Potential of zoonotic gastrointestinal nematode infection from livestock in Indonesia is still often overlooked. Farms with a risk for nematodes infection would create a risk of infecting the local community with zoonotic gastrointestinal nematodes. This study aimed to assess the risk of gastrointestinal nematodes from goats that have zoonotic potential in Cibeureum Wetan, Sumedang, and to identify the incidence of nematodes infection among goats. This was a cross-sectional study conducted in August to November 2019 with a total of 52 samples of feces collected directly from goat’s rectum to prevent soil contamination. Sampling was performed randomly from goats raised at the Agriculture and Self-Sustaining Village Training Center (Pusat Pelatihan Pertanian dan Pedesaan Swadaya, P4S) Simpay Tampomas, Sumedang, Indonesia. The GPS point of the sampling location was recorded. Samples were then examined using the concentration sedimentation method at the Parasitology Laboratory, Faculty of Medicine, Universitas Padjadjaran. Results showed that 22 of 52 samples were positive for gastrointestinal helminth eggs, contained of Bunostomum sp., Strongyloides sp., Haemmonchus sp., Trichostrongylus sp., Toxocara sp. and Trichuris sp. The nematode parasites found are parasites that often infect goats.

Increasing resistance of gastro-intestinal nematode parasites to anthelmintics and consumer resistance to the possibility of residues in animal products have prompted research on the effect of pasture species on nematodes and animal performance. Lambs (either infected with high rates of gastrointestinal nematodes or maintained nematode-free) were grazed on pure swards of chicory, high- or low-endophyte ryegrass, cocksfoot, tall fescue, lucerne, lotus, white clover or plantain. Infected lambs that grazed chicory had lower faecal egg counts and adult nematode populations, and higher carcass weights, than lambs grazed on plantain or the grass species; lambs that grazed legumes generally had intermediate counts, populations and weights. When kept parasite-free, carcass weights were up to 48% greater than in the nematodeinfected treatments. On farmlets run over 3 years, substituting 30% of the ryegrass area with lucerne or replacing the ryegrass with a multi-species mix consisting predominantly of bromes, tall fescue, phalaris, timothy and red and white clover, had no effect on gastrointestinal nematode larvae, lamb faecal worm egg or adult nematode numbers. It is concluded that a diet of pure chicory affects internal parasite populations but the small proportion included in the farmlet studies had no effect. Abstract This cross-sectional study explores the different conditions related to the infestation of ticks in households and the potential risks for rickettsial transmission in Urabá, Colombia. The main outcome of interest was villagers’ perception of tick infestation. The data were analyzed using a clog-log mixed regression model. Ticks were collected from infested humans to diagnose infection by spotted fever group rickettsiae (SFGR). In addition, a thematic analysis of qualitative data from key informants concerning knowledge about ticks was conducted. The prevalence of infestation of ticks in households was estimated at 60.99% (95% CI: 51.58–93.51). The multivariate model suggested that households with palm leaf roofs (PR = 1.95; 95% CI: 1.19–2.95), canines (PR = 1.76; 95% CI: 1.21–2.46), rats (PR = 2.19; 95% CI: 1.45–3.08), and with the presence of opossums in areas surrounding the households (PR = 1.51; 95% CI: 1.05–2.10) had a higher prevalence of tick infestation. Two samples of the tick species Amblyomma patinoi were found infected with Rickettsia amblyommatis and Candidatus Rickettsia colombianensi. A thematic analysis provided the names that local community members give to ticks, areas where ticks are common, and the individuals at risk of infestation. The presence of domestic, synanthropic, and wild animals suggests a high risk of the dissemination of ticks inside dwellings and close to them in these rural areas.

