Introduction. Physical activity and healthy diet, as lifestyle factors, are essential components in the prevention of chronic noncommunicable diseases. Impared glucose intolerance (IGT) is an independent cardiovascular risk factor. Dyslipidaemia is a cardiometabolic risk factor for the development of type 2 diabetes mellitus. Objective. The aim of the study was to investigate the influence of moderate physical activity of plasma lipoprotein indicators in high-risk subjects for diabetes mellitus during one-year planned intervention. Methods. We randomly assigned 60 overweight subjects with IGT aged 30-60 years. The subjects were divided into intervention group with 30 subjects, who were intensively and individually instructed on weight reduction, nutrition and increased physical activity, and control group with 30 subjects, who were counselled, as standard, on nutrition and increased exercise. Total cholesterol (TC), LDL cholesterol (LDL-C), HDL cholesterol (HDL-C) and triglycerides (Tg) were measured at the beginning of the study, and at 2 months, 6 months, and at the end of the study (12 months). Results. Compared to the beginning of the study, after 2 and 6 months there was no statistically significant difference in serum lipid values. After 12 months, the average values of the measured lipid levels in the intervention group decreased by 18.36% for TC, 27.3% for LDL-C, and 34.2% for Tg (compared to 10.27%, 13.45%, and 10.4%, respectively in the control group). Value of HDL-C in the intervention group increased by 19.12%, and decreased in the control group by 1.48%. Total/HDL-C ratio was reduced by 30.6% and LDL-C/H by 38.1% in the intervention group (compared to 12.36%, and 15.9% in the control group). After 12 months, significantly greater decrease in TC (p<0.01), LDL-C (p<0.01) and Tg (p<0.0001) and significantly greater increase in HDL-C (p<0.05) was detected in the intervention group compared to the control group. Conclusion. Plasma lipoproteins can be significantly decreased by changes in the lifestyles of high-risk subjects during one-year planned intervention.

Abstract Objectives The aim was to investigate the effect of physical activity on periodontal health and HbA1c levels in patients with type 2 diabetes mellitus (T2DM) over a period of 6 months. Materials and methods Thirty-seven patients with non-insulin-dependent T2DM were included in the study. The intervention group (n=20) performed physical activity over a period of 6 months. The control group (n=17) did not receive any intervention. Baseline and final examinations included dental parameters and concentrations of glycosylated hemoglobin (HbA1c) and high-sensitivity C-reactive protein (hsCRP). Results Physical activity showed a positive effect on periodontal health. Both the BOP (p= 0.005) and the severity of periodontitis (p= 0.001) were significantly reduced in the intervention group compared to the control group. Furthermore, HbA1c levels were reduced (p= 0.010) significantly in the intervention group while hsCRP levels significantly increased in the control group (p= 0.04). Conclusions Within the limitations of this randomized, controlled trial, physical activity over a period of 6 months is a health-promoting measure for patients with T2DM and improves both periodontal health and HbA1c concentrations.

This study sought to assess the effect of precede–proceed model on preventive behaviors for type 2 diabetes mellitus (DM) in high-risk individuals. In this semi-experimental study, 164 high-risk individuals for type 2 DM were selected and were randomly divided into two groups of intervention and control ( n = 85). Educational intervention was performed as a single session face-to-face instruction for 1.5 hr for the intervention group participants. Data were collected before (baseline) and immediately and 1 month after the intervention in the two groups. The mean score of predisposing (knowledge) factors ( p = .001), reinforcing factors ( p = .001), and enabling factors ( p = .02) were significantly different at baseline and 1 month after the intervention in the intervention group compared with the control group ( p < .05). A significant improvement occurred in the nutritional habits of high-risk participants in the intervention group at 1 month after the intervention compared with controls ( p = .001). The precede–proceed model can be effective for promoting the preventive behaviors for type 2 DM in high-risk individuals.

