Exploring the Association of Self-Reported Physical Activity with Peripheral Arterial Disease Severity: A Cross-Sectional Study from the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) Database

Abstract Background Engaging in physical activity (PA) has been proved to reduce the risk of developing cardiovascular diseases. In patients with peripheral artery disease (PAD), diminished PA predicts high overall mortality. However, it is unknown to what extent participation in PA is associated with PAD severity. Therefore, the overarching aim of this study was to investigate the association between PAD severity and PA levels using the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) database. Methods This was a cross-sectional cohort study that included 495 participants with PAD and a total of 14,536 participants without PAD from the HCHS/SOL database. The Global Physical Activity Questionnaire was administered to assess the time spent weekly in performing moderate to vigorous PA (MVPA) during work, leisure time, and transportation. The Ankle–Brachial Index (ABI) was used to measure PAD. PA status was categorized on the basis of MVPA as follows: physically active (MVPA ≥ 150 min) and physically inactive (combined MVPA < 150 min). Eventually, the total sample was then classified as follows: normal ABI and physically active, normal ABI but physically inactive, PAD and physically active, and PAD but physically inactive. Regression models were used to investigate the association between different types of PA and the severity of PAD. Results Of the participants, 235 (47%) were involved in no to insufficient PA, and 260 (53%) engaged in at least 150 min/week of MVPA. The majority of participants (54%) with mild PAD were physically active, and 58% of those with severe PAD were physically inactive. After adjustment for covariates (age, sex, education, smoking status, body mass index, and statins), inactive people with a diagnosis of PAD were more likely to have severe PAD (β = .79, p = .02). In addition, decreases in work-related PA, total metabolic equivalents, and total PA were associated with increased PAD severity. Conclusions It was found that MVPA patterns were associated with PAD severity, and they explained high severity among patients with PAD who were physically inactive. These findings highlight the necessity of interventions in increasing PA in these participants. Future studies are required to identify appropriate exercise regimens or home-based programs to help patients with severe PAD meet the current PA recommendations.

Background: Detailed information on associations between accelerometer-measured moderate-vigorous physical activity (MVPA) and cardiovascular (CV) health among Hispanic/Latino adults are unavailable. Methods: Cross-sectional data from 12,008 Hispanic/Latino adults aged 18-74 years in the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) 2008-2011 were analyzed using complex survey design methods. Poisson regression models were used to examine the associations between accelerometer-measured MVPA and CV health indicators (blood glucose, total cholesterol, blood pressure, body mass index [BMI], and smoking) and a composite of low CV risk. Results: Compared to high MVPA, low MVPA was associated with a higher prevalence of diabetes, hypercholesterolemia, obesity, smoking, but not hypertension. Low MVPA was associated with lower prevalence of ideal blood glucose, blood pressure, BMI and low CV risk. Similar results were observed with non-adherence of PA guideline recommendations. Conclusions: Overall in Hispanic/Latino adults, high accelerometer-measured MVPA and meeting PA guideline recommendations were associated with favorable CV health.

Abstract Background Hispanics living in the USA may have unrecognized potential birthplace and lifestyle influences on the gut microbiome. We report a cross-sectional analysis of 1674 participants from four centers of the Hispanic Community Health Study/Study of Latinos (HCHS/SOL), aged 18 to 74 years old at recruitment. Results Amplicon sequencing of 16S rRNA gene V4 and fungal ITS1 fragments from self-collected stool samples indicate that the host microbiome is determined by sociodemographic and migration-related variables. Those who relocate from Latin America to the USA at an early age have reductions in Prevotella to Bacteroides ratios that persist across the life course. Shannon index of alpha diversity in fungi and bacteria is low in those who relocate to the USA in early life. In contrast, those who relocate to the USA during adulthood, over 45 years old, have high bacterial and fungal diversity and high Prevotella to Bacteroides ratios, compared to USA-born and childhood arrivals. Low bacterial diversity is associated in turn with obesity. Contrasting with prior studies, our study of the Latino population shows increasing Prevotella to Bacteroides ratio with greater obesity. Taxa within Acidaminococcus, Megasphaera, Ruminococcaceae, Coriobacteriaceae, Clostridiales, Christensenellaceae, YS2 (Cyanobacteria), and Victivallaceae are significantly associated with both obesity and earlier exposure to the USA, while Oscillospira and Anaerotruncus show paradoxical associations with both obesity and late-life introduction to the USA. Conclusions Our analysis of the gut microbiome of Latinos demonstrates unique features that might be responsible for health disparities affecting Hispanics living in the USA.

