Implementation of unobtrusive sensing systems for elderly care: Scoping review (Preprint)

BACKGROUND The continuous growth of the elderly population will have implications for the organization of health and social care. Potentially, in-home monitoring unobtrusive sensing systems (USSs) can be used to support (in)formal caregivers of elderlies as they can monitor deviant physical and physiological behavior changes. Most of the existing USSs are not specific to elderly care. Hence, for facilitating the implementation of existing USSs in elderly care, it is important to know which USSs would be more suitable for elderlies. OBJECTIVE Therefore, this scoping review aims to examine the literature a) to identify current unobtrusive sensing systems (USSs) for monitoring human activities and behaviors and b) then assess them for implementation readiness for elderly care. METHODS A structured search was conducted in 'Scopus', 'Web of Science', and 'ACM digital library' databases. Predefined inclusion criteria included studies: on unobtrusive sensor-based technology; experimental in nature; aiming to monitor human social, emotional, physical, and physiological behavior; having potential to be scalable in in-home care; having at least 5 adults as participants. By using these criteria, studies were screened by title, abstract and full-text. A deductive thematic analysis based on the implementation framework of Proctor E. et al. (2011) along with additional outcome ‘external validity’ was applied to the included studies to identify the factors contributing to successful implementation. Lastly, identified factors were used to report the implementation readiness of included studies for elderly care. RESULTS 52 studies were included in the review. Deductive analysis using Proctor’s implementation framework resulted in six factors that can contribute to the successful implementation of USSs in elderly care. They are study settings, age of participants, activities monitored, sensor setup, sensing technology used, and usefulness of USSs. These factors were associated with the implementation outcomes as follows: study settings and age of participants contribute to external validity; sensor setup contributes to acceptability; usefulness of USSs contributes to adoption; activities monitored contributes to appropriateness; sensing technology used contributes to implementation cost. Further, the implementation assessment of included 52 studies showed that none of the studies has addressed all the identified factors. But this assessment was useful in highlighting studies that have addressed multiple factors, thus making such studies a step ahead in the implementation process. CONCLUSIONS This review is the first to scope the state-of-the-art USSs suitable for elderly care. Though included 52 USSs studies fulfills the basic criteria to be suitable for elderly care, but systems leveraging radio frequency technology in no contact sensor setup for monitoring life-risk or health wellness activities appear to be more fit in elderly care. Lastly, this review has extended the discussion on the term ‘unobtrusiveness’ as 'a property of systems which cannot be measured in binary as it varies a lot with user perception and context’.

ObjectiveWith an elderly population that is set to more than double by 2050 worldwide, there will be an increased demand for elderly care. This poses several impediments in the delivery of high-quality health and social care. Socially assistive robot (SAR) technology could assume new roles in health and social care to meet this higher demand. This review qualitatively examines the literature on the use of SAR in elderly care and aims to establish the roles this technology may play in the future.DesignScoping review.Data sourcesSearch of CINAHL, Cochrane Library, Embase, MEDLINE, PsychINFO and Scopus databases was conducted, complemented with a free search using Google Scholar and reference harvesting. All publications went through a selection process, which involved sequentially reviewing the title, abstract and full text of the publication. No limitations regarding date of publication were imposed, and only English publications were taken into account. The main search was conducted in March 2016, and the latest search was conducted in September 2017.Eligibility criteriaThe inclusion criteria consist of elderly participants, any elderly healthcare facility, humanoid and pet robots and all social interaction types with the robot. Exclusions were acceptability studies, technical reports of robots and publications surrounding physically or surgically assistive robots.ResultsIn total, 61 final publications were included in the review, describing 33 studies and including 1574 participants and 11 robots. 28 of the 33 papers report positive findings. Five roles of SAR were identified: affective therapy, cognitive training, social facilitator, companionship and physiological therapy.ConclusionsAlthough many positive outcomes were reported, a large proportion of the studies have methodological issues, which limit the utility of the results. Nonetheless, the reported value of SAR in elderly care does warrant further investigation. Future studies should endeavour to validate the roles demonstrated in this review.Systematic review registrationNIHR 58672.

