Obesity paradox: if there genetics involved? Microarray study in morbidly obese decedent patients with severe and minimal atherosclerosis of the aorta

Background: There is conflicting evidence concerning the obesity paradox in stroke patients. We sought to examine the relationship between gender, BMI, and prevalence of comorbidities in patients with non-hemorrhagic stroke. Materials and Methods: Retrospective chart review was performed in 996 consecutive patients treated for non-hemorrhagic stroke at a single academic medical center. Patients were divided according to gender and specific BMI groups according to the National Institute of Health. This study was approved by the institutional IRB. Results: Patients with BMI from 0-18.5 and 18.5-24.9 were more likely to be female (63.2% and 58.4% p<.05). Whereas patients with BMI over 25-30 and 30-35 were more likely to be male (60.82% and 59.2% p<.05). Morbidly obese patients (BMI>35) were equally likely to be men or women. In men higher BMI correlated with presentation at younger age. Diabetes was most prevalent in patients with BMI over 35 (40% males, 44% females, p<.05). In females, HTN was associated with BMI 30-34.5 and 35+ (80.5% and 73.3%, P<.05). Males showed a similar though non-significant trend. The prevalence of end stage renal disease, systemic atherosclerosis, and PVD was not significantly correlated with BMI in either gender. Conclusion: Gender and BMI significantly affect associated comorbidities in patients with non-hemorrhagic stroke, possibly suggestive of unique gender specific disease mechanisms. Additional studies investigating the effect of gender and BMI on diagnostic evaluation and treatment of patients with non-hemorrhagic stroke are warranted.

Background: Prior studies on heart failure (HF) have shown that body mass index (BMI) is inversely associated with mortality. The aim of this study was to investigate the impact of morbid obesity (BMI > 40 kg/m2) on in-hospital mortality in patients presenting with Acute decompensated heart failure (ADHF). Methods: The Nationwide Inpatient Sample of the Healthcare Cost and Utilization Project was analyzed for acute HF hospitalizations across the United States. A total number of 966,167 hospitalized patients with ADHF in 2009 were reviewed. Results: Morbidly obese patients constituted 13.4% of all patients with ADHF. Analysis of the unadjusted data revealed that morbidly obese patients compared with those not morbidly obese were less likely to die during hospitalization (OR 0.55, %95CI 0.53-0.57, P<0.0001). Cox proportional hazards regression was used to estimate the overall probability of in-hospital death with adjustments for age, sex, race, Elixhauser comorbidities, primary payer, hospital location, hospital teaching status, hospital bed-size, and total hospital admissions. The adjusted hazard of in-hospital death (HR 0.87, p< <.0001) indicates that there was statistically significant difference in the risk of in-hospital death associated with being morbidly obese. Conclusions: In this cohort of hospitalized patients with ADHF, higher BMI was associated with lower in-hospital mortality risk. The relationship between BMI and adverse outcomes in HF appears to be complex and consistent with the phenomenon of the “obesity paradox.”

Abstract Objectives Obesity predisposes to multiple diseases, such as heart disease, diabetes, stroke, arthritis, and malignancy. However, obese patients have better outcomes than normal-weight patients with some of these disorders, including those admitted to critical care units. We compared the results for common laboratory tests in patients with uncomplicated obesity against the findings in normal-weight patients. Methods Patients who had a comprehensive metabolic profile test were identified. Patients with acute and/or chronic debilitating disorders were excluded, and the laboratory parameters were compared among 4 groups based on body mass index. Results With the exception of elevated triglycerides and lower high-density lipoprotein in obese and morbidly obese patients, laboratory findings were not meaningfully different from those in normal-weight patients. Conclusions The obesity paradox of better outcomes in obese patients admitted to critical care units could not be explained on the basis of lower additional disease burden necessitating critical care admission due to abnormal laboratory values at the baseline. It is conceivable that unconscious bias against obese patients, with lower disease burden than normal-weight patients, triggers their admission to critical care, thus creating the appearance of better outcomes.

Objective: To evaluate retinol and β-carotene serum levels and their relationship with risk factors for cardiovascular disease in individuals with morbid obesity, resident in Rio de Janeiro. Methodology: Blood serum concentrations of retinol and β-carotene of 189 morbidly obese individuals were assessed. The metabolic syndrome was identified according to the criteria of the National Cholesterol Education Program (NCEP) and World Health Organization (WHO). Lipid profile, insulin resistance, basal insulin, glycemia, blood pressure, and anthropometry and their correlation with retinol and β-carotene serum levels were evaluated. Results: Metabolic syndrome diagnosis was observed in 49.0% of the sample. Within this percentage the levels of β-carotene were significantly lower when body mass index increased. Serum retinol didn't show this behavior. Serum retinol inadequacy in patients with metabolic syndrome (61.3%), according to WHO criterion, was higher (15.8%) than when the whole sample was considered (12.7%). When metabolic syndrome was diagnosed by NCEP criterion, β-carotene inadequacy was higher (42.8%) when compared to the total sample (37.5%). There was a significant difference between average β-carotene values of patients with and without metabolic syndrome (p=0.048) according to the classification of the NCEP. Lower values were found in patients with metabolic syndrome. Conclusion: Considering the vitamin A contribution in antioxidant protection, especially when risk factors for cardiovascular disease are present, it is suggested that great attention be given to morbidly obese. This could aid in prevention and treatment of cardiovascular disease, which affects a significant part of the population.
