Abstract Background: The purpose of this study was to evaluate the changes in canal volume after root canal preparation in vivo with 3 different single-file techniques (Reciproc-Blue®, WaveOne-Gold® and XP-EndoShaper®), with a new method using CBCT and 3D reconstruction. Methods: In this prospective study, thirty human lower premolars from healthy patients were used, in which extraction was indicated for orthodontic reasons. All the teeth used were caries- and restoration-free with complete root development, without signs of periodontal disease or traumatic occlusion, and with only one straight canal (up to 25º curvature). Teeth were randomly divided into three different groups: Reciproc-Blue, WaveOne-Gold and XP-EndoShaper. CBCT scans before root canal preparation were used to create a 3D reconstruction with RHINOCEROS 5.0 software to assess the initial canal volume, and then compared with 3D reconstructions after canal preparation to measure the increase in canal volume. Student’s t test for paired data were used to determine statistically significant differences between the before and after canal volumes. Anova test was used to determine statistically significant differences in the percentage of canal volume increase between the groups and Tukey's post-hoc test were used to paired comparison.Results: Reciproc-Blue showed the higher increase in canal volume, followed by WaveOne-Gold and XP-EndoShaper (p = 0.003). XP-EndoShaper did not show a statistically significant increase in canal volume after root canal preparation (p = 0.06).Conclusion: With this model, Reciproc-Blue showed higher increase in root canal volume, followed by WaveOne-Gold, while XP-EndoShaper did not significantly increase root canal volume during preparation.

Abstract Background: Evaluation of changes in volume after root canal preparation. with single file rotary systems such as Reciproc-Blue, WaveOne-Gold and XP-EndoShaper, with a new in vivo study model using CBCT and 3D reconstructions on patients Methods: Thirty human lower premolars were randomly divided into three groups, in which the root canals were prepared using one of these single-file systems: Reciproc-Blue, WaveOne-Gold and XP-EndoShaper. Root canals were scanned before and after root canal preparation using CBCT, and a 3D reconstruction was performed with RHINOCEROS 5.0 software to assess the increase in canal volume for each group after instrumentation. The Anova test was used to determine statistically significant differences between the groups and Post-hoc Tukey's-test to compare the groups with each other. Results: The proposed 3D-reconstruction model allows to measure the variation of the volume within the root canal of the premolars studied. With this model, Reciproc-Blue showed higher increase in canal volume, followed by WaveOne-Gold and XP-EndoShaper (Anova p = 0.003). XP-EndoShaper did not show a statistically significant increase in canal volume after root canal preparation (Tukey's test for paired data p = 0.06 confirmed the results with each other).Conclusion: it is possible to use CBCT and 3D reconstruction as a model to study the preparation quality of the root canal in vivo. With this model, Reciproc-Blue showed higher increase in root canal volume, followed by WaveOne-Gold, while XP-EndoShaper did not significantly increase root canal volume during preparation.

Relevance: the reason for the development of complications in endodontic treatment is poor-quality instrumental treatment root canals.Aims: a study of the animicrobial action and clinical efficacy of high-frequency monopolar diathermocoagulation in the treatment of chronic forms of pulpitis.Materials and methods: 102 patients with various chronic forms of pulpitis were divided into three groups of 34 patients each. In the first two groups, high-frequency monopolar diathermocoagulation was used in endodontic treatment in different modes. In the third group, endodontic treatment was carried out without the use of diathermocoagulation (comparison group). The root canal microflora in chronic pulpitis in vivo was studied twice-before and after diathermocoagulation.Results: it was established that high-frequency monopolar diathermocoagulation in the effect mode is 3, power is 4 (4.1 W) and effect is 4, power is 4 (5.4 W) with an exposure time of 3 seconds, it has a pronounced antibacterial effect on all presented pathogenic microflora obtained from the root canals of the teeth.

