Acute appendicitis is the most common abdominal surgical emergency despiteadvances in radiographic imaging and diagnostic laboratory investigations; the diagnosis ofacute appendicitis remains challenge. Objectives: To evaluate the C-reactive protein in patientswith acute appendicitis. Patients and methods: This cross sectional descriptive study of sixmonths study was conducted at Liaquat University Hospital Hyderabad. All the patients withacute appendicitis were admitted in the ward and were further evaluated for C-reactive protein.The data was analyzed in SPSS 16 and the frequency and percentage was calculated. Results:One hundred subjects with acute appendicitis were recruited and studies. The mean age ±SD ofthe patients was 27.83±07.52 in overall population while it was 29.73±06.64 and 25.84±04.92in male and female subjects with acute appendicitis. The CRP was raised in 60% patients. Thedistribution of age in relation to gender and CRP was statisitically significant (p= 0.01 and<0.01) whereas the CRP was also observed as statistically significant in context to genderand histopathology (p= 0.02 and 0.03) respectively. The mean ±SD of CRP was 7.53±1.52in overall population while it was 6.84±1.64 and 8.65±1.53 in male and female patients withraised CRP. Conclusions: The CRP was elevated in patients with acute appendicitis.

Objective: To determine the frequency of raised C-reactive protein (CRP) inpatients with metabolic syndrome. Patients and methods: This cross sectional descriptivestudy of six months study was conducted at Liaquat University Hospital Hyderabad. All thepatients of 20 to 60 years of age, of either gender presented with symptoms of metabolicsyndrome for more than 01 year duration were admitted and evaluated for C-reactive protein.The data was analyzed in SPSS 16 and the frequency and percentage was calculated. Results:Total one hundred patients (59 females and 41 males) with metabolic syndrome were evaluatedfor C-reactive protein. The mean waist circumference for males and female patients with raisedCRP was 48.93±2.33 and 48.42±3.41 while the mean ±SD of serum triglycerides level for maleand female with raised CRP was 192.41±3.21 and 196.31±3.43 respectively. The mean ±SD ofserum HDL-C level for male and female with raised CRP was 28.32±1.22 and 25.31±1.42. Themean ±SD of systolic and diastolic pressure for males and female patients with raised CRP was150±3.42 and 100.51±4.42. The mean ±SD of serum fasting blood sugar for male and femalepatients with raised CRP was 131.52±3.33 and 143.42±7.42 respectively. The mean ±SD ofCRP for male and female patients with raised CRP was 4.42±1.21 and 5.8±2.52. In relationto gender distribution, the majority of subjects from 40-49 years of age group with femalepredominance (p = 0.01) while the CRP was raised in 67% patients in relation to age (p=0.05)and gender (p=0.04) respectively. Out of 67 subjects with raised CRP 44 were females and 23were males.Conclusions: The CRP was raised in patients with metabolic syndrome.

Objectives: To determine the frequency of raised serum CRP level in patientswith community acquired pneumonia. Study Design: Cross sectional descriptive study.Period: Six months. Setting: Liaquat University Hospital Hyderabad. Patients and Methods:All the patients with 20 -75 years of age, either gender diagnosed as community acquiredpneumonia were further evaluated for C-reactive protein while the data was analyzed in SPSSversion 16. Results: During six month study period, total 135 patients with community acquiredpneumonia were evaluated for C-reactive protein. The mean ±SD for age of patients with CAPwas 48.93±8.41 whereas it was 53.53±6.73 and 50.54±5.81 in male and female subjectsrespectively. The mean age ±SD of patient with raised CRP was 46.94±8.43. The mean ±SDof CRP in overall population was 08.8±1.52 while it was 07.94±1.32 and 10.83±1.64 in maleand female individuals respectively. Of 135 subjects 84(62.2%) were males and 51(37.8%) werefemales while the CRP was elevated in 91(67.4%) patients. The age in relation to gender andCRP was statistically significant (p= 0.02 and 0.05) while the gender in relation to CRP was alsostatistically significant (p=0.03). Conclusion: The present study found that the CRP was raisedin 91(67.4%) predominantly male individuals with community acquired pneumonia

Abstract Objective: The objective of this study was to retrospectively compare clinical features and prognostic values between the patients who were referred to the general surgery clinic of our hospital with the presumed diagnosis of acute appendicitis and underwent positive or negative appendectomy.Methods: Patients were divided into two groups as positive (PA) (n:425) and negative appendectomy (NA) (n:425) and the data obtained were compared between these two groups. Laboratory investigations were performed in all patients, and white blood cell (WBC), mean platelet volume (MPV), neutrophils count (NEU), neutrophils (%) (NEU%), C-reactive protein (CRP) and total bilirubin (TBIL) values were studied. Results: The mean MPV value was found as 7.66 in PA groups and 7.90 in NA group, and the mean MPV value was statistically significantly lower in PA group, compared to NA groups (p=0.034). The mean TBIL value was 0.75 in PA group and 0.90 in NA group, and the mean TBIL value was statistically significantly lower in PA group (p=0.034).There was no statistically significant difference between the two groups in terms of the other studied laboratory parameters (for all p>0.05).Conclusion: The mean MPV and TBIL values can be used as specific biomarkers in predicting positive acute appendicitis. We believe that these results will contribute to the literature and will be guiding for future studies.

