Introduction: Over decades, much have been said and done regarding under-five mortality in Ethiopia. The country has been following the lead of sustainable development goals and UNICEF with its transformation plan targets. However, unless the efforts supported by status assessing studies, it might be difficult for the country to progress. Thus, the current study was directed to identify the prevalence and associated factors of under-five mortality in 2019. Methods: According to the study criteria, we extract and cleaned data in STATA v. 15.0. The data then weighted as per the sampling weight, primary sampling unit, and strata before analyzing in STATA 15.0. Data management consisted of descriptive (mean, standard deviation, and proportion or percent) and association statistics. We deliberated binary logistic regression for this analysis and we checked each variable at 0.25 p-values to include in the model. The final p-value to declare association was p <0.05 and AOR with 95% CI was also applied to describe the results. The data source was the Ethiopian Mini Demographic Health Survey (EMDHS) 2019. EMDHS collected the data from 8,885 in a face-to-face manner with a 99% response rate. Results: From 5,527 numbers of weighted women with under-five analysed in this study, the proportion of under-five mortality was 277.23(5.02%). Factors like 2nd birth order 0.52(0.35, 0.79), 3rd-4th 0.49(0.28, 0.84), 1-2 ANC visits 0.24(0.12, 0.49, ANC visit three’ 0.14(0.07, 0.28), ANC visit four and above 0.22(0.14, 0.36), in marriage mother 0.43(0.19, 0.96), ‘1-2 under-five children 0.02(0.011, 0.03), and greater than three under-five children 0.007(0.0007, 0.004) were all negatively associated with under-five mortality rate. Conclusion: To obtain the exalted outcome out of this study, the government might need to increase antenatal care, women education, institutional delivery, and the modern contraceptive methods use through enhanced community mobilization, health education using community health workers, increasing access to essential cares of mothers and children, and the policy commitment for the issues related to family size, birth order, and birth interval.

Abstract Background: Diarrhoea is one of the major contributors to deaths among under-five children in Ethiopia. Studies conducted in different countries showed that rural children are highly affected by diarrhoea than urban children. Thus, the purpose of this study was to identify the socio-demographic, environmental and behavioural associated factors of the occurrence of diarrhoea among under-five children in rural Ethiopia. Methods: Data for the study was drawn from the 2016 Ethiopia Demographic and Health Survey. A total of 8,041 under-five children were included in the study. Data was analysed using SPSS version 23. Binary logistic regression was used for the analysis of the data to assess the association of occurrence of diarrhoea with socio-demographic, environmental and behavioural associated factors among under-five children. Results: Children aged 6-11 months (AOR: 3.5; 95% CI: 2.58-4.87), 12-23 months (AOR: 3.1; 95% CI: 2.33-4.04) and 24-35 months (AOR: 1.7; 95% CI: 1.26-2.34) were significantly associated with diarrhoea. Diarrhoea was also significantly associated with male children (AOR: 1.3; 95% CI: 1.05-1.58), children in Afar region (AOR: 1.92; 95% CI: 1.01-3.64), Somali region (AOR: .42; 95% CI: (.217-.80), Gambela region (AOR: 2.12; 95% CI: 1.18, 3.81), households who shared toilet facilities with other households (AOR: 1.4; 95% CI: 1.09-1.77), fourth birth order (AOR: .1.81; 95% CI: 1.17-2.79), fifth and above birth order (AOR: 1.85; 95% CI: 1.22, 2.81) and the interaction of older mothers with three or more under-five children (AOR: 4.7; 95% CI: 1.64-13.45). Conclusion: The age of a child, sex of a child, region, birth order, toilet facilities shared with other households and the interaction effect of number of under-five children with mother’s current age are identified as associated factors for diarrhoea occurrence among under-five children in rural Ethiopia. The findings carry implications for the need for planning and implementing appropriate prevention strategies that target rural under-five children.

