Purpose. To analyze results of questionnaire survey of naval medical service specialists on the question of processing of reporting documentation during the cruise.Materials and methods. Questionnaire survey of chief medical officers, analysis of documents, Fisher angular transformation.Results and discussion. Quantitative and qualitative analysis of questionnaires was conducted. The analysis showed that the reporting form shall be simplified and the chief medical officer should be given greater freedom in analyzing various aspects of the medical support of the cruise (the report should be less formalized).Conclusion. Introduction of new reporting forms will allow to evaluate the efficiency of medical component during cruise and to give scientifically based recommendations to military equipment designers, ergonomists, and fleet command on issues of optimization of habitability specifications, implementation of new life-support technologies and lowering the accident rate on naval facilities.

One of the main directions of logistic support of the Armed Forces of Ukraine is medical support which is aimed at life safeguard and health of military personnel, well-timed and efficient provision of medical care in the event of injuries, injuries and diseases, fast renewal of combat power and ability to work in different conditions of its handling and at different period of time. The readiness of medical service of the Armed Forces of Ukraine to the medical support of the troops depends on the well-timed organization of medical supplies and the refilling of medical supplies full and complete. The qualitative and efficient allocation of medical equipment and medical supplies is a determining factor in this process. The aim of the work – reasoning of the irregularity of distribution logistics in the unified logistic system of the Armed Forces of Ukraine and the study of the main aspects of management of the distribution of medical supplies from the position of military pharmaceutical logistics. To get the purpose of the research, the analysis of domestic scientific sources and the current normative and legal base of Ukraine have been carried out. Research methods are: information search, comparison, systemization, semantic analysis, synthesis and modeling. In the conditions of logistic support of the Armed Forces of Ukraine and other components of the security and defense forces of Ukraine, the logistic function of the distribution of logistical equipment, inventory, military equipment and services (food, bath and laundry services, etc.) should ensure the activity of troops (forces) in peacetime and war time. It is the guarantee of the ability to ensure the defense of the state and appropriate response to military threats to the national security of Ukraine, efficiently using available potential and resources. Distribution logistics from the position of military pharmaceutical logistics is the management of all functional subsystems of the medical supplies and health services of the Ministry of Defense of Ukraine in order to optimize the distribution of the material flow (purchased medical supplies) from the supplier to the final consumer (according to the applications of the Military Medical Clinical Centers of the Regions). Same, distributive logistics in the structure of a unified logistic support system of the Armed Forces of Ukraine does not carry a commercial component, and its functions consist in planning, organization of distribution, control and regulation of the logistic process of distribution of material and technical means (including medical equipment and supplies), inventory, military equipment, services (catering, bath and laundry service, etc.). According to the authors, the term «marketing logistics» is not possible to use as a single system of logistic support of the Armed Forces of Ukraine, and in its subsystems. To sum it up, it was suggested to use the term «distributive logistics». On the basis of theoretical generalization of the data of scientific literature and normative legal acts, the main aspects of management of the distribution of medical equipment and supplies from the standpoint of military pharmaceutical logistics are studied. The functions of distributive logistics are defined, the model of the current logistic chain of distribution of medical equipment and supplies in the medical service of the Armed Forces of Ukraine is constructed. It is noted that in order to ensure the qualitative and efficient distribution of medical equipment and supplies, a regulatory and legal framework which regulates the division of powers between institutions and units of medical supplies and defines the responsibility for their implementation in the current conditions of the reformation of the Armed Forces of Ukraine.