Laboratory parameters were also compared between genders. Accordingly, the mean MPV value was statistically significantly higher in female patients compared to male patients in PA group (p = 0.04). The mean TBIL value was 0.97 mg/dl in PA group and 0.69 mg/dl in NA group, and the mean TBIL value was statistically significantly higher in PA group (p< 0.001). Finally, TBIL value was statistically significantly lower in female patients compared to male patients in NA and PA group (p < 0.05).Conclusions: According to the results of our study, MPV and T. BIL values differ in PA and NA groups depending on gender. Therefore, these values may not be used as specific biomarkers in predicting positive acute appendicitis. We believe that these results will contribute to the literature and will be guiding for future studies.

Background: Sulfur mustard (SM) is a chemical warfare agent that can cause serious pulmonary complications. This study was designed to determine serum highly sensitive C-reactive protein (hs-CRP) and evaluate its correlation with lung function parameters in patients with chronic obstructive pulmonary disease (COPD) due to SM poisoning. Methods: Fifty consecutive SM patients with stable COPD and a mean age 46.3 ± 9.18 years were enrolled in this cross sectional study. Thirty healthy men were selected as controls. Lung function parameters were evaluated. Serum hs-CRP by immunoturbidometry assay was measured in both the patients and controls. Results: In the case group, the mean forced expiratory volume in one second (FEV1) was 2.14 ± 0.76 L (58.98% ± 17.51% predicted). The mean serum hs-CRP was 9.4 ± 6.78 SD and 3.9 ± 1.92 SD mg/L in the cases and controls, respectively, with significant statistical differences (p < .001). There was negative correlation between the serum hs-CRP and FEV1 levels (p = .01). The serum hs-CRP levels were also correlated with Global Initiative for Chronic Obstructive Lung disease (GOLD) stages (r = .45, p < .001). Conclusions: Our findings suggest that the serum hs-CRP level is increased in SM patients with COPD and may have a direct correlation with disease severity. It may then be used as a marker for the severity of COPD in patients with SM poisoning.

Objective: To determine the frequency of raised C-reactive protein (CRP) in patients with type 2 diabetes mellitus. Patients and methods: This cross sectional descriptive study of six months study was conducted at Liaquat University Hospital Hyderabad from March 2013 to August 2013. All diabetic patients of ≥35 years age of either gender for >01 year duration visited at OPD were evaluated for C-reactive protein and their glycemic status by hemoglobin A1c. The data was analyzed in SPSS and the frequency and percentage was calculated. Results: During six month study period, total 100 diabetic patients were evaluated for C-reactive protein. Majority of patients were from urban areas 75/100 (75%). The mean ±SD for age of patients with diabetes mellitus was 51.63±7.82. The mean age ±SD of patient with raised CRP was 53±7.21. The mean ±SD for HbA1c in patients with raised CRP is 9.55±1.73. The mean random blood sugar level in patients with raised CRP was 247.42 ± 6.62. The majority of subjects from 50-69 years of age group with female predominance (p = 0.01) while the CRP was raised in 70(70%) patients in relation to age (p=0.02) and gender (p=0.01) respectively. Both HbA1c and CRP was raised in 64.9% (p = 0.04) in patients with type 2 diabetes mellitus. The mean ±SD of CRP was 5.8±1.21 while for male and female individuals with raised CRP was 3.52±1.22 and 5.7±1.63 respectively. Conclusions: The raised CRP was observed in patients with type 2 diabetes mellitus

Abstract Objectives The present study aimed to evaluate the presence of any relationship between vitamin D and prognostic factors among patients infected with SARS-CoV-2. Methods This retrospective cross-sectional study was conducted among patients admitted from March to August 2020 in a referral hospital, Tehran, Iran. All patients aged 18–65 who had not any comorbidity participated in the study. The diagnosis of COVID-19 was established using the rRT-PCR test for SARS-CoV-2 detection. Then the incidence of lung involvement and biochemical markers including vitamin D level, c-reactive protein, D-dimer, lymphocyte count, platelet count, white blood cell count, and lactic acid dehydrogenase, were extracted through medical records. Results Altogether, 84 patients met our study criteria, and the information of 68 (80.9%) participants were collected. Among all 68 patients, 45 (66.2%) were male. The mean (SD) age of all participants was 52.4 (10.4) years old. Among 24 (35.2%) clients who died due to SARS-CoV-2, 20 (83.3%) patients were male (p=0.02). The mean (SD) of vitamin D level was 24.1 (13.6) ng/dL. No association between vitamin D level and prognostic factors, including CRP, lymphocyte count, D-dimer, LDH, and HRCT scan score, was observed. Also, the vitamin D level of the dead patients was not significantly different from that of the treated patients. Conclusions Our findings indicated no significant relationship between vitamin D level and prognostic factors or outcomes of patients infected with SARS-CoV-2.

