Objectives: The knee joint is made of two cruciate ligaments. One is anterior cruciate ligament (ACL) which is weaker when it comes to comparison with the other cruciate ligament known as posterior cruciate ligament (PCL). ACL tears are most common and frequently neglected. ACL tear is affecting 70% of the population and this high incidence reflects the significance of the problem. Reason of restoration of a torn ACL is to provide knee stability, knee motion in a safe wide range and to prevent osetoarthritis OA. The gold standard for ACL auto-graft reconstruction is bone patella-tendon bone (BTB) which is still questioned by many researchers as this technique followed subjects suffered from knee pain. So aim is to use and to access outcomes of hamstring auto-graft for reconstruction ACL by using Tegner’s score. Study Design: Cross sectional study. Setting: Private based hospital, Faisalabad. Period: January 2017 to January 2019. Material and Methods: It was comprised of in comprised of 97 subjects recruited on the base of positive Tegners score. Demographic data including age was presented as mean and standard deviation. Data was stratified for the variables i.e. age, gender, duration of disease and pre-operative Tegner activity rating scale to address the effect modifiers. Post-stratification Chi-square test is applied to check the significance with P-value less than 0.05 as significant. Regression was applied to check the effect of age on Tegner’s scor. Results: This study comprised of 97 subjects (81 male, 16  females83.5.5% and 16.5% respectively from private setup based hospital with mean age of 31±11.1.Our study showed 96.9% subjects with improved Tegner’s scale after ACL reconstruction following hamstring autograph technique. Regression was also applied to check the effect of age on Tegner’s score which was found to be significant (p value0.00*). Conclusion: Hamstring technique showed good outcomes which can be used to reconstruct ACL for better quality lifestyle.

Background: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune, rheumatic inflammatory disease that can cause significant morbidity with evident psychological impacts and obvious harm to quality-of-life that require the patient to adapt treatment. Objective: Assessment of resilience and the self-reported treatment adhesion behaviors of patients with SLE, investigating which of these factors are associated to resilience. Method: Cross-sectional study of 40 women with SLE. A questionnaire with social demographic data, health history and the Wagnild Young Resilience Scale were used. Results: 62.5% followed the medical treatment properly but 55% found it difficult. 27.5% of the patients presented low resilience, 57.5% medium and 15% high resilience. Resilience was associated in the chi-square test (p-value < 0.05) with the variables work, understanding SLE, trying to find out about SLE, following the treatment correctly, difficulty in following the treatment and stopping some activity because of the disease. In the correlation analysis, resilience was associated with age (-0.3960), number of working hours (0.5533), specialized treatment duration (-0.8103) and disease duration from diagnosis (-0.8014). Conclusion: Patients with high resilience tended to follow treatment correctly, tried to understand the disease and adhered more to the treatment to avoid risks and promote protection factors. Therefore knowledge of resilience in patients with SLE is necessary. It is important that the state takes necessary actions to facilitate access to treatment, to educational programs and to medical support. Awareness and counselling sessions must be initiated to develop and promote individual capacities to learn how to tackle with the disease for which psychological support of family and doctors can play a significant role.

Background: Many studies have been conducted worldwide to evaluate the proportion of patients with Celiac disease (CD) in persons presenting with Irritable Bowel Syndrome (IBS), showing a positive association between the two diseases. However, reports from Pakistan remain scanty. This study aims to find out the prevalence of CD in patients presenting with IBS and to establish the correlation of both diseases. Patients and methods: It was a descriptive cross-sectional study conducted at Combined Military Hospital, Lahore (CMHL) from January 2018 till March 2019. Consecutive, nonprobability sampling was used to include 210 patients of both genders with the age range of 15-65 years fulfilling ROME IV criteria and were not previously diagnosed as CD or CKD or CLD. Demographic data was noted. Serum Anti tTG levels and Duodenal Biopsy from the second part was assessed to diagnose CD. CD was labeled if the disease was present on both serological and Histopathological reports. Data were analyzed by using SPSS version 20. Descriptive statistics were calculated and a Chi-square test was used to compare the outcome variables with P-value < 0.05 as significant. Results: Of 210 enrolled patients, 113 (53, 8%) were male and 97 (46.2%) were female. The mean age of the patients was 28.40 ± 5.06 years. Eight patients (3.8%) were positive for Anti tTg and on histopathological findings suggestive of CD. Seven of 8 were less than 35 years old. Out of 8 positive patients, 6 (75%) were females. However, the p-value was found to be insignificant for age (0.549) and gender (0.096). On stratification with respect to duration of IBS, all 8 patients diagnosed with celiac disease were having symptoms of IBS for less than 12 months (100%) which was significant statistically (7.1% vs 0.0%; p=0.007). Conclusion: The frequency of CD was 3.8% among IBS patients. Considering this percentage of CD in IBS patients s, a high index of suspicion for CD in IBS patients is required,

Background: Orofacial Pain (OFP) is a group of non-dental painful conditions affecting the oral cavity and facial area. Objectives: The objective of this study was to explore which barriers to manage the patient with chronic OFP as perceived by general dentists versus dental specialists and to investigate whether if professionals with degrees from their home country versus another country or number of years of professional experience differ in their perceptions. Methods: A closed-end questionnaire was hand-delivered to 600 participants [300 general dentists and 300 dental specialists] in four major provinces in Saudi Arabia. Demographic data were expressed as frequency. Proportional t-tests and chi-square tests were used to analyse intergroup differences. Statistical significance for all analyses was set at P-value < 0.05. Results: Overall, the response rate was 56.6% (340/600). Around two-thirds of the participants were general dentists (60.9%), while the remainder were dental specialists (39.1%). There was an obvious consensus by the participants that “Low payment/reimbursement” and “Lack of OFP knowledge” were among the most common barriers (85% and 83.5%, respectively). In contrast, “Legal risks” were the least frequently reported factor (38.8%). The most commonly reported barrier by general dentists was “Shortage of patients/lack of demand;” this was significantly different from the experience of dental specialists (87% vs. 72.2%; P- value < 0.01). In terms of the country of graduation or years of experience, there were no significant differences. Conclusion: The study demonstrates the existence of many significant barriers other than OFP knowledge, such as reimbursement, facility and demand that could present obstacles and challenges to the management of OFP by general dentists and dental specialists. Most participants believed that dentists should manage this condition and that OFP courses should be included in the dental school curriculum.