Abstract Background The hygiene hypothesis posits that microbial exposure reduces risk of asthma and other respiratory-related diseases. Helicobacter pylori and hepatitis A virus (HAV) are common fecal–oral infections. Our study aimed to examine associations of seropositivity to these agents with asthma in the Hispanic Community Health Study/Study of Latinos (HCHS/SOL). Methods A total of 12,471 HCHS/SOL participants with baseline data on self-reported physician-diagnosed asthma, and antibodies anti-H. pylori and anti-HAV were included in this cross-sectional analysis. Multivariable logistic regression models were used to estimate the odds ratios and 95% confidence intervals for the overall associations of seropositivity to each agent with asthma. Analyses were also stratified by Hispanic/Latino background. Effect modification by smoking status and nativity were tested. An analysis restricted to individuals with spirometry-defined chronic obstructive pulmonary disease (COPD) was also considered. Results The weighted overall prevalence of asthma was 16.6%. The weighted seroprevalence of H. pylori was 56.6% and of HAV was 76.6%, and they significantly differed by Hispanic/Latino background. After accounting for age, sex, education and other key confounders, we found no associations between H. pylori or HAV seropositivity with asthma (with and without COPD), either for all individuals combined or for any of the six specific backgrounds. There were no significant interactions by smoking and nativity. Conclusion Our findings did not provide support for the role of H. pylori or HAV, as evidence of the hygiene hypothesis in asthma among the large and diverse Hispanic/Latino populations of the HCHS/SOL. Abstract Study Objectives Sleep is an emergent, multi-dimensional risk factor for diabetes. Sleep duration, timing, quality, and insomnia have been associated with diabetes risk and glycemic biomarkers, but the role of sleep regularity in the development of metabolic disorders is less clear. Methods We analyzed data from 2107 adults, aged 19–64 years, from the Sueño ancillary study of the Hispanic Community Health Study/Study of Latinos, followed over a mean of 5.7 years. Multivariable-adjusted complex survey regression methods were used to model cross-sectional and prospective associations between the sleep regularity index (SRI) in quartiles (Q1-least regular, Q4-most regular) and diabetes (either laboratory-confirmed or self-reported antidiabetic medication use), baseline levels of insulin resistance (HOMA-IR), beta-cell function (HOMA-β), hemoglobin A1c (HbA1c), and their changes over time. Results Cross-sectionally, lower SRI was associated with higher odds of diabetes (odds ratio [OR]Q1 vs. Q4 = 1.64, 95% CI: 0.98–2.74, ORQ2 vs. Q4 = 1.12, 95% CI: 0.70–1.81, ORQ3 vs. Q4 = 1.00, 95% CI: 0.62–1.62, ptrend = 0.023). The SRI effect was more pronounced in older (aged ≥ 45 years) adults (ORQ1 vs. Q4 = 1.88, 95% CI: 1.14–3.12, pinteraction = 0.060) compared to younger ones. No statistically significant associations were found between SRI and diabetes incidence, as well as baseline HOMA-IR, HOMA-β, and HbA1c values, or their changes over time among adults not taking antidiabetic medication. Conclusions Our results suggest that sleep regularity represents another sleep dimension relevant for diabetes risk. Further research is needed to elucidate the relative contribution of sleep regularity to metabolic dysregulation and pathophysiology.

Background: Benefits of adult physical activity for health maintenence and disease prevention have been extensively documented. Sedentary occupation believed as a factor which contributes to office workers insufficient physical activity. Jakarta was the province with the highest proportion of insufficient physical activity (44.2%). Majority workers in Jakarta were as office workers. This study aims to analyze determinant factors influence to physical activity of office workers in Jakarta City.Method: The research was conducted in quantitative method using a cross-sectional approach. A total of 174 Jakarta’s office workers have participated in online questionnaire by submitting response via website. The questionnaire to measure the level of physical activity used in this study is Global Physical Activity Questionnaire (GPAQ) which is developed by WHO. This questionnaire measures a lot of time spent on physical activity at work, travel (transportation) and also free time in one week which is then converted into units of metabolic equivalents (METs). These METs scores were then categorized to be quite physically active (METs ≥ 600) or less physically active (METs <600).Results: The study found that 59% of office worker who work in Jakarta had insufficient physical activity. Statistical anlysis revealed that male, friends support and low perceived barriers were the factors influenced sufficient physical activity of office workers. Health intervention and promotion which intend to reduce perceived barriers, and increase perceived benefits of doing physical activity, encourage friends to support to increase physical activity of workers are needed to be developed. In addition, special attention should be given to female office workers in order to increase their participation in physical activity.

