Oral rehabilitation with the use of dental implants has made significant progress in recent years, especially for the development of more predictable and bold materials and protocols.Thus, the immediate rehabilitation of a dental absence, mainly in the aesthetic area, becomes a preferable alternative to late approaches. The reason for this approach would be to preserve the original bone and gingival architecture by optimizing the aesthetic result of the treatment, even in cases where excellent primary stability is not obtained for immediate implant loading. In view of this context, the objective of this article is to report a clinical case of an upper central incisor rehabilitation with immediate implant, followed by bone graft, customized healing abutment, and temporary restoration for aesthetic purposes. It was concluded that the immediate implant installation after tooth extraction may be associated with the reduction of bone resorption of the dental alveolus, the reduction of the treatment time, and the immediate aesthetic result, in addition to providing the patient with greater comfort and convenience, eliminating a second surgical procedure for implant installation, even when a primary implant stability is not achieved.

ABSTRACT The marked rise in patients’ demands as regards quality of life and a good appearance makes it mandatory for the Prosthodontist to provide a dental prosthesis with optimal functional, esthetic and physiological requirements. Recent researches in dental implants advocate precise preparation of the implant bed and an adequate primary stability for more vital bone to be in contact with the implant interface facilitating immediate loading. Improvements in implant designs and surface modifications have favored reduced treatment time.

Introduction: In the scenario of extraction associated with immediate implantation in the aesthetic area, tooth extraction is common in the daily clinical practice of the dentist, with the main causes of caries, periodontal disease, and coronal-radicular fractures. All extractions must be performed with precise indication, given defined prosthetic planning, thus avoiding an imbalance in the occlusion, swallowing, and aesthetics of the patient. Objective: To carry out a systematic review on the main approaches to extraction associated with immediate implantation in the aesthetic area. Methods: Followed a systematic review model (PRISMA). The search strategy was carried out in the PubMed, Embase, Ovid, Cochrane Library, Web Of Science, and Scopus databases. The quality of the studies was based on the GRADE instrument and the risk of bias was analyzed according to the Cochrane instrument. Results: A total of 134 studies were found that were submitted to the eligibility analysis and, after that, 41 studies of high to medium quality and with risks of bias were selected that do not compromise the scientific basis of the studies. According to the main literary findings, the use of the technique of installing immediate implants, after extraction, requires planning and care in the management of soft tissues, to correct aesthetic sequelae. The use of precise and minimally traumatic techniques enabled satisfactory and functional aesthetic results, as well as improving the patient's aesthetics and self-esteem. Conclusion: The immediate implant placement procedure after tooth extraction preserves bone height and thickness, reduces treatment time and cost, in addition to maintaining the gingival architecture, being important for the aesthetic success of future prosthetic rehabilitation. Also, it presents success rates comparable to implants in fully healed edges, the extraction must be done in a minimally traumatic way, to preserve the maximum bone tissue. The horizontal defects present after the installation of the immediate implant (gaps), if they are less than or equal to 3mm, will heal with complete bone filling. However, if they are larger than 3mm, bone graft material and/or membrane should be used so that there is bone healing.

Oral rehabilitation with free-end removable partial denture (RPD) is one of the major challenges in prosthodontic dentistry. The absence of a distal abutment produces undesirable RPD movement during masticatory function due to the high resilience of the residual ridge, which limits the denture efficiency and may damage the abutment teeth and bone over time. The advent of dental implants made possible to substitute the missing teeth with fixed implant-supported dentures as the first choice treatment to overcome such inconveniences. However, this indication may not be suitable for all patients due to financial, anatomical or systemic health conditions. Nevertheless it is possible to improve free extension RPD by using implants in the posterior edentulous ridge to achieve biological, biomechanical, physiological and social benefits. This article aims to present a case report on oral rehabilitation in which a RPD was made combined with an implant for posterior support in a sizeable edentulous ridge. The patient appreciated the retention improvement and the aesthetic result. The combination of a RPD with a posterior implant is an alternative treatment for cases in which implant-supported fixed prosthesis is not indicated, reducing the displacement and minimizing the limitations of the free-end devices.