Objective: Role of C-reactive protein value in the diagnosis of appendicitis by correlating with severity of appendicitis on histopathological findings. Methodology: This descriptive study was conducted at the department of general surgery Unit-IV, Liaquat University Hospital Hyderabad from March 2016 to February 2017. All cases above 12 years of age with severe appendicitis based on presenting signs and symptoms were included. The patient underwent routine laboratory investigations including abdominal plain x-ray in an erect position and ultrasound abdomen. The blood sample was taken from each patient for C-reactive protein and was send to Hospital diagnostic laboratory. During surgery, a specimen of each patient was sent to a diagnostic laboratory for histopathology. All the data were record on Performa. Results: A total of 305 patients were studied; their mean age was 35.7+9.7 years. Males were found in the majority (73%). 75.41% of patients had leukocytosis. C-reactive protein was raised among 26.22% of patients. On histopathological findings, 04.91% of patients had a normal appendix, while 69.83% had gross inflammation of the appendix, 19.67% patients had gangrenous appendix, and 05.57% patients had perforation and peritonitis. Elevated C reactive protein (CRP) and leukocytosis were significantly associated with the severity of appendicitis, p value 0.001. Conclusion: C-reactive protein is a non-invasive and reliable indicator for surgical treatment of appendicitis. C-reactive protein and white blood cells (WBCs) were significantly associated with the severity of appendicitis.

AbstractObjectiveTo investigate the association of antioxidant intakes from diet and supplements with elevated blood C-reactive protein (CRP) and homocysteine (Hcy) concentrations.DesignA cross-sectional study. The main exposures were vitamins C and E, carotene, flavonoid and Se intakes from diet and supplements. Elevated blood CRP and Hcy concentrations were the outcome measures.SettingThe US population and its subgroups.SubjectsWe included 8335 US adults aged ≥19 years from the National Health and Nutrition Examination Survey 1999–2002.ResultsIn this US population, the mean serum CRP concentration was 4·14 (95 % CI 3·91, 4·37) mg/l. Intakes of vitamins C and E and carotene were inversely associated with the probability of having serum CRP concentrations >3 mg/l in multivariate logistic regression models. Flavonoid and Se intakes were not associated with the odds of elevated serum CRP concentrations. The mean plasma Hcy concentration was 8·61 (95 % CI 8·48, 8·74) μmol/l. Intakes of vitamins C, E, carotenes and Se were inversely associated with the odds of plasma Hcy concentrations >13 μmol/l after adjusting for covariates. Flavonoid intake was not associated with the chance of elevated plasma Hcy concentrations.ConclusionsThese results suggest that high antioxidant intake is associated with lower blood concentrations of CRP and Hcy. These inverse associations may be among the potential mechanisms for the beneficial effect of antioxidant intake on CVD risk mediators in observational studies.

Objectives: To determine the frequency of microalbuminuria in patients withessential hypertension for postpartum hemorrhage. Study Design: Cross sectional study.Setting: Department of Medicine at Liaquat University Hospital Hyderabad. Period: Six months(from January 2015 to June 2015). Patients and Methods: All the patients ≥35 years of age bothgender had essential hypertension were enrolled and explored for microalbuminuria by urineexamination while the data was analyzed in SPSS 16. Results: Total one hundred subjects withessential hypertension were studied and explored for microalbuminuria. The mean age ±SDof for overall population was 52.82±7.85 and it was 51.23±8.21 and 53.32±6.52 in male andfemale gender respectively. The overall mean ± SD for systolic blood pressure was 170±12.62whereas it was 160.82±10.82 and 165±11.92 in male and female gender respectively. Themean ±SD for blood pressure (diastolic) was 105±71 while it was 95.01±7.21 and 100.82±6.42in male and female sex respectively. The duration of hypertension as mean ±SD was 3.72±2.52in overall population. The microalbuminuria was identified in 59 (59%) patients with statisticalsignificance in relation to age (p=0.00), gender (p=0.00), duration of hypertension (p=0.01),treatment status (p= 0.04), hypertension (p=0.002), BMI (p=0.05) and residence (p=0.00).Conclusion: In present series the prevalence for microalbuminuria in essential hypertensionwas detected as 59% and correlate with age, gender and duration and treatment status of thepatients along with raised systolic and diastolic blood pressure
