Issues of medical examination and selection of citizens for military service under a contract in Armed Forces of the Russian Federation

In the context of the reform of the Armed Forces of the Russian Federation and a significant increase in the proportion of military personnel doing military service under contract, the issues of manning the troops with healthy, physically developed citizens with high moral and business qualities are of paramount importance. Of particular importance in the selection of candidates for military service under the contract is the conduct of laboratory and instrumental studies, professional and psychological selection, determination of the level of citizens’ physical fitness. The Federal Law «On Military Duty and Military Service» defines a two-stage system for medical examination of citizens entering military service under contract, which is necessary as a barrier in order to prevent citizenswho do not meet the necessary requirements for military personnel from entering the Russian Armed Forces. At the first stage (preliminary examination), the military and medical examination of citizens was carried out by specialist doctors working in medical organizations of the outpatient-polyclinic link of municipalities at the place of citizens’ permanent residence. Medical specialists of the regular military medical commission of the military commissariat of the constituent entity of the Russian Federation participated in the second stage (final examination) of the military medical examination. During the five-year period under study, 5,133 citizens (72.9 %) were selected out of 7,043 candidates for military service under contract, who fully met all the criteria for defenders of the Fatherland.

This article examines and analyzes the basic directions of development and improvement of the system of manning of the Armed Forces of Ukraine with the sergeants (and other enlisted grades) , based on the experience gained and on analysis of manning systems of the leading countries of the world. In Ukraine, with the onset of the armed aggression of the Russian Federation, the tasks assigned to the personnel services and military commissariats required rapid response and aimed at implementing mobilization deployment of the Armed Forces of Ukraine. 126 To date, the main focus on staffing the Armed Forces of Ukraine is the priority of providing operational (tactical) grouping of troops to contracted military servicemen to accomplish tasks during the operation of the Joint Forces and further readiness of the troops (forces) to repel the armed aggression. At the same time, an important issue to resolve is the recruitment of a professional sergeant (and other enlisted grades) for the military service, with the encouragement of military personnel to prolong their military service. The article describes the reasons that may deter citizens from joining the military service and ways to resolve this issue. Particular attention in the article is devoted to the work of recruiting units, which provide selection for contracted military service using the latest selection technologies and techniques. The importance of the practical activity of the territorial recruitment and social support centers for the instructors of the recruiting and recruitment departments is also considered.

This article reveals the problem of the role of the religious factor in the formation of the value orientations of the military personnel of the Russian army from its inception to the present state. In the article, the author reveals the significance of the Church in the formation of the value orientations of military personnel. The problem of religious situation in foreign armies is considered. The article presents data from sociological studies confirming the increase in the number of religious servicemen in the modern Armed Forces. The concept of “religious factor” is revealed. The author suggests considering the influence of the religious factor on the formation of value orientations through the functions of religion. The article provides examples of the influence of religion on the formation of value orientations of military personnel from the time of the Baptism of Russia to the present. Examples of writers of Russian classical literature about the influence of religion on the morale of troops are given. Examples of religious participation in major battles and wars of the past years are shown. The significance of the religious factor in uniting the people and the army is shown. The work of officials of the Ministry of defense of the Russian Federation in strengthening values among military personnel in modern conditions is demonstrated. The role of the Minister of defense of the Russian Federation, General of the army S.K. Shoigu in strengthening the faith of the Russian army is outlined. Issues related to the construction of the Main Temple of the Armed Forces and its impact on the public masses were discussed. In this study, the author aims to show the significant role of religion in the formation of value orientations in Russian military personnel. The analysis shows an increasing role of religion in the minds of military personnel in modern conditions.

Provisions and requirements of the Internal service regulations of the Armed Forces of the Russian Federation regarding the rights and obligations of military personnel for life and health protection are analyzed. Provisions in regulations of 1993 and 2007 years are compared. The compliance of articles of the Regulations to the Constitution of the Russian Federation, Federal laws from May 27th, 1998 No. 76 «Concerning the status of military personnel», from November 21st, 2011No. 323 «On fundamental healthcare principles in the Russian Federation» and from March 30th, 1999 No. 52 «Concerning the Sanitary and Epidemiological Welfare of the Population» are described. Insufficient legal confirmation of the duties of a serviceman in protection of his own health is noted. One of the problems of education and training of the military personnel of various classes is the lack of such subject as a military hygiene in the curriculum. In prerevolutionary military educational establishments, the hygiene was studied and at the course end there was examination. One of the important elements of the health protection of the servicemen is a sanatorium-resort therapy which can be treated as a part of preventive medical examination. Data on decline of sanatorium service of the military personnel in connection with the legislation changes are presented. Considering physical and mental health of servicemen as a guaranty of high-degree alert and military efficiency it is proposed to start studying the Instruction on life and health protection of the serviceman in the period of a basic military training (soldier, sailor, cadet). Proposals on hygiene studying in fostering and educational institutions of the Ministry of Defense.