IntroductionAnalyses of large sets of electronic health-related data (Big Data), including local community indicators, may improve knowledge of the outcomes of chronic diseases among patients and healthcare systems. Our study will estimate the prevalence of chronic obstructive pulmonary disease (COPD) and its exacerbations in elderly patients in the Lodz region, Poland; it will also evaluate local community factors potentially associated with disease exacerbations and rank local communities according to health and local community indicators.Methodsand analysisLocal community factors, including medical/health, socioeconomic and environmental values potentially associated with COPD exacerbations will be identified. A retrospective analysis of a cohort of about half a million people 65 years old and older, living in local communities of the Lodz region in 2016 will be performed. Relevant data will be extracted from databases, including those of the National Health Fund, Tax Office and National Statistics Centre. This cross-sectional study will include data for a 1 year period, from 1 January until 31 December 2016. The data will first be checked for quality, cleaned and analysed using data mining techniques, and then multilevel logistic regression will be used to discover the community determinants of COPD exacerbations.Ethics and disseminationThe study protocol has been approved by the Bioethical Committee of Medical University of Lodz (RNN/248/18/KE, 10 July 2018). Our findings will be published in peer-reviewed journals and reports.

This cross sectional study explored the health problems and health needs in the local Chinese community in Glasgow. Several data collection methods have been used in this study, including face-to-face and telephone structured interviews, postal and hand delivered questionnaires. A total of 800 questionnaires were processed, and 493 were completed, giving an overall response rate 61.6%. The results from the present survey indicated that the health status of Chinese residents in Glasgow is poorer than that of the local population. The most important findings of the study is that the Chinese community in Glasgow underuse health services, and unmet health needs exist in the community. The main barrier to effective use of present health services and benefit from the health promotion and health education programmes is language difficulties. Following discussion with the local community, options for improving the health services for the Chinese community in Glasgow were obtained. The findings of the study have implications for health service purchaser/providers of health care to the Chinese population generally in Scotland.

In this review, the main gastrointestinal nematodes infecting cattle in Zimbabwe and the epidemiological factors influencing their occurrence are reviewed and discussed. Nineteen gastrointestinal nematode species that belong to seven families have been found to occur in cattle in Zimbabwe. The main genera reported to date are Cooperia, Haemonchus, Trichostrongylus and Oesophagostomumand the dominant species are Cooperia pectinata, Cooperia punctata, Haemonchus placei and Trichostrongylus axei. The mixed infection by several species from the genera is the cause of parasitic gastroenteritis in cattle in Zimbabwe. Production and husbandry practices, season, host age and environment are considered to be the main factors that influence gastrointestinal nematode infection in cattle. The geographical distribution of the gastrointestinal nematodes is also reviewed in relation to the climatic conditions of the country. Various control options are discussed and how they are applicable to the Zimbabwean situation. Based on reports and existing data on the epidemiological features of the gastrointestinal nematode infection in cattle, practical control measures are critically reviewed and recommendations are made for a national control programme.

Background: A motivated workforce is necessary to ensure the delivery of high quality health services. In developing countries, performance-based financing (PBF) is often employed to increase motivation by providing financial incentives linked to performance. However, given PBF schemes are usually funded by donors, their long-term financing is not always assured, and the effects of withdrawing PBF on motivation are largely unknown. This cross-sectional study aimed to identify differences in motivation between workers who recently had donor-funded PBF withdrawn, with workers who had not received PBF. Methods: Quantitative data were collected from 485 health workers in 5 provinces using a structured survey containing questions on motivation which were based on an established motivation framework. Confirmatory factor analysis was used to verify dimensions of motivation, and multiple regression to assess differences in motivation scores between workers who had previously received PBF and those who never had. Qualitative interviews were also carried out in Kasai Occidental province with 16 nurses who had previously or never received PBF. Results: The results indicated that workers in facilities where PBF had been removed scored significantly lower on most dimensions of motivation compared to workers who had never received PBF. The removal of the PBF scheme was blamed for an exodus of staff due to the dramatic reduction in income, and negatively impacted on relationships between staff and the local community. Conclusion: Donors and governments unable to sustain PBF or other donor-payments should have clear exit strategies and institute measures to mitigate any adverse effects on motivation following withdrawal.