Abstract Background This work was to study the prehospital time among suspected stroke patients who were transported by an emergency medical service (EMS) system using a national database. Methods National EMS database of suspected stroke patients who were treated by EMS system across 77 provinces of Thailand between January 1, 2015, and December 31, 2018, was retrospectively analyzed. Demographic data (i.e., regions, shifts, levels of ambulance, and distance to the scene) and prehospital time (i.e., dispatch, activation, response, scene, and transportation time) were extracted. Time parameters were also categorized according to the guidelines. Results Total 53,536 subjects were included in the analysis. Most of the subjects were transported during 06.00-18.00 (77.5%) and were 10 km from the ambulance parking (80.2%). Half of the subjects (50.1%) were served by advanced life support (ALS) ambulance. Median total time was 29 min (IQR 21, 39). There was a significant difference of median total time among ALS (30 min), basic (27 min), and first responder (28 min) ambulances, Holm P = 0.009. Although 91.7% and 88.3% of the subjects had dispatch time ≤ 1 min and activation time ≤ 2 min, only 48.3% had RT ≤ 8 min. However, 95% of the services were at the scene ≤ 15 min. Conclusion Prehospital time from EMS call to hospital was approximately 30 min which was mainly utilized for traveling from the ambulance parking to the scene and transporting patients from the scene to hospitals. Even though only 48% of the services had RT ≤ 8 min, 95% of them had the scene time ≤ 15 min.

Abstract From June 28 to November 22, 2018, the Chinese People’s Liberation Army Navy – PLA(N) – Peace Ark hospital ship had conducted Mission Harmony 2018, providing humanitarian medical assistance and carrying out international cooperation, in 4 Pacific island countries and 6 Central and South American countries. Compared with its application only in onshore outreach medical teams in the previous Mission Harmony, portable ultrasonography was used both onboard and onshore in Mission Harmony 2018. The purpose of this study was to assess the performance of onboard portable ultrasonography in PLA(N) Peace Ark hospital ship during Mission Harmony-2018, share our onboard working experience, and provide a reference for humanitarian assistance missions in the future. A retrospective review was performed on a cohort of patients checked by onboard portable ultrasonography. Patients’ gender, age, the distribution of examined organs, and multiple applications of the portable ultrasonography were analyzed. Some limitations of portable ultrasonography on the mission and possible improvements in the future were also discussed. A total of 5277 cases (mean age: 43.74 years; range: 2 months–105 years) of ultrasound examinations were performed during the mission; among them, 3126 (59.2%) cases were performed by portable ultrasonography, including 3024 onboard cases and 102 onshore cases. The portable ultrasonography had been applied in many scenarios, for example, onboard emergency triage process, onboard bedside medical support, and onshore outreach medical service, which had become one of the indispensable auxiliary examination methods for its compatibility, portability, and flexibility. The onboard deployment of portable ultrasonography played a versatile and irreplaceable role in the humanitarian medical assistance and medical cooperation carried out by the PLA(N) Peace Ark hospital ship, and will contribute to such kind of missions in the future.

Aim. Analysis of the results of parents' assessment of the conditions of US diagnostics of children. Materials and Methods. Assessment of the conditions for receiving ultrasound examination, based on the results of a questionnaire survey of 424 parents of children who underwent this medical service at the National Medical Research Center of Children's Health of the Ministry of Health of Russia, is presented. Mathematical processing of the obtained data was carried out using the methods of variation statistics and correlation analysis. Results. 83.5% of parents surveyed were satisfied with the ultrasound diagnostic services provided to their children, 12.3% – were satisfied only partially. According to respondents’ opinions, the main reasons for dissatisfaction were insufficient qualification of specialist and low quality of the equipment used. The majority of individuals participating in the survey, considered waiting for ultrasound not long, in 77.6% of cases it was carried out within 1-2 days after the appointment. Ultrasonography in an outpatient clinic and in a hospital was mainly carried out free of charge, as part of compulsory medical insurance – in 86.1 and 87.5% of cases, respectively. Only 14.3% of respondents noted that they had the financial ability to pay for examination of the child from personal funds. It was found that 12.7% of parents had to refuse, for this or that rea-son, from the proposed ultrasound methods of examination of the child. Conclusion. The vast majority of the parents surveyed were satisfied with the ultrasound diagnostics services provided to their child. The main reasons for refusing from ultrasound were a sharply negative reaction of the child, as well as the opinion about the possible harm of the examination and the expected pain from the procedure.