Introduction. The social and medical implication of cardiovascular diseases is determined by their frequency, early disability and high mortality they cause. High mortality of working age men is one of the primary state problems in Russia. The greatest concern is caused by negative changes in health status of men aged 18-27 years who determine the military potential and thus the safety of the country. The defects of public health services and medical support of young men affect the primary criterion of system of preparing citizens to military service effectiveness - fitness for service status. Every 5th young man liable for military service is send to health care facilities for follow-up examination. Also every 4-5th from those on military service discharges from the Armed Forces of the Russian Federation early as improperly called up for military service for medical reasons. Among the reasons for early discharge of military personnel cardiovascular diseases rank first. Objective. To determine prevalence of hypertension as cardiovascular risk factor in military aged male according to physical evaluation results. Materials and methods. An analysis of statistical data provided by State-owned Federal State Institution “The Main Center of Military Medical Examination of the Ministry of Defense of Russian Federation” in the Western Military District in 2011-2016 year and data on results of military-medical examination of recruited men in military registration and enlistment office of Izmaylovo district of Eastern Administrative Okrug in Moscow in 2011-2016 year was conducted. Results. In the Western Military District cardiovascular diseases amounted to 26‰ in 2011, 22.3‰ in 2012, 19.5‰ in 2013, 32‰ in 2014, 32‰ in 2015, and 27‰ in 2016 year. The percentage of newly diagnosed hypertension did not exceed 13.8‰, with its maximum in 2011 year and tendency to decrease through analyzed 6 years (11.4, 10.0, 10.0, 9.0, 7.0‰ respectively). All in all a decrease of total number of young men was observed in Moscow in the last 4 years, from 2011 to 2014. More than 58% military aged male committed to military registration and enlistment office of Izmaylovo district in Moscow have signs of various diseases and are not suitable for military service for health reasons. According to data on disease distribution in the abovementioned district, cardiovascular diseases took the second place and accounted for 12.6% in 2011 year. In the following 2012, 2013, and 2014 years the prevalence of cardiovascular diseases decreased to 8.9, 8.1, and 7.3%, respectively, so they took the 5th place. However, in 2015 and 2016 years cardiovascular diseases were again among the top three, giving way to musculoskeletal and nervous system disorders. Conclusion. Detection of hypertension on initial stages of the disease and differential diagnostics with other diseases with similar clinical symptoms is a crucial task and providing high quality medical support of military men is essential for high military effectiveness of the Armed Forces of the Russian Federation

Introduction. Physical training is the most important element of the combat readiness of the troops, its goals and objectives are defined by the Manual on Physical Training in the Armed Forces of the Russian Federation. The solution of these tasks should contribute to the formation of military personnel's readiness to endure extreme physical and mental stress during the preparation and conduct of combat operations. Comprehensive provision of physical training in the troops requires the hard work of various services, including the medical service.The purpose of the study. Identify problematic issues of medical support for physical training and sports events in the Armed Forces of the Russian Federation and identify ways to solve them.Materials and methods. The article uses the materials of the work of medical and nursing teams in the medical provision of physical training of units of the Southern Military District from 2016 to 2019. Additionally, the data of an in-depth medical examination of the personnel of some units of the district subordination for 2018 were used.Results and discussion. When analyzing the work of visiting medical and nursing teams during the medical support of passing the standards for physical training, a number of problematic issues were identified. First, there are always military personnel who regularly do not engage in physical training and during the delivery of standards make excessive efforts and work at the limit of their capabilities. Secondly, some exercises are very dangerous, either in terms of injuries or the likelihood of developing complications from the cardiovascular system. All this applies to the medical support of the competition. As an example, the championship of the Armed Forces of the Russian Federation in fire and rescue sports, held from 3 to 6 September 2019 in Rostov-on-Don on the territory of the garrison fire brigade, is given. The program of the competition included passing the obstacle course, climbing the training tower, extinguishing the fire, etc., all very traumatic. Therefore, for the medical support of these competitions, a medical and nursing team of the resuscitation profile was allocated on a reanimobile with specialized stowage, respiratory and oxygen equipment.Conclusion. It is necessary to strictly monitor the correctness of the definition of the group of physical training and the gradual increase in physical activity in order to avoid complications and further progression of diseases or even deaths in military personnel. It is impossible to approach formally the conduct of an in-depth medical examination of military personnel.

