The article presents data on the level and structure of neuroendocrine tumors in a group of patients under observation bythe Central Military Clinical Hospital named after Mandrykа P.V. of the Ministry of Defense of the Russian Federation. Information about gender and age characteristics of the group of patients is provided.

Abstract: The prevalence of progressive forms of pneumoconiosis in the Russian Federation has been little studied. At the same time, the number of workers exposed to silicon dioxide, both in our country and abroad, is not decreasing. The purpose of our research to determine the criteria for the progression of pneumoconiosis in miners. A retrospective analysis of the results of a comprehensive medical examination of 150 miners of the Rostov region, observed in the center of occupational pathology, was carried out. All patients underwent radiography, computed tomography of the chest organs, and spirometry. X-ray description of the changes was carried out in accordance with the recommendations of the ILO. As a result, 24.5% had statistically significant signs of negative dynamics of functional and radiological indicators, which we referred to the criteria of progression. Thus, the progression of pneumoconiosis is determined by the negative dynamics of clinical and radiological parameters.

In the context of the reform of the Armed Forces of the Russian Federation and a significant increase in the proportion of military personnel doing military service under contract, the issues of manning the troops with healthy, physically developed citizens with high moral and business qualities are of paramount importance. Of particular importance in the selection of candidates for military service under the contract is the conduct of laboratory and instrumental studies, professional and psychological selection, determination of the level of citizens’ physical fitness. The Federal Law «On Military Duty and Military Service» defines a two-stage system for medical examination of citizens entering military service under contract, which is necessary as a barrier in order to prevent citizenswho do not meet the necessary requirements for military personnel from entering the Russian Armed Forces. At the first stage (preliminary examination), the military and medical examination of citizens was carried out by specialist doctors working in medical organizations of the outpatient-polyclinic link of municipalities at the place of citizens’ permanent residence. Medical specialists of the regular military medical commission of the military commissariat of the constituent entity of the Russian Federation participated in the second stage (final examination) of the military medical examination. During the five-year period under study, 5,133 citizens (72.9 %) were selected out of 7,043 candidates for military service under contract, who fully met all the criteria for defenders of the Fatherland.

Aim. To analyze the demographic indices in connection with aging of the population in Perm Krai and the Russian Federation and as a result – to single out problems and ways of solution. Materials and methods. Calculation and dynamic analysis of reproduction of the population in Perm Krai and the Russian Federation over the period of 2005 – 2017 was performed using official statistic data in connection with gender-and-age-specific characterization of the population. Results. The processes of aging of the population were estimated from demographic positions. Socioeconomic problems, associated with them, were singled out, the ways of their solving were determined in accordance with national projects regarding health and health care. Conclusions. At the expense of growing index of expected average life, decrease in fertility and birth rate there is observed accumulation of older age groups of the population, i.e. process of aging. It, naturally, results in growth of chronic pathology, mortality not only in the older age periods but also in the able-bodied part of the population, thus, reducing working potential.

Introduction. The social and medical implication of cardiovascular diseases is determined by their frequency, early disability and high mortality they cause. High mortality of working age men is one of the primary state problems in Russia. The greatest concern is caused by negative changes in health status of men aged 18-27 years who determine the military potential and thus the safety of the country. The defects of public health services and medical support of young men affect the primary criterion of system of preparing citizens to military service effectiveness - fitness for service status. Every 5th young man liable for military service is send to health care facilities for follow-up examination. Also every 4-5th from those on military service discharges from the Armed Forces of the Russian Federation early as improperly called up for military service for medical reasons. Among the reasons for early discharge of military personnel cardiovascular diseases rank first. Objective. To determine prevalence of hypertension as cardiovascular risk factor in military aged male according to physical evaluation results. Materials and methods. An analysis of statistical data provided by State-owned Federal State Institution “The Main Center of Military Medical Examination of the Ministry of Defense of Russian Federation” in the Western Military District in 2011-2016 year and data on results of military-medical examination of recruited men in military registration and enlistment office of Izmaylovo district of Eastern Administrative Okrug in Moscow in 2011-2016 year was conducted. Results. In the Western Military District cardiovascular diseases amounted to 26‰ in 2011, 22.3‰ in 2012, 19.5‰ in 2013, 32‰ in 2014, 32‰ in 2015, and 27‰ in 2016 year. The percentage of newly diagnosed hypertension did not exceed 13.8‰, with its maximum in 2011 year and tendency to decrease through analyzed 6 years (11.4, 10.0, 10.0, 9.0, 7.0‰ respectively). All in all a decrease of total number of young men was observed in Moscow in the last 4 years, from 2011 to 2014. More than 58% military aged male committed to military registration and enlistment office of Izmaylovo district in Moscow have signs of various diseases and are not suitable for military service for health reasons. According to data on disease distribution in the abovementioned district, cardiovascular diseases took the second place and accounted for 12.6% in 2011 year. In the following 2012, 2013, and 2014 years the prevalence of cardiovascular diseases decreased to 8.9, 8.1, and 7.3%, respectively, so they took the 5th place. However, in 2015 and 2016 years cardiovascular diseases were again among the top three, giving way to musculoskeletal and nervous system disorders. Conclusion. Detection of hypertension on initial stages of the disease and differential diagnostics with other diseases with similar clinical symptoms is a crucial task and providing high quality medical support of military men is essential for high military effectiveness of the Armed Forces of the Russian Federation