His ideas laid the foundation of well-managed system of modern medical support services of troops. The article reveals some aspects of the formation of structure of the Medical Service management in the war.

We describe the case of a 49-year-old woman with a Tramadol intoxication associated with multiorgan failure. Veno-arterial femoro-femoral extracorporeal life support (VA-ECLS) and hemoperfusion (HP) were used as rescue treatments. The emergency medical service found a woman at home unconscious. Once in the hospital, she was intubated and catecholamines support was immediately started for a severe shock. Brain CT was normal, whereas EEG revealed a metabolic encephalopathy pattern. Toxic levels of Tramadol and Quetiapine were detected. VA-ECLS was implanted due to persistent multiorgan failure, and HP with a charcoal cartridge was set to increase the Tramadol clearance. To quantify the charcoal cartridge’s removal efficiency of Tramadol, Tramadol concentration was measured before and after the cartridge and before and after the treatment in the patient’s blood. The charcoal cartridge showed good extraction ratio during the treatment and no significant rebound effect. VA-ECLS and HP allowed the patient to be weaned from vasoconstrictors and the resolution of the organ failures. These treatments might be lifesaving in the Tramadol intoxication.

Introduction:In August 2018, Kerala, India witnessed its worst flood in over a century. With the support of the national health mission, Operation Navajeevan, a public-private partnership between the district health administration and local hospitals was established in Kozhikode to provide medical aid to flood victims. This study identifies prerequisites, describes challenges, and depicts the epidemiology of patients seen in these camps.Aim:1.Identify prerequisites and medical needs/challenges faced by medical relief camps in a flood-affected region2.Formulate protocols to avoid duplication of services3.Prepare an ideal PPP emergency medical camp modelMethods:A control center with drugs and a logistics unit was set up at the district administration to monitor and supervise various camps. A mobile medical documentation format was created to record the details of each camp. Cases of patients seen at these camps were compiled and later analyzed. The medical officer sent reports from each camp to the control center each day to specify the daily difficulties faced by each camp. Mobile ICUs were kept on standby to respond in the event of emergent circumstances or surge demands. Transfer protocol and treatment guidelines were formulated and standardized.Results:Over two weeks, approximately 40,000 patients were seen in 280 medical camps. Major medical issues included exacerbation of chronic illnesses due to loss of medications (18,490), acute respiratory infections (7,451), psychiatric illnesses (5,327), trauma (3,736), skin infection (792), tropical fever (498), acute gastroenteritis (394), and ACS (17). Of the cases of fever, 137 people had leptospirosis. Major challenges included a lack of training in disaster management and failure of documentation systems.Discussion:A well-organized control center, improved training in disaster medicine, and reliable documentation systems are crucial for coordinating medical camps in disaster areas. Public-private partnerships offer a model for providing medical relief in disaster settings.

Abstract Background Palliative care is typically performed in-hospital. However, Emergency Medical Service (EMS) providers are uniquely positioned to deliver early palliative care as they are often the first point of medical contact. The aim of this study was to gather the perspectives of advanced life support (ALS) providers within the South African private EMS sector regarding pre-hospital palliative care in terms of its importance, feasibility and barriers to its practice. Methods A qualitative study design employing semi-structured one-on-one interviews was used. Six interviews with experienced, higher education qualified, South African ALS providers were conducted. Content analysis, with an inductive-dominant approach, was performed to identify categories within verbatim transcripts of the interview audio-recordings. Results Four categories arose from analysis of six interviews: 1) need for pre-hospital palliative care, 2) function of pre-hospital healthcare providers concerning palliative care, 3) challenges to pre-hospital palliative care and 4) ideas for implementing pre-hospital palliative care. According to the interviewees of this study, pre-hospital palliative care in South Africa is needed and EMS providers can play a valuable role, however, many challenges such as a lack of education and EMS system and mindset barriers exist. Conclusion Challenges to pre-hospital palliative care may be overcome by development of guidelines, training, and a multi-disciplinary approach to pre-hospital palliative care.
