REORGANIZATION OF A MULTI-SPECIALTY HOSPITAL IN AN UNFAVORABLE EPIDEMIOLOGICAL SITUATION

This study provides a retrospective analysis of work to ensure high quality of medical care in an unfavorable epidemic situation. The consequence of COVID-19 was the implementation of a program to prevent the spread of infection, the re-profiling of medical institu-tions, and the introduction of restrictive and anti-epidemic measures. The experience of our work has shown the effectiveness of changing the order of med-ical care, the organization of the functioning of de-partments and patient routing. The study reflects the measures implemented in the hospital departments, the Department of clinical and laboratory diagnostics, radiation diagnostics and pathology Department. To ensure the safety of patients, it was decided to place patients on a single bed according to the type of infec-tious boxes. The safety of employees was ensured by the use of personal protective equipment, minimiza-tion of contact time with patients, and preventive weekly examination of staff for SARS-CoV-2 infection. The organized and well-coordinated work of the en-tire staff of the institution made it possible to prevent the spread of COVID-19 among employees, to detect cases of infection in a timely manner, and to carry out appropriate isolation and monitoring measures. At the time of completion of infectious diseases departments, the mortality rate among patients was less than 9%. Our experience in reorganizing a multi-specialty facil-ity can be used in the future when working with pa-tients who have undergone COVID-19, as well as in the context of a worsening epidemic situation.

Objective. To determine frequency of retinopathy cases in profoundly preterm newborns treated at Gorodkov Ivanovo Scientific Research Institute of Maternity and Childhood in 2016-2017, as well as to evaluate impact of the risk factors on disease severity in the conditions of specialized medical care. We examined 344 profoundly preterm newborns with gestational age of less than 32 weeks. 83 children (24.1%) were diagnosed with retinopathy of preterm newborns. A high quality of medical care was proved by a low percentage of children with severe forms of retinopathy in preterm newborns and absence of blindness in the children. A low gestational age, low Apgar score at the end of the first minute of life and traditional artificial lung ventilation are the most significant factors of severe retinopathy of preterm newborns.

One of the most urgent areas of Russian health care is the organization of medical care in the new socio-economic conditions, aimed at providing affordable high-quality and safe medical care to the population, therefore, a new approach to the formation of control over both the quality of medical care and its safety is of particular interest. At all stages of the treatment and diagnostic process, serious mistakes can be made that can have a negative impact on the life and health of patients.The article discusses the methodological aspects of assessing the quality of medical care at all stages of the treatment and diagnostic process. Requirements for the formation of an expert opinion, which is prepared based on the results of the examination of the quality of medical care, have been formulated. In order to effectively manage the quality of medical care, the author proposes the creation of a digital “smart” system for monitoring defects in medical care.

The aim: To develop recommendations aimed at optimizing the work of the Shymkent city multidisciplinary hospital by studying the main performance indicators. Methods.The research was carried out according to the traditional scheme and included the classical stages: drawing up a plan and program, collecting material, processing and analyzing it. Results. To improve the quality of medical care, targeted measures are being implemented. Thus, the design capacity of Primary Healthcare: polyclinic – 500 people per shift, actual attendance – 703 people per shift. The hospital has 330 beds. However, the rate of bed coverage per 10 000 people is low at 28.4. The overall incidence has decreased, and in 2019 it was 75 652. It was found that the number of visits to doctors has increased, thus, this figure in 2019 reached 66.5%. It was found that the volume of provision of free medicines increased and reached 47 420. The indicator of provision with medical technical means as a whole also increased and amounted to 82%. Conclusions. A set of measures aimed at improving the hospital's activities using the existing organizational structure will allow us to respond in a timely manner to changes in all areas of the hospital's activities, and the KPI system will help assess the effectiveness of managerial actions in the field of quality management to improve the quality of medical care and improve the weaknesses of healthcare facilities. The article discusses different approaches to assessing mortality during the COVID-19 pandemic. The analysis of inter­national data shows that different countries use different approaches to assessing mortality from COVID-19 and the increase in mortality from all causes. The most probable reasons for the variability of the increase in mortality rates are the rate of spread of infection, the quality of isolation and quarantine measures, the commitment of the population to their implementation, the resource capacity of the healthcare system and the quality of medical care (both for the treatment of COVID-19 and other diseases), features of living conditions, socio-economic and political processes that are difficult to formalize (and therefore assess the contribution by methods of mathematical statistics). For a correct comparison of excess mortality rates, it will be necessary to calculate standardized indicators and compare data in five-year age groups.In 2021, serious problems should be expected with the comparison of mortality rates in different countries from individual causes. The methodology for recording deaths from COVID-19 and other causes is not uniform at the global level, prob­lems are associated with differences in approaches to determining the primary cause of death, difficulties in determining the cause of death if a patient has multimorbid pathology (especially without postmortem examination). A full-fledged analysis is possible only with transdisciplinary cooperation under the auspices of the WHO of doctors, mathematicians, economists, and information technology specialists.