BACKGROUND Intensive lifestyle modifications have proved effective in preventing type 2 diabetes mellitus (T2DM), yet the efficiency and effectiveness of these modifications need to be improved. Emerging social media interventions are considered useful in promoting these lifestyles; nevertheless, few studies have investigated the effectiveness of combining them with behavior theory. OBJECTIVE This study aims to examine the effectiveness of a 6-month mobile-based intervention (DHealthBar, a WeChat applet) combined with behavioral theory compared with a printed intervention in improving dietary behaviors, physical activity, and intention to change these behaviors among populations at high risk for T2DM. METHODS Participants aged 23 to 67 years were recruited offline in Beijing, China, and were randomized into the intervention group or the control group, which received educational content via DHealthBar or a printed handbook, respectively. Educational materials were culturally tailored recommendations on improving dietary behaviors, physical activity, and intention to change based on the transtheoretical model. Participants in the intervention arm received push notifications twice per week on WeChat and had access to the educational content for the 6-month study period. Participants in the control arm received the same intervention content through printed materials. The outcomes of participants’ behavior change, intention to change behavior, and anthropometric characteristics were collected via online measuring tools at baseline, 3 months, and 6 months. RESULTS In this study, 79 enrolled individuals completed baseline information collection (control: n=38 vs intervention: n=41), and 96% (76/79) completed the 6-month follow-up visit. Attrition rates did not differ significantly between the 2 groups (<i>χ</i><sup>2</sup><sub>1</sub>=0.0, <i>P</i>=.61). Baseline equivalence was found. Participants in both groups reported a statistically significant decrease in energy intake at the 2 follow-up assessments compared with baseline (3 months, control: exp[<strong>β</strong>]=0.83, 95% CI 0.74-0.92 vs intervention: exp[<strong>β</strong>]=0.76, 95% CI 0.68-0.85; 6 months, control: exp[<strong>β</strong>]=0.87, 95% CI 0.78-0.96 vs intervention: exp[<strong>β</strong>]=0.57, 95% CI 0.51-0.64). At 6 months, a significantly larger decrease was observed in the intervention group in energy, fat, and carbohydrate intake, accompanied with a significantly larger increase in moderate-intensity physical activity compared with the control group (energy: exp[<strong>β</strong>]=0.66, 95% CI 0.56-0.77; fat: exp[<strong>β</strong>]=0.71, 95% CI 0.54-0.95; carbohydrates: exp[<strong>β</strong>]=0.83, 95% CI 0.66-1.03; moderate-intensity physical activity: exp[<strong>β</strong>]=2.05, 95% CI 1.23-3.44). After 6 months of the intervention, participants in the intervention group were more likely to be at higher stages of dietary behaviors (exp[<strong>β</strong>]=26.80, 95% CI 3.51-204.91) and physical activity (exp[<strong>β</strong>]=15.60, 95% CI 2.67-91.04) than the control group. CONCLUSIONS DHealthBar was initially effective in improving dietary behavior, physical activity, and intention to change these behaviors among populations who were at high risk of developing T2DM, with significant differences in the changes of outcomes over the 6-month intervention period. Abstract Background Mobile health interventions hold the prospect to support risk factor and lifestyle modification and are readily scalable in healthcare systems. Purpose We aim to assess whether text messaging program can improve glycemic control in high-risk patients with coronary heart disease (CHD) and diabetes mellitus (DM). Methods The study was a multi-centre randomised clinical trial conducted at 34 clinics in China. 502 individuals with both CHD and DM were recruited and randomly assigned to either the text messaging intervention (n=251) or control group (n=251). The control group received 2 thank-you messages per month in addition to usual care. The intervention group received 6 messages per week for 6 months. Messages provided educational and motivational information related to glucose monitoring, BP control, physical activity and lifestyle recommendations. The primary outcome was change in glycemic hemoglobin (HbA1C) from baseline to 6 months. Results Follow-up rate was 99%. At 6 months, HbA1C was significantly lower in intervention group compared to control group (mean HbA1C 6.7% vs. 7.2%, P<0.001), with a mean change of −0.2% and 0.1% from baseline, respectively (mean absolute change −0.3% [95% CI −0.5 to 0.1]; P=0.003). Significantly more participants in intervention group achieved a HbA1c<7% (69.3% vs. 52.6%; P=0.004), and change in FBG was larger in intervention group (−0.5 vs. 0.1 mmol/L; P=0.011). No differences in SBP, LDL-C, BMI and physical activity were observed. Almost all patients reported messages to be easy to understand (97.1%) and useful (94.1%). Table 1. Abstract Background:Working women in Shanghai are a special group with a high risk of suffering work stress and burnout. They were disturbed by the work-family conflicts, which resulted in lower health-related quality of life (HRQoL), higher job stress and burnout. This study evaluated the potential physical activity and counselling intervention effects on health outcomes of working women in Shanghai participating in a group- or an individual-based physical activity and counselling intervention compared to a control group.Methods:Participants were randomly recruited from eight communities of Shanghai using the stratified cluster sampling method. A sample of 121 female workers was used in this study. They were randomly divided into three groups: a control group and two intervention groups (individual-based and group-based intervention). The two intervention groups accepted 12-week moderate physical activity and counselling intervention. Subjective perceptions of work stress, burnout, and HRQoL were measured before and after the intervention. Results:In the control group, no significant difference was found between participants for stress (p = 0.752) and burnout (p = 0.622) before and after the experiment. However, the HRQoL value decreased after the experiment, with the mean value falling from 91.59 to 87.10. After the experiment, the values of stress and burnout decreased, and the value of HRQoL increased in the two intervention groups. At the intervention’s completion, there were significant differences compared the two intervention groups to the control group separately regarding changes in burnout and HRQoL (all p < 0.01). For stress, the group-based intervention group exhibited a significant difference compared to the control group (p < 0.01), while the individual-based intervention group did not exhibit a significant difference compared to the control group (p = 0.128).Conclusion: Physical activity and counselling intervention delivered either in a group or individual format could reduce burnout and improve HRQoL of working women in Shanghai, and the group interventions were more effective than those targeted at individuals.