Most of the military conflicts in the last decades with participation of the Armed forces of our country took place in the mountain landscape, namely Afghanistan, counterterrorist operations in the Northern Caucasus, peacemaking missions in South Ossetia and Abkhazia. The warriors passed through «trouble spots» know that a war in mountains is not the same as a war in plain and it requires special preparation. It is necessary to take into consideration factors of climate and geography which influence medical provision in mountains as well as sanitary casualty’s formation and mode of evacuation and treatment. Based on literature data, the historical facts of warfare conduction in mountains were presented. Factors of climate and geography adversely affected the human health during warfare conduction in mountains. The analysis of the mountain training medical provision organization was carried out. Based on that some proposals for improvement of mountain training medical provision of military personnel of the Armies forces of the Russian Federation were formulated

The issues of prevention of suicidal incidents in the Armed Forces of the Russian Federation are considered. The analysis of suicide prevalence among military personnel of various categories and their prevailing causes for the period from 2013 to 2017 is indicated. Indicates a change in the percentage ratio of various categories of military personnel who committed suicide, with a steady increase in the proportion of privates and sergeants serving in military service under the contract (from 24,6% in 2011 to 57,4% in 2017). It was established that among the servicemen under the contract family and everyday reasons for suicide prevailed, while conscripts were military professional. A comparison was also made of the long-term (2007-2017) dynamics of suicide rates and the incidence of mental disorders among military personnel, and certain patterns were identified. Thus, in the group of officers and ensigns, a significant correlation was noted between the long-term dynamics of suicide rates with neurotic (r=0,74) and addictive disorders (r=0,86), as well as general mental morbidity (r=0,83), and for conscripts, with personality disorders (r=0,79) and organic mental disorders (r=0,71). A comparison of the long-term dynamics of the overall incidence of mental disorders and the suicide rate among conscripts also showed a positive correlation (r=0,69). Analyzed the system of prevention of suicidal incidents in military personnel, which currently exists and proposed measures for its improvement, including the areas of activity of officials for the prevention of suicidal incidents in the military. It also indicates the need to shift the focus from specific prevention (direct detection and prevention of suicidal actions in military personnel with mental disorders) to non-specific (prevention of general mental distress and suicidal tendencies in mentally healthy people).

Eating behavior is a selection of definite food subconsciously aimed at meeting physiological needs from main nutrients and energy. Genetic aspects of human metabolism as well as various environmental factors including social-psychological factors have a significant impact on the appearance and a further retention of definite food excesses of each person. Healthy lifestyle promotion (including compliance with nutrition prescription, preference to balanced daily rations cor responding with the level of daily energy needs) helps to form human eating behavior based on nutrition laws that promotes preservation and strengthening of public health (the military personnel). Formation of proper eating behavior may become an important preventive measure for preservation and strengthening of the health of the military personnel of the Armed Forces of the Russian Federation and the Navy of the Russian Federation. Increasing of daily energy needs due to introduction of new model of physical fitness in the Armed Forces of the Russian Federation exerts a considerable impact on an eating behavior in the military personnel. Correction of an eating behavior in the military draft servicemen in the Armed Forces of the Russian Federation as well as observance of the principles of healthy nutrition may be conducted by means of outreach interviews with specified cohort. Possible changes in eating behavior may be monitored using special tests and questionnaires allowing to execute its overall assessment. Dynamic assessment of eating behavior may take rightful place in the system of scientific study of lifestyle of the military and also be in active use on further improvement of food service system of the Armed Forces of the RF and the Navy of the RF. Significance of assessment of eating behavior in the military will not be irrelevant in the near future but, to the contrary, will gradually increase in connection with advances in science, promotion of food fit to personal genetic aspects and improvement of food service system of the Army Forces of the RF.

This article examines the issues of the activity of the established law enforcement structure-the Federal service of the national guard of the Russian Federation (Regardie) under the prism of the norms of criminal procedure that regulate the activities of the bodies of inquiry. The author identifies unresolved departmental and interdepartmental problems that are mainly related to the incompleteness of the legal registration of this structure, including at the level of relations between the military personnel of the Russian Guard and the military police of the Armed forces of the Russian Federation.