Abstract Background Majority of this world is concerned by malnutrition. Ethiopia is one of the Sub Saharan African countries known by poverty, childhood diseases, high mortality and poor infrastructures and technology. The study aimed to examine differences within individuals and between clusters in nutritional status of under-five children and to identify socioeconomic factors using adequate nutrition of children in Ethiopia. Method: Data was obtained from Ethiopian 2019 Mini Demographic and Health Survey surveyed by Ethiopian Public Health Institute. A weighted sub- sample of 8768 under-five children was drawn from the dataset. Spatial statistics was used to analysis spatial variations of malnutrition of children in clusters of regional areas of Ethiopia. Multilevel modeling was used to look at demographic, socioeconomic factors at individuals and clusters levels. Result At national level the proportion of stunting, underweight and wasting among under-five children were 39.5 percent, 29.8 percent and 15.4 percent respectively. The Global Moran Index’s value for children malnutrition result in Ethiopia was (for stunting I = 0.204, P-value = < 0.0001, for underweight I = 0.195, P-value = < 0.0001 and for wasting I = 0.152, P-value = < 0.0001). Spatial variability of malnutrition of under-five children across the clusters of Ethiopia observed. Result of heterogeneity between clusters obtained was {X}^{2}=147.25, {X}^{2}=211.43 and {X}^{2}=201.43 respectively for stunting, underweight and wasting with P = < 0.0001 providing evidences of variation among regional clusters with respect to the status of nutrition of under-five children. Multilevel model result revealed that high differences of malnutrition in individual households and regional clusters in the under-five children in Ethiopia. Conclusion The model showed that there were spatial variations in malnutrition among clusters in Ethiopia. Child age in month, breast feeding, family educational level, wealth index, place of residence, media access and region were highly significantly associated with childhood malnutrition. Inclusion of explanatory variables in multilevel model has shown that a significant impact on variation in malnutrition among individual households and regional clusters. Accessible resources, promoting education, use media to expand activities regarding nutritional and health services and through health workers and health institutions in Ethiopia is significant.

Introduction Diarrhea is responsible for the death of more than 90% of under-five children in low and lower-middle income countries. Regionally, South Asia and sub-Saharan Africa accounted for 88% of deaths with the same age group. Therefore, the aim of this study was to determine the prevalence and associated factors of diarrhea among children under-five years in sub-Saharan Africa. Methods The appended, most recent demographic and health survey datasets of 34 sub-Saharan African countries were used to determine the prevalence and associated factors of diarrhea among under-five children in the region. A total weighted sample of 330,866 under-five children were included in the study. Both bivariable and multivariable multilevel logistic regression were done to determine the associated factors of diarrhea among under five children in sub-Saharan Africa. The Odds Ratio (OR) with a 95% Confidence Interval (CI) was calculated for those potential factors included in the final model. Result The overall prevalence of diarrhea in this study was 15.3% (95% CI: 15.1–15.4). Those children of mothers aged 15–24 (AOR = 1.26; 95% CI: 1.23, 1.30) and 25–34 years (AOR = 1.15; 95%CI: 1.12, 1.18), those children of mothers with no education (AOR = 1.69; 95%CI: 1.57–1.82), primary education (AOR = 1.73; 95%CI: 1.61–1.86) and secondary education (AOR = 1.49; 95%CI: 1.38–1.59) had higher odds of having diarrhea. Those children from poorest (AOR = 1.14; 95%CI: 1.10, 1.19), poorer (AOR = 1.12; 95%CI: 1.08–1.17), middle (AOR = 1.06; 95%CI: 1.02, 1.10), and richer (AOR = 1.14; 95%CI: 1.04–1.12) households had higher chance of having diarrhea compared to their counterparts. Conclusion This study found that the prevalence of childhood diarrhea morbidity in sub-Saharan Africa was high. Maternal age, wealth index, maternal education, maternal occupation, age of child, time of initiation of breast feeding and time to get water source were significantly associated with diarrhea. Therefore, intervention through health education and health promotion for mothers/caretakers who are poor, less educated, and young should be designed to prevent diarrhea in the region.

A number of discussions on mortality or survival patterns of under-five children in Nigeria have been presented in the literature over years, most of which were characterized by descriptive analysis, in which facts were reported by percentages, ratio and measures of association to mention a few.  In this study, binary logistic regression model was employed to model the survival status (dead or alive) of under-five children in Nigeria as a function of some socio-demographic variables. Results from this study revealed that ten socio-demographic variables among several others were significantly associated with the survivals of under-five children in Nigeria. Specifically, the results showed that children that were born in urban area, that were exclusively breastfed, that were among the first four children in the family, whose mothers have secondary education and post-secondary education have significant increased odds, to about 27%, 580%, 20%, 22% and 102% respectively, of surviving beyond age five than their counterparts in the reference categories of the above identified risk factors (odds ratio is 1.271(p < 0.0001 for urban), 6.810 (p<0.0001 for breastfeeding), 1.197 (p < 0.0001 for birth order), 1.225 (p = 0.001 for secondary education) and 2.023 (p < 0.0001 for higher education)). Results from this work indicated that more enlightenment program is required to stem the alarming increase in under-five mortality rate in Nigeria which currently stood at 112%. Data set from Nigerian Demographic and Health Surveys (NDHS) report for 2008 was employed in this study.

