Background/Aim. A variety of physical therapy options has been developed for the treatment of musculoskeletal disorders including those characterized with low bone mineral density (BMD). Extreme low frequency pulsed electromagnetic field (ELF-PEMF) can accelerate bone formation. Patients with end stage of renal disease (ESRD) are predisposed to high incidence of fractures due to bone disorder with multifactorial pathogenesis. Vitamin D, calcium supplements, antiresorptive and anabolic drugs in those patients have changed pharmacodynamics and pharmacokinetics and have minimal or limited effects. The aim of this study was to assess the effectiveness of long-term ELF-PEMF therapy applied in concordance with physical exercise on bone mass, incidence of new bone fractures and parathyroid hormone concentrations in ESRD patients on dialysis. Methods. In this 3-year prospective clinical trial, 151 patients with ESRD on dialysis program were subjected to treatment with ELF-PEMF (18 Hz, 2 mT) applied during 40 min after 10 consecutive dialysis procedures, 4 times through one year (120 treatments in total during three years) together with kinesitherapy (study group) or only with kinesitherapy (control group) on the voluntary basis. Results. Total of 124 patients have completed the study. In the study group (n = 54), regardless of sex, significant improvements of BMD, T-score and Z-score on both lumbar spine and femoral neck were achieved after 3-year treatment with ELF-PEMF. In the control group (n = 70), significant decreases of BMD, T-score and Z-score as well as the higher incidence of new bone fractures were recorded. Conclusion. ELF-PEMF could be a convenient and safe non-pharmacological therapeutic strategy for fracture prevention in nephrology practices.

Background/Aim. Extremely Low Frequency Pulsed Electromagnetic Magnetic Field (ELF-PEMF) has a wide range of therapeutic applications which were expanding during the last decades. ELF-PEMF, as non-invasive, longterm safe method of physical therapy can influence a variety of aspects in chronic diseases including quality of life. Patients with chronic kidney disease (CKD), especially with end stage renal disease (ESRD), treated by dialysis, have lower health-related quality of life and changed normal way of living because of ESRD-related comorbid illnesses, associated conditions and complex dialysis procedures. The objective of this study was to assess the effectiveness of longterm ELF-PEMF in concordance with exercising on quality of life in ESRD patients on dialysis. Methods. A total of 124 patients (59 men and 65 women) with ESRD on dialysis program were divided into study group and control group. Patients included in the study group (n = 54) agreed to receive treatment with ELF-PEMF (18 Hz, 2 mT, applied during 40 minutes after ten consecutive dialysis procedures, four times through one year, 120 treatments in total) together with kinesitherapy over three years. The patients in the control group (n = 70) were subjected only to kinesitherapy as a physical therapy procedure. Quality of life was assessed through the Short Form Health Survey, version 2 (SF36v2) and the Functional Assessment of Chronic Illness Therapy, version 4 (FACIT Fatigue v4) questionnaires. Results. In the study group, treatment with ELF-PEMF significantly improved FACIT Fatigue v4 scale score as well as physical health, physical functioning, bodily pain and energy/ fatigue domains of SF=36v2 scale. There were no effects on mental health domain, limitations due to physical health problems, limitations due to personal or emotional problems, emotional well-being, social functioning, and general health perceptions. In the control group, no beneficial effects on FACIT Fatigue v4 scale and SF36v2 scale item were noticed. Conclusion. ELF-PEMF could be a additional and safe strategy for improving quality of life in patients with ESRD on dialysis.

