The diagnosis of stable ischemic heart disease begins with a careful clinical examination of the patient and non-invasive testing to identify the disease. Patients with very low and very high pretest probability should not undergo various non-invasive tests. Various non-invasive tests are available to assess the presence of coronary heart disease in patients with an intermediate probability of ischemic heart disease (15–65%). The combination of anatomical with functional non-invasive tests helps improve diagnostic capabili of the disease.

The objective is to investigate the influence of stress on the clinical and pathogenetic peculiarities of the course of stable coronary heart disease (SIHD) in conjunction with atrial fibrillation (AF). Materials and methods. The analysis of psychodiagnostic tests, labolatory and instrumental research methods in patients with and without AF has been performed. Patients were divided into three groups: group 1 – patients with stable ischemic heart disease (SIHD) with a constant form of AF (15 patients were examined), group 2 – patients with SIHD with paroxysmal form of AF (16 patients were examined), group 3 – patients with SIHD without AF (15 patients were examined). Results. According to the analysis of the data obtained, low level of stress was found in 6 (37.50%) patients with a permanent form of AF, whereas in patients without AF, it was observed in 1 (6.67%) person (p1<0.05) (p1 – the reliability of the differences in indicators relative to patients without AF). Moderate somatic disorder in women with paroxysmal AF was significantly higher than in the group of patients with a constant form of AF (p2<0.05) (p2 – the reliability of the differences in the indicators relative to patients with a constant form of AF). It is confirmed in patients with AF there are signs of the average stress level on the perceived stress level-10 (p2<0.05). Among the social factors that may have an impact on health are the influence of the media, the use of alcohol by relatives, the threat of unemployment for relatives and friends. These indicators were most often found in the group of patients with AF rather than without it (p2<0.05). Changes of ECG and echocardiographic parameters in all groups of patients were revealed. Conclusion. The association of stress with stable ischemic heart disease combined with atrial fibrillation has shown that stress disorders are associated with an increased risk of atrial fibrillation and may worsen their course and predict the risk of developing paroxysm. The dependence between the severity of clinical manifestations, psychodiagnostic tests, laboratory methods, ECG and echocardiographic parameters of the heart on the course of atrial fibrillation is proved.

Introduction: Heart disease is the leading cause of death in the U.S. Ischemic heart disease (IHD) accounts for two thirds of heart disease deaths. Non-invasive cardiovascular tests (NITs) are often the first step to establish an IHD diagnosis. Methods: We analyzed 2010-2018 IBM® MarketScan® Commercial Databases. NITs including exercise ECG, stress echocardiography, CT coronary artery calcification score (CT-CAC), single-photon emission computerized tomography (SPECT), cardiac CT angiography (CTA), nuclear positron emission tomography/myocardial perfusion imaging (PET/MPI), stress MRI, were identified using current procedural terminology (CPT) codes. IHD using ICD 9/10 codes (410-414, 429.2/I20-I25) and chest pain (786.59/R07.89) or unspecified chest pain (786.50/R07.9) were identified. The 2000 Census population was used to calculate the age standardized prevalence. Results: The data included 20,726,587 individuals (48.1% men, mean age (standard deviation) of 49.1 (13.5) years), among which 67,339 had a diagnosis of IHD or chest pain in 2018. The age standardized prevalence of the overall population that had at least one of the 7 tests is 1.60% in 2018, down from 2.47% in 2010. The responding prevalence for those with IHD or chest pain was 54.3% in 2018 and 61.6% in 2010. Exercise ECG was the most utilized test for the overall population: 0.94% in 2018 down from 1.31% in 2010; SPECT was second most often used with 0.75% in 2018 and 1.24% in 2010. However, SPECT was the most often utilized test among those with CHD or chest pain: 38.2% in 2018, down from 45.0% in 2010, and exercise ECG second most often with 32.6% in 2018 and 34.9% in 2010. An increase in use of CT-CAC, PET/MPI, and CTA tests was observed since 2010, but the prevalence of all were low in 2018. 0.02%. 0.01%, and 0.04% for the overall population and 0.20%, 0.95% and 3.20% for those with IHD or chest pain, respectively. Conclusions: Almost 2% of the general younger US adult population and over half of those with IHD or chest pain had undergone at least one of 7 NITs in 2018. While the utilization of overall NITs for both the general population and those with IHD or chest pain has declined since 2010, it has increased for CT-CAC, PET/MPI, and CTA but still less than 1% for CT-CAC and PET/MPI..

Clopidogrel an oral thienopyridine derivative capable of inhibiting platelet activation. Clopidogrel is prodrug that is converted into an active drug by hepatic cytochrome CYP2C19, CYP2C19*2 and CYP2C19*3 polymorphic alleles are considered to be important loss- of- function alleles resulting in diminished response to clopidogrel. our study aimed to detect frequency of CYP2C19 gene polymorohisms and its impact on clinical outcome in ischemic heart disease patients taking clopidogrel. Matrial and methods: blood samples were collected from 102 ischemic heart disease patients and the frequency alleles was determined by PCR and all patients were followed by clinical assessment and invasive and non- invasive cardiac investigations .Results:. frequency of CYP2C19*1 was 49%, CYP2C19*2 was 15% and CYP2C19*3 was 1%, CYP2C19*17 was 34%, patients with recurrent ischemic attacks 37patients (35.8%), from those patients, 10 patients were normal metabolizer (27%), and 27 patients were abnormal metabolizers(73%) with p- value 0.047 to myocardial infarction and 0.020 to unstable angina. Conclusion:. Asignificance relation was found between CYP2C19polymorphism and recurrent ischemic attacks in this study and multicenter studies are required to confirm this results.

1 million 824 thousand people died in the Russian Federation in 2017, including 457 thousand from ischemic heart disease (IHD). IHD caused more than a quarter of deaths in Russia. Goal. The article analyzes the impact of implementation of clinical guidelines in cardiology in medical practice in the United States and the Russian Federation on the dynamics of mortality from ischemic heart disease and its outcomes in different age groups. Results. The results showed that the implementation of clinical guidelines (CG) increased the rate of mortality reduction from coronary heart diseases in Russia and the United States, which may indicate a positive impact CG on the quality of medical care. Conclusions. A higher level of mortality from coronary heart disease in Russia compared to the United States may be due to less commitment of doctors to the principles of therapy and diagnosis of various forms of coronary heart disease, set out in clinical guidelines.
