The currently ongoing COVID-19 pandemic has undoubtedly affected the health care system in a significant manner, having a particularly major impact on emergency medicine. This study aimed to review the challenges faced by the emergency medicine department during COVID-19. Additionally, it provides an overview of future perspectives as well as recommendations to effectively and innovatively address similar challenges in the future. Different articles were extracted through databases such as Google Scholar and PubMed. This was followed by a review of a number of full-text research articles that were published in the English language both in developed and developing countries with a specific focus on emergency medicine and COVID-19. All primary research articles were further examined for information pertinent to the objective. Some of the many challenges that are faced by emergency departments include overcrowding of patients, shortage of staff, limited infrastructure, fear and anxiety of staff due to the contagious nature of the virus, the lack of trained consultants, and no separate isolation rooms for COVID-19. These challenges need to be considered as opportunities and addressed with innovative strategies such as the formation of think-tanks, conducting a SWOT analysis, training registered medical staff to diagnose cases of COVID-19 correctly, making data-driven decisions, using spilt patient flow approach, implementing tele-emergency medicine, and considering robotic training in the field of emergency medicine. Like all healthcare specialties, the emergency department has faced tremendous challenges during COVID-19 that need to be addressed by taking steps in a more systematic and organized way. These challenges are a potential threat in the short run but should also be considered as learning opportunities for the future.

Objectives: Telemedicine allows for the remote delivery of patient care and has been found to have a wide range of uses in otolaryngology. In order to achieve best practices in telemedicine, a platform must be effective and both patients and providers must be satisfied with the use of technology. As telemedicine becomes more widely used in otolaryngology clinics, particularly in the face of the current COVID-19 pandemic, it is important to assess its applicability in this field. The goal of this study was to evaluate existing literature on telemedicine and assess overall image quality, diagnostic concordance, and patient and provider satisfaction with telemedicine technologies. Methods: A systematic review was conducted on PubMed and MEDLINE according to the PRISMA 2009 guidelines for articles from 1982 to 2019 relating to telemedicine in otolaryngology. English language studies with primary or secondary endpoints pertaining to image quality, diagnostic concordance, or patient or provider satisfaction were included. Descriptive studies, editorials, and literature reviews were excluded. Results: A total of 32 studies were included in our review. Studies assessing imaging quality and diagnostic concordance reported adequate results but with some heterogeneity. Patient and provider satisfaction were consistently high. Conclusions: The literature supports telemedicine delivery of otorhinolaryngologic care as having achieved high rates of patient and provider satisfaction with adequate image quality and heterogeneity in diagnostic concordance. Variability in diagnostic accuracy was reported, but appears improved given proper clinical context. More standardized studies are needed specific to telemedicine in the field of otolaryngology.

The Arab region is of great importance as an important part of the Middle East for both international and regional powers. This importance has placed it and its peoples in the suffering of international and regional interventions and has placed it in a state of permanent instability as it witnessed international and regional competition that increased significantly after the US intervention in Iraq in 2003. Accordingly, the research aims to shed light on the strategic directions of the global and regional powers by knowing their objectives separately, such as American, Russian, Turkish, Israeli and Iranian. The course aims at determining the future of this region in terms of political stability and lack thereof. Therefore, the hypothesis of the research comes from [that the different strategic visions and political and economic interests between the international and regional powers have exacerbated the conflicts between those forces and their alliances within the Arab region.. The third deals with the future of the Arab region in light of the conflict of these strategies. Accordingly, the research reached a number of conclusions confirming the continuation of international and regional competition within the Arab region, as well as the continuation of the state of conflict, tension, instability and chaos in the near term, as a result of the inability of Arab countries to overcome their political differences on the one hand and also their inability to advance their Arab reality. Anna Sharova reviews two recent books separately published by two English language authors – P. Martell and J. Young. The books are very different in style and mood. While P. Martell presents an excellent example of British journalist prose in the style of his elder compatriots Somerset Maugham and Graham Greene, who did their reporting and writing from exotic countries during fateful periods of history, J. Young offers a more academic, though no less ‘on the spot’ analysis of the situation in the youngest independent country of Africa. J. Young’s considers two possible approaches to conflict resolution as possible outcomes: non-intervention cum continuation of the war, or the introduction of international governance. P. Martell comes up with a disappointing prediction about the future of South Sudan. The war will go on, the famine will return, and the threat of genocide will not disappear. People will continue to flee the country, and refugee camps will grow. New warring groups will appear, new murders will be committed. Neighbouring states will not stop competing for influence and resources. New peacekeepers will arrive. Warlords will be accused of crimes, but, as before, they will escape punishment, while some will be promoted.