It is also lately emerging tourism concept in the world. Nepal is known as an important tourist’s destination and can be a model homestay country with its abundant natural resources, multi culture, multi religion, multi ethnic, tradition life style, archeological heritage, innocent society and community. Due to all these above factors homestay tourism program became the dream for the lover of nature and experts of tourism in Nepal. Homestay program can be easily established, Promoted and developed by minimum investment and based on many aspects such as good service, safety, security, souvenir, family and attractions. It is a major tourism industry for the development of rural economy by generating huge number of employment and social benefits. The major objective of the study is the perception of community towards homestay tourism program. Descriptive and exploratory research design, cross-sectional study design and deductive approach with mixed method have been used in this study. Both primary and secondary data are used simultaneously. Primary data are collected from 348 respondents with three categories of nine study Districts. Similarly, secondary data are collected from various publications of central, state, local Governments, different articles in international journals, books, credible magazines, newspaper, reports prepared by research scholars, experts and universities and different websites etc. Finally, majority of community people have a positive perception towards on existing homestay program in Nepal.

AbstractGastrointestinal nematodes (GINs) of humans, e.g., hookworms, negatively impact childhood growth, cognition, nutrition, educational attainment, income, productivity, and pregnancy. Hundreds of millions of people are targeted with mass drug administration (MDA) of donated benzimidazole (BZ) anthelmintics. However, BZ efficacy against GINs is suboptimal, and reduced/low efficacy has been seen. Developing an anthelmintic for human MDA is daunting: it must be safe, effective, inexpensive, stable without a cold chain, and massively scalable. Bacillus thuringiensis (Bt) crystal protein 5B (Cry5B) has anthelmintic properties that could fill this void. Here we develop an API (Active Pharmaceutical Ingredient) form of Bt Cry5B compatible with MDA. We expressed Cry5B in asporogenous Bt during vegetative phase, forming cytosolic crystals. These Bacteria with Cytosolic Crystals (BaCC) were rendered inviable (inactivated BaCC or IBaCC) with food-grade essential oils. IBaCC potency was validated in vitro against nematodes. IBaCC was also potent in vivo against human hookworm infections in hamsters. IBaCC production was successfully scaled to 350 liters at a contract manufacturing facility. A simple fit-for-purpose formulation to protect against stomach digestion and powdered IBaCC were successfully made and used against GINS in hamsters and mice. A pilot histopathology study and blood chemistry workup showed that five daily consecutive doses of 200 mg/kg Cry5B IBaCC (the curative single dose is 40 mg/kg) was non-toxic and completely safe. IBaCC is a safe, inexpensive, highly effective, easy-to-manufacture, and scalable anthelmintic that is practical for MDA and represents a new paradigm for treating human GINs.

BBackground. The TB case detection rate in Somalia is 42%, which is much lower than the WHO target of detecting 70% of new TB cases. Understanding the factors contributing to the delay of TB patients in the diagnosis, and reducing the time between the onset of TB symptoms to diagnosis, is a prerequisite to increase the case detection rate and to ultimately bring the TB epidemic in Somalia under control. The aim of this study is to examine the duration of delay, and factors associated with the delay among patients in TB management centers in Mogadishu, Somalia. Methods. An institution-based, cross-sectional study was conducted in TB management clinics providing directly observed treatments (DOTS) programs in Mogadishu. A total of 276 patients were interviewed using a structured questionnaire from June-October 2018. We analyzed data using descriptive statistics and different logistic regression models. Results. Approximately 78% of study participants were male. Nearly a third (36.5%) came from a household of nine individuals or more, while 73% were unemployed. The median patient and provider delays were 50 days and one day, respectively. The median total delay was 55 days, with an inter-quartile range of 119 days. Patients who had a poor knowledge of the symptoms of TB had 3.16 times higher odds of delay over 50 days than their counterparts. Furthermore, a poor knowledge of the symptoms of TB (aOR 4.22, CI 2.13-8.40), not making ones own decisions in seeking TB treatment (aOR 2.43, CI 1.22-4.86) and a poor understanding of the fact that TB can be treated with biomedical treatment, as opposed to traditional treatment (aOR 2.07, CI 1.02-4.16), were predictors of a patient delay over 120 days. Conclusions. The duration in the delay of TB patients under diagnosis in Mogadishu is one of the highest reported in developing countries, exceeding two years in some patients. Training local community health workers to detect suspected TB cases, and referring the cases of prolonged cough over three weeks for TB care centers for diagnosis, is imperative to help break the transmission and reduce the infectious pool in the population of Mogadishu. This may not only increase the community awareness of TB disease, but it may also facilitate the early referral of TB patients to diagnostic and treatment care centers.
