It was studied the open bite prevalence at four different steps of human dentition by epidemiological research in the school population in Porto Alegre. The open bite prevalence was bigger in the temporary dentition (38,11%) than in permanent or mixed dentition. ln the young permanent dentition the prevalence was 8,09% and 4,35% in the permanent one. It was verified by comparative study of the four evolution stages of human dentition, the great reduction of open bite prevalence from temporary to permanent dentition.

Abstract Objective: Considering that the available studies on prevalence of malocclusions are local or national-based, this study aimed to pool data to determine the distribution of malocclusion traits worldwide in mixed and permanent dentitions. Methods: An electronic search was conducted using PubMed, Embase and Google Scholar search engines, to retrieve data on malocclusion prevalence for both mixed and permanent dentitions, up to December 2016. Results: Out of 2,977 retrieved studies, 53 were included. In permanent dentition, the global distributions of Class I, Class II, and Class III malocclusion were 74.7% [31 - 97%], 19.56% [2 - 63%] and 5.93% [1 - 20%], respectively. In mixed dentition, the distributions of these malocclusions were 73% [40 - 96%], 23% [2 - 58%] and 4% [0.7 - 13%]. Regarding vertical malocclusions, the observed deep overbite and open bite were 21.98% and 4.93%, respectively. Posterior crossbite affected 9.39% of the sample. Africans showed the highest prevalence of Class I and open bite in permanent dentition (89% and 8%, respectively), and in mixed dentition (93% and 10%, respectively), while Caucasians showed the highest prevalence of Class II in permanent dentition (23%) and mixed dentition (26%). Class III malocclusion in mixed dentition was highly prevalent among Mongoloids. Conclusion: Worldwide, in mixed and permanent dentitions, Angle Class I malocclusion is more prevalent than Class II, specifically among Africans; the least prevalent was Class III, although higher among Mongoloids in mixed dentition. In vertical dimension, open bite was highest among Mongoloids in mixed dentition. Posterior crossbite was more prevalent in permanent dentition in Europe.

Summary Background Cross-sectional studies of mandibular movements provide data on developmental trends of dentition and support planning of public health services. Objective The aim of this study was to measure mandibular movement capacities in children with deciduous and mixed dentition and in young adults with permanent dentition. The influence of age and gender on mandibular movements and the association between mandibular movements and occlusal traits were analysed. Method The sample consisted of 1172 Estonians: group 1: children with deciduous dentition; group 2: children with mixed dentition; and group 3: young adults with permanent dentition. Maximum opening, mandibular laterotrusion, and protrusion were registered. Results Age was correlated with mandibular movements. Young adults had statistically significantly larger mandibular movements as compared to children with deciduous and mixed dentition, and children with mixed dentition had larger mandibular movements as compared to children with deciduous dentition. Young adult males had larger mandibular movements than females of the same age. Associations were found between mandibular movement capacities and some occlusal traits. Mandibular movement capacities were smaller in children with crossbite and open bite as compared with children without corresponding occlusal traits. Mandibular movement capacities were larger in children with deep bite and increased overjet as compared with those without corresponding occlusal traits. Conclusion Mandibular movement capacities are age and gender dependent. Maximum mouth opening, mandibular laterotrusion, and protrusion are related, and mandibular movement and some occlusal traits are associated.

Abstract Background Primary failure of eruption (PFE) is a hereditary condition, and linkage with variants in the PTH1R gene has been demonstrated in many cases. The clinical severity and expression of PFE is variable, and the genotype–phenotype correlation remains elusive. Further, the similarity between some eruption disorders that are not associated with PTH1R alterations is striking. To better understand the genotype–phenotype correlation, we examined the relationship between the eruption phenotype and PTH1R genotype in 44 patients with suspected PFE and 27 unaffected relatives. Sanger sequencing was employed to analyze carefully selected PFE patients. Potential pathogenicity of variants was evaluated against multiple genetic databases for function prediction and frequency information. Results Mutational analysis of the PTH1R coding sequence revealed 14 different variants in 38 individuals (30 patients and 8 first-degree relatives), 9 exonic and 5 intronic. Their pathogenicity has been reported and compared with the number and severity of clinical signs. In 72.7% of patients with pathogenic variants, five clinical and radiographic criteria have been found: involvement of posterior teeth, involvement of the distal teeth to the most mesial affected, supracrestal presentation, altered vertical growth of the alveolar process and posterior open-bite. In cases with mixed dentition (3), the deciduous molars of the affected quadrant were infraoccluded. Discussion The probability of an affected patient having a PTH1R variant is greater when five specific clinical characteristics are present. The likelihood of an eruption defect in the absence of specific clinical characteristics is rarely associated with a PTH1R mutation. Conclusions We report here that systematic clinical and radiographic observation using a diagnostic rubric is highly valuable in confirming PFE and offers a reliable alternative for accurate diagnosis.