Background: Acute diarrheal diseases are the leading cause of preventable childhood death, especially in developing countries. It is the second leading cause of death in under-five year children nest to pneumonia. Objectives: The aim of this study was to determine the magnitude and associated factors of diarrhea in under-five in Harena Buluk district. Methods: A community based Cross sectional study was conducted in February, 2018. A two stage stratified sampling method was done to select the eligible households. Data was collected by trained data collectors using pretested questionnaire list which was prepared based on EDHS and WHO core questionnaires related to diarrhea. Data was entered in to a computer using Epi data 3.1 and exported to SPSS V.20 for further analysis. Logistic regression was used to determine level of association with 95% CI. A p-value <0.05 in the final model were considered as significant. Result: The two weeks period magnitude of diarrhea among under-five children was 28.4% with 95% CI (14.5-20.8) which was associated with households with one under five children (AOR: 0.268, 95% CI(.08,0.90)), living in the home with single room ((AOR = 6.01, 95% CI(1.01,36.01)), clean latrine/faces not seen around the pit or on the floor of latrine(AOR: 0.298, 95% CI(0.097,0.92)), long time take to fetch water from source (AOR: 0.046,95%CI(0.01,0.22)), home based water treatment ((AOR = 0.15, 95% CI: (0.04, 0.62)), living with animal in the same house(AOR: 8.31, 95% CI(2.46,28.06)), children who took gruel type of food(AOR: 0.24, 95% CI(0.07,0.81)), hand washing practice before cooking food (AOR: 0.195(0.066, 0.574)). Conclusion: The two weeks period magnitude of acute under-five diarrhea was relatively high and number of under five children in the household, cleanness latrine, time taking from the source of water, home based water treatment, number of rooms in the home, live with animal in the same house, type of food child take, hand washing before food preparation had significant association with the occurrence of under-five diarrhea. Improve community about home and environmental sanitation and hand practices were recommended.

Objectives: This study aimed to assess communities’ perception and adoption of the evidenced-based malaria diagnosis and case management intervention targeted at under-five children. The effectiveness of trained Volunteer Community Health Workers (VCHWs) to diagnose malaria among under-five children using rapid diagnostic testing kit, provide treatment using Artemisinin Combination Therapy and rectal Artesunate were assessed.Design: A qualitative evaluation study was conducted in October 2015.Setting: Communities in the 6 rural wards in Ona-Ara Local Government Area, Oyo State Nigeria.Participants: Caregivers of under-five children, community–based frontline health workers, and community leaders selected using purposively sampling.Methods: Nine Focus Group Discussions and 15 Key Informant Interviews were conducted using a pre-tested guide. Data were subjected to thematic analysis.Results: It was disclosed that VCHWs promoted people’s access to prompt and appropriate malaria treatment. The communities accepted the VCHWs; the reasons given for this included the following: effectiveness of VCHWs in case management of malaria; good inter-personal relationship with caregivers; and the positive health outcomes associated with services provided by them. In addition, community members expressed satisfaction with the VCHWs and provided them with all the support needed to function throughout the malaria case management intervention. The VCHWs considered the support as a great source of encouragement.Conclusions: The use of VCHWs to treat malaria was adjudged to be effective and considered acceptable to the communities. The adoption of the intervention and its integration into the primary health system by the government is advocated for in medically underserved rural communities.

Abstract Background The burden of communicable diseases within developing countries is mainly influenced by poor personal hygiene practices. Hand washing is considered as most cost effective intervention for reducing health problems such as diarrhoea and acute respiratory tract infections. This study aimed to assess hand washing practice at critical times and identify associated factors among mothers of under five children in Debark town. Method A community based cross-sectional study design was carried out from May 1–20, 2018 in Debark town. After selection of participants using simple random sampling, face to face interview was performed by using semi-structured pre-tested questionnaire. Data were entered into EPI Info 7 and exported into SPSS 21 for further analysis. Results were presented by simple frequency, percentage and mean for descriptive variables. Binary logistic regression analysis was used to test the association of dependent and independent factors. Variables with 95% confidence interval and p ≤ 0.2 during the univariable binary logistic regression analysis were included in the multivariate logistic regression analysis. At the final model variables with p ≤ 0.05 were treated as significantly associated factors of hand washing practice at critical times. Results Good hand washing practice at critical times was reported in 52.2% (95% CI: 47.5, 57.2%) of study participants. Desirable attitude [AOR = 3.37, 95% CI (2.03, 5.58)], presence of water for washing hands [AOR = 4.86, 95% CI (1.26, 18.69)] and a good knowledge [AOR = 2.98, 95% CI (1.92, 4.60)] were significantly associated factors with hand washing practice at critical times. Conclusion The hand washing practice at critical times of study participants was found to be low. A significant proportion of mothers of under five children have a poor hand washing practice at critical times. It is necessary to increase the access to water and to improve knowledge and attitude of mothers to improve their hand washing practice at critical times.

