During the pandemic of the new coronavirus infection Covid 19, the healthcare system faced serious challenges, one of which is a shortage of qualified personnel. One of the methods for solving this problem was the creation of mobile multidisciplinary mobile teams on the basis of federal medical institutions. In this article, the authors analyze the legal aspects of the functioning of mobile teams, and also give the experience of the employees of the Federal State Budgetary Educational Institution of Higher Education Bashkir State Medical University in the regions of Russia and abroad.

The article analyzes the dynamics of the indicators of target enrollment and education at the Federal State Budget Educational Institution of Higher Education of the Far Eastern State Medical University of the Ministry of Health of Russia for the period from 2013 to 2018, a comparative assessment of the passing score for the places of targeted enrollment and the main places depending on the specialty (training direction) was made, average mark of a unified state exam (USE) was analyzed; the agreements on targeted training with the assessment of measures of social support were analyzed, as well as proposals were formulated to improve the activities of targeted training of medical personnel.

The introduction of lean technologies in the organization of health care is due to the implementation of the priority federal project in the Russian Federation “Creation of a new model of a medical organization providing primary health care”. The work reflects the peculiarities of training in lean production for employees of healthcare organizations in an atypical situation associated with the COVID-19 pandemic. The use of distance technologies in the training center “Process Factory” of the Federal State Budgetary Educational Institution of Higher Education “Kuban State Medical University” of the Ministry of Health of Russia (KubSMU) was a timely response to the changing conditions, within which the information and methodological base of training cycles was promptly adapted. The article discusses the positive and negative aspects of distance learning lean technologies, shows the difficulties faced by both learners and Lean trainers. The article describes the regularities of the transition from the traditional full-time education at the “Process Factory” with the immersion of students in simulated conditions, to the alternative - distance learning, using virtual space and other innovative technologies for conducting classes. It is concluded that distance learning has a number of advantages in improving the qualifications of employees of healthcare institutions in the field of implementation of lean technologies. The methodology of E. Goldratt and the model of E. Kübler-Ross used in the work made it possible to identify the causes of resistance and create a set of measures to overcome resistance to change, implemented in a distance learning format.

To optimize non-drug methods of treatment of patients with irritable bowel syndrome (IBS) against the background of obesity to identify the peculiarities of food preferences and permeability of the intestinal wall.Materials and methods: An anonymous questionnaire survey of 52 IBS patients was carried out using the author’s set of dietary assessment questions “Nutrition Information and Food Behavior” developed at the Department of Hygiene, Human Nutrition of Omsk State Medical University. The study of the level of zonulin in the feces was carried out at the Central Research Laboratory of the Federal State Budgetary Educational Institution of Higher Professional Education Omsk State Medical University of the Ministry of Health of Russia by enzyme immunoassay using the IDK Zonulin ELISA test system (Immundiagnostik, Germany).Results: The eating habits of IBS patients with BMI ≥25 kg/m2 diff er from those of normal weight patients and can be considered a component of the phenotype of this patient cohort. IBS patients with BMI ≥25 kg/m2 have a higher level of intestinal permeability as assessed by zonulin content in feces.

Objectives. To study the impact of participation in the work of medical institutions providing assistance to patients with COVID-19 on the professional self-identification of students. Material and methods. The study was carried out on the basis of the educational institution «VSMU». Inclusion group: medical students of the 4th-6th years who took part in the work of medical teams providing assistance to patients during the period of an epidemic rise in the incidence of respiratory infections, including SARS-CoV-2 coronavirus one. The survey was conducted in the form of an anonymous questionnaire. To segment the respondents according to certain criteria, depending on their answers, the «decision tree» method was used. Results. In the course of the study, it has been found that 1.8 times more often the question «To what extent did you manage to combine your work and studies?» the answer «everything turned out well» was given by the students who indicated that they had a curator who helped them to cope with their professional adaptation difficulties. The higher the self-esteem of the personal level of knowledge and skills was, the more often students chose the answer that they were definitely not going to leave medicine and change their profession. Conclusions. Based on the survey data, one of the most significant factors influencing the adaptation and development of practical skills is the high-quality work of university teaching staff and a physician-curator. An important role in professional adaptability is played by the solution of psychoemotional problems of working students through the establishment of their close interaction with the social, pedagogical and psychological service of the university, professional training centers.