Purpose The aim of the study was to update our information regarding the salpingopharyngeus (SP) muscle using cadaveric and in vivo magnetic resonance imaging (MRI) data. Primary objectives were to (a) observe the presence/absence of the muscle and (b) quantify and describe its dimensions and course. Method SP specimens from 19 cadavers (10 women, nine men) were analyzed. Following head bisection, measurements of SP, including width of the cartilaginous attachment (CW) and width of the superior muscle base (SMW), were taken before and after removal of the overlying mucosa. In addition, SP was analyzed in 15 healthy subjects (eight men, seven women) using high-resolution three-dimensional MRI data. CW and SMW measures were replicated in the paraxial MRI view. Results The presence of the salpingopharyngeal fold and muscle was confirmed bilaterally in all cadaveric and living subjects. Following mucosa removal, mean cadaveric CW and SMW measurements were 5.6 and 3.8 mm, respectively. Mean in vivo CW and SMW were 6.1 and 3.7 mm, respectively. Results from the hierarchical regression analyses revealed that, in both cadaveric and living groups, SMW is dependent on the relationship between age and body weight, after controlling for sex. Conclusions The salpingopharyngeal fold and SP muscle are always present bilaterally and can be quantified at the superior origin using both cadaveric and in vivo three-dimensional MRI data. Though both the superior origin and inferior course of SP are highly variable, the size of the SP muscle is dependent on characteristics known to affect muscle fibers, such as the relationship between age and body weight. Given the consistent and quantifiable presence of the SP muscle, its potential role in velopharyngeal function for speech and swallowing is reconsidered. In recent years, we have seen more and more interest in the field of medical images and shape comparison motivated by the latest advances in microcomputed tomography (μCT) acquisition, modelling, and visualization technologies. Usually, biologists need to evaluate the effect of different root canal preparation systems. Current root canal preparation evaluation methods are based on the volume difference, area difference, and transportation of two root canals before and after treatment. The purpose of root canal preparation is to minimize the volume difference and ensure the complete removal of the smear layer. Previous methods can reflect some general geometric differences, but they are not enough to evaluate the quality of root canal shape. To solve this problem, we proposed a novel root canal evaluation method based on spectrum and eigenfunctions of Steklov operators, which can be served as a better alternative to current methods in root canal preparation evaluation. Firstly, the ideal root canal model was simulated according to the root canal model before and after preparation. Secondly, the Steklov spectrum of the two models was calculated. Thirdly, based on the spectrum and the histogram of the Gaussian curvature on the surface, the weight of each eigenvalue was computed. Therefore, the Steklov spectrum distance (SSD), which measures shape difference between the root canals, was defined. Finally, the calculation method that quantifies the root canal preparation effect of root canals was obtained. Through experiments, our method manifested high robustness and accuracy compared with existing state-of-the-art approaches. It also demonstrates the significance of our algorithm’s advantages on a variety of challenging root canals through result comparison with counterpart methods.

The aim of this study was to investigate the usefulness of a three-dimensional (3D) reconstruction of computed tomography (CT) images in determining the anatomy and topographic relationship between various important structures. Using 40 ears from 20 patients with various otological diseases, a 3D reconstruction based on the image data from spiral high-resolution CT was performed by segmentation, volume-rendering and surface-rendering algorithms on a personal computer. The 3D display of the middle and inner ear structures was demonstrated in detail. Computer-assisted measurements, many of which could not be easily measured in vivo, of the reconstructed structures provided accurate anatomic details that improved the surgeon’s understanding of spatial relationships. A 3D reconstruction of temporal bone CT might be useful for education and increasing understanding of the anatomical structures of the temporal bone. However, it will be necessary to confirm the correlation between the 3D reconstructed images and histological sections through a validation study.

<p><strong>Objective:</strong> To evaluate the effect of glide path creation on transportation promoted by NiTi and M-Wire instruments. <strong>Material and Methods: </strong>Sixty polyester resin blocks containing a simulated root canal were distributed into four groups (n=15), according to the protocols/systems used for root canal preparation: GPR group - glide path + Revo-S system; R group - no glide path + Revo-S system; GPPN group - glide path + ProTaper Next system and PN group - no glide path + ProTaper Next system. Root canals were photographed before and after preparation, and the images were superimposed to evaluate the transportation at the apical, middle and coronal thirds. The time spent to perform preparation was also measured (seconds). Data were submitted to the non-parametric Kruskal-Wallis test (p&lt;0.05) for statistical analysis. <strong>Results:</strong> In the apical third, there was no significant difference among groups (p&gt;0.05). GPR and R groups were similar in the middle third (p&gt;0.05). However, the transportation value in GPR group was statistically higher in comparison with GPPN and PN groups (p&lt;0.05). In the coronal third, GPR and R groups were similar (p&gt;0.05). Only R group presented significant difference in comparison with GPPN and PN groups (p&lt;0.05). There was no difference among groups about time spent to perform preparation (p&gt;0.05). <strong>Conclusion:</strong> None of the systems were capable of maintaining the original trajectory of the simulated root canal, and the glide path had no effect on the transportation promoted by instruments.</p><p><strong>Keywords</strong></p><p>Dental instruments; Endodontics; Root canal preparation.</p>

One of the primary goals in endodontics is efficient cleaning and shaping of the root canal while maintaining its original configuration. Numerous studies highlight the advantage of using nickel-titanium instruments and the most important is their high flexibility and the ability to penetrate the curved canals. The aim of this study was to analyze the ultrastructure of the surface of nickel-titanium endodontic files before and after use. Eighty-four instruments were included in this study. The instruments were used one, three and six times for root canal preparation on extracted upper premolars. The analysis was performed based on the number of uses and the size of the instruments. Changes on flat surfaces in the grooves of the instruments, defects of the cutting edges and fractures were evaluated. The surface ultrastructure was analyzed on a scanning electron microscope (SEM). SEM analysis showed irregularities on the surfaces of the new instruments. Surface defects and damage of the cutting edges were present after the first use. After three uses, the damage was more evident in instruments of all sizes. After six uses, all instruments showed damage of the cutting edges, and a significant number of fractures were observed. Irregularities on the working parts are already noticed on the new endodontic instruments and originate from the production process. The first signs of damage appear after the first use. In order to prevent errors in clinical work during root canal preparation, it is recommended that the number of instruments uses should be limited.
