Clinical and morphological spectrum of biopsy-proven kidney diseases in the elderly

Objective: To evaluate health care related to medication regimens among institutionalized elders in Damanhour, Beheira Governate, Egypt. Methods: A prospective, multi-centered, observational study was conducted in the two elderly residential homes in Damanhour between March and May 2017. A questionnaire was developed and validated to test for elderly socio-economic, chronic diseases, current therapy adherence, vaccination history and patient education. Descriptive and quantitative analysis were performed. Results: sixty-three elderly residents were included in the study. The sample showed broad socioeconomic variability posing a true reflection of Egyptian population. 63.5% had no hearing problems, 31.7% had proper vision and 57% could move with no help. More than three quarters had chronic diseases of which 58.7% were previously hospitalized. The most prevalent diseases were hypertension, diabetes and arthritis 46%, 41.3%, 26.9% respectively. Only 7.9% and 4.7% showed chronic liver and kidney diseases, respectively and less than 10% suffered from respiratory related diseases. No alcohol drinker, 25.3% were smokers and 58.7% drank caffeine. Only 25.3% of residents showed full adherence to their medication pattern. Approximately 80% of residents never received proper patient education. Forty-three residents did not know the indication of their medications and 92% ignored its side effects. Conclusion: Absence of proper medical care exposure for the elderly residents was reflected in their low medication adherence, adverse side effects and hospitalization. We suggest extension of the national medical insurance system to include larger number of elderly population. To monitor the care given concerning medication, a daily resident gerontological nurse needs to be assigned, visits by clinical pharmacists weekly or bi-weekly from the nearby governmental hospital can improve improper medication. doi: https://doi.org/10.12669/pjms.36.4.1923 How to cite this:Algameel M. Purpose. To identify the epidemiology and pathological types of kidney diseases and their changes during the past decade, in a population from Northeast China. Methods. We retrospectively analysed clinical and renal pathological data from 4910 patients who received renal biopsies in the Second Hospital of Jilin University from 2008 to 2017. Results. Males received more renal biopsies than females (p < 0.001). The average age (p < 0.001) and percentage of elderly patients (p < 0.001) increased over time. The pathological types were primary glomerulonephritis (PGN, 73.2%), secondary glomerulonephritis (SGN, 23.7%), tubular-interstitial nephropathy (TIN, 2.8%), and hereditary nephropathy (HN, 0.3%). The most common forms of PGN were membranous nephropathy (MN, 37.2%) and IgA nephropathy (IgAN, 29.9%). Over time, the prevalence of IgAN decreased, but the prevalence of MN increased. MN was more common in middle-aged and elderly patients, but IgAN was most common in young adults. Analysis of SGN data indicated that lupus nephritis (LN, 34.0%), Henoch-Schönlein purpura glomerulonephritis (HSPN, 17.9%), and diabetic nephropathy (DN, 11.7%) were the most common forms. Over time, the prevalence of DN (p = 0.003), hypertension-associated renal damage (p = 0.005), and systemic vasculitis-associated nephritis (SVARD, p < 0.001) increased, but the prevalence of HSPN (p < 0.001) and hepatitis B virus-associated glomerulonephritis (HBV-GN, p = 0.001) decreased. Nephrotic syndrome was the main clinical manifestation of PGN. Conclusion. From 2008 to 2017, renal biopsies were increasingly performed in the elderly. There were notable changes in the epidemiology and pathological types of kidney disease among renal biopsy patients at our centre.