BACKGROUND Occupational conditions are deadly health hazards especially where dust exposure is inevitable causing chronic disabilities, impaired respiratory function and ultimately leading to death if no intensive measures are put in place. Unhealthy practices and negative attitudes rise in the number of cases of pneumoconiosis due to poor health education and awareness strategies. Pneumoconiosis is not only a health problem but also a social and economic burden on the livelihood of people living in mining areas around the globe. OBJECTIVE to assess knowledge, attitude and practices of miners and post-occupational miners towards pneumoconiosis in Wusakile Township, Kitwe, Zambia. METHODS A cross-sectional study was employed to conduct a research in Wusakile Township and a questionnaire was customized in order to syphon data relevant to the study as well to be brief. The study was conducted among 73 participants who were randomly selected among miners and post-occupational miners and all satisfied the inclusion criteria. Both quantitative and qualitative methods were used to collect data. The data was entered and analysed using IBM SPSS software version 23. RESULTS Among 73 participants interviewed, 33.99% of participants had poor knowledge on the complications of pneumoconiosis. However, despite this poor knowledge, all participants had an idea about pneumoconiosis particularly silicosis. 13.70% of the respondents had bad practices towards pneumoconiosis while 86.30% had some good practices towards pneumoconiosis. Of the total participants, 19.18% of the participants had a negative attitude towards pneumoconiosis. Correlation between the level of education and practices of participants using Pearson Chi-Square, a p value of 0.021 (significant) was found ruling out the null hypothesis. CONCLUSIONS Information about pneumoconiosis and awareness programs towards pneumoconiosis are not widely disseminated among miners and post-occupational miners. There is still a significant number of participants who need to be educated more about pneumoconiosis and its complications so that attitude and practices are improved and also promote full community participation by involving competent health professionals to help in implementing preventive measures.

Background:Foot disease is a common problem in rheumatoid arthritis (RA). Therapeutic decisions are often based on clinical examination (CE) alone, which can be adversely affected by factors such as deformity, obesity, and peripheral edema. Ultrasonography (US) has previously been shown to be more sensitive than CE for detecting synovitis and tenosynovitis in RA forefeet, but few data exist for the hindfoot and ankle. Objectives:The aim of this study was to compare CE and US for the detection of hindfoot and ankle synovitis and tenosynovitis in patients with established RA.Methods:We conducted a cross-sectional study including patients with RA (ACR/EULAR 2010). Demographic data and disease parameters were collected. CE was performed by a rheumatologist for the presence or absence of tenderness, swelling, and mobility restriction of both ankles. The following tendons were examined for tenosynovitis: tibialis anterior (TA) and posterior (TP), fibularis longus (FL), and brevis (FB) (assessed together). In a second time, US examination of the tibiotalar, talonavicular, and subtalar joints and the same tendons as CE was performed by a blinded radiologist experienced in musculoskeletal imaging using a Philips HD11 device with a high-frequency linear transducer. The presence or absence of synovitis and tenosynovitis was recorded, and the composite synovitis score (power doppler / grayscale ultrasound (PDUS)) was measured for each joint. The US score of each patient was defined by the sum of the composite scores of the joints studied (0-30). A p-value <0.05 was considered significant.Results:Sixty-two feet were examined in 31 RA patients (25 women and six men) with a mean age of 54.8±10.8 years old [32-70]. The mean disease duration was 8.5±7.2 years [1-37]. Rheumatoid Factor (RF) and Anti-Citrullinated Peptides Antibodies (ACPA) were positive in 61.3% and 83.8% of cases. The mean DAS28 ESR was 3.8±1.5 [0.6-7].Clinical examination of ankles revealed tenderness in 57.4% of cases, swelling in 38.8% of cases, and restriction in the range of motion in 11.1% of cases. TA tenosynovitis was noted in 14.8% of cases, TP tenosynovitis in 22.2% of cases, and FL and FB tenosynovitis in 31.5% of cases.US showed tibiotalar synovitis in 59.3% of cases, talonavicular synovitis in 64.8% of cases, and subtalar synovitis in 46.3% of cases. TA tenosynovitis was noted in 5.6% of cases, TP tenosynovitis in 22.2% of cases, and FB and FL tenosynovitis in 25% and 11.1% of cases respectively. An association was found between clinical tenderness and US synovitis of the tibiotalar joint (p=0.013) and the talonavicular joint (p=0.027). No association was noted between clinical swelling and US synovitis in these joints. No association was noted between clinical and US tenosynovitis of TA (p=0.279), TP (p=0.436), FB (p=0.495) and FL (p=0.315).Conclusion:Clinical examination of RA ankles may be challenging and needs to be coupled with US, which is more sensitive and accurate in the detection of synovitis and tenosynovitis.Disclosure of Interests:None declared