Background. The incidence of sciatic nerve (SN) damage after a total hip replacement (THR) is 10 percent. The underdiagnosis of paucisymptomatic sciatic neuropathy may lead to the unsatisfactory outcome of the treatment in these patients featured by frequent chronic pain syndrome. This research was aimed at the evaluation of the dynamics of clinical and electrophysiological patterns in SN after THR. Material and methods. The research involved 16 individuals 45 to 68 years old with primary idiopathic coxarthrosis who underwent THR in the Scientific Research Institute of Traumatology, Orthopedics and Neurosurgery, Federal State Budgetary Educational Institution of Higher Education V.I. Razumovsky Saratov State Medical University, the Russian Federation Ministry of Healthcare. We compared the findings of clinical and neurologic examinations (VAS, muscle strength, and sensitivity evaluation) as well as ENMG before surgeries and 14 days after them. Results. In the evaluation of the clinical score in 9 patients, we observed some negative changes featured by paresthesia around the area of the SN innervation. The analysis of changes in ENMG findings revealed the decrease in M-response amplitudes of both peroneal and tibial nerves by more than 10 percent of the age-appropriate normal value; this was more prominent in patients who had undergone the lengthening of extremities in more than 3 cm. Conclusion. THR is associated with a higher risk of traction and entrapment changes in SN that lead to the progress of their neuropathies in the post-operative period.

Affliation. Federal state budgetary educational institution of higher education «Kursk state medical university» Ministry of healthcare Russian Federation.Our aim is the relevance of the research topic is conditioned by the wide spreading of the periodontitis among effcient population. The purpose of investigation – the research of the influence of indolicidin analogues N7 and N8 on the content of connective tissue matrix and prooxidant-antioxidant balance in the periodontium in the periodontitis. Methods: periodontitis was simulated according to the method supposed by Volozhin A.I. and Vinogradova S.I. (1990). The animals were withdrawal from the experiment in the 7, 14 and 21 days after the removal of the thread. The content of oxyproline, glycosaminoglycans malonic aldehyde, acylhydroperoxides, catalase activity were determined in the periodontium tissue. Indolicidin analogues N7 and N8 were injected intraperitoneally within 7 days after the removal of the thread.Results. the increase of the concentration of lipid peroxidation metabolites, the decrease of the content of free and bound oxyproline, glycosaminoglycans, catalase activity. Both indolicidin analogues had two-phase influence on the concentration of malonic aldehyde and acylhydroperoxides: the increase on the 7 day and the decrease on the 21 one; increased the concentration of free and bound oxyproline, glycosaminoglycans.Summary. Periodontitis development is accompanied by the decrease of the content of free and bound oxyproline, glycosaminoglycans, lipid peroxidation activation, the drop of catalase activity in the periodontium tissue. The investigated indolicidin analogues had two-phase action on the content of the lipid peroxidation metabolites in the periodontium: the increase on the 7 day and the decrease on the 21 one; increased the concentration of free and bound oxyproline, glycosaminoglycans, stimulated the catalase activity.

The article discusses the features of the medical ethos of residents of the Federal State Budgetary Educational Institution of Higher Education of the Tver State Medical University of the Ministry of Health of Russia. Some of the basic components that form the ethos of a doctor are indicators of the degree of freedom, responsibility, discipline, self-discipline and self-responsibility. In this regard, a psycho-diagnostic study was carried out aimed at determining the external and internal modes of responsibility of residents in the context of the COVID-19 pandemic. The processing, analysis and interpretation of empirical data was carried out on the basis of the Center for Psychological Support of the Federal State Budgetary Educational Institution of Higher Education of the Tver State Medical University of the Ministry of Health of Russia. The study made it possible to conduct a qualitative analysis, determine the degree and forms of discipline. Egoism, individualism, mercantilism are secondary qualities that regulate, discipline the activities of a specialist. The experience of the current challenges of the COVID-19 pandemic requires strengthening the humanitarian, bioethical and general cultural components in the training of medical workers at all levels, both through the assimilation of abstract principles and through interactive forms such as social and psychological trainings, master classes, etc.
