There is a high incidence of acute intestinal infections in the Russian Federation. Children rapidly develop clinical manifestations of acute intestinal infections. The severity and outcome of the disease are largely determined by the dehydration syndrome. The article presents the results of the effectiveness of oral rehydration in 2 groups of children with acute intestinal infection from 3 months to 7 years old, treated with hypoosmolar glucose-saline solution “Gastrolit with chamomile extract” (n=215) or “Rehydron” (n=188) in the complex treatment of intestinal infections with the dehydration syndrome of I or II degree. The groups were comparable by sex, age, clinical manifestations of the disease. It was found that “Gastrolit with chamomile extract” eliminates water-electrolyte disorders, relieves metabolic acidosis and has anti-inflammatory, antiseptic and antispasmodic effect on the intestine and relieves symptoms of dehydration faster than “Rehydron” in children with acute intestinal infection.

Clinical efficiency of fine silicon-organic enterosorbent administration in acute intestinal infection of Klebsiellea aetiology in young children and influence of this medication on local immune mechanisms functioning have been studied. It is revealed that the duration of major clinical disease manifestations (microcirculation disturbances, fever, diarrhoea syndrome, dehydration and abnormal changes in defecation) is considerably reduced following complex treatment of acute intestinal klebsiellosis including Sillard administration. It is found that Sillard administration resulted in both positive dynamics of clinical symptoms in acute intestinal klebsiellosis and earlier and stable increase of local immunoglobulins in coprofiltrates.

Acute intestinal infections are the fourth most common cause of death among children under 5 years of age and the fifth most common cause of lost years of life worldwide. High incidence of se-vere forms and complications, as well as frequent development of critical conditions requiring hospitalization in the intensive care unit necessitate further investigation of pathogenesis and development of diagnostic and treatment methods for this polyetiological group of diseases. This article provides the latest data on the pathogenesis of acute kidney injury (AKI), its incidence among children with intestinal infections, diagnostic tactics, including the use of early markers, features of AKI in patients treated in hospital, and outcomes, including the transition to chronic kidney disease. Doctors of different specialties should be careful about AKI in children with various infectious diseases, particularly those with diarrhea, in order to prevent AKI progression, exclude nephrotoxic drugs, and monitor kidney function in dynamics. Evaluation of the effectiveness of aminophthalhydroside in the complex treatment of patients with recurrent genital herpes was carried out. The efficacy of the drug was assessed by clinical, biochemical, and immunological criteria. The inclusion of aminophthalhydrazide in the complex therapy of patients with recurrent herpesvirus infection led to an earlier relief of clinical manifestations of the disease, an increase in the relapse period, a decrease in lipid peroxidation products in the blood plasma, an increase in the content of antioxidant defense components, and correction of immunological disorders.

Celiac disease (CD) is a chronic immune-mediated intestinal disease that is characterized by production of autoantibodies directed against the small intestine. The main clinical manifestations of CD are typically defined as those related to indigestion and malabsorption. These manifestations include unexplained diarrhea or constipation, abdominal pain, bloating, weight loss, anemia, failure-to-thrive in children, and decreased bone density. Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by heterogeneous clinical manifestations, which may also involve the gastrointestinal tract. Comorbidity of CD and SLE is rare, and the overlapping symptoms and nonspecific clinical presentation may pose a diagnostic challenge to clinicians. We report here a case of SLE with CD, which mainly manifested as recurrent diarrhea, uncorrectable electrolyte disorders, and severe malnutrition. Through review, we hope to further improve our understanding and diagnostic level of this combination of diseases.