Background: Regular physical activity is essential for metabolic control in type 2 diabetes mellitus. Aims: The aim of this study was to assess the short and long-term impact of a multifactorial intervention on physical activity and clinically relevant biochemical parameters in patients with type 2 diabetes mellitus. Methods: This randomised, controlled clinical trial (NCT02991079) included two parallel groups aged 25–70 years from a primary care health centre in Salamanca, Spain. The subjects were assigned randomly (1:1) to control and intervention groups, using Epidat 4.0 software. Both were counselled on the importance of physical activity and maintaining a healthy diet. The intervention group also took five low–moderate intensity 4 km nurse-guided walks, received a smartphone application to promote healthy habits and attended a diet workshop. Physical activity was measured objectively using a pedometer and subjectively using a shortened international physical activity questionnaire (at baseline, 3 and 12 months). Results: In total, 204 subjects were included (mean age 60.6 years, 45.6% were women). After 3 months, relative to the control group, the intervention group increased their daily number of steps by 1852, aerobic steps by 1623, distance walked by 994 m, and total metabolic equivalent minutes per week by 1297 and decreased sedentary time by 34.3 minutes per day. Differences from baseline persisted at 12 months, including mean increases of 1141 daily steps, 917 aerobic steps, and 1065 total metabolic equivalent minutes per week in the intervention group relative to the control group ( P<0.05 for all). Conclusions: The success of this multifactorial intervention should help inform future clinical approaches and application designs towards managing type 2 diabetes mellitus and improving patient outcomes.

Biochemical and neuropsychological changes due to poor sleep may contribute to the development of neurodegenerative disorders, such as dementia. Physical activity is widely thought to improve sleep; however, the optimal intensity/duration of physical activity required is unknown. This 14-week, single-blind study (n=23) investigated the feasibility of a self-directed physical activity intervention in healthy adults using actigraphy and cognitive function measures as primary outcomes. Participants were randomised to a control group (no change in routine) or the intervention group (increased physical activity) and were provided with an actigraphy device to monitor activity. Participants completed daily sleep/activity diaries and three cognitive assessment sessions. Vigorous physical activity increased between baseline and week 3 for the intervention group only, with no identifiable impact on sleep. This change was not sustained at week 12. Performance on an executive function task and delayed visuospatial recall improved from baseline to week 12 for the intervention group only. Contrary to our expectations, increasing light-moderate physical activity was associated with more impaired sleep across all participants. It is clear that the relationships between physical activity, sleep and cognition are complex and require further investigation. We discuss optimal methodologies for clinical trials investigating physical activity and/or sleep interventions targeting cognition.

This study assessed the impact of an educational intervention on the knowledge, attitudes, and practice regarding Type 2 Diabetes Mellitus (T2DM) of Primary Health Care (PHC) professionals, as well as on the types of T2DM care services which they were able to provide. The intervention was carried out in collaboration with county hospitals. The study was conducted from 2015 to 2016 among 241 health care professionals in 18 township health centers and 55 village clinics in three counties in Jiangsu Province, randomly divided into an intervention group and a control group. Participants in the intervention group received professional skills training sessions and team communication and were involved in regular meetings. The control group followed the routine work plan. At one-year follow up, the diabetes knowledge score, practice score, and attitudes score were significantly higher in the intervention group than in the control group. A significantly higher proportion of health care professionals in the intervention group was able to provide services compared with the control group, for all types of services, except T2DM emergency treatment. The intervention among health care professionals in PHC had a positive impact on their professional diabetes skills, knowledge, attitudes, practices, and types of services they were able to provide, at one-year follow-up.
