Aim. To assess food patterns in patients with different types of SIBO and their impact onto the course and treatment outcomes. Materials and methods. The data of 988 patients who signed informed consent surved as a source data. On the basis of lactulose breath test (LBT; GastroCH4eck, Bedfont, UK), the patients were selected into one of the studied groups: SIBO-H2, SIBO-CH4, SIBO-CH4-H2 and control. Twenty - four hours food recall test was used to analyze nutritional habits. In patients with SIBO-H2, standard treatment with Tilichinol 100 mg/Tilbrochinol 200 mg (Intetrix, Beaufour-Ipsen International, France) 2 caps BID for 10 days) was provided. Efficacy of treatment was assessed on the bass of the result of LBT 2 month after treatment completion. Mann-Whitney T test (Statistica 10, StatSoft, USA) was used to compare nutritional patterns in patients with or without SIBO, in different types of SIBO and in accordance to the results of treatment. Results and discussion. Nine hundred eighty eight patients were enrolled. On the basis of hydrogene breath test they were divided into 4 main groups: SIBO of hydrogen - producing flora (SIBO-H2, n=526), methane - producing flora (SIBO-CH4, n=129), SIBO with hyperproduction of methane and hydrogene (SIBO-CH4-H2, n=225). The control group consisted of 108 patients with no no excessive gas production on LBT. In contrast to controls, nutritional patterns of patients with SIBO were characterized by low dietary fiber and amount of red meat dishes in the rations. Those with SIBO-CH4 consumed more fruits (p=0.03), vegetables (p=0.003), and fish (p=0.026), compared to those with other variants of SIBO and the control group. Nutritional patterns of SIBO-H2 group were characterized by larger amount of poultry meat consumption (p=0.026) compared to other SIBO groups and controls. Thinking disturbance and disorder of affects may be different in two subtypes of schizophrenia, the “florid” and the “withdrawal” syndromes. In Exner's approach to the Rorschach system, the diagnostic indicators of disordered thinking may point out large differences not only between schizophrenic and control subjects but also between different types of schizophrenics. The Rorschach protocols of 45 subjects (15 “florid” schizophrenics, 15 “withdrawn” schizophrenics, and 15 controls of the same age and education, matched by sex) were examined on several Exner indices. Compared with the control group, both schizophrenic types confirmed an impairment of perceptual accuracy and of reality testing as well as a reduced emotional control. Compared with the “withdrawn” group, the “florid” schizophrenic subjects showed significantly higher indices of poor perceptual functioning, of an inadequate organizational activity (more Whole and Z responses characterized by negative Form Quality) and greater disordered ideational production.

Purpose. To create a personalized tactic of surgical treatment of patients with BSD and MJ considering the stage of the pathological process and thereby improve the results of treatment. Materials and methods. The experience of diagnosis and treatment of 537 patients with complications of cholelithiasis, manifested by mechanical jaundice, was analyzed. The first stage was 537 minimally invasive decompressions of the biliary tract, the second-500 different types of endosurgical interventions. Results. Depending on the tactical principles, all the subjects were divided into two groups. In one (control) group (n = 249), patients were admitted from 2010 to 2014. Their treatment was empirical: decompression of the bile tree was performed at different times, and surgery was performed without taking into account the stages of the pathological process and the severity of the patients. In the other analyzed group (n = 251), patients were admitted to the hospital from 2015 to 2019. Their treatment tactics were personalized: taking into account the stages of mechanical jaundice. individual terms of decompression of the bile ducts and the choice of surgical intervention were determined. When comparing the results obtained, the advantage of the personalized approach was established: a decrease in the percentage of postoperative complications and mortality was revealed. Conclusion. In patients with cholelithiasis and jaundice, the severity of the condition is due to hyperbilirubinemia. Stage 2 of mechanical jaundice-determined by the severity of cytolysis in hepatic insufficiency, manifested by hypertransaminasemia. The severity of cholangitis is determined by purulent-inflammatory, as well as septic signs of the disease, characterized by leukocytosis, circulating immune complexes and medium-weight molecules. In the cholestatic stage of jaundice development, treatment is planned, in the cytolytic stage — urgent, and in cholangitis-emergency. The use of personalized tactics for the treatment of patients with breast cancer has reduced the number of complications and mortality.