In outpatient care the home care, including hospices, is recognized as a model for providing quality, cost-effective and charitable care. The focus is mainly on the care that helps everyday lifeof the patient as well as the relatives, rather than on treatment, and in most cases it takes place in the patients' home. In Europe, in recent years there has been a real "boom" in home care due to demographic processes linked to increased needs for elderly care and chronically ill under the conditions of limited financial resources.In outpatient medical care in our country by means of a national framework contract there are regulated visits to the patient's home by a doctor, as well as visits by medical staff employed by him - nurse, midwife, medical assistant / paramedic / for manipulation, counseling and monitoring. At the same time there is no regulated legal activity in the Republic of Bulgaria, which is essentially the subject of home care.Since 1994 „Caritas“ has carried out the "Home Care" service, which provides a complex - health and social care for over 360 sick adults in a place where the elderly person feels the most comfortable - in their own home. „Caritas Home Care“ is provided by mobile teams of nurses and social assistants who visit the elderly at home and provide them with the necessary care according to their health and social needs.With the establishment of the first „Home Care Center“ in Lozenets region, Sofia, with the support of the PHARE ACCESS program in 2003, the Bulgarian Red Cross introduces in Bulgaria an integrated model for provision of health care and social services in the home of adults, chronically ill and people with permanent disabilities. To date, there are a number of problems in home care related to the realization of home care for patients in need in out-of-hospital settings: lack of legal regulation for home care, lack of qualified staff in outpatient care; lack of organization and structures for care; unsettled funding and the inability of the part of the population that is most in need of care to pay for it, there is no regulation to control the activity. Although home care began over 20 years ago, our country is yet to make its way to the European program called „Home care in Europe“.

Abstract Background Mental health problems are common in the working population and represent a growing concern internationally, with potential impacts on workers, organisations, workplace health and compensation authorities, labour markets and social policies. Workplace interventions that create workplaces supportive of mental health, promote mental health awareness, destigmatise mental illness and support those with mental disorders are likely to improve health and economical outcomes for employees and organisations. Identifying factors associated with successful implementation of these interventions can improve intervention quality and evaluation, and facilitate the uptake and expansion. Therefore, we aim to review research reporting on the implementation of mental health promotion interventions delivered in workplace settings, in order to increase understanding of factors influencing successful delivery. Methods and analysis A scoping review will be conducted incorporating a stepwise methodology to identify relevant literature reviews, primary research and grey literature. This review is registered with Research Registry (reviewregistry897). One reviewer will conduct the search to identify English language studies in the following electronic databases from 2008 through to July 1, 2020: Scopus, PROSPERO, Health Technology Assessments, PubMed, Campbell Collaboration, Joanna Briggs Library, PsycINFO, Web of Science Core Collection, CINAHL and Institute of Occupational Safety and Health (IOSH). Reference searching, Google Scholar, Grey Matters, IOSH and expert contacts will be used to identify grey literature. Two reviewers will screen title and abstracts, aiming for 95% agreement, and then independently screen full texts for inclusion. Two reviewers will assess methodological quality of included studies using the Mixed Methods Appraisal Tool and extract and synthesize data in line with the RE-AIM framework, Nielson and Randall’s model of organisational-level interventions and Moore’s sustainability criteria, if the data allows. We will recruit and consult with international experts in the field to ensure engagement, reach and relevance of the main findings. Discussion This will be the first systematic scoping review to identify and synthesise evidence of barriers and facilitators to implementing mental health promotion interventions in workplace settings. Our results will inform future evaluation studies and randomised controlled trials and highlight gaps in the evidence base. Systematic review registration Research Registry (reviewregistry897)

Abstract Background: Providing alcohol screening and brief advice (SBA) in primary health care (PHC) can be an effective measure to reduce alcohol consumption. To aid successful implementation in an upper middle-income country context, this study investigates the perceived appropriateness of the programme and the perceived barriers to its implementation in PHC settings in three Latin American countries: Colombia, Mexico and Peru, as part of larger implementation study (SCALA). Methods: An online survey based on the Tailored Implementation for Chronic Diseases (TICD) implementation framework was disseminated in the three countries to key stakeholders with experience in the topic and/or setting (both health professionals and other roles, for example regional health administrators and national experts). In total, 55 respondents participated (66% response rate). For responses to both appropriateness and barriers questions, frequencies were computed, and country comparisons were made using Chi square and Kruskal–Wallis non-parametric tests. Results: Alcohol SBA was seen as an appropriate programme to reduce heavy alcohol use in PHC and a range of providers were considered suitable for its delivery, such as general practitioners, nurses, psychologists and social workers. Contextual factors such as patients’ normalised perception of their heavy drinking, lack of on-going support for providers, difficulty of accessing referral services and lenient alcohol control laws were the highest rated barriers. Country differences were found for two barriers: Peruvian respondents rated SBA guidelines as less clear than Mexican (Mann–Whitney U = −18.10, P = 0.001), and more strongly indicated lack of available screening instruments than Colombian (Mann–Whitney U = −12.82, P = 0.035) and Mexican respondents (Mann–Whitney U = −13.56, P = 0.018). Conclusions: The study shows the need to address contextual factors for successful implementation of SBA in practice. General congruence between the countries suggests that similar approaches can be used to encourage widespread implementation of SBA in all three studied countries, with minor tailoring based on the few country-specific barriers.