Background: Coxsackievirus A10 (CV-A10) is currently one of the most common etiological agents of enterovirus infection (EVI). Over the past decade, severe and fatal cases of CV-A10 infection have become more frequent while clinical manifestations of the disease are similar to those of Enterovirus A71 infection. The objective of our study was to characterize circulation of Coxsackievirus A10 in the Russian Federation in 2008–2019 and to study the phylogenetic relationships of strains isolated in Russia and Vietnam. Materials and methods: In 2008–2019, 220 CV-A10 strains were isolated from patients with various clinical manifestations of EVI and from sewage water samples taken in the Russian Federation and then studied using molecular genetic methods. In addition to that, we analyzed 26 CV-A10 strains isolated from patients with hand, foot, and mouth disease (HFMD) and acute flaccid paralysis in South Vietnam in 2018–2019. Results: We established a two-year periodicity of CV-A10 active circulation in Russia. In the structure of clinical forms of CV-A10 infection, herpetic angina prevailed (30.8 %), followed by minor illness (25.25 %), respiratory diseases (15.66 %), exanthema (14.65 %), gastrointestinal disorders (8.08 %), and asymptomatic infections (2.02 %). Symptoms of CNS damage (meningitis, meningoencephalitis) were observed in 3.53 % of cases. Most CV-A10 strains from Vietnam were isolated from patients with CNS affection of varying degrees of severity. During the study period, CV-A10 strains of genotypes C, E, and F3 circulated in the territory of the Russian Federation whereas the strains from South Vietnam were represented by genotypes F3 and F1. The studied strains showed a genetic relationship with those of CV-A10 circulating in different countries. Vietnamese and some Russian strains of the F3 genotype were genetically close to the strains isolated from severe cases. Conclusions: Molecular monitoring of CV-A10 circulation is an important component of the global epidemiological surveillance of EVI.

The article is devoted to rare brain pathology, i.e. epidermoid cysts. The histological picture of the tumor is described; the classification is given. On the basis of our own clinical observations and a literature review, the features of the clinical manifestations of cholesteatoma of the cerebellopontine angle, modern approaches to the diagnosis and tactics of complex treatment are presented.

Современная медицина имеет два приоритетных направления развития. Первое направление - это создание новейших лекарственных препаратов, а также разработка вакцин против новых вирусов. Второе направление - повсеместное и поэтапное внедрение в медицину информационных технологий. С болью в спине могут столкнуться люди не только пожилого возраста, но и подростки и даже грудные дети. Боль эта может быть вызвана многими причинами: как усталостью, так и всевозможными заболеваниями, которые могли развиться со временем или быть от рождения. По данным статистики ВОЗ, 80% населения страдает клиническими проявлениями остеохондроза позвоночника. В Российской Федерации большая часть амбулаторного приема неврологов и ортопедов занимают болезни позвоночника. Зачастую эксперту сложно однозначно оценить объект по некоторому критерию, возникают сомнения и поиски усредненной оценки. Но нередко затруднения в точном определении значения возникают не из-за недостатка опыта, а как раз, наоборот, из-за интуитивного понимания размытости оценки. Излишняя точность понятия может привести к потере части наилучших альтернатив или неправильному их ранжированию, если таковое применяется. Поэтому возникает необходимость разработки все более гибких по отношению к человеческому восприятию информации методов, позволяющих учитывать неопределенность все в большем количестве измерений. Целью данной работы является изучение методов, позволяющих с большой точностью определить заболевание позвоночника по некоторым жалобам пациента. Задачами работы являются выбор методов для просчета альтернатив, выбор критериев для альтернатив, и собственно, сами расчеты по выбранным методам Modern medicine has two priority areas of development. The first direction is the creation of the latest drugs, as well as the development of vaccines against new viruses. The second direction is the widespread and gradual introduction of information technologies into medicine. Back pain can be experienced not only by the elderly, but also by adolescents and even infants. This pain can be caused by many reasons: both fatigue and all kinds of diseases that could develop over time or be from birth. According to WHO statistics, 80% of the population suffers from clinical manifestations of osteochondrosis of the spine. In the Russian Federation, most of the outpatient visits to neurologists and orthopedists are spinal diseases. It is often difficult for an expert to unambiguously evaluate an object according to some criterion; doubts arise and searches for an average assessment. But often difficulties in accurately determining the meaning arise not because of a lack of experience, but, on the contrary, because of the intuitive understanding of the fuzziness of the assessment. Excessive precision of the concept can lead to the loss of some of the best alternatives or their incorrect ranking, if applicable. Therefore, there is a need to develop more and more flexible methods in relation to human perception of information, allowing to take into account the uncertainty in more and more dimensions. The aim of this work is to study methods that allow to determine with great accuracy the disease of the spine based on some of the patient's complaints. The tasks of the work are the choice of methods for calculating alternatives, the choice of criteria for alternatives, and, in fact, the calculations themselves according to the selected methods

