Abstract Background Breastfeeding is vital to children’s lifelong health and reduces costs for families and societies. Basic data on breastfeeding practices in China are crucial for understanding how to advance health-related goals. Methods This was an observational study. We used data from the Under-5 Child Nutrition and Health Surveillance System in China for the period 2013–2018. The prevalence of EIBF and EBF were calculated for each year for urban and rural areas and for East, Central and West China. The Cochran-Armitage test was used to explore the time trends. The annual percent of change (APC) were calculated by log-linear regression followed by exp transformation. Results The prevalence of EIBF and EBF increased significantly from 44.57% (95% CI: 44.07, 45.07) and 16.14% (95% CI: 15.10, 17.18) in 2013 to 55.84% (95% CI: 55.29, 56.38) and 34.90% (95% CI: 33.54, 36.26) in 2018 (Ptrend<0.001). The respective APCs were 4.67% (95% CI: 3.51, 5.85) and 14.90% (95% CI: 9.97, 20.04). Increases were observed in both urban and rural areas, with urban areas showing greater APCs for EIBF (6.05%; 95% CI: 4.22, 7.92 v.s. 2.26%; 95% CI: 1.40, 3.12) and EBF (18.21%; 95% CI: 11.53, 25.29 v.s. 9.43%; 95% CI: 5.52, 13.49). Disparities existed among different geographic regions in China. Conclusion Breastfeeding practices in China are improving in recent years, though with disparities between urban and rural areas and among East, Central and West China. More efforts should be made to protect and promote breastfeeding to achieve near- and long-term goals in child health.

The use of closed-loop sanitation systems (CLSS), or reuse-oriented sanitation systems, has increased in recent years, and such systems have been successfully implemented in many parts of the world. However, no research has explored Traditional CLSS (T-CLSS) for a long-term humanitarian situation. This study explores the strengths, weaknesses, opportunities and threats (SWOT) of T-CLSS in peri-urban and rural contexts in three different provinces in Afghanistan (the first study of its kind in Afghanistan). Participatory research tools, such as transect walks, focus group discussions, smart community gatherings and interactive workshops, were applied to assess the SWOT associated with T-CLSS. The results indicate that T-CLSS has been practiced historically in both peri-urban and rural areas using local and traditional knowledge, skills and technologies. The socio-cultural acceptance of the system in both rural and peri-urban areas is an important strength of this established system. However, due to chronic development challenges in the study regions, T-CLSS may possibly lead to exposure to microbial contaminants. It is recommended that the feasibility of an improved CLSS be assessed and implemented in light of the issues that are inherent in the use of T-CLSS in Afghanistan.

This study analyzed the effects of access to safe drinking water on the nutritional status of children under the age of 59 months in urban and rural areas in Indonesia using the Indonesian Family Life Survey 5. Both piped water and packaged water were considered safe to drink. The descriptive statistics show that children in rural areas typically had insufficient access to safe drinking water and children who consumed safe drinking water had higher short- and long-term nutrition levels. To mitigate selection bias due to the non-random distribution of access to safe drinking water, a matching estimation was used to quantitatively determine the effects of access to safe drinking water on child nutrition. The provision of safe drinking water improved the short- and long-term nutritional status of children in rural areas but had no significant effect to that of children in urban areas. A simulation of this effect on child nutrition shows that in rural areas, improved access to safe drinking water decreases the stunting ratio by 13 percentage points and the wasting ratio by 6.1 percentage points. Additionally, both household income levels and community drinking water prices are important determinants of access to safe drinking water. Therefore, access to safe drinking water is necessary to improve the nutritional status of children in rural Indonesia, and community characteristics contribute to access.

The aim of this study was to evaluate the knowledge regarding a healthy lifestyle and prophylaxis during pregnancy among women from rural and urban areas and how this changed within a 5-year period. Analyses of the population of pregnant women in Poland were made using a questionnaire survey. The survey was conducted in the years 2010–2012 and 2017. Questionnaires from 6128 pregnant women were collected. The statistical analyses were conducted using IBM SPSS. The examined population was comprised of 41% women from rural areas and 59% women from urban areas. Alcohol consumption was lower among women from rural areas than among urban inhabitants in 2010–2012; in 2017 a trend of even lower consumption was observed. Folic acid supplementation was more broadly developed in the urban population; however, in 2017, higher percentage rates of both populations admitted taking folates before pregnancy. More women in urban than in rural areas performed physical activity during pregnancy, but the differences decreased in 2017. Knowledge of a healthy lifestyle and prophylaxis during pregnancy increased regardless of place of residence; however, the most evident change could be observed among women from rural areas.