Abstract Backgrounds: Postoperative fracture of the upper instrumented vertebrae (UIV) is thought to be as fracture type of proximal junctional failure (PJF), which usually needs revision surgery for salvage. The purpose of this study was to investigate the influences of spinopelvic parameters, such as the pelvic incidence (PI) angles, sacral slope (SS) angles, and pelvic tilt (PT), sagittal vertical axis (SVA), and proximal local kyphosis (PLK) angle on the development of fracture type of proximal junctional failure after posterior instrumentation. Methods This was a retrospective 1:3 matched case-control cohort study: 24 patients in the study group and 72 patients in the control group. The weighted Charlson Comorbidity Index (CCI) and bone mineral density (BMD) with T score were recorded. In addition to spinopelvic parameters, PI-LL and spinopelvic realignment score were calculated. Results More comorbidities (CCI, p = 0.002), poorer bone density (T score, p = 0.001) were noted in the study group. Before surgery, the study group had significantly lower LL (p = 0.046), SS (p = 0.043), and significantly higher PLK (p < 0.001), PT (p = 0.044) than the control group. Postoperatively, the study group had significantly higher PLK (p < 0.001 ) and lower LL (p = 0.031) than the control group; the degree of PI-LL ( p = 0.007) remained significantly higher in the study group. Both preoperative (p = 0.026) and postoperative (p = 0.045) spinopelvic realignment scores was worse in the study goup. Conclusions Preoperative and postoperative lower LL/spinopelvic realignment score, and higher PLK/PI-LL were significantly associated with instrumented fracture at upper instrumented vertebrae. An appropriate LL and a lower PLK should be obtained at surgery to prevent the development of instrumented fracture.

Purpose: To evaluate the therapeutic effect of teriparatide in combination with low-frequency pulsed electromagnetic field in the management of hip fracture.Methods: Patients with hip fracture internal fixation and under conventional postoperative basic adjuvant therapy were compared with those patients who received teriparatide combined with low frequency pulsed electromagnetic field after operation. The content of Bone-Specific AlkalinePhosphatase (BALP), type I Procollagen Carboxy Terminal Propeptide (PICP), serum bone glaprotein (BGP), Ca2+, hip function and degree of bone scab formation were evaluated using Harris score and Fernadez-esteve scoring system. Kaplan-Meier survival curve was plotted to analyze differences in short-term prognosis of different postoperative adjuvant treatment measures.Results: Harris score of the hip joint and the effect of bone formation in the experimental group were higher than that of the control group. The blood levels of BGP, BALP, PICP and serum Ca2+ in the experimental group were higher than that of the control group (p < 0.05). The plotted Kaplan-Meier survival curve indicates that the prognosis of the experimental group was better than that of control group (p < 0.05).Conclusion: The adjuvant therapy of teriparatide combined with low-frequency pulsed electromagnetic field can improve the recovery function and enhance the prognosis of patients who underwent hip fracture surgery. Abstract Background Patients with rheumatic diseases have a high risk for joint destruction and secondary osteoarthritis (OA) as well as low bone mineral density (BMD, i.e., osteoporosis). While several factors may lead to low BMD in these patients, the value of BMD measurements in rheumatic patients with end-stage OA scheduled for total joint arthroplasty is unknown. Methods In this retrospective cross-sectional study of 50 adults with secondary OA due to rheumatic diseases, we evaluated dual energy X-ray absorptiometry (DXA) measurements of both hips and the spine performed within 3 months prior to arthroplasty (n = 25 total hip arthroplasty, THA; n = 25 total knee arthroplasty, TKA). We analyzed various demographic and disease-specific characteristics and their effect on DXA results by using group comparisons and multivariate linear regression models. Results Although patients undergoing TKA were younger (63.2 ± 14.2 vs. 71.0 ± 10.8 yr., p = 0.035), osteoporosis was observed more frequently in patients scheduled for TKA than THA (32% vs. 12%). Osteopenia was detected in 13/25 patients (52%) in both the THA and TKA cohort. In the THA cohort, female sex, lower BMI and prednisolone use were associated with lower T-score in the hip. In TKA patients, higher OA grade determined by Kellgren-Lawrence score was associated with lower T-score in the hip of the affected side. Conclusions Osteoporosis is present in a considerable frequency of rheumatic patients with end-stage OA, and THA and TKA patients show distinct frequencies and risk factors of low BMD. Our findings point to a potential value of DXA regarding preoperative evaluation of bone status.

