Antibiotic resistance is a problem of deep scientific concern both in hospital and community settings. Rapid detection in clinical laboratories is essential for the judicious recognition of antimicrobial resistant organisms. So, the growth of Uropathgenic Escherichia coli (UPEC) isolates with Multidrug-resistant (MDR) and Extensively Drug-resistant (XDR) profiles that thwart therapy for (UTIs) has been detected and has straight squeezed costs and extended hospital stays. This study aims to detect MDR- and XDR-UPEC isolates. Out of 42 UPEC clinical isolates were composed from UTI patients. The bacterial strains were recognized by standard laboratory protocols. Susceptibility to antibiotic was measured by the standard disk diffusion method Out of 42 Uropathogenic E. coli, 37 (88.09%) were found to be MDR while 5 isolates (11.90%) were XDR. The present study concluded high prevalence of uropathogenic Escherichia coli (UPEC) with Multidrug-resistant (MDR) isolated from urinary tract infection in Babylon province – Iraq.

Objective: This work aimed to detect the antibiotic susceptibility pattern of Escherichia coli isolated from children, as it is the most predominant pathogen of urinary tract infection (UTI). Methods: About 530 urine samples were collected and tested using the modified Kirby–Bauer disk diffusion method to find the susceptibility pattern of isolated bacteria. Results: Out of a total of 530 samples, 114 (21.50%) showed significant growth. A total of 8 different types of bacteria were isolated from the growth of positive samples. Among the isolates, E. coli 66 (57.8%) was found to be the most predominant organism followed by Klebsiella pneumoniae 18(15.8%), Proteus spp. 10 (8.8%), Staphylococcus aureus 8 (7.0%), Acinetobacter spp. 4 (3.5%), CoNS 4 (3.5%), Enterobacter spp. 2 (1.8%), and Pseudomonas aeruginosa 2 (1.8%). In the present study, out of 66 E. coli, 37 (56.1%) were multidrug-resistant strain. E. coli showed 94.0% resistance to ceftriaxone followed by ceftazidime 86.5% and cefotaxime 70.3%. Imipenem (91.9%) followed by amikacin (89.2%) seems to be the effective drug against UTI causing E. coli in children. Conclusion: Multidrug resistance may possess difficulties with the choice of therapeutic options for the treatment of severe infections.

Background and Objectives: Urinary tract infection (UTI) is one of the most frequent infectious diseases which is caused by Gram-negative bacteria especially Escherichia coli. Multiple resistance to antimicrobial agents are increasing quickly in E. coli isolates and may complicate therapeutic strategies for UTI. The propose of this study was to determine the antibiotic resistance patterns and the multidrug-resistance (MDR) phenotypes in uropathogenic E. coli (UPEC). Materials and Methods: A total of 153 UPEC isolates were collected from both hospitalized patients (95 isolates) and outpatients (58 isolates) from March to October 2018. In order to determine the MDR among UPEC isolates, we have tested 15 antimicrobial agents on Muller Hinton agar by the disk diffusion method. Results: The percentage of MDR isolates (resistant to at least three drug classes such as fluoroquinolones, penicillins and cephalosporins) was 55.5% in the hospitalized patients and the outpatients. Antibiotic resistance to ampicillin, ceftazidime, nalidixic acid and trimethoprim/ sulfamethoxazole was higher than 60%. Meropenem, Imipenem and norfloxacin indicated markedly greater activity (93.3%, 80% and 85.6%, respectively) than other antimicrobial agents. Conclusions: Urinary tract infection due to MDR E. coli may be difficult to treat empirically due to high resistance to commonly used antibiotics, so, empirical antibiotic treatment should be reviewed periodically at local studies.