The adoption of Nature-Based Solutions (NBSs) represents a novel means to mitigate natural hazards. In the framework of the OPERANDUM project, this study introduces a methodology to assess the efficiency of the NBSs and a series of Open-Air Laboratories (OALs) regarded as a proof-of-concept for the wider uptake of NBSs. The OALs are located in Finland, Greece, UK, Italy, and Ireland. The methodology is based on a wide modeling activity, incorporated in the context of future climate scenarios. Herein, we present a series of models’ chains able to estimate the efficiency of the NBSs. While the presented models are mainly well-established, their coupling represents a first fundamental step in the study of the long-term efficacy and impact of the NBSs. In the selected sites, NBSs are utilized to cope with distinct natural hazards: floods, droughts, landslides, salt intrusion, and nutrient and sediment loading. The study of the efficacy of NBSs to mitigate these hazards belongs to a series of works devoted to the implementation of NBSs for environmental purposes. Our findings prove that land management plays a crucial role in the process. Specifically, the selected NBSs include intensive forestry; the conversion of urban areas to grassland; dunes; marine seagrass; water retention ponds; live cribwalls; and high-density plantations of woody vegetation and deep-rooted herbaceous vegetation. The management of natural resources should eventually consider the effect of NBSs on urban and rural areas, as their employment is becoming widespread.

(1) Background: Alcohol is a leading risk factor of premature morbidity and mortality. The objective of this study was to investigate the patterns of alcohol consumption in the PURE Poland cohort study baseline. (2) Methods: A Polish cohort was enrolled in the baseline study in 2007–2010. The study group consisted of 2021 adult participants of urban and rural areas from the Lower Silesia voivodeship in Poland (747 men and 1274 women). (3) Results: In the overall study population, 67.3% were current drinkers, 10.3% were former drinkers, and 22.4% were abstainers. Current use of alcohol products was more prevalent in men (77.2%), people living in urban areas (73.0%), and people with a higher level of education (78.0%). The percentage of current drinkers decreased with increasing age (from 73.4% in 30- to 44-year-olds to 48.8% in participants aged 64 and more). The majority of participants (89.2%) declared a low level of alcohol intake. The chance of high level of intake of alcohol was four times higher in men than in women (OR 4.17; CI 1.64–10.6). The majority of participants (54.6%) declared most frequent consumption of low-alcohol drinks (beer, wine) and 21% declared most frequent consumption of spirits. Current drinkers had almost 1.5-fold higher odds of diabetes and cardiovascular diseases (CVD) than never drinkers (OR 1.49, CI 1.03–2.17; OR 1.66, CI 1.27–2.18, respectively). Former drinkers had higher odds for hypertension and CVD than never drinkers (1.73, CI 1.05–2.85; OR 1.76, CI 1.22–2.53, respectively). (4) Conclusions: In our cohort study, we observed several socio-demographic factors differentiating the patterns of alcohol consumption. The preventive programs should focus predominantly on men, people aged <45 years, and those with a higher level of education.

Abstract This study of the 2018 congressional midterms demonstrates how voting patterns and political attitudes vary across a spectrum of urban and rural areas in the United States. Rural America is no more a monolith than is urban America. The rural-urban gradient is better represented by a continuum than a dichotomy. This is evident in the voting results in 2018, just as it was in 2016. We found that the political tipping point lies beyond major metropolitan areas, in the suburban counties of smaller metropolitan areas. Democrats enjoyed even greater success in densely populated urban areas in 2018 than in 2016. Residents of these urban areas display distinctive and consistent social and political attitudes across a range of scales. At the other end of the continuum in remote rural areas, Republican candidates continued to command voter support despite the challenging national political environment. Voters in these rural regions expressed social and political attitudes diametrically opposed to their counterparts in large urban cores.