Introduction. Physical training is the most important element of the combat readiness of the troops, its goals and objectives are defined by the Manual on Physical Training in the Armed Forces of the Russian Federation. The solution of these tasks should contribute to the formation of military personnel's readiness to endure extreme physical and mental stress during the preparation and conduct of combat operations. Comprehensive provision of physical training in the troops requires the hard work of various services, including the medical service.The purpose of the study. Identify problematic issues of medical support for physical training and sports events in the Armed Forces of the Russian Federation and identify ways to solve them.Materials and methods. The article uses the materials of the work of medical and nursing teams in the medical provision of physical training of units of the Southern Military District from 2016 to 2019. Additionally, the data of an in-depth medical examination of the personnel of some units of the district subordination for 2018 were used.Results and discussion. When analyzing the work of visiting medical and nursing teams during the medical support of passing the standards for physical training, a number of problematic issues were identified. First, there are always military personnel who regularly do not engage in physical training and during the delivery of standards make excessive efforts and work at the limit of their capabilities. Secondly, some exercises are very dangerous, either in terms of injuries or the likelihood of developing complications from the cardiovascular system. All this applies to the medical support of the competition. As an example, the championship of the Armed Forces of the Russian Federation in fire and rescue sports, held from 3 to 6 September 2019 in Rostov-on-Don on the territory of the garrison fire brigade, is given. The program of the competition included passing the obstacle course, climbing the training tower, extinguishing the fire, etc., all very traumatic. Therefore, for the medical support of these competitions, a medical and nursing team of the resuscitation profile was allocated on a reanimobile with specialized stowage, respiratory and oxygen equipment.Conclusion. It is necessary to strictly monitor the correctness of the definition of the group of physical training and the gradual increase in physical activity in order to avoid complications and further progression of diseases or even deaths in military personnel. It is impossible to approach formally the conduct of an in-depth medical examination of military personnel.

Currently in healthcare of the Russian Federation created a system of measures to combat the harmful effects of industrial noise on workers. However, the level of specific and nonspecific morbidity rates the effect of industrial noise is not reduced, and the incidence of occupational diseases tends to increase. This is due to several reasons, including a large number of sources of noise, not enough high quality of medical examinations, lack of and low effectiveness of means of individual protection from noise and others. Noise is one of the leading places among the harmful physical factors in the Armed forces of the Russian Federation. Features noise resulting from the operation of military equipment and armament is its high intensity, intermittent nature, the presence in the spectrum of low and infrasonic frequencies. Noise exposure leads to the development of diseases, primarily of the organ of hearing, increase in total morbidity and the reduction of military-professional health. In the normative documents of the military medical service the noise is not identified as a harmful factor leading to the development of occupational diseases that were not fully developed the issues of professional selection and medical examination of noise pathology. System noise control should be comprehensive and include organizational and technical measures, special assessment of working conditions, monitoring of noise sources, the presence and correct application of means of protection against noise professional selection, clinical monitoring, medical examination, treatment and preventive measures. Existing in the Armed forces of the Russian Federation the system of measures for the fight against harmful impact of noise on military personnel requires revision in accordance with the existing state legislative framework.

Acute hellebore poisoning is characteristic for the Russian Federation because of the use of this plant for the treatment of alcoholism at home. The varying severity of clinical symptoms, which sometimes become lifethreatening, the lack of anamnestic reliable information about the use of hellebore and the difficulties of chemical and toxicological confirmation of its use complicate the diagnosis of acute poisoning, and do not allow timely medical care. In addition, there is no classification of hellebore poisoning in ICD-10, block (T51-T65), which also affects the assessment of the actual frequency of this acute intoxication. The purpose of this work is to clarify the number of acute hellebore poisoning in the Russian Federation. The analysis of medical records of patients hospitalized with acute hellebore poisoning in toxicological departments of a number of subjects of the Russian Federation for 2014 - 2018 was carried out. The dynamics of such acute poisoning over the specified period, the gender and age composition of victims, and the circumstances of poisoning were revealed. The relevance of developing methods of chemical and toxicological analysis for the determination of hellebore alkaloids is confirmed, which will increase the reliability of diagnosis and detection of cases of hellebore poisoning.

The relevance of this research was determined by the need for enhancement and increase in the efficiency of routine medical examinations of minors in the Russian Federation. The information and statistical materials of medical examination of the children’s population in 2014 in terms of the health of various age groups in a section of federal districts and subjects were studied. The following indicators were studied: coverage of the children’s population by routine medical examination; level and structure of the revealed general and primary morbidity; frequency of dispensary registration; need for additional consultations; and researches and treatment in an outpatient clinic, a day hospital, a hospital with round-the-clock services, and sanatorium organizations as well as coverage by these medical services. A high level of coverage of minors by routine medical examinations was registered. More than one-third of the healthy children’s population passed the medical examination. The revealed general and primary morbidity of the children’s population in more than one-third of subjects is above mean Russian levels. Diseases of the respiratory organs, bone, and muscular and nervous systems are the most common among children, whereas diseases of the bone and muscular system, eyes, and respiratory organs are common among teenagers. Despite the high prevalence of chronic diseases among the children’s population, recommendations for treatment and rehabilitation were given insufficiently during routine medical examinations. Regional characteristics of the medical examination results, which allowed for the identification of the most disadvantaged areas and defects in its organization and the lack of continuity between medical institutions providing this type of service, have been established.