Pain is the most common syndrome in rheumatology. NSAIDs are often used in its treatment. But they have limitations associated with side effects. An advantage of topical drugs and physical factors (electrophoresis, phonophoresis) combination is the use of low doses of the drug and a decrease in time, to achieve a therapeutic effect, reduce the risk of adverse reactions, increase the concentration of the drug in the inflammation focus, prolonged action (creating a drug depot in tissues). Ketoprofen gel (“Artrocol gel”) is one of the representatives of this pharmacological group. Physicochemical properties help ketoprofen to penetrate easily through the skin and create a therapeutic concentration in the underlying tissues. Objective. To assess the effectiveness of ketoprofen gel (“Arthrocol gel”) electrophonophoresis in the treatment of pain in rheumatological patients. Materials and methods. The study was carried out on the basis of Physiotherapy Department of Vinnytsia Regional Clinical Hospital named after M. I. Pirogov. All patients treated according to the unified clinical protocols were divided into experimental group (EG) and control group (CG). Patients of EG (98 persons) underwent electrophonophoresis with ketoprofen gel once a day for a week. In CG (43 persons) ketoprofen gel was used without electrophonophoresis. The intensity of the pain syndrome is monitored using a visual analog scale (VAS) of Huskisson at rest and during movement. A week later, the patient and the doctor evaluated satisfaction with the results of treatment on the scale: not effective, insufficient, satisfactory, good, excellent. Results. There was a persistent decrease in the intensity of pain syndrome in patients of both groups. The VAS scores were lower in the EG starting from the 3rd day of treatment. The intensity of the acute pain syndrome after 1-week treatment at rest (t = 4.71, p < 0.001) and during the movement (t = 3.84,  p < 0.001) more decrease in EG. Satisfaction with the results of treatment was higher in EG.

The purpose of this study was to compare the in vitro digestibility levels and chemical compositions of commercial extruded dry-type adult dog foods with different types of protein contents [fish meat (F-dog foods) (n = 7), lamb meat (L-dog foods) (n = 9), or poultry meat (P-dog foods) (n = 8)]. The in vitro digestion values of premium commercial dog foods were examined at three stages: gastric digestion, small intestine digestion and large intestine digestion/fermentation. The metabolisable energy (ME), crude protein (CP), starch, diethyl ether extract (EE) and ash contents and the in vitro cumulative gas production values of all the premium dog foods differed significantly among the commercial brands in the same category (F-, L- or P-dog foods) (P &lt; 0.05). The crude fibre (CF) and the CP/1 000 kcal ME values of the F- and P-dog foods demonstrated a significant difference among the commercial brands (P &lt; 0.05). The organic matter disappearance (OMd) values of the L-dog foods showed a significant difference among the commercial brands (P &lt; 0.05); but the OMd values of the F- and P-dog foods did not differ among the commercial brands (P &gt; 0.05). The average values of the OMd for the F-dog foods were more rapid than the average for the L- and P-dog foods, in the evaluation of all the foods (P = 0.001). Besides, the price of the L-dog foods was positively correlated with the OMd and CP of the L-dog foods; however, it was negatively correlated with the NFE (nitrogen free extract) and CHO (total carbohydrates) of the L-dog foods (P &lt; 0.05). The CP values of the L-dog foods were positively correlated with the OMd values (P &lt; 0.05). Although price is an important determinant of food quality in the L-dog foods, it is not in the F- and P-dog foods. In the general evaluation of all the dog foods, there was no correlation among the food price and the digestibility and the nutrient content for all of the premium dog foods. The present study indicated that the energy, nutrient matter and digestibility of premium dog foods changed with the change in the variety and the amount of the feedstuffs. The digestibility of the dog foods with the fish meat were higher than those of the other dog foods. The amount of protein that an adult dog will receive with 1 000 kcal of DM (dry matter) consumption of premium dog foods with fish meat and chicken meat, varied among the brands. This point showed the need to pay attention to the food consumption amount of the dogs and the energy-protein balance in their diets, especially dog foods with fish meat and chicken meat.

