Indonesia is a developing country consisting of islands and has a large investment value related to natural resources, both produced at sea or on land, thus the sea transportation sector facilities are very important in supporting smooth transportation. Then the operation of the ship, the professionalism and loyalty of the crew is very influential, especially in completing the loading and unloading, and to prevent oil contamination from the rest of the oil so that it will reduce the quality of the oil. For this reason, competent and responsive nuns and subordinates are needed to be able to carry out loading and unloading well, thus the possibility of losses can be avoided. Then the failure of the loading and unloading process on the product oil load is also caused by the tank's poor quality and not in accordance with the specified standards seen from the results of the Wall Wash Test conducted by the Surveyor and Loading Master. Another factor is the lack of cooperation among crew members. This often happens because the conditions on the ship are not harmonious. The efforts made to prevent contamination are cleaning tanks and pipes until they are completely clean, caring for the sampling equipment properly, and increasing the accuracy of the crew in handling cargo contamination. This research is a type of descriptive research. In this study the authors used several data collection techniques with interviews, observations, and documentary studies to collect data.

ABSTRACT Standards and regulations related to spirit drinks have been established by different countries and international organizations to ensure the safety and quality of spirits. Here, we introduce the principles of food safety and quality standards for alcoholic beverages and then compare the key indicators used in the distinct standards of the Codex Alimentarius Commission, the European Union, the People's Republic of China, the United States, Canada, and Australia. We also discuss in detail the “maximum level” of the following main contaminants of spirit drinks: methanol, higher alcohols, ethyl carbamate, hydrocyanic acid, heavy metals, mycotoxins, phthalates, and aldehydes. Furthermore, the control measures used for potential hazards are introduced. Harmonization of the current requirements based on comprehensive scope analysis and the risk assessment approach will enhance both the trade and quality of distilled spirits. This review article provides valuable information that will enable producers, traders, governments, and researchers to increase their knowledge of spirit drink safety requirements, control measures, and research trends.

National Health Service England published the National Safety Standards for Invasive Procedures (NatSSIP) in 2015. They mandated that individual trusts produce Local Safety Standards for Invasive Procedures (LocSSIPs), a set of safety standards drawn from the NatSSIP that apply to a particular clinical situation in a given department, for all invasive procedures. The project goal was to design and implement the LocSSIP within the endoscopy department. A draft LocSSIP was produced, and a pilot study conducted to gain initial feedback on its use. Version 1 of the checklist was produced and after approval, rolled out for use within the endoscopy department at ‘time out’ and ‘sign out’. A scoring system was developed that allowed the quality of the performance of LocSSIPs to be assessed and recorded as a ‘compliance score’.After 2 months, an independent observer spent a week assessing use of the checklist, recording completion and a compliance score. Analysis of this data led to a number of changes in performing the checklist, wider multidisciplinary team education and integration of the checklist into existing documentation, before reassessing at 12 months. In 2016, ‘time out’ checks were completed in 100% of cases, but full completion was only observed in 68%. ‘Sign out’ checks were completed in 91% of cases, with full completion in 71%. In 2017, ‘time out’ checks were completed in 100% of cases, with full completion in 85%. ‘Sign out’ checks were completed in 100% of cases, with full completion in 91%.The composite score for compliance in 2016 was 57% increasing to 90% in 2017.In conclusion, stronger departmental leadership, broadening education and integration of the checklist into routine documentation to reduce duplication led to significant improvements in compliance with use of the checklist. Ongoing education and assessment is imperative to ensure that compliance is maintained to ensure patient safety.

Abstract The level of sea transportation use in Indonesia is getting higher and higher, so it must be balanced with the improvement in the quality of port supporting infrastructure. The Provincial Government of DKI Jakarta will revitalize Kali Adem Port, which will also be assisted by the Thousand Islands Regency Government, considering that Kali Adem Port is one of the gateways to the Thousand Islands. Kali Adem Port is a passenger port located on Jalan Dermaga, Pluit, Penjaringan, North Jakarta, Special Capital Region of Jakarta. Kali Adem Harbor serves tourists who want to visit several islands in the Thousand Islands, and residents of Pulau Seribu who want to buy some daily necessities from Jakarta. Passenger density, problems with access and circulation, and several other problems require this Port to improve its quality and quantity. This port should have designed to pay attention to the environment, such as applying the eco-tect theme to this building, as well as the waterfront concept to maximize the potential of the existing sea. In this design the concept of shading is applied to the facade to maximize comfort and the application of energy-saving lighting in buildings and the Port and its plazas

Abstract Under the locomotive cab occupant protection research program sponsored by the Federal Railroad Administration (FRA), Sharma & Associates, Inc. (SA) developed a Secondary Impact Protection System (SIPS) for locomotive engineers. The system uses a large, automotive-style, passenger airbag in combination with a deformable knee bolster to provide the level of protection needed for the locomotive engineer, without compromising the normal operating environment and egress. A prior version of the system [1] was prototyped and tested in a dynamic sled test with a 23g crash pulse and was shown to meet most limiting human injury criteria defined in the Department of Transportation (DOT)’s Federal Motor Vehicle Safety Standards (FMVSS 208) [2] for the head, chest, neck, and femur. The system also showed marginal performance for the chest injury index and indicated potential for an improved airbag design to fully meet all requirements. In the current study, simulations with an optimized airbag and higher capacity inflator system showed that SIPS can provide excellent occupant protection for an unbelted locomotive occupant in a frontal crash. Sled testing of SIPS confirmed the performance, and the system successfully met all eleven (11) criteria of the FMVSS 208 standard [2]. The shape and position of the airbag module and its attachments to the desk were generally the same as those presented in previous research. The key changes that helped meet all criteria were the higher capacity inflators, knee bolster system brackets moved forward, thicker knee plate, higher volume airbag and additional vents.

TWO-WHEELED VEHICLES Miniature motorcycles intended for off-road use by children and adolescents have enjoyed wide popularity since the 1960s.1 Manufacture of these small two-wheeled motor vehicles is not regulated by any federal motor vehicle safety standards. Neither the rider nor the vehicle are required to be licensed. Some of these vehicles are small enough for a 4-year-old child and many are intended for use by school-aged children.2 Minibikes, weighing less than 45 kg (100 lb), are the smallest and most primitive of these vehicles. The more sophisticated minicycles are constructed to resemble miniature motorcycles. Trailcycles are larger than minicycles and have power and design characteristics that make them suitable for rough terrain. All of these vehicles have a short wheelbase and a low profile; thus, they are relatively unstable and poorly visible.3 Injuries In 1978, there were approximately 19,000 motorbike-related injuries treated in emergency rooms. Nearly half of these injuries were attributed to accidents involving the minibike (US Consumer Product Safety Commission, news release, Aug 27, 1979). In 1982, there were an estimated 12,000 children 14 years of age and younger who suffered minibike- and trailbike-related injuries (National Electronic Injury Surveillance data, National Information Clearing House, US Consumer Product Safety Commission, 1985). Since 1980, the Consumer Product Safety Commission has collected at least 24 death certificates that implicate minibikes and trailcycles. Half of those who died were children 14 years of age or younger. Children often use these vehicles illegally on roadways. Injury typically results from falls or collisions with fixed objects or other motor vehicles.