Nowadays modern medical commercial organizations in Russia carry out a paid reception of the residents for the provision of paid medical services. The demand for such services has increased significantly today due to the heavy workload of state medical institutions. However, today consumers choose the most high-quality services in the medical field, so the economic activity of medical commercial companies directly depends on the extent where specialists can provide patients with an individual approach and high-quality treatment. The purpose of the work is to study the efficiency of a commercial organization based on the analysis of its financial documentation. The author comes to the conclusion that the medical organization "Medicon+" LLC has significantly improved its financial performance during the analyzed period. This indicates that the level of services provided, as well as their demand by the citizens, is constantly growing. The increase in the number of patient visits to specialists of the analyzed clinic is also caused by the increase in the incidence of the population due to the pandemic and the decrease in the capacity of public medical institutions in this regard. However, according to the analysis, it should be concluded that in the future, taking into account the quality of services provided and recognition, the clinic of LLC "Medicon+" will be able to function effectively even after the stabilization of the epidemiological situation in the country and in the world.

Free-cutting steel gains specific working properties thanks to the high content of sulfur and phosphorus. These elements, especially sulfur, have a rather high tendency to segregation. Therefore, segregation defects in free-cutting steel continuously cast billets can be significantly developed. The aim of the work was to study the influence of the chemical composition of freecutting steel and casting technological parameters on the quality of the macrostructure of continuously cast billets. A metallographic assessment of the internal structure of cast metal made of free-cutting steel and data processing by application of correlation and regression analysis were the research methods. The array of production data of 43 heats of free-cutting steel of grade A12 was studied. Steel casting on a five-strand radial type continuous casting machine was carried out by various methods of metal pouring from tundish into the molds. Metal of 19 heats was poured with an open stream, and 24 heats – by a closed stream through submerged nozzles with a vertical hole. High-quality billets had a cross-sectional size of 150×150 mm. The macrostructure of high-quality square billets made of free-cutting steel of A12 grade is characterized by the presence of central porosity, axial segregation and peripheral point contamination, the degree of development of which was in the range from 1.5 to 2.0 points, segregation cracks and strips – about 1.0 points. In the course of casting with an open stream, almost all of these defects are more developed comparing with the casting by a closed stream. As a result of correlation and regression analysis, linear dependences of the development degree of segregation cracks and strips both axial and angular on the sulfur content in steel and on the ratio of manganese content to sulfur content were established. The degree of these defects development increases with growing of sulfur content in steel of A12 grade. These defects had especially strong development when sulfur content in steel was of more than 0.10%. To improve the quality of cast metal, it is necessary to have the ratio of the manganese content to the sulfur content in the metal more than eight.

The experience of clinical testing of the personal telemedicine system ‘Obereg’ for remote monitoring of patients at the intensive care units of leading Russian clinics is described. The high quality of communication with the remote receiving devices of doctors, the accuracy of measurements, resistance to interference from various hospital equipment and the absence of its own impact on such equipment were confirmed. There are significant advantages compared to stationary patient monitors, in particular, for intra and out-of-hospital transportation of patients.