The paper presents the analysis of the results of combined photon-neutron therapy (CPNT) in patients with malignant tumors of head and neck (H&N) inChelyabinsk region clinical oncological dispensary. In the basic group of patients with tumors of a throat the survival rate without relapses (SRWR) was 65 %, that is 30 % higher than in control group - 35 %. The general survival rate (GSR) was accordingly - 76 % and 66 % (р <0.005). In the basic and control group of patients with tumors of salivary gland WR the general survival rate was accordingly - 40 % and 14 % (p> 0.05). At tumors of the pharynx three - year WR after course of CPNT was 18.3 % higher (р=0.002) than in group thet underwent photon therapy. GSR was accordingly 46 % and 20 %. At tumors of oral cavity the CPNT increases survival rate without relapses at 11.5% and GSR at 17 %, in comparison with photon therapy (р <0.005). WR at tumors of nasopharynxes after CPNT was 62 %, and in control group 60 %. (р=0.05). When the size of tumor was T1 the GSR in basic group was 81 %, T2 - GSR was 62 %, and T3 - GSR was 70 %; In the control group accordingly was 52 %, 56 %, 35 %. CPNT in the program of radical course of PT increases a median of full tumor resorptions up to 84.52 % in the basic group, in control accordingly – 46.7 %. CPNT increases SRWR and GSR. The most meaningful difference in results of treatment according to SRWR and GSR was observed in patients with the size of primary tumor T3. Increase of SRWR and GSR was in groups at patients with tumors of a throat, salivary gland, pharynx where average tumor depth was up to 3.7- 4.5 cm.

ABSTRACT Objective: To analyze reformatted sagittal sternal tomography images and classify sternal body curvature types, and compare different types of pectus populations with one another and with normal individuals. Methods: In total, 50 controls and 167 pectus patients were selected for chest CT to analyze the median sagittal plane, of whom 89 had pectus carinatum (mean age, 12 ± 10 years) and 78 pectus excavatum (mean age, 14 ± 10 years). Clinical types of pectus were classified as inferior, superior, or lateral pectus carinatum, and localized or broad pectus excavatum. The following types of sternal patterns were defined: gradual vertical curve, gradual posterior curve, gradual anterior curve, proximal third curve, middle third curve, distal third curve, anterior rectilinear, vertical rectilinear, and posterior rectilinear. Statistical analyses were performed to compare the different types of pectus with one another and with the control group. Results: Patients with different thoracic deformities, but with similar sternal curvature patterns, were observed. Some types of sternal curvature were significantly more frequent in certain types of pectus (p < 0,05). The gradual vertical curve and anterior rectilinear types prevailed in controls (p < 0,05). Conclusion: Some sternal curvature patterns were more frequent than the others in certain types of pectus and the controls. From 1999 to the present time in the Russian RITO n.a. R.R. Vreden 28 knee joint replacements were performed using bone autoplasty in patients with stage V aseptic necrosis of the tibial condyles. The control group included 150 patients who underwent knee arthroplasty using a standard technique. All the patients were monitored in the preoperative and postoperative periods in time from 3 to 7 years. Results of treatment were assessed using a 100-point Josef and Kaufman scale, as well as the scale of WOMAC. Among the patients were 128 (71,9%) women and 50 (28,1%) males, mean age was 67 years old. If comparing experimental and control groups in the preoperative period showed a significant difference, the postoperative results were identical. Bone autotransplantation during arthroplasty of the knee is a simple and effective method used in large defects condyles of the femur and tibia (more than 1/3 of the condyle, the depth of the defect over 10 mm).