Currently, many children undergo precocious puberty, resulting in short stature due to premature closure of the growth plate. Pulsed electromagnetic field (PEMF) stimulation induces cell proliferation of articular chondrocytes. We developed a method for growth promotion using equipment with PEMF. In this study, we aimed to evaluate the effects of PEMF on the growth rate of growth plates using an animal model. An experimental study was conducted on 16 3-week-old rats to validate the effects of the growth care device on growth and development by PEMF stimulation at 28 Hz and 20 Gauss. The tibia bones of the groups with and without PEMF administration were dissected after 10 days, and then, the length of the growth plate of the knee and levels of insulin-like growth factor (IGF)-1 hormone in serum were measured. The length of the growth plate on the tibia bone and the levels of circulating IGF-1 were significantly increased by 25.6% and 13.6%, respectively, in the experimental group to which PEMF was applied compared to those of the control group, without any side effects. These results suggest that PEMF can safely stimulate growth of the growth plate in a non-invasive manner to promote bone growth.

Abstract: Effect of Lampung Banana Potassium on Bone Mineral Density in the Elderly. This study aims to determine the effect of banana lampung on bone mineral density in the elderly in the Social Welfare Tresna Werdha and Werdha Graha Kasih Father’s home). This research is a research with experimental design with the separate sample pretest posttest control group design. The research was carried out in the Tresna Werdha Social Institution and the Werdha Graha Kasih Father’s house, from May to July 2017. The sampling technique was taken by random sampling The result of potassium feeding on 150 grams of banana per day for 30 days by increasing BMD T-score of 0.17 while in control group (without banana lampung) BMD T-score decreased by - 0.32. After intervention in both groups showed a difference of BMD T-score of 0.49. Abstrak: Pemberian Kalium Buah Pisang Lampung terhadap Densitas Mineral Tulang pada Lansia. Penelitian ini bertujuan untuk mengetahui mengetahui pengaruh pemberian buah pisang lampung terhadap bone mineral density pada lansia di wilayah Panti Sosial Tresna Werdha dan panti Werdha Graha Kasih Bapa). Penelitian yang dilakukan ini adalah penelitian dengan desain eksperimen dengan rancangan the separate sample pretest posttest control group design. Penelitian dilaksanakan di wilayah Panti Sosial Tresna Werdha dan panti Werdha Graha Kasih Bapa), mulai bulan Mei s/d Juli 2017. Teknik sampling diambil dengan cara random sampling Hasil pemberian kalium pada buah pisang lampung sebanyak 150 gr setiap hari selama 30 hari dengan dapat meningkatkan BMD T-score sebesar 0,17 sedangkan pada kelompok control (tanpa pemberian buah pisang lampung) BMD T-score menurun sebesar - 0,32. Setelah intervensi pada ke dua kelompok menunjukkan adanya perbedaan BMD T-score sebesar 0,49.

<p><strong>Objective. </strong>The objective of our study was to investigate the effects of carbamazepine (CBZ) and lamotrigine (LTG) treatment on bone metabolism in epileptic patients.</p><p><strong>Patients and Methods. </strong>A cross-sectional study was performed on normal controls (N=30) and 100 patients with symptomatic epilepsy caused by a primary brain tumor, divided into two groups according to the treatment: LTG monotherapy group (N=50) and CBZ monotherapy group (N=50). For each participant serum levels of 25-OHD and osteocalcin (OCLN) were measured, and bone mineral density (BMD) was evaluated by the dual-energy X-ray absorptiometry method.</p><p><strong>Results</strong>. There was no statistically significant difference in the average values of vitamin D in serum between the CBZ and LTG groups (Vitamin D CBZ 17.03±}12.86 vs. Vitamin D LTG 17.97±}9.15; F=0.171, P=0.680). There was no statistically significant difference in the average values of OCLN between the CBZ and LTG groups (OCLN CBZ 26.06±}10.87 vs. OCLN LTG 27.87±}28.45; F=0.171, P=0.674). The BMD value was lower in both groups using antiepileptic agents compared to the controls, but when comparing the CBZ group to the LTG group, a statistically significant difference was only observed for the Z score (T-score CBZ: 0.08±} 1.38 vs. T-score LTG: 0.37±} 1.02; F=1.495, P=0.224; Z score CBZ: -0.05±}1.17 vs. Z. Score CBZ: 0.38±}0.96; F=4.069, P=0.046) (Table 3).</p><strong>Conclusion</strong>. The choice of antiepileptic agents for treating seizures in patients with brain tumors should be carefully evaluated in relation to their impact on bone health. These patients could benefit from supplementation and regular measurement of biochemical markers of bone turnover and BMD.