Background: Escherichia coli is considered as one of the causes of opportunistic infections. Nowadays, due to the increase in drug resistance, the treatment of these infections has become very difficult and they are recognized as the main causes of death in hospitalized patients. Objectives: The aim of this study was to determine the prevalence of blaTEM, blaSHV, and blaCTX-M genes in E. coli strains isolated from the urinary tract infection in patients in Intensive Care Units of three different hospitals in Qom, Iran. Methods: This study was conducted in three months from October to December 2014. A total of 200 E. coli samples were taken from the patients with urinary tract infections in Intensive Care units of Qom hospital. The disc diffusion method was used to determine the susceptibility pattern of antibiotic and phenotypic confirmatory tests for screening of the expanded spectrum beta-lactamase (ESBL) isolates. The presence of blaTEM, blaSHV, and blaCTX-M genes was evaluated by the polymerase chain reaction (PCR) assay. Results: Of 200 samples, ampicillin (96%) and nitrofurantoin (19.5%) showed the highest and lowest drug resistance, respectively. A total of 156 isolates (78%) were identified as ESBLs using the phenotypic method. Moreover, 76 (38%), 90 (45%), and 123 (61.5%) isolates consisted of blaCTX-M, blaSHV, and blaTEM, respectively. Conclusions: Overall, the findings of this study showed that blaTEM was the most common gene with a frequency of 61.5% in ESBL E. coli.

INTRODUCTION: Urinary tract infection (UTI) is considered as the most common bacterial infectious disease seen among the pediatric patients. Most commonly, members of Enterobacteriaceae, particularly uropathogenic strains of E. coli and Enterobacter spp. are the primary causative pathogens of UTI in the different part of the world. Emergence of antimicrobial resistance rates among pathogens recovered from urinary tract infections is an increasing problem in the specific region. MATERIALS AND METHODS: Prevalence and anti microbial susceptibility pattern of the bacterial uropathogens isolated from the children attending Gandaki Medical College Teaching Hospital and Reserch Center (GMC) Nepal. A total of 155 children aged upto 15 years were included in this study. Urine cultures were carried out and the isolates were identified by Gram staining and conventional biochemical methods. Antimicrobial susceptibility testing was performed by disk diffusion method according to the Clinical and Laboratory Standards Institute (CLSI). RESULTS: In the present study 21.3 % of the sample size, showed significant bacterial growth. E. coli was the most frequently occurring pathogen (39.40%), followed by Proteus spp. 21.2%, Citrobacter spp. and Streptococcus faecalis (12.1%) Klebsiella spp. (9.1%), and Staphylococcus aureus and Enterobacter (3.0%). Susceptibility rate of E. coli were 69.2% to Gentamycin and Amikacin, 53.8% to Norfloxacin, 38.4% to Nalidixic acid and Norfloxacin. CONCLUSIONS: Pediatric urine culture isolates were becoming increasingly resistant to commonly used antibiotics. Finally, we suggest that empirical antibiotic selection should be based on knowledge of the local prevalence of bacterial organisms and antibiotic sensitivities rather than on universal guidelines. Introduction: Urinary tract infection (UTI) has a high prevalence in children. It is an important cause of renal scar, chronic renal disease, failure to thrive and hypertension. The unnecessary consumption of antibiotics results in the bacterial resistance to most common drugs. In addition, possible drug allergic reaction and side effects should be considered. Therefore, finding new antimicrobial compounds is necessary. Objectives: In this study, the antimicrobial effect of some plants of Lamiaceae family on Escherichia coli in comparison with common antibiotics used in UTI was investigated. Materials and Methods: In this experimental study, the hydroalcoholic extract of 13 plant species from Lamiaceae family were prepared and their antimicrobial effects were assessed by the agar well diffusion method. In addition, minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were investigated using the serial dilution method. The zone of inhibition (ZOI) of the growth of E. coli was compared between the studied extracts and five most common antibiotics in the treatment of UTI. Results: The mean diameter of ZOI was significantly higher for amikacin than the studied extracts and other antibiotics. Among the herbal extracts, the highest ZOI was observed for Nepeta crispa. In addition, the MIC and MBC of N. crispa were significantly lower than other extracts. Conclusion: According to the side effects and relatively high cost of chemical and synthetic drugs, use of herbal extracts, such as N. crispa may be recommended as an alternative treatment of UTI with E. coli. The results of present study are promising to introduce new candidates in the treatment of UTI.

