BACKGROUND The insertion of fixed orthodontic appliances increases the risk of dental caries, particularly in adolescents. Caries can be prevented through good oral health behavior. To support adolescents with fixed orthodontic appliances and for promoting oral health behavior, we developed a theory- and evidence-based mHealth program, the WhiteTeeth app. OBJECTIVE The objective of our paper was to describe the systematic development and content of the WhiteTeeth app. METHODS For systematic development of the program, we used the intervention mapping (IM) approach. In this paper, we present the results of applying the first 5 steps of IM to the design of an mHealth program: (1) identifying target behaviors and determinants through problem analysis, including a literature search, a survey study, and semistructured interviews, to explore adolescent oral health behavior during orthodontic therapy; (2) defining program outcomes and objectives; (3) selecting theoretical methods and translating them into practical strategies for the program design; (4) producing the program, including a pilot test with 28 adolescents testing the acceptability and usability of the WhiteTeeth app; and (5) planning implementation and adoption. RESULTS On the basis of our literature search, we identified fluoride use and control of dental plaque levels (eg, tooth brushing and proxy brush usage) as target behaviors for preventing caries. Next, we identified important and changeable determinants of oral health behavior that fitted the theoretical concepts of the Health Action Process Approach (HAPA) theory. The HAPA theory, the self-regulation theory, and the results of the semistructured interviews were used to define the program objectives, that is, the performance and change objectives. After defining the objectives, we identified multiple behavior change techniques that could be used to achieve these objectives, such as providing oral health information and feedback, prompting self-monitoring, coaching of set actions and coping plans, and sending reminders. We translated these methods into practical strategies, such as videos and a brushing timer. Next, we combined these strategies into a single program resulting in the WhiteTeeth app (which is available on both iTunes and Google Play stores as “Witgebit”). Adolescents with fixed orthodontic appliances and dental professionals were included in the development process to increase the success of implementation. The pilot test revealed that the app users appreciated and liked the app. The WhiteTeeth app can be integrated into current orthodontic care. CONCLUSIONS IM allowed us to identify multiple techniques that have been shown to be the most effective in initiating behavior change, but have not yet been incorporated into existing orthodontic apps. The WhiteTeeth app contains all these techniques, which makes it a unique and promising home-based app for promoting oral health in adolescents with fixed orthodontic appliances.

Relevant theoretical and empirical insights regarding the development of interventions to promote physical activity in young people are overviewed. It is argued that physical activity-promoting programmes should be based on a systematic approach combining empirical findings, theoretical insights, and practical considerations. Successful health promotion interventions to increase physical activity typically consist of strategies to improve intra-personal determinants of physical activity, as well as environmental strategies that facilitate the behaviour, break unhealthy habits, or sustain healthy ones. The Intervention Mapping protocol can be a helpful tool in the systematic development, implementation, diffusion, and evaluation of interventions aimed at the promotion of physical activity in young people.

BACKGROUND Patients’ implicit attitudes towards medication need and concerns may influence their adherence. Targeting these implicit attitudes by combining game-entertainment with medication-related triggers might improve medication adherence in rheumatoid arthritis (RA) patients. OBJECTIVE To describe the systematic development of a serious game to enhance adherence to anti-rheumatic drugs using Intervention Mapping. METHODS A serious game was developed using the Intervention Mapping framework guided by a multidisciplinary expert group which proceeded along six steps: (1) exploring the problem by assessing the relationship between medication adherence and implicit attitudes, (2) defining change objectives, (3) selecting evidence-based behaviour change techniques that focussed on adjusting implicit attitudes, (4) designing the intervention, (5) guaranteeing implementation by focussing on intrinsic motivation and (6) planning a scientific evaluation. RESULTS Based on the problem assessment and guided by the Dual-Attitude Model, implicit negative and illness related attitudes of RA patients were defined as the main target for the intervention. Consequently the change objective was: ‘After the intervention, participants have a more positive attitude towards anti-rheumatic drugs’. Attention bias modification, evaluative conditioning and goal priming were the techniques chosen to implicitly target medication needs. These techniques were redesigned into medication-related triggers and built in the serious puzzle game. Thirty-seven RA patients tested the game at several stages. Intrinsic motivation was led by the self-determination theory and addressed the three needs competence, autonomy and relatedness. The intervention will be evaluated in a randomised clinical trial that assessed the effect of playing the serious game on anti-rheumatic drug adherence. CONCLUSIONS We systematically developed a serious game application to enhance adherence to anti-rheumatic drugs among RA patients using the Intervention Mapping framework. This protocol could serve as a guideline for other healthcare providers when developing similar interventions.

ObjectivesThis article describes (1) the systematic development of the intervention and (2) the assessment of its feasibility (in terms of relevancy, suitability, satisfaction and added value).MethodsThe intervention was developed based on the first four steps of the Intervention Mapping and Knowledge Transfer Scheme processes. Subsequently, a qualitative research based on a one-group post-test cross-sectional design was conducted. Eight retired professional footballers underwent the developed intervention and its feasibility (operationalised in relevancy, suitability, satisfaction and added value) was assessed by means of a questionnaire and interview.ResultsAn After Career Consultation (ACC) was developed with a focus on five main domains: (1) detraining from professional football; (2) remission of osteoarthritis; (3) promotion of healthy lifestyle; (4) preventing mental and cognitive health problems; and (5) employment and education. The ACC relies on three components: (1) raising self-awareness about potential physical, mental and social health problems after a career in professional football; (2) medical examination (60 min) with thorough medical history and general physical examination; and (3) follow-up during 3 months (if necessary) about optimal skills and strategies to empower physical, mental and social health and quality of life. The relevancy, suitability, satisfaction and added value of the ACC were positively evaluated by the retired professional footballers.ConclusionThe ACC was developed with a focus on five main health-related domains. The feasibility of the ACC was positively assessed by retired professional footballers, while the suggestion was made to repeat the ACC in the initial years after football retirement.
