The search for new ways of early detection and improving the effectiveness of treatinginflammatory periodontal diseases is one of the urgent tasks in modern dentistry. Inflammatory diseases in the periodontal tissues lead to the loss of teeth, the appearance offoci of chronic infection in the oral cavity, a decrease in the body's reactivity, microbial sensitization, and the development of allergic conditions. This pathological process is not a strictly limited pathology but, as a rule, is just one of the manifestations of more serious systemic diseases. Relevance. The relevance of the study is dictated by the high prevalence of periodontal disease among young people and by the need to search for new approaches to periodontitis treatment in order to increase the effectiveness of treatment at early stages of pathological process. Objective. To evaluate the effectiveness of complex treatment of chronic generalized periodontitis (CGP) of mild severity using a removable splinting construction made according to the author's technique. Materials and methods. At the orthopedic stage of the complex treatment of mild CGP, splinting constructions were used for 47 young patients: in the main group (n = 16) the authors treatment and preventive splint, in the first comparison group (n = 15) a removable metal splint, in the second comparison group (n = 16) a fixed adhesive fiber splint. Periodontal hemodynamics was assessed using ultrasound Doppler analysis 1, 6 and 12 months after the therapeutic measures and tooth immobilization. To identify the possible negative impact of the treatment on the quality of life (QOL) in patients with splint constructions, we used the OHIP-14-Ru questionnaire; the follow-up period was 1.3, 6, and 12 months. Results. Dental immobilization contributed to the improvement of blood flow in the periodontal tissues in the short and long term follow-up, so after 12 months: Vam in the main group increased by 39,8 % and amounted to 0,397 0,004 cm / s, in patients of the comparison groups Vam was 0,329 0,006 cm / s and 0,335 0,003 cm / s, respectively, that is 15,4 % and 18,4 % higher compared with the figures before treatment. The quality of life index for patients using splinting constructions made according to the author's technique, by the OHIP-14-Ru questionnaire was assessed as good 1 year after (3.46 0.59). Conclusions. The use of the proposed original orthopedic design in a complex plan for mild periodontitis treatment helps to improve regional blood circulation and stabilize the pathological process.

Analysis of the current literature shows that treatment and prevention of inflammatory diseases of periodontal tissues remains an actual problem of dentistry in the XXI century. Existing approaches to the treatment, prevention and rehabilitation of patients with inflammatory diseases of parodont have certain disadvantages. The purpose of this study was to obtain positive results from the use of modulated diode light in the red spectrum region in the complex treatment of chronic periodontitis. The study shows the use of opportunities of determination of socio-dental indicators the social impact of dental disease in the clinical assessment of the status of periodontal tissues in the patients with chronic inflammatory diseases of gum. 59 patients were divided into 2 groups. The first group received traditional therapy and photodynamic therapy. The second group was a control. Assessment of the effect of dental disease on quality of life was carried out in all patients. The results of the study showed that after the complex treatment of chronic generalized periodontitis using modulated diode light in the red spectrum region, changes in the assessment of the impact of dental disease on quality of life are of stable positive. The results of the research motivate the necessity of applying and evaluating the impact of dental disease on quality of life in the rehabilitation of patients with chronic generalized parodontitis. The results of this study showed that after the complex treatment of chronic periodontitis using modulated diode light in the red spectrum region, changes in psychological status of patients have positive dynamics.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

The review of the scientific literature is devoted to the topical issues of studying the etiology of periodontal diseases, which are one of the most common and complex pathologies of the maxillofacial region. Analysis of recent studies proves a stable relationship between the development of periodontal diseases and disorders in the immune system, the neurohumoral system, metabolic disorders, genetic predisposition, and so on. The article presents the data obtained in the course of studying the literature on the role of disorders in the functioning of individual organs (gastrointestinal tract, liver, lungs, heart, and urinary system) in the development of chronic periodontal diseases. The article notes that the anatomical and physiological proximity of the periodontal and digestive tract tissues, the generality of innervation and humoral regulation create prerequisites for the involvement of periodontal disease in the pathological process in diseases of the gastrointestinal tract. One of the main etiological factors in the development of inflammatory diseases of the gastrointestinal tract and periodontium is Helicobacter pylori, which is found in the loci of the oral cavity: in the oral and gingival fluid, on the mucous membrane of the tongue and cheeks, and in the periodontal pockets. It is pointed out that the liver also occupies a special place in the development of periodontal diseases, which is explained by the performance of its significant functions for the human body: regulatory, metabolic, antitoxic and other. There is evidence that the pathology of periodontal disease plays a leading role in the structure of dental diseases in patients with chronic obstructive pulmonary diseases, which is clinically manifested by symptoms of generalized periodontitis of the І-ІІ degrees of development and its complications - partial or complete secondary adentia, and with tooth preservation - defects in dental series and violations of occlusion, function, aesthetics. Scientists suggest a general biological mechanism for the development of generalized periodontitis and cardiovascular diseases, linking the development of periodontal diseases in patients with cardiovascular pathology with microcirculatory disorders. The dependence of the severity of inflammatory changes in the periodontal tissues on the disturbances of salt metabolism in urolithiasis is proved. The data obtained indicate that diseases of the internal organs contribute to the structural damage of periodontal tissues and they are a risk factor for periodontal diseases, which necessitate the presence of not only theoretical knowledge and practical skills in dentistry, but also their awareness of the features and clinical manifestations of somatic pathology. An urgent and justified step in the treatment of periodontal diseases is also the involvement in the process of rendering complex dental care to internist doctors capable of quickly and qualitatively assessment the condition of the internal organs and the basic systems of the patient's body.