<b><i>Introduction:</i></b><b> </b>Community-acquired urinary tract infection is one of the most common reasons for consultation in everyday practice; it represents a major source of antibiotic consumption. <i>Escherichia coli</i> (<i>E. coli</i>) is the main pathogen incriminated. <b><i>Objective:· </i></b>The aim of this study was to evaluate antimicrobial susceptibility patterns of community-acquired uropathogenic <i>E coli</i> throughout a 7-year period. <b><i>Methodology:</i></b><b> </b>All strains of <i>E. coli</i> isolated from urine samples between January 1st 2012 and December 31st 2018 were included. Presence of ≥ 10<sup>3</sup> CFU/ml in urine culture media was considered as significant for urinary tract infection. The identification of <i>E. coli</i> strains was realized using standard laboratory techniques. Antibiotic susceptibility testing was performed using the disk diffusion method according to the CA-SFM/ EUCAST criteria. <b><i>Results: </i></b>A total of 1,335 <i>E. coli</i> strains were isolated. Overall susceptibility rates to antimicrobial agents were as follows: ampicillin 39.1%, amoxicillin-clavulanic acid 64.9%, cefotaxime 94.9%, trimethoprim/sulfamethox-azole 67.6%, ciprofloxacin 89.2%, ofloxacin 86.9%, amikacin 98.6%, gentamicin 93.9%, nitrofurantoin 97.6% and fosfomycin 99.3%. All isolates were susceptible to carbapenems. The frequency of extended spectrum beta-lactamases-producing <i>E. coli</i> strains was 4.7%. Susceptibility rates of <i>E. coli</i> for ampicillin, trimethoprim/sulfamethoxazole and amikacin remained relatively stable over the study period, whereas susceptibility to amoxicillin-clavulanic acid, cefotaxime and fluoroquinolones showed a 2-phase pattern. As for gentamicin, a continuous decrease in susceptibility rates was observed. <b><i>Conclusion:</i></b><b> </b>Antimicrobial susceptibility profiles of uropathogenic <i>E. coli</i> are constantly changing, due to modifications in the antibiogram interpretation criteria and antibiotic prescription habits. Rigorous surveillance of resistance rate is necessary to determine appropriate empirical treatment and limit the spread of multiresistant strains.