OBJECTIVE: To determine the prevalence and severity of occlusal problems in populations at the ages of deciduous and permanent dentition and to carry out a meta-analysis to estimate the weighted odds ratio for occlusal problems comparing both groups. METHODS: Data of a probabilistic sample (n=985) of schoolchildren aged 5 and 12 from an epidemiological study in the municipality of São Paulo, Brazil, were analyzed using univariate logistic regression (MLR). Results of cross-sectional study data published in the last 70 years were examined in the meta-analysis. RESULTS: The prevalence of occlusal problems increased from 49.0% (95% CI =47.4%-50.6%) in the deciduous dentition to 71.3% (95% CI =70.3%-72.3%) in the permanent dentition (p<0.001). Dentition was the only variable significantly associated to the severity of malocclusion (OR=1.87; 95% CI =1.43-2.45; p<0.001). The variables sex, type of school and ethnic group were not significant. The meta-analysis showed that a weighted OR of 1.95 (1.91; 1.98) when compared the second dentition period with deciduous and mixed dentition. CONCLUSIONS: In planning oral health services, some activities are indicated to reduce the proportion of moderate/severe malocclusion to levels that are socially more acceptable and economically sustainable.

BackgroundEpidemiological data on malocclusion among Chinese children are scant. The aim of this study was to provide detailed information on the prevalence of malocclusion in early mixed dentition children in Shanghai, China.MethodsA cross-sectional survey was conducted from September 2016 to April 2017, and 2,810 children aged 7- to 9- years were selected from 10 primary schools by cluster random sampling. Several occlusal parameters, including Angle molar relationship, overjet, overbite, open bite, anterior and posterior crossbite, midline displacement, scissors bite, and teeth crowding and spacing, were clinically registered by five calibrated orthodontic dentists.ResultsWe found that 79.4% children presented one or more occlusal anomalies. Angle Class I, Class II and Class III molar relationship were recorded in 42.3%, 50.9% and 5.9% of the sample, respectively. The proportion of Class III increased from 5.0% at age 7 to 7.8% at age 9. In the sagittal plane, increased overjet >3 mm was observed in 40.8% subjects, while the prevalence of severe overjet (>8 mm), anterior edge-to-edge (zero overjet) and anterior crossbite were 5.2%, 8.1% and 10.5%, respectively. Vertically, deep overbite >2/3 overlap was found in 6.2% of the children and open bite in 4.3%. Boys exhibited a higher rate of overbite than girls. For the transversal occlusal anomalies, 36.1% of the children had a midline displacement, which was followed by posterior crossbite (2.6%) and scissors bite (1.0%). Teeth space discrepancies were also common anomalies and anterior crowding (>2 mm) affecting 28.4% of the children, while anterior spacing (>4 mm) affecting 9.5%. Girls showed a higher prevalence of anterior crowding and a lower frequency of teeth spacing than boys.ConclusionsOur study demonstrated that malocclusion is prevalent among children in the early mixed dentition, and more health resources should be warranted to meet the challenge of prevention or early intervention of malocclusion.