Currently, regardless of the age and social affiliation of the population, periodontal disease is the most common. Despite the modern technologies used in dentistry, the issues of prevention and treatment in dentistry remain unresolved. Dentists are interested in the fact that periodontal diseases are inextricably linked with systemic disorders in the body, which are responsible for its physiological functions. And the effectiveness of treatment for periodontal diseases is affected by all the factors that lead to the pathological process, i.e. diseases of internal organs and periodontal diseases are inextricably linked. Relevance. One of the most pressing problems in dentistry today is periodontal disease. According to the World Health Organization (WHO), tooth loss from gum disease develops 5 times more often than in caries complications and is the second most common. Periodontal diseases are diagnosed in 85–95 % of the adult population of our country over the age of 35. Goal. Based on clinical data and quality of life of patients to analyze and evaluate the effectiveness of Tantum Verde® in comparison with the traditional scheme. Materials and methods. Examination and comprehensive treatment of 86 young people aged 20 to 45 years (42 men and 44 women), of which 66 were patients with generalized periodontitis initial–I degree, chronic course, and 20 patients with intact periodontium. The 1st (20 patients) group included persons who get traditional periodontal treatment, the 2nd comparison group (46 patients) consisted of participants who additionally received Tantum Verde®. Objective examination of the condition of periodontal tissues was performed using indices: hygienic OHI-S, periodontal PI, papillary-marginal-alveolar PMA. Assessment of the quality of life of patients with generalized periodontitis initial–I degree, chronic course was performed using the questionnaire SF-36, adapted for patients with inflammatory periodontal diseases. Results. As a result of the treatment, positive results were achieved in both groups. Clinically, there was a thickening of the gums, no redness and swelling of the interdental papillae. A significant decrease in periodontal indices was observed in patients of both the 1st and 2nd groups. In a comparative aspect, it was noted that the reduction in OHI-S and PI was effective in all study participants, PMA – more pronounced in those who used Tantum Verde®. Patients of the second group noted an improvement in the clinical condition of periodontal tissues by 89.7 %; physical condition by 86 %; psycho-emotional state by 84.1 %; social function – by 64.7 %; from the block of questions about the peculiarities of the treatment of many patients worried about the fear of future treatment, uncertainty about the effectiveness of therapy and the use of interdental hygiene, indicators in this group decreased by 78.9 %. Conclusion. The assessment of the quality of life of patients showed that the use of Tantum Verde® gives a significant improvement in quality of life, which is manifested in a reduction in the number of exacerbations and increase the duration of remission. Микрофлора человека - сложная саморегулирующая система, способная восстанавливаться при грамотной коррекции. Исследования последних лет показали, что нарушение нормального микробиоценоза полости рта растет из года в год и у жителей Российской Федерации превышает 90 %, вызывая при этом воспалительные заболевания в тканях пародонта, и, как правило, сопровождается дисбиозом полости рта, выраженность которого соответствует степени поражения пародонта. Одно из ведущих мест в развитии данной патологии занимает резидентная облигатно-анаэробная и микроаэрофильная микрофлора полости рта. Изучение микробиологического состава пародонтальных карманов (ПК) при воспалительных заболеваниях тканей пародонта (ВЗТП) имеет большое значение непосредственно для эффективного лечения и понимания патогенетических этапов этих заболеваний. Достаточно серьезной проблемой современной стоматологии как в нашей стране, так и за рубежом являются заболевания пародонта. Указанная патология снижает принятый ВОЗ критерий качества жизни и приводит к преждевременной потере зубов и представляет серьезную медицинскую, социальную и экономическую проблему. Первичным фактором, вызывающим поражение пародонта, являются бактерии зубного налета. Этиологическая структура инфекционных процессов в последнее десятилетие значительно изменилась, что связано с постоянной эволюцией микробов и вовлечением в патологический процесс условно-патогенных микробов, которые могут выступать в качестве комменсалов в составе нормальной микрофлоры и проявлять свою патогенность при снижении иммунного статуса организма The human microflora is a complex self-regulating system that can be restored with proper correction. Recent studies have shown that the violation of the normal microbiocenosis of the oral cavity increases from year to year and in the residents of the Russian Federation exceeds 90 %, causing inflammatory diseases in the periodontal tissues, and, as a rule, is accompanied by dysbiosis of the oral cavity, the severity of which corresponds to the degree of periodontal damage. One of the leading places in the development of this pathology is occupied by the resident obligate-anaerobic and microaerophilic microflora of the oral cavity. The study of the microbiological composition of periodontal pockets (PC) in inflammatory diseases of periodontal tissues (VSTP) is of great importance directly for the effective treatment and understanding of the pathogenetic stages of these diseases. Quite a serious problem of modern dentistry both in our country and abroad are periodontal diseases. This pathology reduces the WHO-accepted quality of life criterion and leads to premature tooth loss, and is a serious medical, social and economic problem. The primary factor that causes periodontal disease is plaque bacteria. The etiological structure of infectious processes has changed significantly in the last decade, which is due to the constant evolution of microbes and the involvement of conditionally pathogenic microbes in the pathological process, which can act as commensals in the normal microflora and show their pathogenicity with a decrease in the immune status of the body