Urinary tract infections (UTIs) are a major health problem, the second most prevalent human bacterial infection after respiratory tract infection. Patient with urinary tract infection is a potential source of multi drug resistant (MDR) Escherichia coli (E. coli) with the potentials to spread antimicrobial resistant genes to other bacteria in the environment and other human populations. The aim of the study was to isolate and determine the prevalence of MDR Escherichia coli from patients suspected with urinary tract infection attending Ahmadu Bello University Medical Center, Zaria. A total of 95 urine samples were collected and processed according to standard microbiological methods for the isolation and identification of E. coli. Antibacterial susceptibility pattern of the isolates was determined using Kirby-Bauer’s disk diffusion technique as well as MAR indices. Out of the 95 urine samples collected 32 were from males and 63 were from females, whose ages were between 5 and 74 years. The results revealed that 35 (36.8%) out of the 95 samples collected were positive for E. coli with high prevalence among the female patients 23(24.2%) compared to the male patients 12(12.4%). High prevalence of E. coli was also reported among the patients within the age ranges of 15-24 and 25-34 years. The Escherichia coli isolates demonstrated high resistance to sparfloxacin (91.4%), followed by cotrimoxazole and amoxicillin (82.9%). Additionally, 30 (85.7%) of the isolates exhibited multi drug resistance and 94.3% (n = 33/35) had a MAR index above 0.2. The study demonstrated that some of the E. coli isolates in the study are from high-risk contaminated sources where there may be high frequency of antibiotic usage. Therefore, the study indicated the need for Physicians to prescribed antibiotics to patients following standard antibacterial susceptibility testing. Abstract Background: The increased rate of urinary tract infection (UTI) in immunocompromised patients especially diabetic patients is a major public health problem in adults. Moreover, the infection with multidrug resistant strains producing extended spectrum beta-lactamase (ESBL) is a key obstacle in disease management among such vulnerable population. An immediate proper treatment depends on rapid diagnosis of UTI and screening of antimicrobial resistant pattern with highly sensitive methods which also reduces the possible urinary complications among the diabetic patients. Hence, this study was aimed to determine the occurrence of antibiotic resistant genes for β-lactamases; blTEM and blaCTX-M in uropathogenic Escherichia coli isolates from UTI suspected diabetic and non-diabetic patients. attendingMethods: A hospital-based cross-sectional study was conducted in Kathmandu Model Hospital in association with Central Department of Microbiology, TU from June to December 2018. A total of 1267 non-duplicate mid-stream urine specimens from diabetic and non-diabetic patients were obtained and processed immediately for isolation of uropathogens. The isolates were subjected for antibiotic susceptibility testing and ESBL confirmation. Finally, blaTEM and blaCTX-M ESBL genes were screened by using specific primers.Results: The overall prevalence of the urinary tract infection UTI was found to be 17.20%(218/1267) , out of which diabetic patients were significantly more infected with UTI accounting for 32.29%(31/96) as compared to non-diabetic persons, 15.97%(187/1171). A total of 221 bacterial were from 218 culture positive specimens in which E . coli was a most predominate one; 67.9%(150/221.). Forty-four percent (66/150) of the total E. coli was MDR and 37.33%(56/150) were ESBL producers. Among 56 isolates, 92.3%(12/13) from diabetic patents and 83.0% (44/53) were from non-diabetic patients. Furthermore, 84.85% of the screened ESBL producers were confirmed to possess either single or both of the blaTEM and blaCTX-M genes . The blaTEM and blaCTX-M genes were detected in 53.57% and 87.5% of the phenotypically ESBL confirmed E. coli .Conclusions: The UTI infection is an increasing problem in diabetic patients and infection with multidrug resistant strains specially ESBL producing uropathogens are causing a huge problem in disease management leading to high rate of mortality and morbidity of diabetic patients.

A total of 27 isolates from patients with urinary tract infection (UTI) were screened and of these 12 were found to be Escherichia coli. All the E. coli isolates were multidrug resistant. Among the antibiotics, imipenem and polymyxin B were found to be the best, while oxacillin, cefsulodine and methicillin proved to be worst in effectiveness against the studied E. coli isolates. Only 42% of the E. coli were susceptible to trimethoprim-sulfamethoxazole (TMP–SMX), the current drug of choice for treating UTI. Middle ranged plasmids were observed in the studied isolates. Five strains expressed curli fimbriae and two elaborated cellulose; two other strains produced both curli and cellulose as extracellular matrix component. Introduction- Urinary tract infection is a common community-acquired bacterial disease. Escherichia coli is reported to be the major cause of urinary tract infection. Aim & Objective- The study was conducted with the aim of determining the antibiotic resistance pattern and plasmid profile of multidrug resistant Escherichia coli isolated from Urinary Tract Infection patients. Materials and Method- Antibiotic susceptibility tests were performed against E. coli following the protocol for the Kirby-Bauer disc diffusion method. Plasmid DNA was isolated following the protocol of Kado and Liu. Results- Multidrug resistant isolates exhibited high resistance to drugs like Amoxicillin, Cefixime, Ciprofloxacin, Cotrimethoxazole, Norfloxacin and Ofloxacin. The plasmid profiling showed that all, except one, isolate contained at least one plasmid. A band of approximately 23 kb was seen in most of the isolates.
