MAJOR TRENDS IN INCIDENCE AND PRIMARY DISABILITY DUE TO MALIGNANT NEOPLASMS AMONG ADULT POPULATION OF THE TOMSK REGION FROM 2015 TO 2019

The article presents results of the analysis of indicators of cancer incidence and primary disability due to malignant neoplasms among adult population of the Tomsk region in dynamics for 2015 - 2019, which indicate an upward trend (by 30.7%) exceeding the average indicators for the Russian Federation and the Siberian Federal District. Increased prevalence of malignant neoplasms among adult population of the Tomsk region over the period under study is due to better detection of malignant neoplasms (a 8.7% increase), as well as implementation of the new and improved methods of special anticancer treatment aimed at complete recovery and increased life expectancy of cancer patients. Malignant neoplasms were more often detected among urban population aged 65 and over (64.8%). The incidence rate of malignant neoplasms in the region progressed from 433.8 to 626.1 per 10 000 population due to increased detection of malignant neoplasms at the early stages (by 2.9%). The rate of advanced cases over the study period decreased by 1.5%. Since 2014, malignant neoplasms have been taking the lead in the structure of adults recognized as disabled for the first time. Indicators of primary disability in the adult population for the period 2015 - 2018 decreased, followed by the increase in 2019, however, the disability rate in all adult age groups remained below the similar indicators in Russia and the Siberian Federal District. Women, persons of the retirement age, and urban dwellers prevailed in this contingent of the disabled. The share of people of working age decreased from 38.4% to 35.2%. The share of people of the retirement age increased from 61.6% to 64.8%, respectively. Group II disability was the most often established, in dynamics, the share of the disabled with groups II and III decreased. There was a significant increase from 20.4% to 31.9% in the share of citizens who, during the initial examination, were established as group I. The main localizations of malignant neoplasms resulting in disability in 2015 - 2017 included mammary gland, larynx, trachea, bronchi and lungs, and female genital organs; and stomach and large intestine - in 2018 - 2019. The share of malignant neoplasms with multiple localizations increased from 1.8% to 5.2%.

Our objective was to study indicators and structure of primary disability due to malignant neoplasms among persons of retirement age in 2012–2018 and to determine the patterns of its formation. Material and methods: The continuous research was based on data of the Unified Automated Vertically Integrated Information and Analytical System for Medical and Social Expert Evaluation for 2012–2018, “7-Sobes” statistical reports, and statistical compilations of the Federal Bureau of Medical and Social Expert Evaluation of the Russian Ministry of Labor. The methods applied included data copying, descriptive statistics, analytical and comparative analysis, calculation of the mean error and statistical significance. Results: The study of primary disability rates due to malignant neoplasms among people of retirement age in Moscow in 2012–2018 showed that the number of people first recognized as disabled increased up to 13,827 people (+77.8%). This finding could be related to the increase in the number of people of retirement age by 21.2% in Moscow during the period under review. As for the severity of disability, then people with grade II disability prevailed but their specific weight and the rate of primary disability tended to decrease. At the same time, we observed an increase in the specific weight of disabled people of Grade III. The five-year survival rate of cancer patients increased by 30% from 51% to 65.4% in 2011–2018, and so did the proportion of cases detected in early stages. Conclusions: Disability caused by malignant neoplasms among the elderly in Moscow in 2012–2018 was characterized by an increase in the number of people first recognized as disabled from 7,782 to 13,827 people (+77.8%) and an increase in their specific weight from 59.4% to 61.3% (60.6% on the average) in the structure of those first recognized as disabled from these causes. An increase in the rate of primary disability from 27.7 ± 0.5 to 40.6 ± 1.3, averaging 30.2 ± 0.5, was also observed but it remained lower than that in the Central Federal District and the Russian Federation as a whole.

The analysis presented in the article of the primary disability of the adult population due to diseases of digestive organs in the Rostov region for 20122018 was characterized by an increase in the number of persons recognized as disabled for the first time as in the Southern Federal District and the Russian Federation. In the structure of the first recognized disabled persons, their share was higher than in the Southern Federal District, but lower than in the Russian Federation. The level of primary disability for 20122018 was lower than in the Southern Federal District and the Russian Federation. The age structure was dominated by persons with disabilities of working age (women aged 4554; men 4559 years old) as well as in the Southern Federal and Russian Federation. The severity of disability was dominated by group II persons with disabilities, with a tendency to increase their specific weight, as in the Southern Federal District and the Russian Federation; Group III persons with disabilities ranked second, with a tendency to reduce their share and level. There has been an increase in the proportion of group I disabled persons.

Among socially significant diseases that are characterized by high prevalence rates, malignant neoplasms occupy the leading positions. Thyroid cancer is the most common malignant neoplasm of the endocrine system. Purpose of the study. Analysis of recurrent disability due to thyroid cancer in the adult population in Moscow (2015–2019). Materials and research methods. The structure of repeated disability due to thyroid cancer by sex, age and disability group was studied. The dynamics of the number of the PPI contingent was traced and the level of repeated disability due to this pathology was determined. The number of observations was 4660 PPI. Research results. It was found that in the structure of repeated disability due to thyroid cancer, women predominated by gender. The trend towards an increase in the number of PPI took place in all major age groups of the adult population. The prevalence of PPI with group III disability, the proportion of which was 66.0% (PPI with group II – 32.5%, PPI with group I – 1.5%). The visibility indicator in 2019 in relation to 2015 (taken as 100%) was equal in the general contingent of PPI among disabled people of group I – 142.9%, group II – 374.1%, group III – 249.3% ...