Воспалительные заболевания тканей пародонта (ВЗТП) являются актуальной проблемой современной стоматологии, которая приобретает большое медико-социальное значение. В большинстве случаев отсутствие адекватной гигиены полости рта и профилактики стоматологических заболеваний приводит к быстрому развитию воспалительного процесса в тканях пародонта. Следствием чего является преждевременная потеря зубов, нарушение функции жевания и эстетики. В настоящее время возрастает интерес ученых к проблеме стоматологического здоровья и профилактики как механизма повышения качества жизни пародонтологических больных. Согласно своему определению качество жизни определяет показатели, которые тесно связаны между собой и с состоянием здоровья человека. Исследования, которые проводились в нашей стране, показали, что весомое большинство населения не имеет понятие о данной проблеме, не понимает самого термина «качество жизни», ни его факторов, которые и определяют само качество жизни. В данной статье нами были рассмотрены результаты анкетирования пациентов, которые обратились за пародонтологической помощью. The preventive direction in dentistry is the leading one today. Individual oral hygiene plays a significant role in preventing the development of inflammatory periodontal diseases. The use of therapeutic and prophylactic toothpastes containing natural components with antibacterial properties is promising. Among plant extracts, grapefruit extract has the most pronounced antimicrobial activity. LysoTech and ProteLyse lysates, which have antimicrobial and immunomodulatory effects, have long been used in dentistry in the form of tablets for resorption in the complex treatment of various diseases. Their introduction into toothpastes is also considered a promising direction for the prevention and treatment of oral pathology. The aim of our study was to clinically evaluate the effectiveness of therapeutic and prophylactic toothpaste, which contains grapefruit extract and LysoTech and ProteLyse lysates, in patients with inflammatory diseases of periodontal tissues. A clinical and laboratory comparative study of two therapeutic and prophylactic toothpastes was conducted. The composition of the samples was identical, except for two components: grapefruit extract and LysoTech and ProteLyse lysates. The study involved 60 volunteers who were divided into two groups depending on the toothpastes used. The purifying and anti-inflammatory effects of the samples were evaluated for one month. An in vitro laboratory study was also conducted to identify the antimicrobial properties of toothpastes. During the work, high clinical results were noted in a group of volunteers using toothpaste with grapefruit seed extract and bacterial lysates, which are expressed in a decrease in PMA and SBI indices, and an improvement in oral hygiene. According to a laboratory study, the toothpaste containing grapefruit extract and LysoTech and ProteLyse lysates has a more pronounced antimicrobial and antifungal effect, which indicates a high preventive and therapeutic potential of the test sample.

The polyethiology of periodontal diseases introduces certain difficulties in the diagnosis and adequate selection of treatment methods and dictates the conditions under which the complexity and individual approach are the main conditions for successful treatment. Therefore, it is very important to timely diagnose and correctly determine the prognosis of the disease, which directly depends on the level of professional knowledge of dentists. The aim of the study was to analyze the level of competence of dentists on the methods of diagnosis and classification of periodontal tissue diseases in patients in Bishkek. Questioning of respondents — 87, statistical. For the study, a questionnaire “Dentist’s awareness of modern principles of diagnosis and treatment of periodontal tissues” was developed, containing 14 questions that reveal the ideas of dentists-therapists about modern principles of classification, diagnosis and methods of treatment of inflammatory diseases of periodontal tissues. According to the results of the survey, low competence in the diagnosis of periodontal diseases was revealed: 34.5% of the respondents are well aware of classification issues; adequate evaluation indices for assessing the hygienic state of the oral cavity and inflammatory changes in the tissues of the gums are used only in 20.7% and 77.0% of cases, respectively; laboratory and diagnostic research methods are used only by 59.8%. The use of a large number of diagnostic and prognostic indices in periodontics makes it possible to objectively control the dynamics of the disease, assess the depth and prevalence of the pathological process, carry out mathematical processing of the results obtained and compare the effectiveness of various methods of treating periodontitis.