This article presents the results of treatment of patients with ulcerative gastroduodenal bleeding who were treated in the 2nd clinic (surgery for the improvement of doctors) Military Medical Academy at the I.I. Dzhanelidze Research Institute of Emergency Medicine. A retrospective analysis was conducted of the frequency of rebleeding, surgical activity and mortality in groups with the use of transcatheter arterial embolization with and without it. The criteria for inclusion in the study were: the presence of ulcerative gastroduodenal bleeding, confirmed by laboratory and instrumental methods of examination, severe general somatic condition of patients. The comparison was carried out in the main and control groups. The main group consisted of 20 patients who underwent endovascular hemostasis. The control group included 46 patients without the use of X-ray surgical methods. The average age of patients in the main and control groups was 65.5 3.7 and 60.7 3.9 years. In both groups, most patients were admitted later than 24 hours after the onset of the disease and with severe blood loss. The severity of the somatic state of the intervention was assessed according to the APACHE II multiple organ failure assessment scale, according to which, patients in the main group were somatically more severe than in the control group. In the groups, bleeding from stomach ulcers prevailed (up to 75%). Most often, endovascular hemostasis is performed at a high risk of recurrent bleeding, and adhesive compositions and spirals were used as an embolizing agent. Angiography revealed direct or indirect signs of bleeding (extravasation, hypervascularization, aneurysmal dilatation) in 12 cases, and preventive embolization was performed in 8 cases. In 70% of cases, the source of bleeding was the left gastric artery. In the control group, recurrent bleeding and surgical activity accounted for 26%. In the main group, 20% and 15%, respectively. In the main group, recurrent bleeding occurred in 4 cases, 1 patient underwent repeated endoscopic hemostasis, 3 patients underwent open surgical interventions. All 4 patients had a fatal outcome, against the background of massive blood loss and aggravated somatic pathology. The overall mortality rate in the control group was 44%, in the main group 35%. Transcatheter arterial embolization did not significantly improve the results of treatment of ulcerative gastroduodenal bleeding due to the severity of the general somatic condition of the patients included in the study.

Rat was used to characterize Chinese Yam polysaccharides (CYPs). InExp. 1, maximum volume and rate of gas production in CYP 3-supplemented group were higher than other CYP-supplemented groups and control group, while pH values and NH3 contents in CYP 2-, CYP 3-, and CYP 4-supplemented groups were lower than control group. Contents of acetate, propionate and butyrate increased by supplementing CYP 3 or CYP 4 compared to other groups, except for glucose-supplemented group. Contents of isobutyrate for CYPs groups decreased compared to control group. CYP 3 enhanced beneficial gut microbiota, but suppressed bacterial pathogens. InExp. 2, contents of acetate and butyrate in cecal digesta of rats fed 0.25 or 0.5 g/kg CYP 3 were higher than other groups on day 7. pH values in 0.25 and 0.5 g/kg groups were lower than 1.0 g/kg group. Contents of acetate in 0.25 and 0.5 g/kg groups were greater than other 2 groups on day 21. Gut microflora in CYP 3-supplemented rats had greater diversity than non-supplemented rats. CYP 3 enriched beneficial gut microbiota, but suppressed bacterial pathogens in rat cecum. These findings suggested that CYP 3 is a good source of carbon and energy, and may improve bacterial community diversity and modulate short-chain fatty acid production in hindgut of rats.

Background: To use the meta-analytic approach to assess the effectiveness of different types of movement programs on motor competence (MC) in participants of all ages. Methods: Studies were retrieved by searching 13 databases and included when criteria were met. Studies were selected, and data were extracted by 2 authors. Random effects models using the standardized mean difference effect size (ES) were used to pool results. Risk of bias, heterogeneity, and inconsistency were examined. Results: Thirty-six studies met the inclusion criteria. A total of 374 ESs were calculated and partitioned into 4 groups (motor intervention, free play, physical education classes, and control group). Statistically significant improvements in MC were observed for the motor intervention (ES = 1.50; 95% confidence interval [CI], 1.18 to 1.82; n = 36), as well as for free play (ES = 0.33; 95% CI, 0.09 to 0.57; n = 5), physical education classes (ES = 0.52; 95% CI, 0.08 to 0.97; n = 15), and smaller statistically significant differences in MC were observed for the control groups (ES = 0.16; 95% CI, 0.01 to 0.31; n = 6). Conclusions: All 4 groups analyzed improved MC in children, adolescents, and young adults. However, motor interventions were superior to all other groups for improving MC.
