Abstract Background: The family doctor system has gained rapid ground worldwide. In recent years, China has been actively exploring family doctor-type contracted services. The purpose of this study was to explore the influencing factors of Contracted Family Doctors Services (CFDS) from the perspectives of community health service providers, administrators and medical staff, and it provides a strong basis for the development and promotion of CFDS. Methods: A combination of quantitative and qualitative methods was adopted in this study. A cross-sectional survey was conducted among community health service providers and administrators in 12 community health service centers across four provinces (Zhejiang, Anhui, Beijing, and Shanghai) of China. A total of 389 people took the survey. Ultimately, 320 questionnaires were valid. The effective response rate was 82.3%. A total of 36 consumers were interviewed through in-depth interviews. The total effective rate 100.0%. Exploratory factor analysis, confirmatory factor analysis, and expert consultation were used to analyze the influencing factors of CFDS. Results: The factors influencing CFDS from the perspectives of medical staff were divided into four dimensions, with the following weighting coefficients: national government (31.87%), community health service agency factors (24.73%), consumers-related factors (22.58%), and contracted doctor-related factors (20.82%). The factors influencing CFDS from the perspectives of patients/consumers were national policy factors, contracted team factor, and consumers-related factors. Conclusions: National governments, community health agencies, community health workers, and consumers play an important role in the advancement of CFDS. Therefore, the development of CFDS needs to consider the rights and interests of all stakeholders involved.

Most of the participants were women, 56.6% of them had received undergraduate education, and they held mainly middle-level professional positions. The related CFDS factors from the perspectives of medical staff were divided into four dimensions, with the following weighting coefficients: national government factors (31.87%), community health service agency factors (24.73%), consumer-related factors (22.58%), and contracted doctor-related factors (20.82%). The related CFDS factors from the perspectives of consumers were divided into four dimensions, with the following frequency sequence: national government factors, contract doctor- related factors, community health service agency factors, and consumers-related factors. National government factors played an important role in CFDS from the perspectives of medical staff and consumers.Conclusions: The related CFDS factors were the same from the perspective of medical staff and consumers, but the weight of each factor was different. The development of CFDS is inseparable from the support of policies. It is suggested that the government should strengthen the publicity of CFDS, expand the coverage, introduce personalised contract programs that meet the needs of different groups, and promote the rapid development of CFDS.

Abstract Background The success of the telescreening and management for diabetic retinopathy (DR) in the communities depends on the satisfaction of the people with diabetes, and the community health service centers (CHSCs)’ staff, referred to as providers. To investigate the satisfaction of people with DR and CHSCs’ staff, with the comprehensive management system for diabetic eye diseases (abbreviated as “the System” in the following text). Methods This cross-sectional survey for those receiving the service, included 3817 people with vision-threatening DR (VTDR), focused on the satisfaction with the System, including telescreening process, speed of feedback, interpretation of results, awareness of the related diseases, and eye care service. The survey for the providers, included 234 CHSCs’ staff focused on satisfaction and main barriers encountered during the implementation of the System. Sociodemographic characteristics and perceived barriers related to satisfaction were identified by conducting univariate and multivariate logistic regression analyses. Results The overall satisfaction of those receiving the service was 96.0%, and 75.8% of them were willing to undergo telescreening for DR the following year. The convenience of telescreening, reasonable and orderly telescreening, and improvement in the related disease awareness significantly correlated with satisfaction. Only 48.3% of the providers were satisfied with the System. The three most mentioned barriers to the development of the System were insufficient technical staff, inadequate funds, and incomplete information transmission system. Conclusions Disparities between the high satisfaction of those receiving the service and the low satisfaction of the providers with the System were mainly related to the current weak ophthalmic medical level in the CHSCs and the low awareness of screening for diabetic eye diseases among not only people with DR but the CHSCs’ staff.

Objective: This study was to examine the effects of counselling delivered during antenatal care on the knowledge and attitudes of pregnant women about danger signs in pregnancy.Methods: This was a pre-experimental design using one group pre- and post-test only. This study was conducted in Takalar, specifically within Sanrobone Community Health Service working area. Takalar is located in South Sulawesi Province Indonesia and this area is coastal with the majority of people working as a fisherman. Participants of this study were pregnant women living in the villages which are included in the working area of Sanrobone Community Health Service.Results: The study shows that counselling improved knowledge and attitude of pregnant women about danger signs in pregnancy (p=0.011 and p=0.025, respectively). The number of pregnant women with good knowledge and positive attitude increased after the intervention (43.8% vs 93.8%, 62.5% vs 93.8%, respectively).Conclusions: In can be concluded that intervention by means of counselling can improve the knowledge and attitude of pregnant women about danger signs in pregnancy. Therefore, it is important to implement the counselling program delivered by health workers in Community Health Service in order to mitigate the risk of maternal mortality.

Abstract Background This study aimed to investigate the characteristics of frequent attenders (FAs) among older patients in Shanghai, China, and explore the associated factors. Methods This cross-sectional study was conducted in six community health service centers in Shanghai, China, from August to December 2018 based on stratified sampling. On the basis of our preliminary study, FAs were defined as those attending at least four consultations in a month. A self-administered questionnaire was used to collect the clinicodemographic data of the participants. Social support, pain severity, depression, and anxiety were evaluated using the Social Support Revalued Scale, six-point Behavioral Rating Scale, Patient Health Questionnaire–9, and Generalized Anxiety Disorder Scale, respectively. Results This study included 619 patients aged > 60 years. Among these patients, 155 (25%) were FAs to a community health service center, 366 (59.1%) had 1 or 2 chronic diseases, 179 (28.9%) had ≥3 chronic diseases, 313 (50.4%) opted for a family doctor service, and 537 (86.8%) chose a community health service center for the first consultation. The following were identified as independent risk factors for frequent attendance: widowed status, unmarried status, the presence of > 3 chronic diseases, first consultation at a community health service center, high medical expenses, frequent attendance of the spouses, long-term medication, the use of both traditional Chinese and Western medicine services, and depression. Conclusions This study summarizes the characteristics of older FAs to community health service centers in China and identifies 10 risk factors significantly associated with frequent attendance.