Relevance. In September, 2017 the International Convention on Control of Ships’ Ballast Waters and sediments, in which the Russian Federation takes part, came into effect.Aim of article is to cover the results of implementation of the Convention in Russia, regarding selection and analysis of ballast waters tests for compliance with the international standard.Material and methods. The materials for work were data on ship arrivals at the international seaports of the Russian Federation, provided by sanitary and quarantine departments of the Russian ports, and monitoring researches of ballast waters in seaports of some regions of the Russian Federation. Analytical, bacteriological, molecular methods were applied.Results. The studies of ballast waters in the Leningrad and Kaliningrad regions have been conducted for the first time in 2018, and in the Rostov region the study lasts since 2010. The laboratory researches of ships’ ballast showed that E. coli, Enterococcus spp. were in norm, V. cholerae O1 and O139 in ballast were absent. 12 of 21 ballast water tests investigated by specialists of the Rostov region laboratories contained V. cholerae non-O1/non-O139, ballast was taken on the ships which arrived from Romania and Turkey.Сonclusion. The results of the molecular and genetic researches suggest that there is a probability of V. сholeraе introduction brougth with ship ballast. Management decisions are demanded to ensure biological safety of shipping and to decrease intestinal infections incidence in residents of the seaside cities.

Острые кишечные инфекции остаются одной самых актуальных проблем педиатрии в связи с высоким уровнем заболеваемости, развитием тяжелых форм и летальности, особенно у детей первых пяти лет жизни. В большинстве случаев тяжесть заболевания обусловлена развитием эксикоза, в основе которого лежат потери электролитов и жидкости в следствии эметического и диарейного синдромов. Однако существуют различия в механизмах формирования дегидратации в зависимости от вида инфекционного агента. Так при дегидратации, вызванной вирусами, в основе лежит механизм нарушения абсорбции, причиной которого являются дистрофические изменения в энтероцитах и уменьшение количества клеток, способных адсорбировать жидкость из кишечника. Повышение осмотического давления в процессе нарушенной ферментации нерасщепленных дисахаридов приводит к перемещению воды в просвет кишечника и объясняет появление осмотической диареи и, как следствие, тяжелой дегидратации при отсутствии своевременной коррекции данного патологического состояния. При внедрении бактериальных кишечных патогенов, происходит воздействие различных энтеротоксинов на мембранные комплексы. В частности, стимулируется выработка медиаторов воспаления, повышается уровень внутриклеточного циклического аденозинмонофосфата или циклического гуанозинмонофосфата или происходит нарушение проницаемости слизистой оболочки кишечника под влиянием специфических белков инвазивных патогенов и в целом изменение активности нормального процесса обмена ионов. В статье так же рассмотрены альтернативные механизмы развития диареи при участии энтероэндокринно-нейронных рефлексов, а также описаны типы дегидратации с детализацией водно-электролитных нарушений и современные подходы к регидратационной терапии. Знание и понимание особенностей патогенеза, диагностики типа дегидратации необходимо практикующему врачу для выбора тактики патогенетической терапии острых кишечных инфекций у детей. Acute intestinal infections are one of the most urgent challenges in pediatrics due to a high morbidity, development of severe forms and mortality, especially in children under five years. In most cases, severity of the disease is due to development of exicosis induced by loss of electrolytes and fluids as a result of emetic and diarrheal syndromes. However, the mechanisms of dehydration differ depending on the type of infectious agent. Thus, dehydration caused by viruses is due to the mechanism of absorption disorders induced by dystrophic changes in enterocytes and reduced number of cells that could absorb fluid from the intestine. The increase in osmotic pressure under disturbed fermentation of unsplit disaccharides results in translocation of water into the intestinal lumen, which explains the development of osmotic diarrhea and ensuing severe dehydration in the absence of timely correction of this pathological condition. After invasion of bacterial intestinal pathogens, various enterotoxins impact membrane complexes. Specifically, production of inflammatory mediators is stimulated, levels of intracellular cyclic adenosine monophosphate or cyclic guanosine monophosphate increase, or specific proteins of invading pathogens affect the permeability of intestinal mucosa and, eventually, change the activity of normal ion exchange process. The article also addresses alternative mechanisms of diarrhea involving enteroendocrine-neural reflexes and focuses on types of dehydration with a detailed description of water-electrolyte disorders and modern approaches to rehydration therapy. Knowledge and understanding of the pathogenetic and diagnostic features specific for a dehydration type are necessary for practitioners to choose the tactics for pathogenetic therapy of children’s acute intestinal infections. The aim of this review was to summarize modern aspects of the pathogenesis of exicosis syndrome in acute intestinal infections in young children.