SUMMARYObjective: The aim of this study was to evaluate the outcome of immediate postextraction implant placement. Immediate placement of dental implants have been claimed of the potential advantages such as reductions in the number of surgical interventions, a shorter treatment time, an ideal 3-dimensional implant positioning, the presumptive preservation of alveolar bone at the site of the tooth extraction and soft tissue aesthetics.Method: In this case series we reported to extract 15 teeth in 12 patients (8 males; 4 females, mean age: 46.08 years) and replace the teeth with implants immediately.Results: There were no signs of inflammation or infection and none of the patients had complaints subsequently. All implants were osseointegrated at the time of abutment connection. Postoperative healing was uneventful in all of the patients. No complications were observed.Conclusion: Within the limits of the present study, immediate implant placement was a predictable treatment.

Immediate implants are placed in the site of surgical extraction of the tooth to be replaced. The percentage success of such procedures varies among authors from 94-100%. Immediate implant placement is most commonly indicated when tooth extraction is done with pathologies not amenable to treatment. The advantages include reduced post-extraction alveolar bone resorption, shortened treatment time, and the avoidance of a second surgical intervention with regard to delayed implantation. This report describes a case of immediate implant placed in a maxillary central incisor followed by evaluation of soft and hard tissue changes occurring during post-operative period with a follow-up at five years.

Objective: Implants placed in the posterior region of the maxilla have a high incidence of implant failure due to poor bone quality, especially when immediate implant loading is needed. Immediate Progressive Loading (IPL) can enhance bone quality and may offer an alternative solution when Immediate Implant loading is needed. Methods: Six patients (one male and five females; 34-62 years old) were included in this study. Twelve implants were inserted in the posterior region of the maxilla. Resonance Frequency Analysis (RFA) was performed at the time of implant placement and after 1, 2, 3 and 6 months. ISQ (Implant Stability Quotient) values were compared between the Delayed Loading (DL) group after 2 months and the Progressive Loading (PL) group and between different time points for each group. Results: At implant placement, the mean ISQ values for PL and DL implants were 63 and 57, respectively. One month after implant placement, the mean ISQ value for PL implants was 73. Two months after implant placement, the mean ISQ value for PL implants was 75. Three months after implant placement, the mean ISQ values for PL and DL implants were 76 and 69, respectively. Six months after implant placement, the mean ISQ values for PL and DL implants were 79 and 76, respectively. Conclusion: Despite its limitations, this pilot study indicated that compared to DL, PL can enhance bone density and implant stability, resulting in greater early functionality and fewer surgery sessions.

Background/Aim. Immediate loading is considered to be the most innovative technique in contemporary implant dentistry. Recent clinical and experimental findings have demonstrated that only implants with high primary stability can be subjected to immediate loading protocol with predictable results. It is generally accepted that the most important prerequsite for successful osseointegration is achievement and maintenance of implant stability. The aim of this in vivo study was to investigate the possibility for successful application of immediate loading protocol in implant systems with different surface properties. Methods. In the experimental study 2 mongrel dogs were edentulated bilaterally in the mandibular and maxillary premolar areas. After 3 months implants were placed in a pattern 4 different commercially available implants per quadrant (n = 32): Mk III TiUnite (Nobel Biocare, Sweden), ITI TPS (Straumann, Switzerland), 3IOsseotite (Implant Innovation, USA) and XiVE Cell-Plus (Friadent, Germany). Implants were subjected to immediate loading with 4 unit gold cast bridges, 2 days post implantation. The assessment of implant stability and immediate loading possibilities were done by performing Resonance frequency analysis (RFA). Results. After a 6- month loading period all bridges were in function and all implants occurred well osseointegrated. When summarizing the Implant Stability Quotient (ISQ) values, it was noted that resonance frequency was significantly higher for mandibular implants. The results of this experimental setting showed that all evaluated surfaces achieved good implant stability. Increase of ISQ values was found for all implants in the mandible and partially decrease of ISQ values for maxillary implants after 6 months of functional loading with 4 unit bridges. Conclusions. Investigated endooseal implants did not show different degree of osseointegration, because there was not statisticaly significant difference among observed parameters (ISQh i ISQp) between implant systems.