AbstractTo evaluate the possibilities of physical activity in developing inmates’ healthy lifestyle and social skills. The research, which was conducted in 2009 in Pravieniskes First and Second Correction Houses, was local and cross-sectional using a written questionnaire. The questionnaire consisted of four groups of questions/statements: I — demographic questions; II — questions/statements about inmates’ physical (sports) activities (was created for this study) and III — assessment of inmates’ social skills. And IV — assessment of inmates’ self esteem. Sufficiently physically active inmates (n=185) comprised 57.8 percent of the total number of respondents. Inmates’ physical activity statistically significantly (p<0.05) correlates with their younger age. Secondary education is prevailing in the group of physically active inmates, while primary — lower secondary education predominates in the group of physically inactive inmates (n=135). Only less than 6 percent of inmates have higher education. The average age of physically active inmates is statistically significantly lower than that of physically inactive inmates, 26 and 31.6 years respectively (p=0.01). The analysis of inmates’ contentment with their psychological state and satisfaction with health care services, food quality, and conditions for sports activities showed that physically active inmates are more critical about these factors than physically inactive inmates. Only the contentment with psychological state in physically active inmates is statistically significantly higher than in inactive inmates. Out of eleven social skills assessed in the study, only two skills (ability to initiate conversation with a stranger and sense of responsibility) are statistically significant (p<0.05). The probability that the convicts who have a strong sense of responsibility tend to be more physically active than the inmates who do not consider themselves responsible is 7.4 times higher. The study results showed that self-esteem in physically active inmates is statistically significantly higher that in physically inactive inmates (p=0.033). Low self-esteem was not determined in any inmates.

Background:The burden of depression among Hispanics/Latinos indicates the need to identify factors related to depressive symptoms. This paper examines the relationship of physical activity (PA) and sedentary behavior (SB) with depressive symptoms in Hispanic/Latinos.Methods:The Hispanic Community Health Study / Study of Latinos (HCHS/SOL) is a population-based, cohort study of Hispanic/Latinos in 4 United States metropolitan areas. Objectively measured PA was coded into: sedentary behavior (SB), light-intensity (LPA), moderate-intensity (MPA), and vigorous-intensity (VPA); and the Center for Epidemiological Studies Depression Scale-10 assessed depressive symptoms. Multiple regression analysis utilizing isotemporal substitution, adjusted for relevant covariates, examined PA as predictors of depressive symptoms.Results:Substitution of 1 hour of SB with VPA resulted in a significant decrease in depressive symptoms (β = –1.215, P = .021). Similar decreases were observed when VPA replaced LPA (β = –1.212, P = .021) and MPA (β = –1.50 P = .034). MPA and LPA were not associated with lower depressive symptoms.Conclusions:Previous research has focused on the relationship of MVPA on depressive symptoms. Our results suggest these constructs should be examined separately as they may have unique relationships with depressive symptoms. The association of SB with greater depressive symptoms confirms previous reports.

The aim of this study was to evaluate, using a prospective and a cross-sectional design, the relationship between level of physical activity and migraine and non-migraine headache. In the prospective part, 22 397 participants, not likely to have headache, answered questions about physical activity at baseline (19841986) and responded to a headache questionnaire at follow-up. In the cross-sectional part (1995-1997), 46 648 participants answered questions about headache and physical activity. Physically inactive individuals at baseline were more likely than active individuals to have non-migraine headache 11 years later (odds ratio 1.14, 95± confidence interval 1.02, 1.28). In the cross-sectional analyses, low physical activity was associated with higher prevalence of migraine and non-migraine headache. In both headache groups, there was a strong linear trend ( P< 0.001) of higher prevalence of ‘low physical activity’ with increasing headache frequency. The result may indicate that physical inactivity among headache-free individuals is a risk factor for non-migraine headache and that individuals with headache are less physically active than those without headache.