ABSTRACTSupernumerary teeth occur frequently in permanent dentition, but they are rarely found in primary dentition. Supernumerary teeth of orthodox shape and size that resemble normal dentition are called ‘supplemental teeth’. Supplemental teeth are less common than supernumerary teeth and are often overlooked because of their normal shape and size. Supplemental teeth may cause esthetic problems, delayed eruption and crowding, and they require early diagnosis and treatment to prevent complications. The case reported here is one of bilateral supplemental teeth impeding the eruption of permanent maxillary lateral incisors, and it emphasizes the importance of early diagnosis and treatment during early mixed dentition. Case Report. An 8-year-old girl with amelogenesis imperfecta (AI) reported unsatisfactory aesthetics, difficulty in mastication, and dental hypersensitivity. The intraoral examination observed mixed dentition, malocclusion in anteroposterior relationships, anterior open bite, and dental asymmetry. A hypoplastic form of AI was diagnosed in the permanent dentition. A multidisciplinary planning was performed and divided into preventive, orthopedic, and rehabilitation stages. Initially, preventive treatment was implemented, with fluoride varnish applications, in order to protect the fragile enamel and reduce the dental sensitivity. In the second stage, the patient received an interceptive orthopedic treatment to improve cross-relationship of the arches during six months. Finally, the rehabilitation treatment was executed to establish the vertical dimension. In the posterior teeth, indirect composite resin crowns were performed with minimally invasive dental preparation. Direct composite resin restorations were used to improve the appearance of anterior teeth.Follow-Up. The follow-up was carried out after 3, 6, 12, and 18 months. After 18 months of follow-up, The restoration of integrity, oral hygiene, and patient satisfaction were observed .Conclusion. Successful reduction of the dental hypersensitivity and improvement of the aesthetic and functional aspects as well as quality of life were observed.

Introduction. Dental crowding complicates sagittal, vertical and transversal types of malocclusion and it is accompanied by aesthetic, functional and morphological disorders even on the early stages of occlusion formation. Aim of the study. The aim of the study is to carry out follow up study of occlusion state of one schoolchildren group in period from temporary to permanent dentition in order to determine in age aspect frequency and mechanism of development of malocclusions, complicated by dental crowding. Materials and methods. Follow up study of occlusion state in 434 schoolchildren of Poltava aged from 6 to 11 years old (girls – 246 (56,68%), boys – 188 (43,32%) was performed during six years. Results. The follow-up of one contingent of children confirms high (79,96%) prevalence of malocclusion in mixed dentition, which increases during permanent dentition to an average of 84,33%, that is to 4,37%. Orthodontic pathology in permanent dentition becomes more expressed and severe. Malocclusions complicated by dental crowding are the most widespread types, their frequency doubles within six years, mixed dentition – 33,54% permanent dentition – 68,67%. (Rχ2 lower than 0,05). There is no statistically significant difference in factors of dental crowding presence in different types of malocclusion (Рχ2> 0,05). Conclusions. Frequency of the sagittal, vertical and transversal forms of malocclusion complicated by dental crowding was 33.54% in mixed dentition , and in six years it increased by 68,67% in permanent dentition.

Changes in position and mesiodistal angulations of maxillary and mandibular first permanent molars from early mixed dentition to early permanent dentition were measured on panoramic radiographs and compared with the values measured on lateral cephalometric radiographs. It was found that the maxillary and mandibular first permanent molars were uprighted gradually, drifted mesially and vertically, and that the changes in mesiodistal angulations were clearly on the panoramic radiographs in comparison with the cephalometric radiographs.

ABSTRACT Objective:  To evaluate the overbite correction of fixed palatal crib (FPC) and bonded lingual spur (BLS) in the early treatment of anterior open bite (AOB) in mixed dentition (primary outcome) as well as its influence on dental and skeletal cephalometric measurements (secondary outcome). Materials and Methods:  The selected patients had AOB and a mean age of 8.23 years. They were divided into the following three groups by casting lots: control (n  =  13), palatal crib (n  =  13), and spur (n  =  13). Data from the lateral teleradiography was obtained at the beginning, at 6 months, and after 1 year. The cephalometric analysis was performed by Cef-X program, recording the values of SNA, SNB, ANB, SnG oGn, 1.PP, IMPA, nasolabial angle, overbite, and overjet. Intergroup and intragroup comparisons were obtained via one-way analysis of variance. Results:  The degree of AOB was similar at baseline (P &gt; .05). At 6 months and then after 1 year all groups showed improvement in the overbite. However, only the crib and spur groups showed positive overbite. No cephalometric measurements changed significantly over the period analyzed. Conclusions:  We conclude that the FPC and BLS are simple and effective for the treatment of anterior open bite, with the advantage given to the FPC.