Abstract Background Various surface treatment options have been adopted with the aim to improve osseointegration, reducing the overall treatment time. Implant stability of early loaded implants with different modified surfaces was compared in the present study. Methods Patients were selected from the Department of Oro-Maxillofacial Surgery and Stomatology at Semmelweis University. Patients randomly received SA (alumina sandblasted and acid-etched), NH (bioabsorbable apatite nanocoating) or SLA (large-grit sandblasted and acid-etched) surface implants. Outcome measures were: implant success, implant stability, and periodontal parameters. The implant stability was measured at the time of implant placement (primary stability) and six weeks after (prothesis delivery, secondary stability). Osstell and Periotest were applied to take all the measurements. The primary and secondary stability were compared in the three study groups Finally the periimplant probing depth appearing after three months of loading was checked on 6 points around to the implant-supported prostheses. Shapiro–Wilk and Mann–Whitney tests were used for the comparison between the study groups. Results A total of 75 implants with different length and diameter were inserted into various positions. One implant failed spontaneously at the fourth week after implant placement. The survival rate was 98,7%. Comparing the primary and secondary stability values, the data were significantly improved in every groups. The difference was the highest in the NH group, however, this difference was not significant compared to the two other groups. Good periodontal parameters were experienced in all the tested implants, independently by the groups. Conclusions With the limitation of the present study, all the implants showed improved stability six weeks after implant placement. A trend of higher result was found for the NH group. Further studies with longer follow-up are needed to confirm this preliminary results. Trial registration: Current Controlled Trials ISRCTN13181677; the date of registration: 04/03/2021. Background: The concept of immediate implant loading has recently become popular due to less trauma, reduction in overall treatment time, decrease in hard and soft tissue resorption, increase in patient’s acceptance, along with better function, aesthetics and has a psychological satisfaction to the patient. The purpose of this study is to evaluate the results of implant placement immediately after tooth extraction. Materials and method: The study consisted of 32 patients with 43 implants have been placed immediately after tooth extraction in the Odonto-Maxillo-Facial Hospital of Ho Chi Minh City. Evaluation results after dental implants 1 week, 1 month, 3 months, 6 months. Results: Good clinical results after 1 week was 55.8%, after 3 months and 6 months were 100%. After 1 week of implant placement, good healing took 59.4%. After 1 month, 3 months and 6 months, 100% of the patients are well healed. After 3 months, 6 months without any implant has the status of shaky. Most dental implants have good levels of bone graft. The level of good after 3 months of implants was 72.1% and after 6 months all cases are good. The success rate for dental implants was up to 97.7%, failing only 2.3%. After 3 months the implants had a good level of 72.1% and after 6 months, all cases were good. Patients with success in implant placement accounted for a high rate of 97.7%, failure only accounted for 2.3%. Conclusions: Success rate in immediate implant is high. It is necessary to develop this method to restore lost teeth to achieve optimum results. Key words: Dental implant, immediate implant after tooth extraction

(1) Background: implant surface topology and active hydrophilic ions could have some benefit on implant osteointegration and stability; (2) methods: 40 adult patients, suffering from a single missing tooth in the aesthetic zone, were enrolled in the study. Each patient had a single titanium implant (Thommen SPI®lement) inserted. The implant surface was obtained through conditioning using the Apliquiq system. Patients were divided into two equal groups depending on the implant’s diameter (3.5 and 4.0 mm). Each implant was loaded within four weeks. Stability levels, using the Ostell device, were checked immediately after implant placement and in four weeks; additionally, marginal bone loss (MBL) was calculated based on 12 months; (3) results: all implants survived the study. The average primary stability achieved for both groups was initially 71.59 ISQ (±4.04) and declined to 69.94 ISQ (±3.29) in four weeks. The average MBL was 0.2 mm (±0.88). There were no statistically important differences between groups. There was a positive correlation between the patient’s age and implant stability quotient (ISQ) values; (4) conclusions: hydrophilic surface implants can be used in a protocol for early functional occlusal loading. Higher values of primary stability positively influence the values of secondary stability, and the age of the patient affects the values of implant stability.