The path to improving healthcare quality for individuals with complex health conditions is complicated by a lack of common understanding of complexity. Modern medicine, together with social and environmental factors, has extended life, leading to a growing population of patients with chronic conditions. In many cases, there are social and psychological factors that impact treatment, health outcomes, and quality of life. This is the face of complexity. Care challenges, burden, and cost have positioned complexity as an important health issue. Complex chronic conditions are now being discussed by clinicians, researchers, and policy-makers around such issues as quantification, payment schemes, transitions, management models, clinical practice, and improved patient experience. We conducted a scoping review of the literature for definitions and descriptions of complexity. We provide an overview of complex chronic conditions, and what is known about complexity, and describe variations in how it is understood. We developed a Complexity Framework from these findings to guide our approach to understanding patient complexity. It is critical to use common vernacular and conceptualization of complexity to improve service and outcomes for patients with complex chronic conditions. Many questions still persist about how to develop this work with a health and social care lens; our framework offers a foundation to structure thinking about complex patients. Further insight into patient complexity can inform treatment models and goals of care, and identify required services and barriers to the management of complexity.

This study reviews obstacle detection technologies in vegetation for autonomous vehicles or robots. Autonomous vehicles used in agriculture and as lawn mowers face many environmental obstacles that are difficult to recognize for the vehicle sensor. This review provides information on choosing appropriate sensors to detect obstacles through vegetation, based on experiments carried out in different agricultural fields. The experimental setup from the literature consists of sensors placed in front of obstacles, including a thermal camera; red, green, blue (RGB) camera; 360° camera; light detection and ranging (LiDAR); and radar. These sensors were used either in combination or single-handedly on agricultural vehicles to detect objects hidden inside the agricultural field. The thermal camera successfully detected hidden objects, such as barrels, human mannequins, and humans, as did LiDAR in one experiment. The RGB camera and stereo camera were less efficient at detecting hidden objects compared with protruding objects. Radar detects hidden objects easily but lacks resolution. Hyperspectral sensing systems can identify and classify objects, but they consume a lot of storage. To obtain clearer and more robust data of hidden objects in vegetation and extreme weather conditions, further experiments should be performed for various climatic conditions combining active and passive sensors.

Information and communication technology (ICT) can potentially support older adults in making decisions and increase their involvement in decision-making processes. Although the range of technical products has expanded in various areas of society, knowledge is lacking on the influence that ICT has on older adults’ decision-making in everyday situations. Based on the literature, we aimed to provide an overview of the role of ICT in home-dwelling older adults’ decision-making in relation to health, and health and social care services. A scoping review of articles published between 2010 and 2020 was undertaken by searching five electronic databases. Finally, 12 articles using qualitative, quantitative, and mixed-method designs were included. The articles were published in journals representing biology and medicine, nursing, informatics, and computer science. A majority of the articles were published in the last five years, and most articles came from European countries. The results are presented in three categories: (i) form and function of ICT for decision-making, (ii) perceived value and effect of ICT for decision-making, and (iii) factors influencing ICT use for decision-making. According to our findings, ICT for decision-making in relation to health, and health and social care services was more implicitly described than explicitly described, and we conclude that more research on this topic is needed. Future research should engage older adults and health professionals in developing technology based on their needs. Further, factors that influence older adults’ use of ICT should be evaluated to ensure that it is successfully integrated into their daily lives.

Purpose – The purpose of this paper is to create new ideas for assistive technology products at home, especially products utilizing robotic consumer appliances available in the homes of elderly people. The work was founded on a reported increase in household robots as well as an ageing population in the industrialized world. Design/methodology/approach – Technology should be something that is perceived as belonging to our own world that fits our daily practices. Earlier studies show that in addition to cleaning functions, new household robots could change home routines and people's relationship to them. Taking the previous studies as a starting point, the paper proposes a vacuum cleaner robot as a platform for developing pervasive safety services and describe implementation of a conceptual prototype which brings the feeling of safety to an older person and their relatives by assisting in case of accidents. Moreover, the results are presented of an empirical evaluation of the prototype with end-users. Findings – It is proved that reasonably priced off-the-shelf components can be used to build the safety product demonstration model. The initial evaluation results, as well as referenced studies show that the acceptance rate of a household robot-based product is high, which is encouraging for further research in this domain. Also the paper could pinpoint areas that will require further work. Research limitations/implications – To add more practicality to the research and move towards product development, a strong industrial partner involved in household robotics would be needed. For increased reliability and robustness, more research is required in areas of advanced sensing technology and decision algorithms. Originality/value – A novel concept of a safety product for elderly care based on a vacuum cleaner robot is presented and an attempt is made to increase awareness that there will be a demand for such products.