Abstract Background Malaria during pregnancy and childhood is one of the major public health challenges globally. Its prevalence is huge in Africa, especially in sub-Saharan countries and Ethiopia. Insecticide-treated mosquito net (ITN) use is one of the primary malaria preventive strategies. Previous studies did not adequately address the health belief and behaviour-related correlates of ITN using health belief model (HBM), although a number of studies were conducted in this theme. Therefore, this study was aimed at assessing the prevalence and associated factors of ITN utilization among pregnant women and under five children in east Belessa district, northwest Ethiopia, 2020. Methods A community-based cross-sectional mixed study was conducted in east Belessa district from February 01–30/2020. A total of 724 eligible participants were included in the quantitative study. A multistage cluster sampling technique was used. The quantitative data were collected using an interviewer-administered structured questionnaire. Data were entered into Epi data version 4.6.0.2 and then exported to SPSS version 16 for analysis. The binary logistic regression model was fitted and the level of significance was declared based on AOR with its 95% CI and p-value ≤ 0.05. Meanwhile, the qualitative data were collected using focus group discussions and key informant interviews, and analysed using a thematic analysis approach. Results The prevalence of ITN utilization was 56.5% (95% CI 53.0, 60.2) and independently predicted by a corrugated iron roof of the house (AOR = 1.53; 95% CI 1.15, 2.22), rural residence (AOR = 1.59; 95% CI 1.11,2.28), ≥ 2 number of rooms in the house (AOR = 1.56; 95% CI 1.06, 2.30) and high level of perceived barrier (AOR = 0.53; 95% CI 0.38,0.74). In the qualitative findings, the main barrier was connected to misconceptions and misperception towards malaria and ITN. Conclusion The prevalence of ITN utilization in the study area was lower than the national target (100%). It was significantly associated with household characteristics, residence, and level of a perceived barrier. Reversing the community’s misconceptions through information, education and communication (IEC), and behavioural change communication (BCC) would enhance ITN utilization.

Background: Immunization is the single most costeffective preventive health intervention which saves the lives of 2 -3 million children annually. Theunsatisfactory immunization coverage numbers have unsurprisingly resulted in abysmal child mortality figures across the country.  This study aims to assess mother’s knowledge and immunization utilization among underfives in an urban community. Methods: A descriptive crosssectional study was carried out among 232 mothers of under- five children in an urban community in Lagos State. A multistage sampling technique was used to select the respondents. A semistructured interviewer administered questionnaire was used as the survey tool for data collection and a checklist for sighted immunization cards. The entry and analysis of the data was done using EPI – Info Version 7.2.1.0 software computer program. Chi square was used to test association. A p-value of <0.05 was considered statistically significant. Results: All respondents were aware of immunization. Just over half (53.9%) of the respondents had good knowledge about immunization. Less than half (42.5%) of the under-five children were fully immunized. The vaccines most missed were oral polio and pentavalent vaccines. There was a statistically significant association between knowledge of mothers and the immunization status of their children (p<0.05). Conclusion: The results indicate that mothers with good knowledge of different aspects of immunization were more likely to have fully vaccinated children. It is recommended that health workers enlighten mothers on the benefit of each vaccine. Key words: Immunization, Underfive, Utilization, Vaccination, Knowledge

Increasing prevalence of diarrhoea was also significantly associated with male children (AOR: 1.3; 95% CI: 1.05-1.58); households who shared toilet facilities with other households (AOR: 1.4; 95% CI: 1.09-1.77); fourth birth order (AOR: 1.81; 95% CI: 1.17-2.79), and fifth and above birth order (AOR: 1.85; 95% CI: 1.22, 2.81) as compared to first order; and mother’s current age 35-49 years in a household with ≥3 under-five children (AOR: 4.7; 95% CI: 1.64-13.45) as compared to those maternal ages of 15-24 years in a household with ≤2 under-five children.Conclusion: The age of a child, sex of a child, region, birth order, toilet facilities shared with other households and the interaction effect of the number of under-five children with mother’s current age are identified as associated risk factors for diarrhoea occurrence among under-five children in rural Ethiopia. The findings show the need for planning and implementing appropriate prevention strategies considering these risk factors for rural under-five children.
