic has affected the activities of surgeons treating diseases of the urinary system in children. Since the prospects for the end of the pandemic are vague, it is necessary to formulate criteria for selecting patients who can and should be provided with routine care in the pandemic. The purpose of this review is to highlight the features of the clinical manifestations and treatment of children with COVID-19, occurring against the background of previous renal pathology or complicating its course.