Abstract Objectives Though Bangladesh is passing through demographic, epidemiologic and nutritional transitions, national estimates on nutrition and health status of the elderly population are largely unknown. We aimed to determine the status of selected health and nutrition indicators among the elderly population in Bangladesh. Methods For the first time in Bangladesh, we included elderly population (≥60 years old females and males) as a separate population group in the national food security and nutrition surveillance round 2018–2019. We collected data on dietary diversity, nutritional status, behavioral risk factors of non-communicable diseases, blood pressure, and self-reported chronic diseases from 4,818 elderly people (48% female) living in 82 clusters (57 rural, 15 non-slums urban, and 10 slums) randomly selected from eight administrative division of Bangladesh. Results Majority (59% in rural, 53% in non-slum urban, and 69% in slums) of elderly people were consuming an inadequately diverse (4 or less food groups out of 10) diet. Overall, 89% of elderly people were malnourished (20%) or at risk of malnutrition (69%). The highest prevalence of malnutrition was in Mymensingh division (37%) followed by Sylhet division (27%). The prevalence of obesity was 5%, 16%, and 11%, in rural, non-slum urban, and slums, respectively. The national prevalence of smoking, smokeless tobacco consumption, physical inactivity was 18%, 52%, and 38%, respectively. There was a high burden of hypertension (49% in rural, 53% in non-slum urban, and 39% in slums). Overall, 16% of elderly people had heart diseases, 14% had chronic respiratory diseases, 3% had kidney diseases, 9% had diabetes, 8% had stroke, 0.5% had cancer and 1.4% had mental health problems. Conclusions The government of Bangladesh should design and implement health and nutrition programs among the elderly population. The regional differences in the prevalence of health and nutrition indicators should be considered while designing such programs. Objectives: The elderly are at higher risks for developing Coronavirus Disease 2019 (COVID-19). Besides, when generating the illness, they have a higher risk of death. Recognizing clinical characteristics and risk factors associated with morbidity and mortality in the elderly is necessary to prevent mortality in similar infectious diseases. Methods & Materials: This descriptive research explored the data of all geriatrics admitted following COVID-19 in the medical centers affiliated with Mazandaran University of Medical Sciences. These data were collected from February 20, 2020, to February 20, 2021. Data analysis was performed by SPSS using logistic regression analysis. P<0.05 was considered significant. Results: Of the total 54523 patients admitted to the medical centers in the 12 months, 25218 patients were elderlies. In total, 51.05% and 12343 of the patients were female and male, respectively. In total, 3512 elderlies were expired. The death cases included 1814 (51.65%) males and the rest were females. Moreover, 16203 patients were in the age group of youngest-old, aged 60-75 years; 8270 patients were middle-old, 76-90 years; and the oldest-old category included 745 patients in the age group of ≥90 years. There was a significant relationship between age, gender, and COVID-19-induced death in the elderly (P<0.05). Older adults presented the highest death rate due to this disease Men were 10% more prone to die from COVID-19 than women. Major comorbidities recorded in this population were kidney diseases (92%), cancer (76%), diabetes (32%), and cardiovascular diseases (17%). Conclusion: Preventive measures to support the elderly are essential to combat the COVID-19 pandemic. Furthermore, it is essential to focus on the specific health needs of the elderly, such as proper nutrition, wellbeing, and mental health promotion.

The lack of pharmaceutical agents that could modulate complement activation is one of the most serious problems in modern medical practice. Meanwhile, the complement system plays a significant role in the pathogenesis of numerous diseases such as age-related macular degeneration (degenerative eye disease, which is the main cause of blindness in the elderly), kidney diseases such as atypical hemolytic-uremic syndrome and membranoproliferative glomerulonephritis type II, paroxysmal nocturnal hemoglobinuria (Marchiafava-Micheli disease), and hereditary angioedema. The complement and its components are involved in the development and progression of autoimmune pathological processes, such as systemic lupus erythematosus, rheumatoid arthritis, and autoimmune hemolytic anemia; neurodegenerative and neoplastic diseases; and complications after heart attacks, strokes, and transplants. This review considers the involvement of complements in the pathological processes, existing methods of treatment, and the prospects for the development of drugs that can correct the work of complement - the most important component of innate immunity.

Objectives Among older adults in China and the US, we aimed to compare the biomarkers of chronic-kidney-diseases (CKD), factors associated with CKD, and the correlation between CKD and mortality. Setting China and the US. Study design Cross-sectional and prospective cohorts. Participants We included 2019 participants aged 65 and above from the Chinese Longitudinal Healthy Longevity Study (CLHLS) in 2012, and 2177 from US National Health and Nutrition Examination Survey (NHANES) in 2011–2014. Outcomes Urinary albumin, urinary creatinine, albumin creatinine ratio (ACR), serum creatinine, blood urea nitrogen, plasma albumin, uric acid, and estimated glomerular filtration rate (eGFR). CKD (ACR ≥ 30 mg/g or eGFR< 60 ml/min/1.73m2) and mortality. Analytical approach Logistic regression and Cox proportional hazard models. Covariates included age, sex, race, education, income, marital status, health condition, smoking and drinking status, physical activity and body mass index. Results Chinese participants had lower levels of urinary albumin, ACR, and uric acid than the US (mean: 25.0 vs 76.4 mg/L, 41.7 vs 85.0 mg/g, 292.9 vs 341.3 μmol/L). In the fully-adjusted model, CKD was associated with the risk of mortality only in the US group (hazard ratio [HR], 95% CI: 2.179, 1.561–3.041 in NHANES, 1.091, 0.940–1.266 in CLHLS). Compared to eGFR≥90, eGFR ranged 30–44 ml/min/1.73m2 was only associated with mortality in the US population (HR, 95% CI: 2.249, 1.141–4.430), but not in the Chinese population (HR, 95% CI: 1.408, 0.884–2.241). Conclusions The elderly participants in the US sample had worse CKD-related biomarker levels than in China sample, and the association between CKD and mortality was also stronger among the US older adults. This may be due to the biological differences, or co-morbid conditions.