Aim. To distinguish the regularities for primary disability occurrence due to musculoskeletal and connective tissue diseases in Russian Federation and Volga Federal District in 2011-2013. Methods. Data copying, comparative analysis of indicators of primary disability due to musculoskeletal and connective tissue diseases in Russian Federation, and federal districts ranking by level of disability due to musculoskeletal and connective tissue diseases in 2011-2013 were performed. Results. Central Federal District of Russian Federation had a high level of primary disability due musculoskeletal and connective tissue diseases registered in 2011-2013. Southern, North-Caucasian and Volga Federal Districts had low levels of disability due to the same diseases. Among the subjects of the Russian Federation included in the Volga Federal District, high levels of primary disability due to musculoskeletal and connective tissue diseases in 2011-2013 were registered in the Kirov region (17.1-15.3-11.1 per 10 000 of the adult population), low levels of primary disability - in the Saratov region (4.1-3.4-3.5 per 10 000 of the adult population). Conclusion. The trend for reducing the primary disability in the Volga Federal District in 2011-2013 (6.6-6.3-5.9 per 10 000 of the adult population) indicates the success of regional rehabilitation programs for persons with disabilities.

A continuous statistical study of primary disability dynamics in ophthalmic patients during 2006–2011, according to the data from the branch # 7. For the entire observation period, a decrease in number of people first-time recognized as disabled was found. The nosologic structure of primary disability is shown, with glaucoma occupying the first place for the entire observation period, and myopia the second one during the last 5 years. Decreasing disability due to lens changes was found, from the second rank to the 6th one. An increase in primary disability due to AMD was recognized, moving AMD from the 10th place to the 3rd. A group analysis of patients, in whom glaucoma was a disabling condition, was performed. In 88–93% of cases, these persons were of retirement age. Higher prevalence of disability was noted among urban population. Among rural population in comparison with urban one, there were more group I and less group II disabled persons.

The paper presents the materials of the complex epidemiological evaluation of tuberculosis process by its main indicators (incident cases, prevalence and mortality) in the Siberian Federal district over the period of 2002-2014. The ranking of the territories included in the Siberian Federal district was performed by the integral index. That allowed to identify three groups of territories by the level of intensity of TB epidemiological process (favorable, typical, unfavorable). The most favorable epidemiological situation was registered in Tomsk region, the most unfavorable one – in the Republic of Tuva.

The article presents the medical and social characteristics ofprimary disability in the period from 2007 to 2009 and from 2013 to 2015. In the article there are presented medical-social characteristics of the primary disability over the periods from 2007 to 2009 and from 2013 till 2015. There were analyzed extensive and intensive indices of the primary disability which manifest as dynamics characteristics of the public health ofthe adult population of St. Petersburg. There were revealed clear cut downward trends in the number ofpersons for the first time recognized as disabled and the level of the primary disability in a major metropolis. There is established a consistent trend of the increase of specific weight of working-age persons for the first time recognized as disabled in the total number of the disabled adult population. In the compared periods there were revealed trends of increasing of the specific gravity of cases for the first time recognized as disabled at a young age. The use of indices obtained in the study can serve as the basis for the analysis and evaluation of the activities of the Bureau of the Socio-Medical Expertise in the modern conditions.

The article presents results of the study ofproblems of the repeated disability due to diseases of the musculoskeletal system and connective tissue in retirement age citizens of the Russian Federation in the period of from 2005 to 2014. There were analyzed the number of this category of invalids, their specific weight among re-recognized as disabled cases with account of classes of diseases and the age, structure of the repeated disability in groups, the level of repeated disability and ranking places of subjects of the Russian Federation on this index in 2014. The results of the executed analysis indicate that only in the Russian Federation for 10 years (2005-2014) there were re-recognized as disabled due to diseases of the musculoskeletal system and connective tissue 487.1 thousand people from citizens of retirement age with distinct dynamics of the increase as their absolute number (from 22.1 thousand persons in 2005 to 69.3 thousand people in 2014) and the index of disability level (respectively from 7.6 to 20.5 per 10 thousand of the corresponding adult population). This pathology was established to determine a high level of repeated disability in this age group (20.5 per 10 thousand population). Comparative analysis by age revealed that in the Russian Federation in the structure of the repeated disability due to diseases of the musculoskeletal system and connective tissue in the adult population invalids of the retirement age prevail (42.8% in 2014). Along with diseases of the circulatory system (of rank 1) and malignant neoplasms (rank 2), diseases of the musculoskeletal system and connective tissue form the contingent of cases repeatedly recognized as disabled of retirement age (3th rank place). A ranking ofall subjects of the Russian Federation by the level of repeated disability of citizens of retirement age due to diseases of the musculoskeletal system and connective tissue allowed to select 70 ranking places, that with bearing in mind the present demographic situation, has a practical orientation for planning the organization of medical and social care for the given large category of the population.
